RDW and MCV in Differentiation of Iron Deficiency Anemia (철결핍성빈혈 진단과 RDW, MCV)

  • RDW and MCV are thought to be the highly sensitive blood cell parameters in the differentiation of iron deficiency anemias. Through the medical records of 227 anemic patients and the physical checking results of 143 healthy persons in a General Hospital during the recent five years, the authors evaluated various blood cell parameters including RDW and MCV. Iron deficiency anemia, aplastic anemia and other anemias associated with chronic disease were shown as the three major causes of anemias in Korea. In the patients of iron deficiency anemia MCV was very low(62.9$\pm$13.7fl), while RDW was very high(19.3$\pm$4.8) showing much lower MCV and much higher RDW in severe IDA compared with in mild IDA. To differentiate iron deficiency anemias form other anemias, a discriminant function was developed from some blood cell parameters like MCV, MCH, MCHC, RDW and platelets(D.F.=0.26-0.012MCV -0.130MCH +0.073MCHC +0.052RDW+0.003PLT).

Effect of Deer Blood on Aplastic Anemia Induced Mouse (녹혈이 빈혈유발 Mouse에 미치는 영향)

  • Hematopoietic stem cells in bone marrow form all kinds of blood cells. In traditional medicine, functions of bone marrow cells are very similar to those of Essence(精) which is a fundamental factor of physical development and reproduction. Our experiment examined the effect of deer blood on aplastic anemia induced mouse using cyclophosphamide 150 mg/kg i.p injection before experiment and then another cyclophosphamide 120 mg/kg i.p injection on day 10. Then we administrated dried deer blood in distilled water for 5 days, 9 days and 10 days. We examined blood and marrow samples. In results, deer blood showed a trend of effectiveness on recovery of red blood cells and erythropoietin although they were not statistical significant. And deer blood did not show changes in CD34.

  • Anemia in fish can be easily recognized by examining the gill color and physical properties of sampled blood. The number of erythrocytes is generally within 1-3 ${\times}10^6$. Hematocrit (Ht) reportedly ranges 35-45% in many of reports which is nearly the same as that in mammals. Anemia in fish can also be classified as hemorrhagic, hemolytic and hypoplastic ones. Fish are also expected to have similar pathological effects by anemia as in mammals. From the fact that fish can survive under extremely low hematocrit below 10% and antarctic icefish (Chanenocephalus aceratus) has no erythrocytes, different pathological effects were expected. Anemia is considered to have one of the most important health parameters, based on the pathological aspect. Therefore, by reviewing clinical and histopathological findings of most of fish diseases already known and then adding our results of experimental anemia, diseases and factors related to anemia were summarized and the pathological effects was discussed.

  • Patients with aplastic anemia were treated with a combination of depo-testosterone cyclopentylpropionate(Upjohn) and dexamethasone. In 7 of 15 patients treated, there was response in which either a significant increase in hemoglobin concentration, a prolonged interval or a cessation of blood transfusion requirement developed during androgen therapy. Younger patients with cellular marrow appeared to be better responding to androgen. EEI(Effective Erythropoietic Index) formulated by Gardner & Nathan(1966) which was a helpful measurement as to whether patients with myelofibrosis whould respond to androgen, was evaluated in patients with aplastic anemia. It was concluded that EEI as well as ferrokinetics indices (Plasma-$^{59}Fe$-disappearance rate, RBC $^{59}Fe$ net incorporation) did not significantly correlate with the degree of response to androgen in aplastic anemia.

