Management of Premaxilla in Patient with Bilateral Complete Cleft Lip and Palate (양측성 완전 구순구개열 환자에서 전상악골에 대한 처치)

  • 전방으로 심하게 돌출된 전상악골로 인하여 구순성형술 및 비성형술의 결과가 악화 될 수 있다. 따라서 변위된 전상악골의 이상적인 위치로 재위치 시키려는 다양한 노력이 시도되어 왔다. 에디오피아와 같은 개발도상국에서는 어른이 되어서도 수술을 받지 못하는 구순구개열 환자가 많이 있다. 성인이 될 때까지 수술받지 못한 양측성 완전 구순구개열 환자에서는 근육, 골, 피부, 점막의 연속성이 없어서 전상악골이 심하게 전방으로 혹은 하방, 좌우측으로 변위된 경우가 대부분이다. 이 경우 구순성형술이 거의 불가능하며, 시도된다 할지라도 돌출된 전상악골 때문에 양쪽 구륜근을 봉합하여 주기가 대단히 어렵다. 따라서 이상적인 결과를 얻기 위해서는 구순성형술 전 혹은 동시에 전상악골의 재위치 술식이 필수적이다. 저자는 한국국제협력단에서 국제협력의사로 선발되어 에디오피아에서 30 개월간 근무하였다. 그 동안 다양한 양측성 완전 구순구개열 환자에서 전상악골의 재위치 술식을 경험하였다. 저자가 경험한 전상악골의 재위치 술식(전상악골의 재위치와 골이식술 동시 시행, 전상악골의 재위치와 구개열 성형술 동시시행)에 대하여 문헌고찰과 함께 보고하고자 한다.

ARREST OF ROOT DEVELOPMENT AFTER SURGICAL REPOSITIONING OF THE INVERTED MAXILLARY CENTRAL INCISOR : CASE REPORT (역위 매복된 상악 중절치의 외과적 재위치 후 치근 발육 정지)

  • Impaction of the maxillary central incisor may cause social, esthetic, and functional problems in children. There are various means of treatment for the inverted maxillary central incisor, such as extraction, surgical opening followed by orthodontic traction surgical repositioning or intra-alveolar autotransplantation prior to extraction. In this case, we surgically repositioned the inverted maxillary central incisor to normal semi-erupted position in a 5-year-old boy The developmental stage of the inverted tooth was Nolla's 6.5, which indicates formation of less than one third of the root. After surgical reposition, we did follow-up for 21 months, expecting spontaneous growth Unfortunately, poor prognosis was noted further root was not observed. Such failure seems to originate from possible injury on Hertwig's epithelial root sheath by surgical trauma. We performed surgical repositioning to retain the tooth instead of extraction. However, arrest of root development occurred which is one of the critical complications. In order to increase the success rate of the surgical reposition procedure, minimal surgical trauma is required as well as selection of adequate indication and decision of proper time of treatment considering the stage of root development.

  • 저자들은 1980 년대 후반부터 역사학적 논쟁들을 통계적 수학적 방법을 이용하여 분석해보고자 하는 시도들을 하여 왔다. 최근에는 역사학의 논쟁점 중이 하나인 화랑세기의 진위 여부를 분석하기 위한 일련의 연구를 시도하였다. 이러한 일련의 시도의 일환으로 이 연구는 기획되었다. 이 글에서는 일본의 고대 기년 추정 방법을 참고하면서 세대 평균치와 재위 평균치의 추정이라는 방법을 통하여 "삼국사기" 초기 기년 문제에 대한 해명을 시도해 보고자 한다. 종래에는 세대 평균치라는 개념을 사용하지 않아서 편차가 심한 왕별 재위기간 평균치만 사용하여 재위기간 추정의 오차가 컸다. 또한 여러 가지 추론들이 주관적인 판단에 의지한 부분이 많아서 폭넓은 지지를 얻지 못한 경우가 많았다. 따라서 세대 평균치와 재위 평균치는 반드시 일치하지 않는다는 점을 분명히 하는 한편, 양자를 결합한 추정 방안을 마련해 볼 것이다.

TREATMENT OF A PERMANENT INCISOR COMPLETELY INTRUDED BY TRAUMA : A CASE REPORT (외상으로 완전 함입된 영구전치의 치험례)

  • Traumatic intrusive luxation, an occurrence common in the primary dentition but one that occurs rarely in the permanent dentition, has a poor prognosis. There have been many treatment approaches such as allowing the tooth to reerupt spontaneously, surgical repositioning and immediate luxation, surgical luxation, and orthodontic repositioning; but all have their own drawbacks. Meanwhile, Turley et al. (1987) have proposed surgical and orthodontic combination therapy to treat intrusion. Surgical and orthodontic combination therapy means to apply the orthodontic traction force immediately after surgical luxation. If ankylosis occurs, orthodontic force may be applied after re-luxation repeatedly. But in cases of complete intrusive luxation, it would be not feasible to bond an orthodontic button or bracket on the tooth directly. Thus, in this case, traction of the tooth was attempted after surgically repositioning it close to the probable original socket site to promote better healing.

