Analysis of the Entrance Surface Dose (ESD) and Contrast Usage of Trance Femoral Cerebral Angiography (TFCA) and Cerebral Computed Tomographic Angiography (CCTA) for Cerebrovascular Disease Examining (뇌혈관 질환 검사를 위한 뇌혈관 조영술(TFCA)과 뇌혈관 전산화 단층 촬영 검사(CCTA)의 입사표면선량(ESD) 및 조영제 사용량 분석)

  • A typical cerebrovascular disease among cerebrovascular diseases is vascular diseases such as cerebral infarction, cerebral hemorrhage, cerebral aneurysm, cerebrovascular stenosis. If the disease occurs and causes cerebral damage, it may be difficult to recover completely. So that, Must continue to perform health care through examination early. In particular, Because most cerebrovascular disease examining use radiation equipment and Thus this study was to find out how to select about the optimal examining method and X-ray dose decrease method among different examining method though comparison and analysis for the entrance surface dose (ESD) on cerebrovascular examining with Trance Femoral Cerebral angiography (TFCA) and Cerebral Computed Tomographic Angiography (CCTA). Also, want to find out how to select about the optimal examining method for worried patient that contrast medium side effect though measuring and evaluating for contrast usage. Data were collected from 70 patients (43 males and 27 females) who underwent CCTA at Yeosoo region hospital from June 2018 to December 2018 and 61 patients (34 males and 27 females) who underwent TFCA at Pyeongtaek region hospital from June 2018 to November 2018. ESD analysis method collected retrospective data though M-view and PACS PLUS program, Used contrast usage measuring method did reality measuring method. In the analysis using SPSS, the ESD of TFCA was $245.74{\pm}71.91$, which was $32.05{\pm}7.74$ lower than the dose of $277.79{\pm}79.65$ of CCTA ESD, and statistically significant at t = 3.249, p = 0.017 (p<0.05). As a result of the comparison of the total amount of contrast agent, the mean contrast agent used in TFCA was $55.05{\pm}17.68ml$, which was about 14.95 smaller than the amount of contrast agent used in CCTA, and statistically significant t = -4.548, p<0.001. In conclusion, the ESD of TFCA was statistically significantly lower than that of CCTA, and also the used contrast usage was significantly tiny than that of CCTA. Therefore, Select the method to increase the utilization of TFCA for cerebral disease examining, we can consider X-ray dose decrease method at the same time as to decrease side effect of contrast medium.

  • This study aimed to propose a methodology for quantitatively analyzing problems resulting from the performance and combination of the ionization chamber when using an automatic exposure control (AEC) and to optimize the performance of the digital radiography (DR). In the experimental method, the X-ray quality of the parameters used for the examination of the abdomen and pelvis was evaluated by percentage average error (PAE) and half value layer (HVL). Then, the stability of the radiation output and the image quality were analyzed by calculating the entrance surface dose (ESD) and entropy when the three ionization chambers were combined. As a result, all of the X-ray quality of the digital radiography used in the experiment showed a percentage average error and a half value layer in the normal range. The entrance surface dose increased in proportion to the combination of chambers, and entropy increased in proportion to the combination of ionization chambers except when three chambers were combined. In conclusion, analysis using entrance surface dose and entropy was found to be a useful method for evaluating the performance and combination problems of the ionization chamber, and the optimal performance of the digital radiography can be maintained when two or less ionization chambers are combined.

Radiological Perspectives for Diagnosis of Vasospastic Angina with Coronary Angiography (이형성 협심증 진단 조영 검사의 방사선학적 관점)

  • If complete coronary artery occlusion occurs due to severer coronary spasm, malignant arrhythmias can lead to death. Therefore, early screening for coronary artery spasm angina is essential. Among the test methods, the drug injection test through coronary angiography is generally performed. Therefore, the purpose of this study was to evaluate the advantages of ergonovine drug test for vasospasitc angina examination during coronary angiography, such as the relationship between the procedure time, contrast medium usage, and radiation exposure effects of coronary angiography. Follow-up data of 142 patients who underwent coronary angiography and variant angina examination from september 2021 to february 2023 were used. As a result of analyzing contrast usage dose and dose area product and air kerma dose and number of imaging series and procedure time, variant angina examination was statistically significantly higher than coronary angiography. (p<0.001) In conclusion, variant angina examination other than coronary artery angiography are radiologically negative. Therefore, we think it is better to avoid excessive inspection. Nevertheless, in the case of the provocation test, the longer the examination time, the higher the fluoroscopy time and the amount of contrast medium used, so it is better to conduct the test as quickly as possible or shorten it.

