간호사의 시간선택제 근무에 대한 인식과 근무방안 (Nurses' Perception of Flexible Working and Its Operational Guideline)

  • 본 연구의 목적은 간호사의 시간선택제 근무에 대한 인식을 조사하고 활성화를 위한 합리적인 근무방안을 제시하는 것이다. 자료는 무작위 할당표출법을 이용하여 전국의 532명의 간호사를 대상으로 2015년 2월 10일부터 24일 까지 설문조사를 하였다. 자료 분석을 위해 SPSS 21.0 프로그램을 사용하였다. 본 연구의 결과에서 시간선택제 근무 중 단축시간제에 대한 필요성($M=3.89{\pm}0.87$)과 지원 의향(46.2%)이 가장 높게 나타났으며, 야간전담제, 휴일전담제, 2교대제 순이었다. 지원 선택의 이유로는 교대근무의 힘든 업무와 육아 병행의 어려움이 가장 높게 나타났다. 시간선택제 근무시행에 따른 긍정적인 영향으로는 자기계발 활동이나 여가활동의 증가로 삶의 질을 높일 수 있고, 육아와 일-가정 양립이 가능한 점이었으며, 부정적인 영향으로는 비정규직으로 채용될 우려와 급여가 감소할 것이라는 의견이 가장 높게 나타났다. 본 연구의 결과를 토대로 시간선택제 근무를 시행하기 위한 선결조건으로 시간선택제 근무 유형에 따른 보상과 근무조건에 대한 표준화된 운영 지침을 제시함으로써 추후에 시간선택제 간호사의 균등한 근무조건을 마련하는데 기여할 수 있다.

  • 본 연구는 간호사의 전문적 업무에 대한 범규범과 임상 현실의 실제적 차이에서 오는 법적 혼란 문제를 외국의 사례와 비교함으로써 이를 비판적으로 고찰하는 것을 그 목적으로 한다. 간호사의 업무 확대의 역사적 배경을 비교하고, 주요 국가의 전문간호사 등의 자격 및 업무에 대한 법적 근거 등을 살펴본 후, 이를 기초로 우리 법제의 발전 방향을 모색하고자 한다. 연구 방법은 비교 제도론적 연구 방법을 적용하였다. OECD 국가 중 미국, 호주, 일본을 선정하여 그 전문간호사 등에 관한 법제를 비교하였다. 미국, 호주, 일본 등에서 간호사의 전문적인 업무는 고령화 사회, 만성 질환, 의사 부족 등으로 인한 의료환경의 변화에 의해 출현하였다. 이들의 전문적인 업무에 관한 법적 근거, 자격인정 및 자격인정제도의 발전과 그 업무 범위 등을 비교하여 살펴보았다. 한국은 미국 등 외국과 비교하여 아직 법적 근거가 부재하며, 전문간호사 등의 업무 범위에 관한 법원의 입장과 실제 업무 간의 격차를 좁히지 못하고 있다. 비교법적 검토를 통해 전문간호사 등에 관하여 한국 실정에 맞는 법안 마련과 업무 범위 개발에 대한 시사점을 줄 수 있을 것이라 기대한다.

결핵관리전담간호사의 직무만족도 (Factors Affecting Tuberculosis (TB) Control Nurses'Job Satisfaction)

  • Purpose: The purpose of this study was to investigate factors associated with perceived job satisfaction of TB control nurses in Korea. Methods: This study employed a descriptive survey research design using data for 189 TB control nurses. Variables included in this study were demographic, social, and work-related characteristics (including job stress), and job satisfaction. ANOVA, t-test,and multiple regression analyses were performed using SPSS/WIN 19.0 program. Results: The mean value of perceived job satisfaction was lower in the subjects ($2.9{\pm}0.41$) than in general nurses working in other departments. In multiple regression analysis, factors associated with TB control nurses' job satisfaction were job stress (${\beta}$=-.281, p<.001), marital status (${\beta}$=-.229, p<.001), and the number of TB control nurses employed (${\beta}$=.200, p<.001). Conclusion: The results of this study showed that job satisfaction was associated more significantly with job-related characteristics than with non-job-related variables in workplace. Along with increased TB control nurse staffing, efforts should be made to reduce negative work-place issues such as job stress and inappropriate pay to improve the subject's job satisfaction.

