The quality of medical care can be defined as four types such as effectiveness, efficiency, adequacy, and scientific-technical quality. For the management of scientific-technical aspects, medical institutions annually disseminate the latest knowledge in the form of conservative education. However, there is an obvious limit to the fact that the latest knowledge is distributed quickly enough to the clinical site with only one-time conservative education. If intelligent information processing technologies such as big data and artificial intelligence are applied to the medical field, they can overcome the limitations of having to conduct research with only a small amount of information. In this paper, we construct databases on which the existing medicine prescription adaptations can be extended. To do this, we collect, store, manage, and analyze information related to oriental medicine at domestic and abroad Journals. We design a processing and analysis technique for oriental medicine evidence research data for the construction of a database of oriental medicine prescription extended adaption. Results can be used as a basic content of evidence-based medicine prescription information in the oriental medicine-related decision support services.
Purpose: This study aimed to assess the educational needs for nurses who care for terminal cancer patients and their families. To identify top needs along with key issues in consideration to develope hospice training program and provide specific recommendations. Methods: A cross-sectional design with nurses from nine universities' hospice specialist courses and seven cancer centers was used. Data were collected via e-mail or mail service from March to April in 2008. One hundred seventy three questionnaires were returned (return rate: 73.6%), and 156 questionnaires were eventually analyzed. The questionnaire consisted of Mason and Ellershaw's The Self-efficacy in Palliative Care (SEPC) and self-reporting confidence and educational needs in hospice care. Results: The mean age of the participants was 37.94 years, 82.1% were staff nurses, and 44.9% completed over six months hospice education. Mean$\pm$standard deviation score for total SEPC was $2.67{\pm}.62$, which was lower than average (score 3), with communication score being the lowest ($2.49{\pm}.69$). The lowest self-reporting confidence score was $2.03{\pm}.77$ in hospice administration and management, followed by providing complement therapy ($2.34{\pm}.77$), bereavement care ($2.34{\pm}.71$), lymph edema management ($2.35{\pm}.79$), and care planning ($2.36{\pm}.81$). The participants reported that additional education is needed in all topics, with pain management score being the highest ($3.71{\pm}.50$), followed by pain and symptom evaluation ($3.67{\pm}.52$), care for dying ($3.67{\pm}.52$), and communication and counseling ($3.63{\pm}.53$). There were significant subgroup differences in SEPC and self-reporting confidence between groups who completed 6 months hospice education or not, however, no significant difference in educational need between the groups. Conclusion: This study showed the need for developing hospice training program to improve compentency of nurses in hospice palliative care.
Purpose: Colorectal ranter is the 4th leading cause of cancer death in Korea and the prevalence is increasing continuously. This study was aimed to figure out the problems through the clinical consideration about terminal colorectal ranter patients who had died in hospice unit. Methods: We retrospectively reviewed the medical records in 78 patients with colorectal ranter who had admitted, received palliative care, and died in a hospice unit between April 2003 and November 2006. Results: The median age of patients was 59.6 years with 45 men (58%) and 24 women (42%). The median survival in hospice and palliative care was 36 days. The median hospitalization was 22 days. The most prevalent reason for admission was pain (38 patients, 49%), and the most common symptom was also pain (70 patients, 90%). Forty eight patients (62%) took analgesics before hospice referral. Twenty seven patients (65%) of 45 patients with intestinal obstruction have been performed palliative procedures. Median survival of patients with palliative procedure was higher than that of no palliative procedure group (47 days vs 19 days, P-value=0.005). Conclusion: The duration of hospice and palliative care was not enough to care the terminal colorectal cancer. Therefore, we suggest that proper education and information should be provided to physician, patients and their family members for effective hospice and palliative care.
