Purpose: In this study, the diameter of each implant driver was measured and compared to find out the compatibility of implant drivers. Materials and methods: Drivers from 12 implant systems being used in Dankook University Dental Hospital were included in this study. The shapes of the implant drivers were segregated, and the effective length and the diameter of upper, middle, lower part of driver tips were measured (n=10). The measured data were mathematically analyzed for its compatibility. Results: A driver with the smallest diameter (1.17 mm) had the highest compatibility at the upper part of driver tip. This driver could be used for a bigger driver up to 1.35 mm in diameter. There were several driver groups which had the same diameter so as to be interchangeable each other. In the middle part, the smallest diameter measured was 1.2 mm and this was able to replace a driver up to 1.40 mm diameter. Since the diameter generally became thicker from upper part (the tip of driver) to the lower part (the shank of driver), some drivers with bigger diameter at the upper part so which was failed to show any compatibility became compatible with a driver which had smaller diameter at the upper part but wider in the middle part. The compatibility of torx shape drivers were affected by the inner diameter of the drivers not only by the outer diameter. Furthermore, the inner diameter of torx drivers decided the compatibility between torx and hex drivers. Conclusion: From the study it was found that compatibility in drivers existed among certain implant systems and to check its compatibility the diameter at a certain effective length should be measured. However, there has been not enough studies about long-term use of compatible drivers, so effects of using compatible drivers on drivers and implants are unknown. Therefore, usage in inevitable cases only is recommended and further study is needed.
Purpose: The purpose of this study is to ascertain the stability of the implant by comparing the effects of the change of implant diameter, length and design on implant stability quotient. Materials and methods: To remove the variable due to the difference of bone quality, the uniform density (0.48 g/$cm^3$) Polyuretane foam blocks (Sawbones$^{(R)}$, Pacific Research Laboratories Inc, Vashon, Washington) were used. Implants (Implantium$^{(R)}$, Dentium, Seoul, Korea) were placed with varying diameters (${\phi}3.8$, ${\phi}4.3$ and ${\phi}4.8$) and length (8 mm, 10 mm and 12 mm), to assess the effect on implant stability index (ISQ). Also the influence of the design of the submerged and the non-submerged (SimplelineII$^{(R)}$, Dentium, Seoul, Korea) on ISQ was evaluated. To exclude the influence of insertion torque, a total of 60 implants (n = 10) were placed with same torque to 35 N. Using Osstell$^{TM}$ mentor (Integration Diagnostic AB, Sweden) ISQ values were recorded after measuring the resonant frequency, one-way ANOVA and Tukey HSD test results were analyzed. (${\alpha}$=0.05). Results: 1. The change of the diameter of the implant did not affect the ISQ (P>.05), but the increase of implant length increased the ISQ(P<.001). 2. The change in implant design were correlated with the ISQ, and the ISQ of submerged design was significantly higher than that of the non-submerged design(P<.05). Conclusion: In order to increase implant stability, the longer implant is better to be selected, and on the same length of implant, submerged design is thought to be able to get a higher ISQ than the non-submerged.
