  • Spondylolysis and spondylolisthesis are two conditions that directly involve changes in the vertebra. Spondylolysis is defined as a defect in the pars interarticularis, the region of the lamina between the superior and inferior articular facets. Progression of the defect can result in spondylolisthesis, which is defined as a subluxation or 'slippage' of two adjacent vertebrae. In the low back it occurs most commonly at the lumbosacral level; next in frequency is spondylolisthesis of the fourth lumbar vertebra on the fifth. To provide the reader with information about the aetiology and anatomical consideration of spondylolysis and spondylolisthesis type. Spondylolisthesis has recently been classified by Wiltse and others into five types based on the suspected aetiology; dysplastic, isthmic, degenerative, posttraumatic, pathologic and postsurgical. Of these five types, isthmic spondylolisthesi and degenerative spondylolisthesis, both of which are frequently associated with low back and lower limb pain.

Arthroscopic Reconstrucion in Megafrequency of Recurrent Anterior Shoulder Dislocations (관절경을 이용한 고 빈도 재발성 전방 견관절 탈구의 재건술)

  • Purpose: To evaluate the short-term follow up outcome and the effectiveness of arthroscopic vertical shift of anteroinferior capsulolabral complex, plication of AIGHLC (anterior band of inferior glenohumeral ligament complex) and thermal capulorraphy, posteroinferior suture plication, rotator interval closure as an adjuncts in recurrent instability of megafrequency with night time dislocation. Materials & Methods: From March 1998 to February 2004, we have had 18 shoulders out of 156. All of the cases have been night time dislocation above more one time. The age were average 29.4$(21{\sim}37)$ year old. The average follow up were 21.1 $(12{\sim}45)$months We checked Rowe score and ROM at pre-operation, post-operation 6 months,1 year & last follow up period. Results: Above good results were 16 cases(88.9%). The excellent were 6 cases, good were 10, fair 1, poor 1. Conclusions: In recurrent anterior shoulder dislocation in greater than 50 frequency, arthroscopic stabilization can be an alternative technique for selected patients against open inferior capsular shift. But more long ter n follow up and large materials will be needed in the future study.

Arthroscopic Anterior Cruciate Ligament Reconstruction using Fresh Frozen Achilles Allograft - Clinical results, 2nd look Arthroscopic and Histologic findings - (신선 동결 아킬레스 동종건을 이용한 전방 십자 인대 재건술 - 임상적 결과, 2차 관절경 및 조직학적 소견 -)

  • Purpose : We observed usability of allograft by evaluating the clinical results, second look arthroscopic and histologic findings at last follow up after anterior cruciate ligament(ACL) reconstruction using by fresh frozen Achilles allograft. Materials and Methods : We analyzed in 58 patients(59 cases) by subjective and objective parameters, Telos stress arthrometer and Modified Feagin Scoring System. The average age and follow up period was 25 years(ranging from 18 to 49 years), and 15 months(ranging from 12 to 19 months). Among them we studied second look arthroscopic and histologic findings in 16 patients. Results : The mean Lysholm Score wits improved from 60 to 88.2 as well the anterior translation was improved from 7.1 min to 2.3mm at last follow up. The second look arthroscopic findings were close to normal in ligament thickness, tension and showed revasculization at the holly attachment site. Light microscope(LM) findings showed dense cellularity like a normal ligament. In electron microscope(EM) collagen fibrils showed parallel arrangement longitudinally and unimodal pattern diameter close to normal tissue in the transverse section. Conclusion : We think that the fresh frozen Achilles allograft is a good substitution of autograft in cruciate ligament reconstruction.

A Study on the Standardization of the Test Method Upon Testing the Anterior Cruciate Ligament Damage Using TELOS (TELOS를 이용한 Knee Stress (Lachman)검사의 표준화에 대한 연구)

  • This study aims to find out the standardized test criteria regarding patients with the anterior cruciate ligament damage by identifying the degree of the flexion gap of femur and tibia upon bending of the anterior cruciate ligament in order to carry out the accurate test of such impairment. On the standardized test method and judgement criteria upon the anterior cruciate ligament test using Telos, it has been shown that there was no significant difference in the results according to the position of a fixed roller in the anterior cruciate ligament test for normal patients. However, in a test for patients who had undergone the anterior cruciate ligament reconstruction, it has been shown that the measured values of the anterior cruciate ligament tended to be pushed when the position of a fixed roller was less than 1cm in the test according to the position of a fixed roller of Telos (less than 1cm, more than 3cm), and this was statistically significant. The anterior cruciate ligament test (knee stress test) is a limited method used in orthopedics and rehabilitation medicine, and there have been no standardized test guidelines available yet although numerous ligament measurement tests have been performed. In addition, since the measured values are often different depending on testers even on the test that is expected to give the same result, the reproducibility of the test is still low. Accordingly, it is considered that the criteria for the anterior cruciate ligament test need to be established, and this would contribute to the accuracy of the diagnosis through the accurate test and standardized measurements.