전담간호사의 직무 스트레스, 감정노동과 긍정심리자본이 직무만족도에 미치는 영향 (Effect of Job Stress, Emotional Labor, and Positive Psychological Capital on the Job Satisfaction of Physician Assistants)

  • Purpose: The purpose of this study was to identify the effects of job stress, emotional labor, and positive psychological capital on the job satisfaction of physician assistants (PAs). Methods: The sample consisted of 166 PAs working in general hospitals in B city, Y city, and C city. Data were analyzed through the SPSS/WIN 18.0 program using t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficients, and multiple regression. Results: The factors of job stress (${\beta}=-.488$, p<.001), positive psychological capital (${\beta}=.188$, p=.004), and total clinical career (${\beta}=-.147$, p=.014) had the greatest influence on the level of job satisfaction experienced by the PAs. The total explanatory power was 49.9%. Conclusion: It was found that job stress, positive psychological capital, and total clinical career were influencing factors in the job satisfaction of PAs. Therefore, to improve their job satisfaction, it is necessary to develop strategies to manage the job stress experienced by PAs, and to strengthen and develop positive psychological capital.

전담간호사의 감성지능과 셀프리더십이 직무만족에 미치는 영향 (Effects of Emotional Intelligence and Self-Leadership on Job Satisfaction among Physician Assistant Nurses)

  • Purpose: This study aimed to identify the effects of emotional intelligence and self-leadership on job satisfaction among physician assistant nurses. Methods: The participants were 146 physician assistant nurses working at two university hospitals. Data were collected from August 1-September 31, 2020 and analyzed through t-test, ANOVA, Scheffé test, Pearson correlation, and hierarchical regression analysis using SPSS/WIN version 26.0. Results: Factors affecting job satisfaction were self-leadership (β=.30, p=.003), "less than 1 year of experience as a physician assistant nurse" (β=.27, p=.025), and emotional intelligence (β=.25, p=.007), and the explanatory power was 34.4% (F=6.03, p<.001). Conclusion: Our study shows that self-leadership and emotional intelligence play a significant role in the job satisfaction of physician assistant nurses; thus, strengthening these two factors is crucial to improve the nurses' job satisfaction. The results of this study may serve as basic data for the development of strategies to enhance job satisfaction among physician assistant nurses.

전담간호사의 역할갈등과 전문직 자아개념이 직무만족에 미치는 영향 (Influence of Role Conflict and Professional Self-Concept on Job Satisfaction of Physician Assistant Nurses)

  • Purpose: This study is investigate the role conflict, professional self-concept, and job satisfaction degree of the physician assistant nurses, and to grasp the factors affecting job satisfaction. Methods: Data were collected from September to October, 2018 and 156 physician assistant nurses working at one certified tertiary hospital, four general hospitals, using self-report questionnaires. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffe's test, Pearson's correlation coefficient and stepwise multiple regression. Results: The degree of job satisfaction of the subject was 2.76 out of 5. The influential factors in the job satisfaction of the subjects were professional self-concept (β=.51, p<.001), and role conflict (β=-.29, p<.001), affected job satisfaction by 53.5%. Conclusion: In order to improve the job satisfaction of physician assistant nurses, the role conflict should be solved and a plan to form a positive self-concept should be sought. Therefore, through the identification of the roles that are appropriate to the situation in Korea and the legal and institutional positioning, they will need to be recognized and developed.

제주특별자치도 종합병원의 감염관리 현황 (Status of Infection Control in Jeju-area General Hospitals)

  • 제주지역 종합병원 감염관리 현황을 조사하여, 병원감염관리 실태와 문제점을 파악하고자 하였으며 감염관리의 중요성을 인식시키고 감염관리 실무자로서 임상병리사의 역할 방안을 모색하기 위하여 제주특별자치도 제주시와 서귀포시에 위치한 종합병원 7개 기관을 대상으로 감염관리 현황을 설문지를 통하여 조사하였다. 제주지역 의료기관의 감염관리는 모든 기관이 감염관리실을 운영하고 전담인력을 구성하고 병원감염관리지침을 보유하고 수행하는 우수한 점을 보였지만, 대부분의 기관이 간호사 1인 구성의 감염관리실을 운영하여 전담인력 확보 및 미생물 관련 배양 판독, 다제내성균의 출현빈도, 감염통계 등 미생물 검사에 대한 내용을 보고 받는데 많은 부분 어려움을 호소하였다. 이에 감염관리의 실질적인 업무를 담당할 수 있는 임상병리사가 의료법 시행규칙에 있는 감염관리에 경험과 지식이 있는 사람으로 충분한 자질과 경험이 있다고 생각되며 감염관리실을 운영함에 있어 감염관리간호사와 미생물에 전문적인 지식을 갖춘 임상병리사(감염관리임상병리사)가 전담자로서 업무를 수행한다면 지금보다 더욱 발전된 감염관리실 운영이 될 것으로 보이며, 향후 감염관리를 제대로 하기 위해서는 전국적으로 종합병원을 비롯한 취약한 중소병원의 감염관리에 대한 전폭적 지원과 감염관리 전문 의료시스템 도입이 중요한 과제라 사료된다.

