Purpose: A comprehensive analysis of demographical, disease, functional status and fall risk related factors identified factors associated with falls in elderly hypertensive. Method: A descriptive research design was used. The participants were 124 persons aged 65 years or older registered at the community center in Daegu city. The data were collected from October, 2008 to February, 2009. Frequency, Fisher's exact test, $X^2$-test, t-test, and logistic regression were done using the SPSS V17.0. Results: Ninety (72.6%) subjects had experienced falls. The occurrence differed according to number of medications, activities of daily living and competence of vision. Logistic analysis revealed number of medication and impaired vision as independent risk factors for subsequent falls. Conclusion: Supportive nursing for the elderly needs to focus on dizziness and impaired vision to prevent falls in community-dwelling elderly with hypertension.
Purpose: Ageing in place may improve the quality of life of frail elderly and decrease their costs of services. The purpose of this study was to examine the factors that influence the institutionalization of elderly using home care services in a Korean long-term care insurance system. Methods: This study used the data of '2009 Satisfaction survey of Korean long-term care system'. The survey proceeded to use a sampling data based on region, level of long-term care need, and insurance type among the beneficiaries between August and September 2009. The onset dates of institutionalization of 1,095 participants were ascertained from long-term care insurance claim data. This study calculated the hazard ratio through the Cox Proportional Hazard Model. Results: A total of 176 subjects who were institutionalized in nursing homes were included. There were higher risks in the group that included those who were 85 years and over, had dementia or fracture, used home-visit nursing service, and were not supported by direct family. Conclusion: The results of this study have policy implications to supplement the home care service system and postpone nursing home institutionalization of elderly.
Objectives: This study was conducted to analyze the symptoms and care needs of home-based hospice palliative patients in Busan and to provide a basic reference for developing practical guidelines for their care. Methods: By examining the registration cards of 409 hospice palliative patients, who were registered in community health centers in Busan as of 2016, this study retrospectively analyzed their characteristics, symptoms and care needs. Results: The average age was 70.6 years, 59.4% were receiving medical benefits, and 48.4% lived alone. As per the data obtained from the Palliative Performance Scale, many were able to mobile. Fatigue was the most severe and depression and anxiety were reported together, and their care needs were also high. Most subjects reported mild or low pain, but care needs were high. Furthermore, the medical benefits group showed a high level of symptoms and care needs across areas. Conclusions: To help subjects to live in their homes for as long as possible, it is necessary to identify symptoms and care needs and provide services in accordance with their severity and situation. Thus, it is necessary to develop practical guidelines for standardized community hospice palliative care services.
Purpose: The purpose of this study was to analyze cancer symptoms, perceived health status, and nursing services for community dwelling cancer patients who are registered in a public health center. Methods: The subject of the study were 561 community dwelling, and home-based cancer patients who were registered in 8 different public health centers in Daegu, Korea. The data collection was performed from September 28 to October 10, 2009. Analysis of data was done by using descriptive statistics, t-test and ANOVA with SPSS program. Results: The mean score of cancer symptom index was 1.63. The level of fatigue was the highest in cancer symptom index. The mean score of perceived health status was 6.92. The exercise guidance was identified as the most frequently practiced nursing service. The scores of cancer symptom index were significantly different by economic status, marital status, living condition, the status of cancer, and metastasis. The scores of perceived health status was significantly different by education, economic status, type of social insurance, time of cancer diagnosis, status of cancer, and metastasis. Conclusion: The above findings indicate that it is necessary to develop a special nursing intervention differentiated according to the time points of cancer diagnosis, status of cancer. In addition, nurses should apply it in their practice to ameliorate fatigue for community dwelling cancer patients who are registered in public health center.
Purpose: The purpose of this study was to examine the current status of home care nursing services provided by community health nurses and to identify barriers to the services. Methods: A cross-sectional survey was conducted with three types of community health care nurses. Participants were 257 nurses, 46 of whom were hospital based home care nurses, 176 were community based visiting nurses, and 35 were long term care insurance based visiting nurses. A structured questionnaire on 7 domains of home care nursing services with a 4-point Likert scale was used to measure activities and barriers to care. Data were analyzed using SPSS WIN 21.0 program. Results: Hospital based home care nurses showed a high level of service performance activity in the domain of clinical laboratory tests, medications and injections, therapeutic nursing, and education. Community based visiting nurses had a high level of service performance in the reference domain. Long term care insurance based visiting nurses showed a high level of performance in the service domains of fundamental nursing and counseling. Conclusion: The results show that although health care service provided by the three types of community health nurse overlapped, the focus of the service is differentiated. Therefore, these results suggest that existing home care services will need to be utilized efficiently in the development of a new nursing care service for patients living in the community after hospital discharge.

