Changes in periodontal tissue and denture around the implants in the mandibular milled-bar implant overdenture: A 12-year follow-up (하악 Milled Bar 임플란트 피개의치에서 12년 간의 임플란트 주변 치주조직 및 의치의 변화)

  • The mandible has a smaller support area than the maxilla, and in particular, in patients with generalized alveolar bone resorption, the stability of the denture and the masticatory efficiency are significantly low in the case of conventional complete denture, due to the movement of the tongue and mandible. In these patients, implant overdenture is evaluated as a highly predictable treatment method with high retention and stability, and excellent pronunciation and masticatory force. In this clinical case, patient had an edentulous maxilla and generalized alveolar bone resorption in mandible. Therefore, complete denture was placed in the maxilla and implant overdenture using milled bar attachment on 4 implants was placed in the mandible. During the 12-year follow-up period, changes in periodontal tissue and denture around the implants were periodically managed and observed.

  • Implant overdentures are widely used as a treatment method to restore oral function in completely edentulous or partially edentulous patients with severe bone resorption. Using a milled bar, it is mechanically advantageous as the implant fixtures are splinted. Applying additional attachments to the bar has the advantage of dispersing the stress applied to the implant. In this case, a patient who used implant overdentures using 4 implants wanted to fabricate a new prosthesis due to repeated fractures of the denture and weakened retention. Milled bar with ADD-TOC attachment and zirconia prosthesis were fabricated by CAD-CAM method and mechanically and aesthetically satisfactory results were obtained.

  • Loss of tooth results in remodeling and resorption of surrounding alveolar bone which causes atrophic edentulous ridge and gradually decreasing gingival attachment. As a result, edentulous patients face difficulty in using dentures due to pain, decrease of support, decline of masticatory efficiency of complete denture. To improve this, overdenture with implant in the mandible and attachment are considered as a treatment of choice as a favorable treatment. In this case, a patient with edentulous ridge for long period is rehabilitated by complete denture in maxilla and implant overdenture using Locator$^{(R)}$ attachment in mandible.

Factors associated with the survival rate and the marginal bone loss of dental implant over 7-years loading (7년 이상 기능한 임플란트의 변연골 흡수와 생존율에 영향을 주는 요인)

  • Purpose: The purpose of this study was to analyze the factors affecting the survival rate and the marginal bone level of dental implants that have functioned over 7-years. Materials and Methods: In 92 patients, 178 dental implants were included. Implant-related factors (diameter, length, prosthetic splint), patient-related factors (gender, smoking, plaque index, compliance to supportive periodontal therapy) and surgery-related factors (proficiency of surgeon, bone graft) were evaluated via clinical and radiographic examination. The marginal bone level was determined by intraoral standard radiography at the mesial and distal aspects of each implant using an image analysis software program. Results: The survival rate of all the implants was 94.94% and the marginal bone level was $0.89{\pm}1.05mm$, these results are consistent with other studies that present long-term good clinical results. Implant length and plaque index among several factors were statistically significant for implant survival rate (P < 0.05). Smoking and the presence of regeneration surgery were statistically significant for the marginal bone level (P < 0.05). Conclusion: Dental implant that have functioned over 7-years showed favorable long-term survival rates and marginal bone level. Implant length and plaque control should be considered for improving the long-term clinical results. It is needed that careful application of bone regeneration technique and smoking control for maintaining of marginal bone level.

  • 건강한 치아는 '오복' 중의 하나로 꼽힌다. 그 이유는 아마도 '잘 먹기'가 그만큼 중요하기 때문일 것이다. 우리 몸의 모든 기관이 정상적으로 작동하기 위해서 영양분이 제대로 전달되어야 하고 그 시작은 '음식물 씹기'에서 시작되기 때문이다 치아의 가장 중요한 임무는 삼키기 쉽도록 음식을 자르고 부수는 것이다 거기에 더해 음식의 맛도 느낄 수 있게 도와준다. 여러 개의 치아를 잃게 되면 임플란트나 틀니치료를 받게 된다. 하지만 아무리 좋은 임플란트, 틀니라 하더라도 건강한 치아와 같을 수는 없다. 치아를 건강하게 보존하고 유지하는 것이야말로 '건강함'을 유지하기 위한 시작이라 볼 수 있다.

A 15-year clinical retrospective study of Br${\aa}$nemark implants (Br${\aa}$nemark 임플란트의 15년 임상적 후향 연구)

  • Purpose: This study was to compare the cumulative survival rate (CSR) of Br${\aa}$nemark machined surface implants and TiUnite$^{TM}$ imlants and to analyze association between risk factors and the CSR of the implants. Materials and methods: A retrospective study design was used to collect long-term follow-up clinical data from dental records of 156 patients treated with 541 Br${\aa}$nemark machined and TiUnite$^{TM}$ implants at Korea University Guro hospital in South Korea from 1993 through 2008. Machined implant and TiUnite$^{TM}$ implant were compared by CSR. Exposure variables such as gender, systemic disease, location, implant length, diameter, prosthesis type, opposing occlusion type, date of implant placement, type of edentulous space, abutment type, existence of splinting with natural teeth, and existence of cantilever were collected. Life table analysis was undertaken to examine the CSR. Cox regression method was conducted to assess the association between potential risk factors and overall CSR (${\alpha}$=.05). Results: Patient ages ranged from 16 to 75 years old (mean age, 51 years old). Implants were more frequently placed in men than women (94 men versus 63 women). Since 1993, 264 Br${\aa}$nemark machined implants were inserted in 79 patients and since 2001, 277 TiUnite$^{TM}$ implants were inserted in 77 patients. A total survival rate of 86.07% was observed in Br${\aa}$nemark and Nobel Biocare TiUnite$^{TM}$ during 15 years. A survival rate of machined implant during 15 years was 82.89% and that of TiUnite$^{TM}$ implant during 5 years was 98.74%. The implant CSR revealed lower rates association with several risk factors such as, systemic disease, other accompanied surgery, implant location, and Kennedy classification. Conclusion: Clinical performance of Br${\aa}$nemark machined and TiUnite$^{TM}$ implant demonstrated a high level of predictability. In this study, TiUnite$^{TM}$ implant was more successful than machined implant. The implant CSR was associated with several risk factors.