Clinical Study of the Treatment of Spondylolisthesis (척추전방전위증의 한방 치료에 관한 임상적 고찰)

  • Objectives : This study was performed to evaluate the clinical results of the spodylolisthesis patients treated with oriental medical methods. Methods : We diagnosed spondylolisthesis by L-spine simple x-ray. 28 patients with spondylolisthesis were hospitalized at WonKwang University KwangJu Oriental Medical Hospital from April 1997 to January 2000 and treated by acupuncture, moxibustion, bed rest, herb-medicine and physical therapy. Results : 1. Distribution showed female predominence(82.1%) in general. 2. In the duration of symptoms, the largest group was the group of "acutest" (32.1%). 3. In the distribution of causes, the largest group was "overwork" (35.7%). 4. Isthmic type is the most common(75%) in etiology. 5. Isthmic type involved the 5th lumbar vertebra in 57.1% and degenerative type involved the 4th lumbar vertebra in 57.1%. 6. Most of cases were grade I(96.4%) in degree of slipping. 7. The common symptoms were radiating pain(92.9%) and low back pain(78.6%). 8. Therapeutic efforts above "good" by Kim's criteria was 20 cases(71.4%) Conclusions : The result of treatment by oriental medical methods is satisfactory for the treatment of spondylolisthsis.

Comparative Medium Term Results of Arthroscopic ACL Reconstruction with Quadrupled Semitendinosus Tendon versus BPB tendon (4겹의 반건양건과 골-슬개건-골을 이용한 관절경적 전방 십자 인대 재건술의 중기적 치료 결과 비교)

  • Purpose : The purpose of this study was to compare the postoperative success and stability of arthroscopically assisted anterior cruciate ligament(ACL) reconstructions using central one third bone-patellar-tendon bone(BPB) autograft versus quadrupled semitendinos-us(ST) autograft as the medium term review. Materials and Methods : Eighty patients(40 BPBs,40 STs) with isolated ACL injury were available for a mean follow up of 49.4 months(BPB) and 48.8 months(ST). There was no significant difference between the two groups with respect to age and sex. We compare the final results between two groups with respect to subjective Lysholm score, objective laxity including anterior drawer test, Lachman test, pivot shift test, KT-2000 measurements and International Knee Documentation Committee(IKDC) evaluation system. Results : Postoperatively, positive anterior drawer test was found in $22.5\%\;and\;27.5\%$, positive Lachman test was found in $30.0\%$ and $25.0\%$ and positive pivot shift test was found in $15\%\;and\;20\%$ of the ST and BPB group, respectively(p>0.05). Mean side to side difference of KT-2000 at 20 lbs was 2.2 mm for the ST group and 2.1 mm for the BPB group. There was no significant difference between the two groups about Lysholm score(>0.05). Anterior knee pain was knee common in the BPB group. Eighty-three percent of the patients were nearly normal according to the IKDC grade in both groups. Conclusion : We consider that autogenous semitendinosus tendon is a good alternative subsititute in ACL reconstruction together with the bone-patellar tendon-bone autograft.

Relationship between Graft Appearance on Follow-up MRI and Knee Stability after Double Bundle ACL Reconstruction (이중 다발 전방십자 재건술 후 이식건의 자기공명영상 추시와 슬관절 안정성과의 관계)

  • Purpose: This study examined the relationship between graft appearance on follow-up magnetic resonance imaging (MRI) and knee stability after double bundle anterior cruciate ligament (ACL) reconstruction. Materials and Methods: For each patient, 1.5 tesla MRI's were obtained. The signal intensity of grafts was divided into 3 grades by Sononda's classification. The course of grafts was divided into two patterns: straight and curved. We assessed Lachman test, KT 2000 arthrometer and anterior drawer stress radiograph using Telos$^{(R)}$ in $30^{\circ}$ knee flexion for anterior stability and evaluated pivot shift test for rotatory stability. The correlation between graft appearance on MRI and the results of knee stability tests was evaluated. Results: The anteromedial (AM) graft was evaluated as being grade 1 in 66.7%, grade 2 in 26.7%, and grade 3 in 6.7% of the cases and the posterolateal (PL) graft was assessed as being grade 1 in 63.3%, grade 2 in 33.3%, and grade 3 in 3.3% of the cases according to the signal intensity. The AM graft was evaluated as being straight in 83.3% and curved in 16.7% of the cases, and the PL graft was assessed as being straight in 86.7% and curved in 13.3% of the cases according to the course. The course of AM graft was correlated with the results of anterior stability tests and the course of PL graft was correlated with the result of rotatory stability test. However, the signal intensity of grafts was not correlated with the results of anterior stability and rotatory stability tests. Conclusion: The course of AM is correlated with anterior stability and the course of PL is correlated with rotatory stability on follow-up MRI after double bundle ACL reconstruction.