전문 및 전담 간호사의 신체검진 수행정도 및 심화교육 요구 (Use of Physical Assessment Skills and Education Needs of Advanced Practice Nurses and Nurse Specialists)

  • Purpose: The study was done to investigate physical assessment skills used by, and educational needs of, advanced practice nurses (APNs) and nurse specialists in Korea. Methods: A total of 123 APNs and nurse specialists working in five major hospitals in Seoul were surveyed from July 15 to August 20, 2007. Results: Fourteen skills out of 126 items were reported as being performed on a regular basis by participants. The majority of these skills involved general observation. Forty-six skills were rarely used. Some participants showed a lack of confidence in certain assessment skills, such as in doing a rectal or pelvic exam, and the use of some assessment equipment. Over 90% of participants required in-depth education on health assessment provided by specialists or nursing professional organizations. Conclusion: More educational opportunities in physical assessment should be provided including education programs based on the nurses' skill levels and needs. This effort will help to increase confidence of APNs and nurse specialists in physical assessment skills, ultimately resulting in better nursing outcomes.

야간전담간호사와 3교대간호사의 표준 교대근무 지표(Standard Shiftwork Index) 차이 (A Comparison of Standard Shiftwork Index between Night Shift Fixed Nurses and Rotating Shift Nurses)

  • Purpose: The purpose of this study was to compare the working status and Standard Shiftwork Index (SSI) between night shift-fixed nurses and rotating shift nurses. Methods: Participants were 367 nurses working as registered nurses at 7 different medical institutions. The data were collected from August 8, 2016 to September 30, 2016. Results: Night shift fixed nurses were more satisfied with work schedule than rotating shift nurses. Rotating shift nurses reported higher SSI scores, such as chronic fatigue, musculoskeletal problems, than night shift fixed nurses. In SSI's sub-items, rotating shift nurses reported higher scores, such as the amount of sleep, feeling well after sleep, heartburn or stomach-ache and heartbeat irregularities, than night shift fixed nurses. Conclusion: The results indicated that night shift fixed system might be an effective method to improve nurses' work satisfaction. It is necessary to figure out any suitable night shift fixed system based on the characteristics of each nursing organization. The SSI might be an useful tool to measure the nurses' satisfaction in reference to their work shift.

보건소간호사와 보건진료전담공무원의 문화역량 및 문화간호역량 교육요구 비교 (Difference in the Cultural Competency and Cultural Nursing Competence Educational Needs of Public Health Nurses and Community Health Practitioners)

  • Purpose: The purpose of this study was to investigate the level of cultural competency and cultural nursing competence educational needs of public health nurses (PHNs) and community health practitioners (CHPs) and to uncover the difference between the two groups. Methods: The subjects of this study were 217 PHNs and 165 CHPs. Data were collected using a structured questionnaire. Descriptive statistics, an independent two-sample t-test, the x2 test, one-way ANOVA, and the Scheffé test were used for data analysis. Result: There was no statistically significant difference between the cultural competency of CHPs (4.27±1.00) and that of PHNs (4.10±1.07) which were at a moderate level. Among the subareas, cultural awareness and sensitivity were above average, but cultural skills and knowledge showed relatively low scores. There was no statistically significant difference between the cultural nursing competence educational needs of CHPs (3.85±0.65) and PHNs (3.90±0.70) and these were at a relatively high level. Among the subareas, knowledge of key concepts, cultural communication, and attitudes and skills were above average. Conclusion: The results of this study highlight the need to develop and implement educational programs to enhance the cultural competency of PHNs and CHPs nationwide using various teaching-learning methods.
