Perfusion RRI of the Brain Using Oxygen Inhalation (산소 호흡을 이용한 뇌의 관류 자기공명영상)

  • Purpose : To know the possibility of clinical application of MRI using oxygen inhalation as a perfusion MRI Materials and methods : Two healthy volunteers and three patients of one moyamoya disease, one acute infarction and one meningioma were studied using a 1.5 Tesla MRI unit. Oxygen (15 liters/min) mixed with room air was given using face mask from 8 second to 35 second during the study. Images were acquired 25 times (scan time per study were 1.6 seconds) using susceptibility contrast EPI (echo planar image) sequence. Difference maps were acquired by early (study 12-18), and late (study 19-25) O2 inhalation image groups minus pre-O2 inhalation image group (study 3-9) with a Z-score of 0.7-1.0 using VB31C program of Magneton Vision. The resulting perfusion images were created by superimposition of difference maps on corresponding T1 weighted anatomic images. On moyamoya patient, similar perfusion images were acquired after Gd-DTPA injection, and compared with O2 inhalation perfusion images. Results ; The author can get the perfusion images of the brain by oxygen inhalation with susceptibility contrast EPI sequence at the volunteers, and the patient of moyomoya disease, acute infarction and meningioma. On moyamoya patient, perfusion images with O2 inhalation are similar with perfusion images by Gd-DTPA injection. Conclusion 1 This study has demonstrated that the susceptibility contrast EPI by oxygen inhalation can be used as the clinically useful perfusion MRI technique

  • MRI plays an important role in abdominal imaging because of its ability to detect and characterize focal lesions. However, MRI examinations have several challenges, such as comparatively long scan times and motion management through breath-holding maneuvers. Techniques for reducing scan time with acceptable image quality, such as parallel imaging, compressed sensing, and cutting-edge deep learning techniques, have been developed to enable problem-solving strategies. Additionally, free-breathing techniques for dynamic contrast-enhanced imaging, such as extra-dimensional-volumetric interpolated breath-hold examination, golden-angle radial sparse parallel, and liver acceleration volume acquisition Star, can help patients with severe dyspnea or those under sedation to undergo abdominal MRI. We aimed to present various advanced abdominal MRI techniques for reducing the scan time while maintaining image quality and free-breathing techniques for dynamic imaging and illustrate cases using the techniques mentioned above. A review of these advanced techniques can assist in the appropriate interpretation of sequences.

The effect of Simulation-based learning scenario using standardized repiratory patients on learning satisfaction, clinical skill competency and self-efficacy in Health-related department students (호흡기계 표준화환자를 활용한 시뮬레이션 기반 시나리오 학습이 보건계열 대학생의 학습만족도, 임상수행능력과 자기효능감에 미치는 효과)

  • This study was conducted to examine the effects of a simulation-based learning scenario using standardized respiratory patients regarding emergency care on learning satisfaction, and self efficacy, clinical skill competency of health related department students. A quasi-experimental non-equivalent control group pre-post test design was used. A total of 50 students, 25 students for the experimental group and 25 students for the control group, The experimental group received the 2 educations sessions and 1 evaluation session with 180 minutes for each session. It was implemented in November, 2013. Data were analysed using frequency, ratio and t-test by the SPSS/Win 18.0. The experimental group who had the simulation-based learning scenario using standardized respiratory patients showed significantly higher learning satisfaction and self efficacy, clinical skill competency compared with the control group who had a traditional simulation education. Through this study, simulation-based learning scenario using standardized patients was verified to be an effective teaching method to be grow up on professional competency of health related department students. Also the simulation-based learning scenario using standardized patients should be developed in the various fields.