목적: 노인 환자에서 식립된 임플란트의 생존율을 조사하고 다양한 요인에 따라 생존율에 미치는 영향을 후향적으로 평가하고자 하였다. 재료 및 방법: 2001년 1월부터 2007년 12월까지 조선대학교 치과병원 치주과에 내원한 65세 이상 노인 환자 56명에게 식립된 138개의 임플란트를 대상으로 하였다.(남자 38명, 여자 18명, 평균 연령$69.38{\pm}3.91$세) 임플란트 생존율은 환자 성별 및 연령, 전신질환 유무, 치아 상실 원인, 식립 위치, 임플란트 직경 및 길이, 골질, 골이식 여부, 상부 보철물 유형에 따라 조사되었다. Kaplan-Meier survival analysis을 통해 각 요인에 따른 누적 생존율을 조사하였고, chi-square test를 통해 누적 생존율과 각각의 요인 간의 통계학적 유의성을 평가하였다. 결과: 보철물 시적 후 5 ~ 140개월 간 추적 관찰을 시행하였다. 65세 이상 노인 환자 56명에게 식립된 총 138개의 임플란트 중 추적 관찰 기간 동안 5개의 임플란트가 실패하였다. 그 중 63개의 임플란트는 환자가 정기적인 내원 약속에 응하지 않아 도중에 조사대상에서 제외되었으며, 결과적으로 5년간 누적 생존율은 94.9%였다. 임플란트 생존율에 영향을 미치는 다양한 요인 중 골질 (P=0.037) 및 상부 보철물 유형 (P=0.015) 간에 누적 생존율이 통계적으로 유의한 차이가 있었다. 그외 연령, 성별, 전신질환 유무, 치아상실 원인, 식립 위치, 임플란트 직경 및 길이, 골이식 유무 관련 요인과 누적 생존율 사이에는 통계적으로 유의한 차이가 없었다. 결론: 여러 한계가 있었지만 노인 환자의 무치악 부위에 있어 임플란트는 장기간에 걸쳐 예지성 있고 받아들여질 만한 치료이면서 적은 합병증과 실패율을 가진다.

Purpose: The aim of the study was to evaluate preprosthetic initial survival rate and factors associated with survival of osseointegrated implants placed in edentulous area of maxilla and mandible and to suspect the possible causes leading to failure. Materials and Methods: A total of 2158 endosseous implants that had been inserted between 2004 through 2013 were placed in 770 patients. The clinical comparisons were performed to evaluate implant loss in relation to age and gender of patients, position, system, length and diameter of implant, and bone graft technique. Results: According to position, the survival rates were 98.23% in maxillary anterior site, 96.98% in maxillary posterior site, 97.85% in mandibular anterior site and 98.76% in mandibular posterior site (P < 0.05). According to diameter of implant, the survival rates were 100% under 3.0 mm, 97.09% between 3.0 to 3.5 mm, 98.19% between 3.5 to 4.0 mm and 98.29% between 4.0 to 4.5 mm but relatively lower survival rate was 75% in 5.0 mm-over (P < 0.05). The survival rates of implants were 89.51%, 98.28%, 98.34% and 99.27% in the group with isolated sinus graft, with isolated GBR, with sinus graft and GBR simultaneously and without bone graft, especially (P < 0.05). Conclusion: This study establishes a relationship between survival rate of implant and position, diameter of implant system and bone graft technique. In conclusion, there were low survival rates in maxillary posterior site, in dental implants with wide diameter of 5 mm-over, and in the group with isolated sinus graft.
The dental osseointegration implant should be enough to endure occlusion load and it's required to have efficient design and use of implant to disperse the stress into bones properly. Solidworks as a finite element analysis program for modeling and analysis of stress distribution was used for the research. The simple crown model was designed on applying conjoined condition with tightening torque of 20 Ncm of a abutment screw between a cement retained implant abutment and a fixture. A $45^{\circ}$ oblique loading from lingual to buccal side on buccal cusps of crown and performed finite element analysis by 100 N of external load. The results by a analysis for stress distribution of supporting bones of fixture were as below. The von Mises stress was concentrated on the upper side of supporting compact bone regardless of the diameters and lengths of fixture, and the efficiency result of stress reduction was increase of fixture's diameter than it's length. Therefore, it's effective to use wider fixture as possible to the conditions of supporting jaw bone.