Acute Lymphoblastic Leukemia with Bone Marrow Infiltration and Hypoplastic Anemia in a German Shepherd (German Shepherd에서 골수침윤과 재생불량성빈혈을 동반한 급성림프모구성백혈병 1증례)

  • A male German Shepherd dog, $2{\frac{1}{2}}$years of age, was admitted with sudden anemia, weakness, hyperpyrexia, anorexia and lethargy. The patient showed hypoplastic anemia, thrombocytopenia, absolute and relative lymphocytosis, absolute and relative granulocytopenia, hypoalbuminemia, slight hepatic disorder, slight azotemia, hematuria and proteinuria by the screening examination. The bone marrow aspiration smear showed high cellularity, severs infiltration of lymphoblasts and prolymphocytes, and mitotic figures of lymphoid cells. The liver aspiration smear demonstrated infiltration of lymphoblasts. Acute lymphoblastic leukemia was diagnosed as none of the superficial lymph nodes showed enlargement and marked functional disorder of important organs other than the liver was not found. The patient was treated with vincristine, cyclophophamide, predniosolone for chemotherapy and blood transfusion and either ampicillin or cefoperazone for supportive treatment. But the patient did not show marked remission and died 9 days after the start of the chemptherapy. The necropsy was not permitted.

  • A case is reported of aplastic anemia occuring during Methimazole therapy. Since this seems to be a kind of self-limiting disease having considerably rapid recovery, much more detailed supportive therapy is considered to be crucial rather than any other corticosteroid or the androgen therapy.

  • Aplastic anemia is a rare disease, which is characterized by pancytopenia and hypocellular bone marrow without infiltration of abnormal cells or fibrosis. The incidence in Asia is higher than in the West and new cases are diagnosed at a rate of 5.1 per million pediatric populations per year in Korea. The pathophysiology is understood roughly by defective hematopoiesis, impaired bone marrow micro-environment and immune mechanism. Treatments are performed on basis of pathogenesis and selected depending on the severity. Immunosuppressive therapy with antilymphocyte or antithymocyte globulin and cyclosporine is effective in the majority of patients but has some problems including relapse or clonal evolution. Recently, there have been clinical trials of immunosuppression with hematopoietic growth factors or other drugs. Allogeneic hematopoietic stem cell transplantation (HSCT) is curative in children with severe aplastic anemia. The overall survival in HSCT from HLA-identical sibling is higher than alternative donor, including HLA matched unrelated donor or cord blood. We have to consider quality of life after HSCT because of high survival rate. However, chronic graft versus host disease and graft failure are important factors that affect the quality of life and overall survival. We need further investigation to make new regimens aimed at overcoming these risk factors and perform clinical trials.

Clinical Observation on a Case of Aplastic Anemia Treated by Herbal Acupuncture (재생불량성(再生不良性) 빈혈(貧血)의 약침치료(藥針治療) 경과 1례 보고)

  • Objective : In order to report the process of treatment of Aplastic anemia by herbal acupuncture Methods: This case was diagnosed as deficiency of Yang of spleen and kidneys, deficiency of Ki and blood, and fevescence from the deficiency of Urn. Hominis Placenta herbal acupuncture and Bee-venom acupuncture were choosen as the method to treat the case. Results: 1. The nocturnal fevescence and indigestion was decreased during the times when Hominis Placenta herbal acupuncture was used once a day. 2. The mobility of the joints was improved and the inflamation on the joints was decreased during the times when Bee-venom acupuncture was used every other day. 3. The interval between the transfusion was extended during the times when Hominis Placenta herbal acupuncture and Bee-venom acupuncture were used by turns. Conclution : Hominis Placenta herbal acupuncture and Bee-venom acupuncture sucessfully imporved the symtoms of patient with Aplastic anemia

  • The major manifestations of aplastic anemia are characterized by persistent bone marrow aplasia and peripheral pancytopenia due to defective stem cells in which differentiation is not curtailed or preferential elimination of stem cells with self-renewal capacity, which results in serious complications such as bleeding and infection. Recent advances in the therapeutic modalities, such as bone marrow transplantation and immunosuppressive therapy, and in supportive measures as component transfusion and infection control with better quality of antibiotics resulted in an improved survival and subsequently increased possibility of complications which need surgical intervention. Considering the fact that surgery may not only control complications, but offers the opportunity to give effective therapy for aplastic anemia and therefore improves chances for survival, it is strongly suggested that active surgical intervention should be performed. We report a case of patient with aplastic anemia who had been performed surgical intervention of partial maxillectomy for the squamous cell carcinoma on maxilla without serious complications during perioperative and postoperative period.