Most terminally ill cancer patients experience various physical and psychological symptoms during their illness. In addition to pain, they commonly suffer from fatigue, anorexia-cachexia syndrome, nausea, vomiting and dyspnea. In this paper, I reviewed some of the common non-pain symptoms in terminally ill cancer patients, based on the National Comprehensive Cancer Network (NCCN) guidelines to better understand and treat cancer patients. Cancer-related fatigue (CRF) is a common symptom in terminally ill cancer patients. There are reversible causes of fatigue, which include anemia, sleep disturbance, malnutrition, pain, depression and anxiety, medical comorbidities, hyperthyroidism and hypogonadism. Energy conservation and education are recommended as central management for CRF. Corticosteroid and psychostimulants can be used as well. The anorexia and cachexia syndrome has reversible causes and should be managed. It includes stomatitis, constipation and uncontrolled severe symptoms such as pain or dyspnea, delirium, nausea/vomiting, depression and gastroparesis. To manage the syndrome, it is important to provide emotional support and inform the patient and family of the natural history of the disease. Megesteol acetate, dronabinol and corticosteroid can be helpful. Nausea and vomiting will occur by potentially reversible causes including drug consumption, uremia, infection, anxiety, constipation, gastric irritation and proximal gastrointestinal obstruction. Metoclopramide, haloperidol, olanzapine and ondansetron can be used to manage nausea and vomiting. Dyspnea is common even in terminally ill cancer patients without lung disease. Opioids are effective for symptomatic management of dyspnea. To improve the quality of life for terminally ill cancer patients, we should try to ameliorate these symptoms by paying more attention to patients and understanding of management principles.

To n the field of nuclear medicine, with regard to checking regular patients, from the moment they register up to the doctor's diagnosis, the person in charge of the checks can find errors in the diagnosis, reexamine, reanalyze the results or save images to PACS. Through this process, the results obtained from the readings are delayed due to checks and additional tests which occur in hospitals, causing patient satisfaction and affected reliability. Accordingly, the purpose is to include visual inspection of the results to minimize error, improve efficiency and increase patient satisfaction. Nuclear medicine and imaging tests from examines at Asan Medical Center, Seoul, from March 2008 to December 2008, were analyzed for errors. The first stage, from January 2009 to December 2009, established procedures and know-how. The second stage from January 2010 until June 2010 conducted Pre-and Post-filtering assessment, and the third stage from July 2010 until October 2010 consisted of cross-checks and attaching stickers and comparing error cases. Of 92 errors, the 1st, 2nd and 3rd stage had 32 cases, and there were 46 cases after the 4th stage, with the overall errors reduced by 74.3% from 94.6%. In the field of general nuclear medicine, where various kinds of checks are performed according to the patient's needs, analysis, image composition, differing images in PACS, etc, all have the potential for mistakes to be made. In order to decrease error rates, the image can continuously Cross-Check and Confirm diagnosis.
To date, detection of microbial populations in dairy products has been performed using culture media, which is a time-consuming and laborious method. The recently developed chromogenic media could be more rapid and specific than classical culture media. However, the newly developed molecular-based technology can detect microbial populations with greater rapidity and sensitivity than the classical method involving culture media and chromogenic media. This molecular-based technology could provide various options for monitoring the characterization of different states of bacteria and cells. Thus, it could help upgrade the processing system of the dairy industry so as to maintain the safety and quality of dairy foods. Among the various newly developed molecular-based technologies, flow cytometry can potentially be used for monitoring microbiological populations in the dairy industry if official international standards are available for this purpose. When omics technology would have biomarker identification, it could be regarded as the rapid and sensitive analytical methods. Methods based on PCR, which has become a basic technique in microbiological research, can be developed and validated as alternative methods for quantification of dairy microorganisms. This review discusses methods for monitoring microbiological populations in dairy foods and the limitations of these studies, as well as the need for further research on such methods in the dairy industry.

