The patient population of U. S. state mental hospitals has changed drastically since the 1960s, when the deintstitutionalization movement began. This paper is designed to look at what happened to the number of inpatients of state hospitals in California during the last 150 years and, from this, to explore implications for the future of the mental health system in Korea, especially for the viability of mental hospitals. The data had been collected by field research(visits to state hospitals and State Department of Mental Health, and interviews with mental health administrators) and accessing statistical publications and various reports. Since the first state hospital opened in 1851 the statewide inpatient population of individuals who were mentally disabled has grown and peaked at 37,489 in 1959. The number of patients in state hospitals, however, began declining in the early 1960s and was reduced to 10,874 by 1971, and to 4,973 by 1986. As of 1997, there were only 4, 263 inpatients remaining in the state hospital system. This dramatic decrease slowed down somewhat in 1980s and 1990s, but this trend seems irreversible except for the inpatients referred by the court. Now the beds in state hospitals are filled with more and more forensic patients, which constitutes nearly 70% of the total inpatient population. Based on these findings, it is well expected that the number of inpatients of mental hospitals in Korea will also be reduced in a significant way as the community-based mental health care system is gradually replacing the traditional one. Mental hospitals need to introduce more diversified programs for the care of the mentally ill, and concurrently more vigorous aftercare programs are required in the community.
Objective: The purpose of this study was to look at a systematic review on the relationships of saccadic eye movements and cognition. Through this comprehensive study, we have to compare the studies. Methods: We systematically examined papers published in journal from 1995 to 2015, using KERIS. Main words to examine are saccadic eye movements, cognitive function, cognition, relationship, saccade program, etc. Results: 6 studies were selected, All of them were saccadic eye movements. Eye movements and cognitive function have correlation. Age from 5 to 65 years from the studies, they found the relationships between cognition and saccade(p<.05). However, the results demonstrated some limitations. First, there were variation in large of age spectrum. Second, the populations were small. Therefore, in the future study, saccadic eye movements study apply to variety of interventions to separate experimental group and control group as cohort study, and majority of patients is needed on occupational therapy. Conclusions: In the future study, the use of eye movements apply to a variety of interventions and majority of patients is needed for the effect of cognitive function improvements. If occupational therapists can obtain knowledge and make a protocol for cognitive program.
Is to identify how this affects the physically disabled to participate in the program 12 weeks Park Golf psychological well-being and happiness, the purpose of this research is subjective. How to study subjects, only 40-year-old disabled man more than 24 people total delay experimental group and 12 patients(failure cut seven, delayed dysfunction 5) and the control group and 12 patients(failure cut six, delayed dysfunction in 4, two people were involved in the joint disorder). 3 times a week(Mon, Wed, Fri), was carried out 50 minutes into 12 weeks of the experimental period, was located at River Park Golf Course A test place. We calculate the pre-and post-test data mean and standard deviation using SPSS Statistics 21.0 statistical data processing program, binary repeated measures ANOVA to analyze the effects on the psychological well-being of the disabled and subjective effects euphoria Park Golf Participation(was performed 2-way [2] RM ANOVA). First results in psychological well-being of the two groups according to Park Golf participate in group comparisons before and after the exercise involved only fun, immersive and shows were not significantly different, within each group enjoyment, competence, self-realization, all the children of the immersion showed a significant difference in the factors. Second, before and after participation in exercise, there was a significant difference between groups in subjective happiness of two groups according to Park Golf participation, the two groups were not significantly different within. Taken together the results to see more, showed that the positive effects on the psychological well-being and subjective happiness Park Golf participation is the Physically Disabled.
The aim of this study is to examine changes in swallowing function and quality of life with therapeutic singing-based music therapy for patients with dysphagia. The music therapy program was based on the previous study (Kim, 2010), and designed to improve breathing, phonation, and swallowing functions focusing on laryngeal elevation. Three patients with dysphagia participated in this study and each participant received a total of 11 or 12 individual music therapy sessions and each session was conducted for 30 minutes. In this study, three kinds of measurements were used. First, the measures of maximum phonation time (MPT), fundamental frequency, average intensity, jitter, shimmer, noise to harmonics ratio (NHR) by Praat test, second, laryngeal-diadochokinesis (L-DDK) to investigate laryngeal elevation, and last, the Swallowing-Quality of Life (SWAL-QOL) was measured. These cases have shown improved breathing, phonation, swallowing function, and the scores of SWAL-QOL in all of the patients. It suggests that this music therapy intervention was effective on laryngeal elevation, and the music intervention with therapeutic singing can be effectively implemented in further research for patients with dysphagia.

The purpose of this study was to investigate the status of studies on swallowing rehabilitation interventions for stroke patients that had been conducted in the past 5 years (2010-2014) in Korea and to examine their findings. A literature search was conducted through RISS, National Assembly Library, and KISS, and 12 papers were analyzed. Four articles were from nursing science, and 8 from other disciplines. Qualitative evaluation of the articles was conducted by using an evaluation framework that was designed by the researcher, which took into account the checklist presented by the Scottish Intercollegiate Guideline Network (2013). In the results of the qualitative evaluation on the papers, 4 studies scored 7 out of 10 points, 6 studies scored between 5 and 6 points, and 2 studies scored 4 points. Intervention sessions were conducted for 25 minutes, on average, each at a frequency of 4.1 sessions per week, for a total of 33.7 sessions. This study found that direct swallowing training and comprehensive intervention of respiratory and oral, including pharyngeal were effective in relieving difficulties in swallowing. In the future, such findings are expected to be used in nursing interventions for establishment of the basis for evidence-based nursing practice.
