Current Status (1999) of Neonatal Surgery in Korea (1999년도 한국 신생아외과 현황 -대한소아외과학회 회원대상 전국조사(제 2 차)-)

  • To understand the current status of neonatal surgery in Korea, a survey was made among the 36 members of the Korean Association of Pediatric Surgeons. The response rate was 75 % (26 surgeons in 20 hospitals). Five hundred fifty three neonatal surgical patients treated in 1999 were analyzed. Regional numbers of patients were closely related to the regional population in most areas. Ano-rectal malformations (17 %), pyloric stenosis (16 %), Hirschsprung's disease (13 %), atresia/stenosis of the gut (11 %), esophageal atresia (8 %) were the most common anomalies treated. The majority of operations were done within the first week of life. Seventy one per cent of cases were major life threatening or so-called neonatal index cases. Over-all mortality was 8 per cent. Higher mortality was observed in patients with diaphragmatic hernia (26 %), gastro-intestinal perforation (18 %), NEC (18 %), and esophageal atresia(14 %). Higher mortality was observed in patients with extremely low birth weight (33%) and low birth weight (18 %). Associated anomalies were observed in 20 %. Prenatal ultrasound was performed in 36 per cent with sensitivity of 20 %. Result of this study was compared to the previous report (1994) and that of Japan (1998).

Constraints to the Transmission and Solutions of the Important Intangible Cultural Properties Related to Dance Successors (중요무형문화재 무용전승자의 이수제약과 극복 방안)

  • This paper is intended on providing solutions for overcoming constraints to the transmission of the Important Intangible Cultural Properties related to dance successors. For accommodating this approach, six career dancers, from the Dance Society of the Important Intangible Cultural Properties, were invited as the research participants for this study. The In-Depth Interviews and the classification analysis are used as a way of structuring and understanding my observations about these tasks. These interviews have also been verified by an expert group. In conclusion, this study identifies numerous obstacles which can restrict the transmission of the Important Intangible Cultural Properties are as follows; first, the dance successors are constrained by personal reasons such as physical strength and mutual understanding at home; second, the dance successors are affected by the structural constraint of programs such as required time for the completion of training, efficiency of the training program and the educational environment; third, the dance successors are constrained by personal relations such as the relationship between themselves, or between the dance successors and holders of Important Intangible Cultural Properties. Suggested solutions for overcoming the constraints to training programs are; building small sized training centers throughout the country; making training time realistic for individuals; and making programs to train the dancers as well as making it sociable.

  • To understand the current status of neonatal surgery in Korea, a survey was made among the 27 members of Korean Association of Pediatric Surgeons. Response rate among surgeons was 78 percent, eighteen hospitals participated in this study. Five hundred and three cases of neonatal surgical patients were analyzed. In Korea, about 50% of cases were treated at the hospital in the capital city area. Regional number of patients were closely related to the regional population. Imperforate anus(19%), atresia/stenosis of gut(12%), and Hirschsprung's diseases(12%) were sitting at the top in the list. Majority of operation was done within the first week of life, especially during the first 24 hour period. Eighty per cent was major or so called index cases. Mortality in so-called index cases was 17%. High mortality was observed in patients with diaphragmatic hernia(47%), gastrointestinal perforation(65%) and esophageal atresia(28%). Low birth weight babies showed higher mortality in gastrointestinal perforation, esophageal atresia and abdominal wall defect. These were compared to 1993 survey of Japanese Society of Pediatric Surgeons.

A study on the Methodology for Vitalization of the Mini-cluster Network in Industrial Parks (산업단지 미니클러스터 네트워크 활성화 방법론)

  • The cluster project in Korea has restructured to develop the cluster into a core engine for the pan-regional development policy and connect industrial parks across the country. Korean government has made great efforts to attract research institutes, support organizations and universities in industrial parks and enhance growth potential through a close cooperation network. Especially A mini-cluster is an integrated group joined by companies, universities and research institutes and support organizations. It has been developing business opportunities through industry-academia-research cooperation and providing assistance through network activities. Mini clusters have implemented a variety of network activities. This paper proposed three strategies to accomplish self-sustainable growth of mini-clusters. First of all, mini-cluster needs to provide useful contents that can attract the participation of members. Then it is necessary to promote mini-cluster initiative. Finally, mini-clusters need to make up a cooperative culture.

  • The current status and problems of UST management in gas stations were reviewed, and suggestions were made for possible improvement of UST management. Regulations and programs relevant through the whole life cycle of the UST, such as construction, installation, operation, and disclosure are insufficient to prevent oil release. The UST requirements are less stringent compared to those of the U.S. and EU members. Current soil test does not seem to be practically effective in detecting soil contamination caused by oil release. The potential for subsurface contamination due to oil release from the UST is estimated from available data other than soil test results. Much higher following future directions and suggestions are made to improve current unsatisfactory UST management: Firstly, increasing the UST requirements - establishing more stringent standards for new UST facilities, and adding new regulatory requirements for existing UST facilities; secondly, replacing current soil test with the tank and piping tests; thirdly, reinforcing programs for supervising the tank construction and installation; fourthly, constructing a system in which independent gas stations can properly manage the USTs; and lastly, educating UST owners and operators, and constructing DB of UST facilities.