  • 생물은 자기 복제를 통한 생장이나 생명유지를 위해 에너지를 필요로 한다. 화학영양생물은 화학에너지를 발효 혹은 호흡을 통해 생물학적 에너지로 전환시키며, 광영양생물은 광합성 작용을 통해 광에너지를 이용한다. 발효, 호흡, 광합성은 모두 산화-환원 반응을 통해 이루어진다. 생물의 모든 에너지 전환반응은 산화-환원 반응, 즉 전자의 흐름으로 이루어지며 생명현상이 에너지를 필요로 하기 때문에 생명현상은 전자의 흐름으로 이루어진다고 할 수 있다. 모든 생물이 에너지 전환 반응에 산화-환원 반응을 이용한다는 말은 생물이 많은 종류의 산화-환원 효소를 보유하고 있다는 뜻이며, 실제 많은 종류의 산화-환원 효소가 발견되고 연구되었다.

Case of Treatment Using Adaptive Servo-Ventilation in a Patient with Central Sleep Apnea after a Lateral Medullary Infarction (외측 연수 경색에 의해 발생한 중추성 수면 무호흡 환자의 자동-적응형 양압기를 사용하여 치료한 경험)

  • Central sleep apnea (CSA) is characterized by respiratory failure of at least 10 seconds without any effort of the chest and abdomen in the absence of upper airway resistance during sleep. In this case, the patient experiences respiratory failure that does not meet the CSA diagnostic criteria and CSA symptoms. Magnetic resonance imaging diffusion-weighted imaging (MRI DWI) scans revealed a lateral medullary infarction. Continuous positive airway pressure (CPAP) was applied as a primary treatment for CSA and respiratory failure. During the titration of CPAP, the apnea-hypopnea index (AHI) and arousal index (AI) were worse than the results before its use (AHI: 42.5/hr→82.8/hr, AI: 21.7/hr→40.8h). As a result, adaptive servo-ventilation (ASV) was chosen as the secondary treatment. Compared to the night-polysomnography results before the ASV treatment, the AHI improved (42.5/hr→8.6/hr). Therefore, ASV is a potential treatment for CSA and respiratory failure in these patients.

Education Effect in Basic CPR for the Dental Hygiene Students (치위생과 학생을 대상으로 한 기본 심폐소생술 교육효과)

  • This study was to examine the knowledge, attitude, self-efficacy, and capability of skill for Dept. of Dental Hygiene students in performing basic skill of cardiopulmonary resuscitation as the following AHA's CPR 2005 Guide-Lines. And we hope to know the influence of those affect on the correctness of chest thrusts, the correctness of artificial respiration, and the correctness of skill performing. As a SPSS 14.0 program's analyzing results, we came to the following conclusions : 1. In the basic CPR's knowledge, the scores of knowledge rose from 5.93 before education to 12.46 after education. In attitude, the scores of attitude rose from 2.52 to 3.71 after education. In self-efficacy's scores, rose from 3.22 to 3.26 too, and all components have statistically significant differences(P < .05). 2. In performing CPR, the correctness of chest thrusts is 68.90%, artificial respiration is 19.00%. 3. Total average score of the capability of skill is 4.51. 4. Only self-efficacy affected the correctness of chest thrusts after education(P < .05). 5. Attitude after education affected both the correctness of artificial respiration and skill performing(P < .05).

Respiratory Assist by Use of Electrical Diaphragmatic Pacing (전기자극에 의한 횡격막 조율을 이용한 호흡보조장치)