Objective : This study was to evaluate the effects of a Neurofeedback(NFB) and Computer Assisted Cognitive Rehabilitation(CACR) training to improve on cognition and affected arm function in stroke subjects. Methods : Participants were randomly allocated to three groups: NFB(n=14), CACR(n=14) and control(n=16). All groups received occupational therapy and physical therapy for 5 session 30 minutes per week during 6 weeks. Also NFB and CACR group practiced additional NeuroComp training and RehaCom training for 30 sessions 30 minutes during 6 weeks. Results were evaluated by cognition, affected arm function. Results : There were significantly increased by CACR training that outcomes of MMSE-K(p<.05). And there weren't significantly difference by NFB and CACR training that outcomes of the affected arm function. And a difference between three groups wasn't found. Conclusion : The NFB and CACR training improves cognitive function. These results suggest that NFB and CACR training is feasible and suitable for individuals with stroke.
The purpose of this study was to investigate the effects of virtual reality game based training on balance and upper limb function in subacute stroke patients. Thirty patients with subacute stroke were randomly assigned to experimental groups(n=15) and control groups(n=15) applying virtual reality game-based training programs. Intervention is applied three times a week for 6 weeks, 30 minutes for 1 time. In the study group, there was a significant improvement in balance ability, upper extremity function, and trunk impairment scale(p<.05), and the difference between the two groups was significant in the BBS, TUG, TIS(p<.05). Based on these results, the virtual reality game based training program is clinically useful exercise program for subacute stroke patients.
Objectives: The purpose of this pilot study was to examine the effects of a mindfulness-based Body-Mind Intervention Program using marine resources on the improvement of quality of sleep, mood symptoms, and cognitive function in Korean female emotional labor workers. Methods: Twelve female workers who experienced excess emotional labor participated at the training camp program for five days in Danghangpo-ocean park, Goseung-gun, Gyeongnam Province in South Korea. Participant sleep quality, mood symptoms, and cognitive functioning before, after, and 1.5 months later were evaluated and analyzed. Results: After participating in the marine resource program, participants reported significantly decreased sleep latency. Global sleep quality, cognitive functions (attention, flexibility, and inhibition control), and mood states, including depression, tension, anger, fatigue, were also improved. These effects were generally maintained after 1. 5 months (PSQI t = 2.63, p = 0.02 ; HAM-D t = 5.92, p < 0.001). Conclusion: A Body-Mind Intervention Program using marine resources was effective in relaxing emotion-related tension and improving cognitive function. To advance this pilot study, it is necessary to carry out further research to investigate the use of marine resources in mental health interventions.
The purpose of this study were to compare the differences of isometric lumber extension strength and subjective pain degrees between obesity patients group and normal body fat group in disc surgical operated patients. The research purposed to indicate how isometric lumbar extension exercise for 12 weeks affected to lumbar strength and visual analogue scale of patients suffered by chronic back pain. The subjects were 65 low back pain patients(male 30, female 35)who had disease on lumbar in W hospital. The lumbar extension strength was measured at seven degrees of angles, which were $0^{\circ}$$12^{\circ}$$24^{\circ}$$36^{\circ}$$48^{\circ}$$60^{\circ}$ and $72^{\circ}$ before and after the exercise program. We got the results of subjective pain degree using the modified visual analogue scale(VAS) of Lawlis et al(1989) and measured the maximal isometric lumbar strength of all subjects using MedX lumbar extension machine. Results were as follows; After the exercise, the lumbar extension strength of normal body fat patients groups included males and female were greater than that of the obesity patients groups in all angles(p<.05). The visual analogue scale of chronic back pain patients was decreased significantly after the exercise(p<.05). The results showed the significance between the lumbar extension strength and the visual analogue scale of chronic back pain patients and showed that the isometric lumbar extension exercise decreased the subjective pain degrees of visual analogue scale with and increased lumbar extension strength. The correlation between the visual analogue scale and the %body fat of chronic back pain patients was no significant after exercise. Therefore, the lumbar extension strength exercise is needed for improvement of back strength, decrease of %body fat.
Purpose: The purpose of this study was to analyze the clinical result and the gait patterns for patients who had undergone the arthroscopic posterior cruciate ligament (PCL) reconstruction using Achilles allograft. Materials and Methods: Among the patient who had undergone the arthroscopic PCL reconstruction using fresh-frozen Achilles allograft between March 2004 to March 2005, we selected 12 patients who compliance to our rehabilitation program. Clinical result and gait analysis were carried out at 36 months postoperatively. There were measured by using range of motion (ROM), Lysholm knee score, Tegner activity score, IKDC score, posterior stress test, and posterior translation measured by using Telos stress arthrometer. Kinematic gait analysis was carried out using 3-dimensional gait analysis system. Results: The PCL reconstruction surgery yielded statistacally significant results in all of clinical evaluation. In gait analysis, the average knee flexion angle increased during stance phase and decreased during the swing phase, but was not statistically significant and there were no significant difference between both sides of their knees overall. Conclusion: Since there was no statistically significant difference between the injured and uninjured sides as a result of the gait evaluation of the patients who had arthroscopic reconstruction surgery on their PCL injured knee and conformed to the rehabilitation program, systemic and strict rehabilitation ought to be important in the reconstruction surgery.