  • Recently, There has been a growing interests in exposure dose to the patient who take a examination using radiation. The radiological technologists should be concerned about the exposure dose to patients and make an efforts to reduce the patient dose without decreasing the image quality. In the case of foreign, the exposure dose of general X-ray examination have been managed by standard value of exposure dose using dose area product (DAP) and entrance surface dose (ESD) dosimeter. This study is to compare DAP and ESD in skull anterior posterior (AP), chest posterior anterior (PA), and abdomen AP projections of phantom by using DAP and ESD dosimeter. In the results, there were no differences between DAP and ESD dosimeter.

  • Voxel head phantom for overcoming the limitation of mathematical phantom in depleting anatomical details was constructed and example dose calculation for BNCT was performed. The repeated structure algorithm of the general purpose Monte Carlo code, MCNP4B was applied for yokel Monte Carlo calculation. Simple binary yokel phantom and combinatorial geometry phantom composed of two materials were constructed for validating the voxel Monte Carlo calculation system. The tomographic images of VHP man provided by NLM(National Library of Medicine) were segmented and indexed to construct yokel head phantom. Comparison of doses for broad parallel gamma and neutron beams in AP and PA directions showed decrease of brain dose due to the attenuation of neutron in eye balls in case of yokel head phantom. The spherical tumor volume with diameter, 5cm was defined in the center of brain for BNCT dose calculation in which accurate 3 dimensional dose calculation is essential. As a result of BNCT dose calculation for downward neutron beam of 10keV and 40keV, the tumor dose is about doubled when boron concentration ratio between the tumor to the normal tissue is $30{\mu}g/g$ to $3{\mu}g/g$. This study established the voxel Monte Carlo calculation system and suggested the feasibility of precise dose calculation in therapeutic radiology.

A Comparative Study of Image Quality and Radiation Dose according to Variable Added Filter and Radiation Exposure in Diagnostic X-Ray Radiography (진단용 X-선 촬영시 부가 필터 및 노출의 변화에 따른 피폭선량 및 영상 화질 비교 연구)

  • To know which parameters were acceptable for achieving lowest radiation exposure to the patients and highest image quality at the diagnostic X-ray radiography, we measured the patient radiation dose and image quality in transmitted PACS (Picture Archiving and Communication System) at variable combinations of the added filters. As a result, the Dose Area Product (DAP: $mGy{\cdot}cm^2$) and Entrance Surface Doses (ESDs: $mGy$) was lowest at 1 mmAl + 0.2 mmCu and highest at 0 mmAl. The histogram of the image quality by transmitted PACS was not significantly different at variable combinations of exposure parameters on the MATLAB. In conclusion, this study can be helpful for expecting radiation dose-exposure and control exposure parameters for the diagnostic X-ray radiography.

A Monte Carlo Study of Dose Enhancement with kilovoltage and megavoltage photons (몬테칼로 기법을 이용한 kV, MV X선에서의 선량증가 효과 비교 평가)

  • Monte Carlo simulations were used to assess dose enhancement effects for 60-, 90-, 120-, and 150-kV X-rays, and for 6- and 15-MV X-rays. The MCNPX code was used for a computer simulation of the ICRU slab phantom, and gold, gadolinium, and iron oxide (Fe2O3) were employed as dose enhancement agents. In consideration of the buildup region of the incident energy, agent concentrations of 5, 10, 15, and 20 mg/g were inserted on the surface of the phantom at a depth of 5 cm. Based on baseline values obtained in the absence of dose enhancement agents, a quantitative analysis was performed by evaluating depth-dependent changes in the absorbed energy and the dose enhancement factor (DEF). A higher concentration of dose enhancement agents led to a greater dose enhancement effect with iron oxide, gadolinium, and gold in descending order. For kilovoltage (kV) X-rays, as the incident energy was decreased and as the energy became closer to the ionization potential of the atoms in the enhancement agent, the dose enhancement effect increased. In the megavoltage (MV) X-ray range, dose enhancement was higher at 6 MV compared with 15 MV. However, the overall dose enhancements were significantly lower compared to the results obtained with kV X-rays.