  • Intrusive luxation that takes approximately three percent of permanent teeth commonly occures at anterior teeth. This intrusion frequently leads to pulp necrosis, root resorption, marginal bone loss and these complications are influenced by depth of intrusion and stage of root development. Various treatment approaches have been suggested to manage of intrusive luxation. Techniques aiming to reposition the intruded tooth include an observation for spontaneous re-eruption, surgical or orthodontic repositioning. We report two cases with clinically satisfactory results for traumatically intruded maxillary central incisor. In one case which has a large open apex and mild intrusion depth, we observed for spontaneous eruption and then repositioning by forced eruption method. In other case, which has been completely intruded, was repositioned by surgical extrusion and followed by apexification.

  • Delayed eruption of a maxillary incisor results in midline shift, the space occupied by adjacent teeth and different levels of alveolar height. Extraction or surgical/orthodontic therapy is the most common treatment for a impacted maxillary incisor. Surgical repositioning provides another option for treatment of this problem. The advantages of this approach include immediate esthetic improvement, use of a single and simplified surgical procedure, simple and short orthodontic therapy, a normal gingival margin and the possibility of the developing root adapting to the new position. Autotransplantation of an immature tooth provides for possible adaptation of the developing root apex to the new position. A root with an open apex has good chance of pulp revascularization after transplantation.

  • Obstructive sleep apnea (OSA) is a disorder characterized by repetitive episode of upper airway collapse during sleep. Recent studies showed that not only the anatomic factors but the physiologic factors of the upper airway also have effcts on the occurrence of apnea and that the genioglossus muscle also plays an important role in the maintenance of the upper airway. A variety of therapies were performed to treat OSA, and among them the use of mandibular repositioning appliances showed reasonable results. But there is still a lack of research on the structural and physiological mechanism upon the use of mandibular repositioning appliances. The author selected 26(male 17, female 9) OSA patients that came to the Yonsei University Dental Hospital, Department of Orthodontics, and 20 normal adults (male 10, female 10) and took cephalometric radiographs of them in a supine position before and after the placement of the mandibular repositioning appliance to see the structural changes of the upper airway and compare the therapeutic effects between the two groups. We also studied the waking genioglossus muscle activity in OSA patients and investigated the difference in the electromyogram of the genioglosssus muscle upon the change in body posture and the use of mandibular repositioning appliance. Following results were obtained. 1. Among the cephalometric measurements of the upper airway structure, the length of the soft palate, maximum thickness of the soft Palate and SPAS, MAS, VAL, H-H1, MP-H showed statistically significant differences between the normal and OSA groups, but the IAS and EAS showed no statistically significant differences between the two groups. 2. In both the normal and OSA groups, as the epiglottis moved forward on wearing the mandibular repositioning appliance, the epiglottis level of the upper airway increased and the maximum thickness of the soft palate changed and the hyoid bone also moved forward, but the IAS in both groups showed various results and the effect of the mandibular repositioning appliance on the structure of the upper airway was different in the two groups. 3. Upon changing the position, the electromyogram of the genioglossus muscle showed a increasing tendency but there was no statistically significant differences, and when the mandibular repositioning appliance were worn there was a statistically significant increase in the electromyogram of the genioglossus muscle in both the upright and supine positions. The mandibular repositioning appliances not only have an effect on the anatomical structure of the upper airway but also on the physiology of the upper airway. There are different responses to the use of mandibular repositioning appliance between the normal and OSA groups therefore it could be considered to have the different physiology of the upper airway between the two groups.

  • In the primary dentitions, the majority of dental injuries involve the anterior teeth, especially the maxillary primary central incisors. When injuries affecting primary and permanent teeth are compared, it appears that trauma to the primary dentition is usually confined to the supporting structures, i.e. luxation and exarticulation, while the largest proportion of injuries affecting the permanent dentition is represented by crown fractures. But, cases reporting trauma affecting primary molars are unusual in the literature and several reports describe fractures of posterior teeth. The main goal of this report is to describe the repositioning treatment using removable appliances to an uncommon case of lingual displacement of primary molar that happened to a 4 year 5 month-old female child.

  • Impaction is defined as the cessation of the eruption of the tooth caused by a clinically or radiographically detectable physical barrier in the eruption path or by the ectopic position of tooth germ. Besides the third molars and the maxillary canines, the most common impacted tooth is the second premolar. The overall frequency of premolar impaction has been reported to be 0.5%. In some cases, orthodontic traction and surgical repositioning may be indicated. When impacted second premolar is involved with prolonged retained second primary molar, extraction of primary molar and space maintenance lead to eruption of second premolar. In these cases, all patients visited to department of pediatric dentistry of Kyungpook National University Hospital for the chief complaint of unerupted second premolar. Extraction of prolonged retained second primary molar and space management are tried for spontaneous eruption of impacted second premolar tooth. The results were as follows: 1. When impacted second premolar is involved with prolonged retained second primary molar, minimal treatment via elimination of primary molar leads to successful results. 2. Proper space management and periodic radiographic examination are required before eruption of second premolar. 3. Sufficient time must be allowed for confirm of tooth movement before orthodontic traction or surgical repositioning. 4. The result is more successful in incomplete root development.