The purpose of this study was to understand the meaning of experiences in home visiting nursing service among the elderly in community provided by nursing students. Method: This is a phenomenological study to investigate the experience of community-based patients. Data were collected through in-depth interviews from the 4th of March to the 15th of December 2003. Individual interviews were conducted with 8 patients on subjective experiences. Data were analyzed through Colaizzi's method, in which meaningful statements were extracted. Result: Four categories were identified from nine theme clusters. The four categories were 'thankfulness', 'dependence', 'sorrowfulness' and 'the sense of being'. Conclusion: These results show the importance of visiting nursing service for the elderly in community
The purpose of this study was to identify the relationships among the perceived health status, activities of daily living, and depression for the elderly at home. The subjects of this study were 403 elderly over 60 years at home. The data were collected through personal interviews using structured questionnaires. The data were analyzed by descriptive statistics and Pearson correlation, using the SPSS 12.0 program. The results of this study were as follows. 1) It was seen that the degree of perceived health status is poor in 62.3%. 2) They could lead an independent daily living in 58.1%. 3) It was seen that contraction of depression was 43.8%. 4) There was a significant relationship among ADL and perceived health status, ADL and depression. However, there was no significant relationship among perceived health status and depression. Based upon above findings, this study give useful information for elderly health policy and intervention program.
This study examined the factors affecting forms of long-term care service use by elderly and the forms of use are classified facility care service, home care service, and unused. It is used data from the 2nd pilot program for the Long Term Care Insurance scheme and it is analysed 5,497 cases. Multi-nominal regression is used. According to the results, women use formal service more than man do, and wowen use facility care than home care. Those who eligible for National Basic Livelihood Security System(NBLSS) are shown to have higher use of formal care(especially facility care) than the middle income class, and the low income class than the middle income class has lower use of formal care. In addition, higher the family care is available, lower the taking part in the service. The big cities and mid sized cities than rural are used the formal service and moreover mid sized cities are used facility care than home care. Furthermore, the level of care need is determinants of service use and function of ADL, IADL, and abnormal behavior is also determinants of formal service(especially facility care). But nursing need and rehabilitation need are not determinants of formal service use. Based on the results, the recommendations are developed and implemented for the improvement the elderly long-term care insurance.
Purpose: This study aimed to investigate possible ways to expand the services of home-visit nursing through a review of the progress, achievements, and obstacles of home-visit nursing; a pilot project of an integrated home-service; the application of the Omaha System; as well as a case analysis of providing home-visit nursing services. Method: An integrated review was conducted using various source materials, including laws, previous studies, and a case analysis. Results: In case analysis of providing visiting nursing service, rehabilitation nursing, end-of-life nursing, and dementia care showed high nursing needs. It was necessary that the various home visit nursing services in the intervention area of the Omaha System, administrative services, case management, and center operation activities were all included in the payment systems of long-term care insurance. Conclusion: In the future, home visit nursing services of long-term care insurance should be reborn in the form of a center for integrated case management in the community, which would set long-term goals until the time of a client's death and encompass the realm of human rights for health, quality of daily life, and a dignity of life.
Purpose: The purpose of this study was to test the effect of Taping therapy on Range of Motion, pain, and depression in stroke patients in the home without complete recovery as a means of nursing intervention. Method: Twenty seven subjects out of fifty four people who were attending in the stroke self-help group in one community health center in S city were asked to participate in this Quai-experimental study. The Taping therapy was a method that stick to the illness area and the point of pressure pain, the elastic and cross tape without medicine treatment with domestic products. Nursing intervention was independently completed by researcher once a week over two period of 12 weeks from September 2001 to March 2002 year. Data were analyzed using the SPSS win. The homogeneity between the experimental group and control group was test by x2 and t-test. The difference of experimental before and after were tested by the unpaired t-test. Result: The shoulder joint flexion and abduction, the elbow joint flexion and extension, the hip joint flexion, and the flexion of knee joint in the ROM of the experimental group were significantly improved over those of the control group. In difference of pain, rest and painful movement, the experimental group were significantly decreased over those of the control group. The difference of depression in experimental before and after was significantly decreased in the experimental group over those of the control group. Conclusion: The Taping therapy intervention proved effect pain relief and depression decrease as well as promote of range of motion.