Purpose: Resonance frequency analysis, Periotest, and removal torque (RT) test were known as the methods to assess implant stability. The results of these methods are affected by the bone condition, implant diameter and shape. The purpose of this study is to access the meaning and the correlationship of the resonance frequency analysis, Periotest and RT test in osseointegration simulated acrylic resin when the engaged bone thickness and peri-implant bone defect are changed. Materials and methods: To simulate osseointegration, the fixture was fixed to an aluminum mold with a screw. Acrylic resin powder and liquid were poured into the mold for polymerization. The engaged resin thickness with implant was controlled. Simulated cortical bone thicknesses were 1, 3, 5 and 10 mm. Additional 1, 3 and 5 mm peri-implant bone defects were simulated. Three types of implants were used; 4 mm diameter implants of straight shape, 4 mm diameter implants of tapered shape and 5 mm diameter implants of tapered shape. Five fixtures per each type were tested in respective bone condition. Resonance frequency analysis and Periotest were evaluated in all bone conditions. Peak removal torque was measured at simulated cortical bone thicknesses of 1 and 3 mm. The statistical analysis was performed with the Kruskal-Wallis test, Mann-Whitney U test, and Spearman test using a 95% level of confidence. Results: With increasing engaged bone depth, the Implant Stability Quotient (ISQ) values increased and the Periotest values (PTVs) decreased (P<.001, P<.001). With increasing peri-implant bone defect, ISQ values decreased and PTVs increased (P<.001). When the diameter of implant increased, ISQ values increased and Periotest values (PTV) decreased (P<.001). There was a strong correlation between ISQ values and PTVs (r = -0.99, P<.001). Furthermore, the peak removal torque values had weak correlations with both ISQ values and PTVs (r = 0.52, P<.001 ; r = -0.52, P<.001). Conclusion: This study confirmed favorable implant stability with increasing engaged bone depth and implant diameter and decreasing peri-implant bone defect. ISQ values and PTVs showed strong correlation with each other and not with the peak removal torque values.
Purpose. This retrospective study was to investigate the survival and complication rates of posterior single implant according to the implant diameters, lengths and position. Materials and methods. Patients who had been restored single implant prosthesis in the posterior area by the three prosthodontists in the department of prosthodontics, Daejeon dental hospital of Wonkwang university, in the period from February 2014 to May 2018 were selected for the study. A total of 505 patients with 697 implants were observed. The survival and complications of implants were investigated using electronic medical records and radiographs. Fixture diameters, lengths, position, patient's sex and age were assessed as possible factor affecting the survival and complications of implants. Results. 3-year cumulative survival rates of posterior single implants were 98.5% and 5-year cumulative survival rates were 94.4%. 5-year cumulative survival rates were higher in implants with diameter > 4.0 mm (97.0%) than implants with diameter ≤ 4.0 mm (89.5%), and in females (98.8%) than males (92.4%). There were statistically significant differences (P < .05). The mechanical complication rate was 20.1% and the biological complication rate was 4.7%. Complications occurred in order of abutment screw loosening (7.5%), decementation (6.3%), proximal contact loss (3.7%) and so on. Abutment screw loosening occurred more frequently in the lower molar region (10.5%), in males (9.5%) than females (5.1%), and in patients aged < 65 years (9.4%) than patients aged ≥ 65 years (5.1%). There were statistically significant differences (P < .05). Conclusion. The 5-year cumulative survival rates were higher in implants with diameter > 4.0 mm than implants with diameter ≤ 4.0 mm and in females than males. Abutment screw loosening which was the most commonly occurring complication occurred more frequently in the lower molar region, in males than females, and in patients aged < 65 years than patients aged ≥ 65 years. There were statistically significant differences.
Purpose: After the introduction of concept of osteointegration, dental implantology have been successful procedure in the dental field. Recently, it has shown successful results when used to restore single tooth missing. Considering the difference in bone quality of the mandible and maxilla, and the increased occlusal force in the posterior region, the success rates in each region may be different. In this study, success rates of single implants placed in the mandibular first and second molar areas were analyzed. Materials and methods: The subjects were patients (284 patients, 308 implants) who had been operated with single implant installation from 2002 to 2009 in seven dental clinics in Daegu city. One hundred sixty eight implants were placed in the mandibular 1st molar and 140 implants were placed in the mandibular 2nd molar. They were analyzed according to implant site, age, sex, length and diameter. Results: The survival rates of single implant of this study were 97.6% in the mandibular 1st molar and 92.9% in the mandibular 2nd molar. In the mandibular 1st molar, 4 implants were failed. In the mandibular, 2nd molar, 10 implants were failed. Conclusion: The restoration of the mandibular 1st molar using single implant was found to be clinically acceptable treatment and showed higher survival rate than mandibular 2nd molar single implant. Single implant in mandibular 2nd molar needs careful consideration of poor bone quality, risk of overloading and anatomical structure of the mandible.