After large-scale reforestation in the 1960s and 1970s, forests in Korea have gradually been aging. Net ecosystem CO2 exchange of old-growth forests is theoretically near zero; however, it can be a CO2 sink or source depending on the intervention of disturbance or management. In this study, we report the CO2 budget dynamics of the Gwangneung deciduous old-growth forest (GDK) in Korea and examined the following two questions: (1) is the preserved GDK indeed CO2 neutral as theoretically known? and (2) can we explain the dynamics of CO2 budget by the common mechanisms reported in the literature? To answer, we analyzed the 15-year long CO2 flux data measured by eddy covariance technique along with other biometeorological data at the KoFlux GDK site from 2006 to 2020. The results showed that (1) GDK switched back-and-forth between sink and source of CO2 but averaged to be a week CO2 source (and turning to a moderate CO2 source for the recent five years) and (2) the interannual variability of solar radiation, growing season length, and leaf area index showed a positive correlation with that of gross primary production (GPP) (R2=0.32~0.45); whereas the interannual variability of both air and surface temperature was not significantly correlated with that of ecosystem respiration (RE). Furthermore, the machine learning-based model trained using the dataset of early monitoring period (first 10 years) failed to reproduce the observed interannual variations of GPP and RE for the recent five years. Biomass data analysis suggests that carbon emissions from coarse woody debris may have contributed partly to the conversion to a moderate CO2 source. To properly understand and interpret the long-term CO2 budget dynamics of GDK, new framework of analysis and modeling based on complex systems science is needed. Also, it is important to maintain the flux monitoring and data quality along with the monitoring of coarse woody debris and disturbances.
To improve germinability of carrot seeds of 'Inari', 'Mansan', 'Mussang', and 'Hongsim' cultivars, various levels of solid matrix priming (SMP) and the physiological changes of seeds associated with SMP were examined. As the ratio of water in a seed-Micro Cel E-water mix and priming duration increased, seed germinability enhanced. However, there was radicle protrusion during SMP in some cultivars, indicating the existence of cultivar different variations of the optimum conditions. The optimum ratio for a seed-Micro Cel E-water mix was 5 : 3 : 10.5. In this mix, the optimum durations for the treatment were 3 days for 'Mansan' and 5 days for other cultivars. SMP at $20^{\circ}C$ to $25^{\circ}C$ resulted in a higher germination percentage and a shorter $T_{50}$. Seeds absorbed water quickly within an hour and the percentage of moisture was stable at 43 to 49 after 24 hours of the treatment. Conductivity of the mix declined for the first 6 hours to $100{\mu}S{\cdot}cm^{-1}$ with little changes thereafter. On the other hand, the pH of the mixed increased slightly during the early phase, and then returned to the initial level as the treatment proceeded.
As the physical and mechanical properties of lightweight air-mixed soil change in the procedure of transportation of mix truck, it is necessary to assure whether the properties during construction satisfy those in design. In this study, variations of properties of mixed soil after transportation by mix truck are proved by field test. Lightweight air-mixed soil used field test the unit weight of $9.0{\pm}1.0kN/m^3$, the flow value of $190{\pm}20mm$ was produced. To analyze variations of properties of mixed soil the unit weight and flow value of the sample before and after transport was measured unconfined compressive strength tests were performed. Mixing time was 19~175 minutes diversified. As the test results, it is known that the density, the flow value and the unconfined compressive strength of lightweight air-mixed soil change by transportation, but these values satisfy the specifications of material of air-mixed soil. After transportation the average value of the unit weight and flow value change in the flow of the $(+)0.10kN/m^3$, 4.8 mm respectively, the average change in the unit weight and the flow value due to the mixing time was constant. And unconfined compressive strength of 28-day specimen increases from 20 to $150kN/m^2$. But, these values do not have some clear relationship with the transportation time within 175 minutes which is longest test time. Consequently, Within 175 minutes the changes of properties by transportation are too small to show some problems in the construction field.
The research subject is 8 mothers who put their physical/mentally impaired children in a group home, as the research method, this study conducted an in-depth interview survey. The participants of this study collected data from the in-depth interview on 8 mothers who put their disabled children to the group home. In the result of open coding, total 34 concepts, 28 subcategories and 13 categories were derived. The core category in the selective coding was 'establishing restorative relationship through entering a group home after encountering the limit of nurture.' Practical Strategies include the following; first, it's necessary to provide psychology counselling consequent on a mother's nurturing phase, and this study proposes a program for a father having a child with disability; in addition, there is the necessity of having to arrange the differentiated facility for physically/mentally impaired people, which meets the needs of the relevant people. At a level of policy, this study suggested the necessity of having to take into account the minimization of poverty problem facing a family having a disable child through the caring card, necessity of the use of good-natured card, and medical-social-welfare-based intervention, expansion of facility-touring class installation, and use of adult guardianship system, and differential payment of disabled child nurturing allowance consequent on income quantile, etc.