  • Korea's social economy promotion policy is a government-led policy, and its success or failure depends on how well it is known to the residents. The Jeonnam Social Economy Internet Shopping Mall was also established as a government-led road, but its popularity is low. This study examines the influence between reliability and loyalty of the 'Jeonnam Social Economy Shopping Mall'. 267 samples were used and verified with the SPSS 22.0 program. As a result of the study, the characteristics of shopping malls had a significant effect on customer trust and loyalty, and played a mediating role between reliability and loyalty. The residential area and the purpose of purchase had a moderating effect. The limitation of research is that comparative research with other shopping malls is insufficient. In the future, comparative evaluation should be carried out in relation to research on social economy enterprise products by region.

The Effect of Ukulele Performance on Stress and Daily Hassles and Life Satisfaction of Middle-aged Woman (우쿨렐레 연주활동이 중년여성의 일상생활 스트레스 및 삶의 만족도에 미치는 영향)

  • This study has researched the effect how ukulele performance of middle-aged women effects daily hassles and life satisfaction. This study has surveyed middle-aged women from 40 to 64 years old aimed at 300 members in ukulele performance club throughout the country using structured questionnaire. Collected data have been analyzed through multiple regression analysis and regression analysis of 3 steps with spss 22.0 statistical program. The analyzed results are as follows. First, the number of times and the period of playing the ukulele by middle-aged women have a negative effect on their daily hassles. Second, the number of times and the period of playing the ukulele by middle-aged women have a positive influence on their life satisfaction. Third, the period of playing the ukulele has a partial mediated effect on life satisfaction. The number of playing has a complete mediated effect on life satisfaction.

  • The first national survey on esophageal atresia and tracheoesophageal fistula was conducted to access the current status of its incidence, clinical manifestation, preoperative diagnosis and management, type of its anomaly, associated anomalies, and surgical results and course. The 43 members of the Korea Association of Pediatric Surgeons received questionnaires and registration forms to be filled out on each patient who were born during the three years from January 1, 1992 through December 31, 1994. Questionnaires composed of six broad areas which include 1) preoperative diagnosis and management, 2) surgical technic, 3) long gap, 4) postoperative management, and 5) complications and courses. A total of 148 cases was returned by 28 members working at 23 institutions and 27 members returned questionnaires. We obtained the following results by analysis of the 148 cases of tracheoesophageal anomalies. The incidence of tracheoesophageal anomaly was about 1/10,000-11,000 in 1994, which is one third of that of anorectal malformations in Korea and the distribution of the patients was almost proportionate to the size of each province. Both sexes are about equally affected. Majority of the members make diagnosis of tracheoesophageal anomaly by taking a simple infantogram with a radiopaque tube in upper pouch and a little under half(46%) prefers to perform echocardiography as a part of preoperative management to identify congenital heart disease and lateralize the aorta. Esophageal atresia with distal TEF(87.5%) was by far the most common and there were pure esophageal atresia(5.6%), H-type TEF(2.1%), and so on. About half(49%) of the patients had one or more associated anomalies in addition to tracheoesophageal anomalies. Congenital heart disease was associated in 46 cases(31%), anorectral malfomations in 19 cases(13%), musculoskeletal anomalies in 15 cases(10%), genitourinary anomalies in 10 cases(7%) and gastrointestinal anomalies in 7 cases(5%). Postoperatively, parenteral nutrition and assisted ventilation were given in 66% and 52% of patients respectively. Ninety three(74%) of 126 cases who underwent sugical procedure, experienced one or more complications such as respiratory complication(65%), leak(22%), stricture(21%) and so on. The survival rates related to the Waterston risk categories were 90.2% in group A, 71.4-75% in $B_1$, $B_2$, and $C_1$, groups, and 28% in group $C_2$, and the overall survival rate was 71.4%. Thirty six(28.6%) of 126 cases died of pneumonia/sepsis(12 cases), respiratory failure(12 cases), and congenital heart disease(4 cases). With short term follow-up, 69% of patients have been excellent whereas remainders of the cases have suffered from some sort of morbidity related to gastroesophageal reflux, recurrent respiratory infection, and esophageal stricture.

Intestinal Atresia - The Second National Survey (선천성 장폐쇄증 2차 조사 - 대한소아외과학회 회원을 주 대상으로 한 전국조사 -)

  • The members of the Korean Association of Pediatric Surgeons conducted a retrospective study of two hundred and twenty-two cases of intestinal atresia for the period from January 1, 2007 to December 31, 2009. Seventeen hospitals were involved. There were 76 duodenal, 65 jejunal, and 81 ileal atresias (3 colonic). The male to female ratio was 0.85:1 in DA and 1.34:1 in JIA. Ninety-four patients(43.3 %) were premature babies (DA 40.3 %, JA 64.6 %, IA 28.8 %), and 70 babies (32.0 %) had low birth weight (DA 38.7 %, JA 44.4 %, IA 16.0 %). Antenatal diagnosis was made in 153 cases (68.9 %). However, 27 infants (17.6 %) with antenatal diagnosis were transferred to the pediatric surgeon's hospitals after delivery. Maternal polyhydramnios was observed in 81 cases (36.59 %) and most frequent with proximal obstruction. In forty-four cases (19.8 %), only simple abdominal film was taken for diagnostic study. The associated malformations were more frequently observed in DA - 61.8 % in DA and 22.6 % in JIA. Meconium peritonitis, small bowel volvulus and intussusception were more frequently associated with ileal atresia. The overall mortality rate was 3.6 %.