  • Background: Electrical breathing pacing has many advantages over mechanical ventilation. However, clinically permanent diaphragmatic pacing has been applied to limited patients and few temporary pacing has been reported. Our purpose is to investigate the feasibility of temporary electrical diaphragm pacing in explothoracotomy canine cases. Methods: Five dogs were studied under the general anesthesia. Left 5th intercostal space was opened. Self designed temporary pacing leads were placed around the left phrenic nerve and connected to the myostimulator. Chest wall was closed after tube insertion with underwater drainage. Millar catheter was introduced to the aorta and right atrium. Swan-Ganz catheter was introduced to the pulmonary artery. When the self respiration was shallow with deep anesthesia, hemodynamic and tidal volume were measured with the stimulator on. Results: Tidal volume increased from 143.3$\pm$51.3 ml to 272.3$\pm$87.4 ml(p=0.004). Right atrial diastolic pressure decreased from 0.7$\pm$4.0 mmHg to -10.5$\pm$4.7 mmHg(p=0.005). Pulmonary arterial diastolic pressure decreased from 6.1+2.5 mmHg to 1.2$\pm$4.8 mmHg(p<0.001). The height of water level in chest tube to show intrathoracic pressure change was from 10.3$\pm$6.7cmH$_{2}$O to 20.0$\pm$5.3 cmH$_{2}$O. Conclusion: Temporary electrical diaphragmatic pacing is a simple method to assist respiration in explothoracotomy canine cases. Self designed pacing lead is implantable and removable. Negative pressure ventilation has favorable effects on the circulatory system. Therefore, clinical application of temporary breathing pacing is feasible in thoracotomy patients to assist cardiorespiratory function.

  • 복부의 자기공명영상(Magnetic resonance imaging; MRI)은 주로 간 질환의 검사에서 다른 영상검사로 해결되지 않은 궁금점을 풀기 위한 보조적인 검사로 주로 사용하였으나, 최근 들어서는 병변의 발견 및 감별진단에 직접적인 검사로도 자주 이용하고 있으며, 담췌관 질환이나 위장관 검사 등에도 적용범위가 넓어지고 있다. 이는 그간의 하드웨어 및 소프트웨어상의 발달로 인해, 위장관 연동운동이나 호흡에 따른 인공물을 억제하면서도 해부학적인 세부구조는 자세하게 나타낼 수 있는 우수한 영상을 얻을 수 있음으로써 가능하게 되었다. 고속영상은 영상의 질을 우수하게 할 뿐 아니라, 검사시간을 단축시켜서 더 많은 환자를 검사할 수 있게 하고, 한자가 검사에 더 잘 적응할 수 있게 하여준다. 단발(single-shot)기법의 고속 T2강조영상은 담췌관이나 위장관등 그간 MRI를 적용하기 어려웠던 부위에 대한 검사가 가능하게 하였으며, 고속의 3차원 T1강조 펄스대열(pulse sequence)은 단면에 따른 오기록(misregistration)이나 운동인공물이 적으면서도, 향상된 다평면적인 영상구성을 얻을 수 있게 하였다. [l]. 본 강좌에서는 최근 수년간 복부 MRI의 응용을 증가할 수 있게 한 영상장치 및 펄스대열의 발전과 최근 국내에 시판이 허용되어 사용이 증가되고 있는 새로운 간자기공명조영제에 관해서 살펴보고자 한다.

Feasibility of $In$ $vivo$ Proton Magnetic Resonance Spectroscopy for Lung Cancer (폐암의 생체 수소자기공명분광법의 실행가능성)

  • Purpose : To investigate the feasibility of in vivo proton magnetic resonance spectroscopy (MRS) for evaluation of lung cancer. Materials and Methods: This prospective study was approved by the institutional review board of our hospital and informed consent was obtained in all patients. Ten patients (7 men, 3 women; mean age, 64.4) with pathologicallyproven lung cancer (mean, 56.8 mm; range, 44-77 mm) were enrolled to 1.5 T MRS using a single-voxel respiration-triggered point-resolved spectroscopic sequence. Technical success rate and the reason of technical failure, if any, were investigated. Results: Out of 10 lung cancers, analyzable MRS spectra were obtained in 8 tumors (technical success rate, 80%). Two MRS datasets were not able to be analyzed due to serious baseline distortion. Choline and lipid signals were detected as major metabolites in analyzable MRS spectra. Conclusion: In vivo proton MRS method using a single-voxel respiration-triggered point-resolved spectroscopic sequence is feasible in obtaining the MR spectra of lung cancer because these spectra were analyzable and high success rate was shown in our study although there was the limitation of small patient group.
