DENTAL TREATMENT OF PATIENTS WITH DOWN SYNDROME UNDER GENERAL ANESTHESIA (다운증후군 환자의 전신마취 하 치과치료)

  • Background: Down's syndrome, or trisomy 21, is the commonest congenital chromosome anomaly. With improvement in medical care, these patients increasingly reach adulthood in spite of their physical maldevelopment and mental retardation. And, the number of those who required general anesthesia for dental treatment is increasing. Methods: We reviewed the 26 cases of 22 patients with Down's syndrome who underwent outpatient general anesthesia for dental treatment at the clinic for the disabled in Seoul National University Dental Hospital. Results: The mean age was 22 years. They all had severe mental retardation and some had congenital heart anomaly, epilepsy, hypothyroidism, acute leukemia, autism, cleft palate, and chronic renal failure. For anesthesia induction, 4 cases was needed physical restriction, but others showed good or moderate cooperation. Drugs used for anesthesia induction was thiopental (17 cases) and sevoflurane (9 cases). All patients received nasotracheal intubation and 3 cases needed difficult airway management. Mean total anesthetic time was $166{\pm}60$ min and staying time at PACU was $92{\pm}48$ min. There was no death or long term hospitalization because of severe complications. Conclusion: If general anesthesia is needed, pertinent diagnostic tests and workup about anomaly, and appropriate anesthetic planning are essential for safety.

  • A critical pathway (CP) defines the optimal care process, sequencing and timing of intervention by multi-disciplinary health care teams for a particular diagnosis and procedure. It plays an important role as a cost-effective health care delivery system and a tool for quality control of medical and dental services by means of standardizing medical practices. The aim of this study is to investigate the satisfaction of patients and medical/dental staff after implementation of a critical pathway for dental treatment of disabled children under general anesthesia and its cost effectiveness. Ten patients who underwent dental treatment under general anesthesia were included in the CP group between August and December 2006. The pre-CP group included 20 patients who underwent the same procedure from February 2003. The satisfaction of parent of child patient and medical staff members were compared between two groups. The parents' satisfaction was significantly improved after the implementation of CP and medical/dental staff members were highly satisfied with the usefulness of the critical pathway. In conclusion, the critical pathway for the dental treatment of disabled treatment under general anesthesia can highly improve the satisfaction of parents and medical/ dental staff members.

Effects of Family Presence Intervention on Anxiety, Delirium, Pain and Length of Time in Recovery Room of Post-operative Elderly Patients in Post-anesthesia Care Units (회복실 보호자 상주가 전신마취 노인수술환자의 수술직후 상태불안, 각성섬망, 통증 및 회복실 체류시간에 미치는 영향)

  • Purpose: The objectives of this study were to assess the influence of family presence in the PACU (Post Anesthesia Care Unit) on anxiety, emergency delirium, pain and length of stay in the recovery room for elderly patients undergoing surgery for which general anesthesia has been used. Methods: The study was a nonequivalent control group pre-post test design. Eighty elderly patients over 65 years who underwent surgery under general anesthesia were recruited. Forty were assigned to the experiment group, patients together with a family member and 40 to the control group, with no family member present. Patients' anxiety, emergency delirium, pain and length of time in the recovery room were evaluated at 10 minute and 30 minute after arrival in the PACU. Results: Patients with family members in the PACU showed significantly decreased levels of anxiety at 10 and 30 minutes and significantly lower levels of emergency delirium and pain at 30 minutes. However there was no difference between the 2 groups for length of time in the recovery room. Conclusion: The results of this study indicate that being with family members in the PACU after surgery under general anesthesia is effective for reducing elderly patients' anxiety, delirium and pain during time in the recovery room.

Rectal Prolapse in Children (어린이 탈직장의 경화요법)

  • Because rectal prolapse in pediatric age was known to have a self-limitting natural history in weeks to years, this disease is prone to be regarded as a minor condition to the most of surgeons. But to the children and the parents who have to be suffered each time could be a heavy distress. Even though operative or nonoperative methods can be applicable for treatment, the main problem is in surgeon's side, whose preference is based on the experiences of adult patients. The authors have experienced 16 cases of ano-rectal prolapse for 9 years since 1986. Eleven of them were true rectal prolapses. In 7 cases of true type, injection therapy has been tried. One ml of five percent phenol in glycerine was injected into the submucosal layer of the ano-rectal angle level at both lateral and posterior sides. After first trial of each cases, 5 of them were cured completely so far. Recurrences were in two cases, but one of them was temporary to be subsided afterward. Complete bowel cleansing and adequate sedations were required as preoperative preparations. Two days' oral antibiotics and two weeks' laxatives for free of defecation straining were recommended after the procedure. The safety of sclerosis was supported by the experimental histology. In pediatric rectal prolapse, sclerosis seems to be a safe and effective treatment of choice without any significant morbidity.

Ossifying Epulis in a Thoroughbred Foal (더러브렛 망아지에서 발생한 골화성 치은종)

  • A 4-month-old female thoroughbred foal was presented with large mass on mandibular gingiva. The enlarged mass was firm and yellowish pink in color, and occupied the whole mandibular arcade. The filly could not close her mouth with her lips. Radiographs showed radioopaque mass under the mandible. The mass was surgically excised from mandible. Histopathologically, gingival mass was characterized by hyperplastic gingival epithelium, well vascularized collagenous stroma and large area of bony tissues. Based on the gross and histopathologic findings, this case was diagnosed as ossifying epulis in the mandibular gingiva. This is the first report of ossifying epulis in a thoroughbred horse in Korea.

