응급실 기반 자살 시도자 사후 관리사업의 등록률에 영향을 미치는 인자 (The Factors Affecting the Registration Rates for Emergency Department Based Post-suicidal Care Program)

  • Purpose: The aim of this study was to investigate the independent factors associated with the registration rate for the community-based post suicidal care program in the emergency department (ED). Methods: This prospective observational study was conducted between March and December 2013 at the academic ED at the tertiary urban hospital. During the study period, the pre-designed registry was recorded. The variables examined included the following: patients' demographic data (Sex, age, address, type of insurance, marital status, level of education, and history of previous psychiatric disease), suicide-related data (suicidal methods, combined drink of alcohol and number of previous attempts), and management-related data (disposition at ED, physician's training level, etc.). Univariated and multivariated logistic regression analyses were performed for identification of factors affecting the registration rate for the community-based post suicidal care program. Results: A total of 163 suicides were included during the study period. Of these, 33 (20.2%) patients were registered in the post-suicide care program. Factors including a patient's address (OR: 14.92, 95% CI: 3.606-61.711), immediate intervention by psychiatric healthcare center (OR: 5.05, CI: 1.688-15.134), admissions in hospital (OR: 3.69, CI: 1.286-10.605), and history of previous psychiatric disease (OR: 3.52, CI: 1.216-10.201) showed significant association with registration for the program. Conclusion: The community-based post-suicidal care program, which is available 24 hours a day, should be operated in each district in order to increase the registration rate. Emergency physicians should actively consider the inpatient treatment program for suicidal patients and strongly recommend registration to the program, particularly for patients without previous history of psychiatric disease.

가족의 거부로 인한 성소수자의 정신건강에 관한 연구: 합의적 질적 연구(CQR) (Mental Health In LGBTs Resulting From Family Rejection: Consensual Qualitative Research)

  • 본 연구는 성소수자들이 가족에게 커밍아웃 후 경험하는 가족의 반응을 알아보고, 가족의 태도가 성소수자의 정신 건강에 어떠한 영향을 미치는지를 연구하였다. 가족에게 커밍아웃한 만 19~30세의 남녀 성소수자 12명을 대상으로 심층 인터뷰를 진행하였으며, 추출된 자료는 합의적 질적 연구방법으로 분석하였다. 커밍아웃에 대한 가족의 태도는 대부분 부정적인 양상으로 거부적 또는 회피적 태도를 보였고, 일부만 수용적 태도를 취하는 것으로 나타났다. 이에 성소수자들도 맞대응을 하기도 하고, 설득, 전문가 상담 참여, 포기 및 회피, 또는 가출과 같은 대처 행동을 취하기도 하였다. 가족의 태도가 성소수자에게 미치는 영향은 대부분 분노, 슬픔, 소외감, 우울함, 불안감 및 두려움, 트라우마, 무력감, 자존감 하락, 알코올 의존, 자살생각 및 시도와 같은 부정적인 심리적 영향이었으며, 수용적인 태도를 보인 가족의 경우 성소수자가 안정감을 느끼는 것으로 나타났다. 연구 참여자들은 사회의 부정적인 태도보다 가족의 부정적 태도에 의해 더 상처를 받는 것으로 보고하였다. 성소수자가 커밍아웃 이후 가족으로부터 어떠한 태도를 경험하고 그들이 느끼는 심리적 어려움은 무엇인지 구체적으로 파악하였으며, 커밍아웃 전후의 과정과 성소수자의 극복방안 등을 심층적으로 보여주었다. 성소수자 개인이나 가족의 상담을 위한 실천적 전략을 마련하는데 도움을 줄 수 있을 것으로 보인다.

  • Purpose: Suicide ranks among the top causes of death among youth in South Korea. This study aimed to identify the characteristics of suicidal individuals treated at emergency departments between 2011 and 2020. Methods: A retrospective analysis was conducted using data from January 2011 to December 2020 in the Injury Surveillance Cohort, a prospective registry. Patients' sex, age, mortality, methods of self-harm, and previous suicide attempts were analyzed. The methods of self-harm were categorized into falls, asphyxiation, blunt injuries, penetrating injuries, poisoning, and others. Sub-groups with and without poisoning were compared. Results: The proportion of self-harm/suicide attempts increased from 2.3% (2011) to 5.0% (2020). The mortality rate decreased from 10.8% (2011) to 6.3% (2020). Poisoning was the most common method (61.7%). Mortality rates ranged from 42.0% for asphyxiation to 0.2% for blunt injuries. Individuals in their 20s showed a marked increase in suicide/self-harm attempts, especially in the last three years. A large proportion of decedents in their 70s or older (52.6%) used poisoning as a method of suicide. The percentage of individuals with two or more previous attempts rose from 7.1% (2011) to 19.7% (2020). The death rates by poisoning decreased from 7.7% (2011) to 2.5% (2020). Conclusion: Our findings underscore the urgent need for targeted interventions and suicide prevention policies. Managing and reducing suicide and self-harm in emergency settings will require a focus on poisoning, the 10-29 age group, and the elderly. This paper will be valuable for future policies aiming to reduce the societal burden of suicide and self-harm.