Retrospective Study of GS II Implant(Osstem) with an Internal Connection with Microthreads (Micro thread를 포함한 GSII RBM임플란트(Osstem)의 후향적 임상연구)

  • Since the introduction of the concept of osseointegration in dental implants, high long-term success rates have been achieved and accepted as viable option for the treatment of fully and partially edentulous patients. Although the use of domestic implants have increased dramatically, there are few studies on domestic implants with clinical and objective long-term data. 96 endosseous implants placed in 31 patients at Wonkwang University Sanbon Dental Hospital were examined to determine the effect of various factors on implant survival rate and marginal bone loss, through clinical and radiographic results. The design of endosseous implant used to this study is straight with the microthread.(GS II RBM Fixture) 1. 3 fixtures were lost, resulting in 96.9% cumulative survival rate. 2. Survival rate in fifties was significantly lower (93.6%) and no significant difference in marginal bone loss was found according to gender. 3. Survival rates were 95.6% in the maxillary molar area and 97.3% in the mandible molar area. 4. No significant difference in survival rate was found according to presence of bone grafts, type of prostheses, implant position, and length and diameter of implant. 5. A factor influencing marginal bone loss was presence of type of prostheses, while facters such a length, diameter of fixture and bone grafts had no statistically significant effect on crestal bone loss. This study indicates the amount of marginal bone loss around implant has maintained a relative stable during follow-up periods.

Intrusion phenomenon of natural tooth bounded by implant-prostheses: a clinical report (임플란트 사이에 있는 자연치 함입 증례)

  • In case of implant-tooth connected prosthesis, a natural tooth tends to intrude. There are several mechanisms that explain an intrusion phenomenon. So it is reco mmended not to connect an implant with a natural tooth. A 68-year-old female had upper left $2^{nd}$ premolar and $2^{nd}$ molar extracted and underwent implant surgery on the missing area. We made an implant prosthesis and treated upper left $1^{st}$ molar with a gold crown. 2.5 year later, the patient complained about loose proximal contact and food impaction between upper left $1^{st}$ molar and $2^{nd}$ molar. Mesial side of upper left $2^{nd}$ molar implant prosthesis was soldered so that proximal contact became tight again. But after 7 months, about 2 mm intrusion of upper left $1^{st}$ molar occurred, and the patient felt periodontally originated pain on intruded upper left $1^{st}$ molar. After the gold crown on upper left $1^{st}$ molar was removed, extrusion occurred and pain was relived.

Oral rehabilitation using implant supported fixed dental prostheses in a growing patient who underwent mandibulectomy and fibular free flap (하악골 절제술 및 비골유리피판을 시행한 성장기 환자에서 임플란트를 이용한 고정성 보철 수복 증례)

  • Functional and esthetic recovery through reconstruction of the oral and maxillofacial system is important for patients who underwent mandibulectomy due to lesions such as oral cancer. The fibula free flap, which is frequently used in the reconstruction of the mandible, is easy to prosthetic restoration with implant placement. However, patients with growing periods have difficulty in treatment with implants when the size of the fibula is small and residual growth remains. This case is an 11-year-old female patient who underwent mandibulectomy due to osteosarcoma and reconstruction of the mandible using a fibular free flap. Functional and esthetic recovery were obtained through prosthetic restoration using implant-supported fixed prostheses. Implantation site, number of implants, and type of prostheses were considered. Implant-supported fixed prostheses were delivered at 13 years old, and after an additional 1 year observation, stable results were obtained. However, observing changes in the maxillofacial system through further examinations is necessary.

Esthetic restoration in mandibular anterior region with one-piece implant and immediate loading (하악 전치부에서의 일체형 임플란트 식립 후 즉시부하)

  • Mandibular anterior region has high implant survival rates and better accessibility compared with any other region. Even if there are possibilities for perforation on lingual cortical bone due to improper drilling and bleeding caused by lingual artery damage, mandibular anterior region is a safe region because less amount of major anatomical structures exist compared with other regions. However, because of narrow bucco-lingual width of alveolar ridge, it is challengeable to obtain esthetic implant prosthesis. Although patients are less sensitive subjectively, mandibular anterior region is as difficult as maxillary anterior region in that implant placement location plays a critical role on the prognosis of implant prosthesis. One-piece implant is a very useful option for mandibular anterior region. Considering the narrow roots and thin alveolar bone of mandible, it is clinically difficult for implant diameter to be greater than 3mm In this case, we could approach the esthetic restoration in mandibular anterior region with one-piece implant and immediate loading.