Tardy Ulnar Nerve Palsy Secondary to the Anconeus Epitrochlearis Muscle - 2 Case Report - (활차상 주근에 의한 지연성 척골신경 마비 - 2예 보고 -)

  • Purpose: Here we present tardy ulnar nerve palsy cases that were caused by the anconeus epitroch-learis muscle. Materials and Methods: A 37 year old female patient presented with tardy ulnar nerve palsy findings for the elbow. During surgery, an atypical anconeus epitrochlearis muscle was observed, which originated from the triceps aponeurosis and inserted into the medial intermuscular septum. Muscle resection, nerve decompression and ulnar nerve anterior transposition were done. A 35 year old male patient presented with tardy ulnar nerve palsy findings for the elbow. During surgery, a typical anconeus epitrochlearis muscle was found. Muscle resection, nerve decompression and ulnar nerve anterior transposition were done. Results and Conclusion: Tardy ulnar nerve palsy can be associated with and caused by either an atypical or a typical anconeus epitrochlearis muscle.

Radiologic Features and Surgical Outcome of Juxtafacet Cyst Associated with Degenerative Lumbar Disease (퇴행성 요추 질환에서 발생한 후관절 근접 낭종의 방사선적 특징 및 수술의 결과)

  • Purpose: The purpose of this study was to evaluate the radiologic features of juxtafacet cyst and determine the correlation between these features and clinical outcome. Materials and Methods: We analyzed a total of 23 patients. The degree of facet joint degeneration was classified using the Fujiwara method. The facet joint angles were measured with an magnetic resonance imaging to determine whether there was a difference between the cystic lesion that was occupied and the cystic lesion that was not occupied. Disc degeneration was measured by the Pfirrmann classification method. The clinical result was evaluated using the Oswestry disability index score and visual analogue scale. Results: The L4-5 level of juxtafacet cyst was mostly affected, as found in previous studies. Facet joint arthritis was more severe within the side with the cystic lesion. Significant correlation was found between disc degeneration and juxtafacet joint cyst. All patients underwent wide decompression and fusion. Clinical result was excellent. No patients had signs of recurrence during the follow-up periods. Conclusion: Juxtafacet cyst has a significant correlation with facet joint degeneration. Therefore, aggressive surgical treatment-not just simple cyst excision-should be considered as the treatment option for juxtafacet cyst associated with degenerative lumbar disease.

Operative Treatment of the Displaced Bucket Handle Tear of the Medial Meniscus (내측 반월상 연골의 전위된 양동이 손잡이형 파열의 수술적 치료)

  • Purpose : The purpose of this study is to compare with result of repair and resection in displaced bucket handle medial meniscal tear. Materials and Methods : From Sep. 1998 to Dec. 2001, we experienced 46 cases, 45 patients. We classified them into repair group (group I, 23 cases) and resection group (group II, 23 cases). We analyzed the time interval between injury and operation, zone of tear, the status of reduction and quality of displaced fragment of all cases. Average follow-up period is 29, 28 months, respectively. We evaluated the final results of both groups as Lysholm knee score, radiologic changes and 2nd look arthroscopy. Results : Mean age was 25 years old in both group, respectively. Mean interval between injury and surgical treatment was 12 and 17 weeks. respectively. In group I, 6 of 7 cases were evaluated as clinical success in red-red zone, 12 of 15 cases were assessed as clinical success in red-white zonal tear, stable reduction. Another case which is evaluated as clinical failure showed red-white zonal tear, unstable reduction and poor quality meniscal fragment. 5 cases showed variant degrees of tibio-femoral compartment symptom. In group II, red-white zone (9 cases) and white-white zone (14 cases) were treated as resection and got good results. Conclusion : Considering zone of tear, reducibility and quality of meniscal fragment before treatment will improve the success rate of meniscal repair.