  • Purpose: This paper describes a electron field presence of magnetic field, intensity and shape surface dose variation to clinical application possibility. Materials and Methods: The using 6 MeV electron and $10{\times}10\;cm^2$ field size, 9 hole to shielding block make the by measure the film, when the magnetic field position inside and outside of the X-Omat film and parallel plate ionization chamber using the surface dose measured. Results: Present of 4 cm to the side at angle about 3 degree from beam center, use of ring type magnetic is 0.9% increase the surface dose, lens block located in the magnetic field the surface dose 1.58% increase, half magnetic field's position on the side of them at the field center of the 3.6% increase of the surface dose. Conclusion: Surface dose variation is with magnetic field about the mean electron beam of progress direction change, orbit region patient's is inconvenient without surface dose increase percentage case goodness will be used as a useful way.

Comparisons of Image Quality and Entrance Surface Doses according to Care Dose 4D + Care kV in Chest CT (Chest CT에서 Care Dose 4D+Care kV에 따른 화질과 입사표면선량 비교)

  • This study compared DLP values along with phantom entrance surface doses and the image quality of chest CT scans made using a Care Dose 4D+Care kV System, scans that are made using only the Care Dose 4D function, and scans that are made with changes made by applying 80 kVp, 100 kVp, 120 kVp, and 140 kVp to the Care Dose 4D and tube voltage to search for methods to maintain the highest image quality with minimal patient doses. It was shown that DLP values decreased 6.727% when scans were taken with Chest Care Dose 4D + Care kV semi 100 and 6.481% when scans were taken with Chest Care Dose 4D + Care kV. With Chest Non as a standard, skin surface doses decreased 16.519% when scans were taken with Chest Care Dose 4D + Care kV semi 100 and 15.705% when scans were taken with Chest Care Dose 4D + Care kV. With comparisons of image quality, when comparisons were made with Chest Non, comparisons made of SNR values and CNR values in all scanning conditions including Care Dose 4D + Care kV showed that there were no significant differences at P>0.05. Imaging using Chest Care Dose 4D + Care kV in chest CT showed that exposure doses decreased similarly to result values gained from the best conditions through manual adjustments of kV and mAS, and there were no significant differences in image SNR and CNR. If the Chest Care Dose 4D + Care kV function is used, image quality is maintained and patient exposure to radiation can be reduced.

Prediction of Entrance Surface Dose in Chest Digital Radiography (흉부 디지털촬영에서 입사표면선량 예측)

  • The purpose of this study is predicted easily the entrance surface dose (ESD) in chest digital radiography. We used two detector type such as flat-panel detector (FP) and IP (Imaging plate detector). ESD was measured at each exposure condition combined tube voltage with tube current using dosimeter, after attaching on human phantom, it was repeated 3 times. Phantom images were evaluated independently by three chest radiologists after blinding image. Dose-area product (DAP) or exposure index (EI) was checked by Digital Imaging and Communications in Medicine (DICOM) header on phantom images. Statistical analysis was performed by the linear regression using SPSS ver. 19.0. ESD was significant difference between FP and IP($85.7{\mu}Gy$ vs. $124.6{\mu}Gy$, p=0.017). ESD was positively correlated with image quality in FP as well as IP. In FP, adjusted R square was 0.978 (97.8%) and linear regression model was $ESD=0.407+68.810{\times}DAP$. DAP was 4.781 by calculating the $DAP=0.021+0.014{\times}340{\mu}Gy$. In IP, adjusted R square was 0.645 (64.5%) and linear regression model was $ESD=-63.339+0.188{\times}EI$. EI was 1748.97 by calculating the $EI=565.431+3.481{\times}340{\mu}Gy$. In chest digital radiography, the ESD can be easily predicted by the DICOM header information.