  • Hemophilia is a group of gender-linkage inherited bleeding disorders that impair the body's ability to control blood clotting or coagulation. This sex-linked disorder is transmitted on the X chromosome. These genetic disorders have lower blood plasma clotting factor level of coagulation factor. Most common form is Hemophilia A and B. Restorative dental care and simple surgery for the hemophiliac patient are quite often neglected for fear of bleeding during procedures. Even dental specialist avoid these patients and make them severe problem patients. On the dental treatment especially, invasive procedure, special considerations of bleeding control are required. Inter-consultation with the hematologist will provide orientation on the best approach to dental treatment, such as the need of replacement therapy, the modification of antifibrinolytic therapy, the application of local hemostatic methods. In this case reports, we successfully treat early childhood caries of patient with hemophilia B under the general anesthesia.

  • The patients with primary lung cancer who have symptoms of angina pectoris commonly have underwent coronary artery bypass grafting (CABG) prior to lung resection. Recently, there are attempts to conduct simultaneous operation of CABG and lung resection to reduce disadvantages of staged operation, such as repeated general anesthesia, two wound incisions and delay of lung operation. These attempts generally report good results. Three patients underwent lung resection and CABG simultaneously, and had good post operative status without recurrence of cancer and angina pectoris. We conceive these as worthy cases to report.

Surgical Treatment of Occluded Aberrant Left Subclavian Artery with Right-sided Aortic Arch -A case report- (우측 대동맥궁을 가진 환자에서 이상 기시된 좌쇄골하 동맥 폐색의 수술적 치료 - 1예 보고)

  • A 57-year-old man with numbness and paresthesia of left arm is presented. There was no pulse in the left arm was absent and his chest radiograph suggested right-sided aortic arch. The aortogram showed right-sided aortic arch with Kommerell's diverticulum. The proximal portion of left subclavian artery was totally occluded and blood was being supplied through vertebral arteries to distal subclavian artery. He underwent bypass grafting between both subclavian arteries by an expanded polytetrafluoroethylene graft. Because the size of Kommerell's diverticulum was small, it need to be observed closely.

A Study on the Penetration of Dexamethasone into Oral Mucosa with the Use of Iontophoresis (이온영동법에 의한 Dexamethasone의 구강점막에의 침투에 관한 연구)

  • 이온 영동법은 전기력의 도움으로 이온화된 물질의 신체조직내 침투를 증가시키는 술식으로서 전신적 부작용은 줄어드는 반면, 국소부위의 약물농도를 증가시킬 수 있다는 장점 때문에 효과적인 국소요법으로 인정받고 있다. 치의학 분야에서는 과민상아질의 치료를 위해 불소 이온영동법이 빈번히 이용되어져 왔으며, 국소마취제나 항바이러스 제재의 도포시에도이용되었다.또,이온 영동법에 의한 스테로이드 투여로 피부나 구강점막의 염증성 질환의 효과적 치료를 보고한 많은 문헌이 있으나, 이온영동법에 의한 스테로이드의 구강점막에의 침투량이나 분포에 관해서는 거의 소개된 바가 없는 실정이다. 본 연구는 방사선 동위원소가 부착된 dexamethasone을 이온영동법을 이용하여 가토의 협점막에 침투시킨후 자기방사선 술식에 의해 그 침투량과 분포를 대조군과 비교 평가하였으며 다음과 같은 결과를 얻었다. 1. 이온영동법은 단순 국소도포에 비해 dexamethasone과 0.1M 인산소다 완충용액의 혼합액(dexamethasone in 0.1M sodium phosphate buffer solution)의 가토 협점막 침투량을 증가시켰으며, 양극을 사용하였을 때 더 효과적이었다. 2. Dexamethasone과 0.1M 인산소다 완충용액의 혼합액 투여 4시가, 24시간후 까지도 양극 잉온영동법이 효과적이었으며 은입자의 감소는 투여 4시간부터 24시간 후 사이에 주로 일어났다. 3. 인산소다 완충용액의 첨가는 양극 및 음극에 의한 이온영동법 모두에 효과적이었으며, 양극에 가장 효과적이었고 단순도포군에는 영향을 미치지 않았다. 4. 이온영동법에 의한 스테로이드 투여는 피부뿐만 아니라구강점막 염증성 병소의 효과적 치료술식으로 여거질 수 있다. 시와 maximal clenching시 사이의 치아 접촉시간에서도 유의한 상관관계를 보였다.

Spontaneous Submucosal Dissection of the Esophagus -Report of 1 Case- (자발성 점막하 식도 박리의 치료 - 1례 보고 -)

  • A 55-year-old male was admitted to our hospital complaining of chest discomfort, dysphagia and severe pain on swallowing. Esophageal manometry showed that there was no relaxation of lower esophageal sphincter to swallowing. There was double barrelled esophagus or mucosal stripe appearance on esophagogram. Endoscopy revealed an appearance as if an esophago-tracheal fistula had been made at the level of the upper thoracic esophagus. Also, two slit-like mucosal tears was seen on the lower esophagus. There after, fasting and total parenteral nutrition for several weeks failed to bring about any changes in his symptoms. So, as treatment, primary closure of the upper opening of the false lummen was performed under general anesthesia. Soon after the surgical procedure, the patient's symptoms were improved except for mild dysphagia. He was discharged after oral intake had been juduciously commenced with fluids and soft diet subsequently. During follow-up in out-patient department, he had no specific symptoms including fever or dysphagia and massive dissection of the esophagus was improved on esophagogram. We report the experience of a case of spontaneous submucosal dissection of the esophagus which required conservative and surgical management
