Pathology and Classification of Focal Segmental Glomerulosclerosis (초점성 분절성 사구체 경화증의 병리와 분류)

  • Focal segmental glomerulosclerosis (FSGS) is the name of the primary glomerular disease as well as the terminology to describe the secondary phenomena of any other glomerular diseases. It is characterized by sclerosis, hyalinosis, foam cell infiltration, vacuolar change of podocytes, and halo formation in the glomerulus. Throughout the interstitium, lymphocytes infiltration, tubular atrophy and vascular changes are accompanied. Occasionally, IgM and/or C3 depositions are noted in the sclerotic areas. Electron microscopically, diffuse effacement of foot processes are seen in non-sclerotic area like minimal change disease. Podocyte injury patterns including vacuolar changes are frequently examined. Recently, Columbia group has suggested morphologic classification of FSGS and they demonstrated very good prognosis of tip lesion and poor prognosis of both collapsing and cellular types. However, the pathogenetic classification has been suggested by others; hyperfilteration, podocyte injury, genetic lesions etc. Further studies are necessary to understand and treat this disease.

  • Membranoproliferative glomerulonephritis (MPGN) is a progressive primary glomerulonephritis characterized by mesangial proliferation with increased mesangial matrix, subendothelial immune deposits, mesangial interposition and a double contour feature of the glomerular basement membrane. The glomerular involvement in MPGN is usually diffuse, however, cases of focal or segmental MPGN have been reported by several authors. We report a case of focal segmental MPGN with prolonged hypocomplementemia for ,3 years in a 5 years old girl. (J Korean Soc Pediatr Nephrol 2005;9:237-244)

An Effects of Radiation Dose Assessment for Radiation Workers and the Member of Public from Main Radionuclides at Nuclear Power Plants (원전에서 발생하는 주요 방사성핵종들이 방사선작업종사자와 원전 주변주민의 피폭방사선량 평가에 미치는 영향)

  • In a primary system at nuclear power plants (NPPs), various radionuclides including fission products and corrosion products are generated due to the complex water conditions. Particularly, $^3H,\;^{14}C,\;^{58}Co,\;^{60}Co,\;^{137}Cs,\;and^{131}I$ are important radionuclides in respect of dose assessment for radiation workers and management of radioactive effluents. In this paper, the dominant contributors of radiation exposure for radiation workers and the member of public adjacent to NPPs were reviewed and the process of dose assessment attributable to those contributors were introduced. Furthermore, the analysis for some examples of radiation exposure to radiation workers and the public during the NPP operation was carried out. This analysis included the notable precedents of internal radiation exposure and contamination of demineralized water occurred in Korean NPPs. Particularly, the potential issue about the dose assessment of tritium and carbon-14 was also reviewed in this paper.

The Results of Primary Repair in Acute Injuries of the Posterior Cruciate Ligament (후방 십자 인대 급성 손상에서 일차 복원술의 결과)

  • Background: We analyze the functional evaluation, posterior stability after surgery of acute injuries in the femoral detachment and tibial avulsion fracture of posterior cruciate ligament. Materials and Methods: Twenty two patients who underwent primary repair were followed more than one year after operation (mean follow-up 33.7 months). The patients were evaluated with KT 2000 arthrometer and posterior stress test for posterior stability. Lysholm knee score was used for knee function. Results: Lysholm score revealed 85.3 in femoral detachment group and 91.1 in tibial avulsion fracture group (P<0.05). Posterior displacement of the posterior stress test showed 17.2 mm at initial injury and 8.6 mm at last follow up in femoral detachment group, 16.8 mm at initial injury and 7.1 mm at last follow up in tibial avulsion fracture group. There revealed the tendency of the improved posterior stability of the tibial avulsion group compared with femoral detachment group, but there showed no statistical significance in KT 2000 arthrometer and posterior stress x-ray. Conclusion: Functional results of tibial avulsion fracture group revealed the better outcome compared with femoral detachment group, but the degree of posterior stability in tibial avulsion fracture group showed no statistical significance in posterior stability.

Psychophysiological Characteristics of Insomnia Patients Measured by Biofeedback System (바이오피드백을 이용하여 측정한 불면증 환자의 정신생리적 특징)

  • Background and Objectives: Insomnia is the most prevalent sleep disorder in the general population and is considered to be a disorder of hyperarousal. The aim of this study was to measure the psychophysiological responses in insomnia patients using a biofeedback system, and to compare them with results from normal healthy subjects. Materials and Methods: Eighty patients with primary insomnia (35 males and 45 females, average age $49.71{\pm}12.91years$) and 101 normal healthy controls (64 males and 37 females, average age $27.65{\pm}2.77$) participated in this study. Electromyography (EMG), heart rate (HR), skin conductance (SC), skin temperature (ST), and respiratory rate (RR) were recorded using a biofeedback system during 5 phases (baseline, stress 1, recovery 1, stress 2, recovery 2) of a stress reactivity test, and average values were calculated. Difference in values between the two groups in each corresponding phase was analyzed with independent t-test, and change in values across phases of the stress reactivity test was analyzed with paired t-test (all two-tailed, p<0.05). Results: Compared to normal controls, insomnia patients had higher EMG in all 5 phases (baseline : $7.72{\pm}3.88{\mu}V$ vs. $4.89{\pm}1.73{\mu}V$, t = -6.06, p<0.001 ; stress 1 : $10.29{\pm}5.16{\mu}V$ vs. $6.63{\pm}2.48{\mu}V$, t = -5.84, p<0.001 ; recovery 1 : $7.87{\pm}3.86{\mu}V$ vs. $5.17{\pm}2.17{\mu}V$, t = -5.61, p<0.001 ; stress 2 : $10.22{\pm}6.07{\mu}V$ vs. $6.98{\pm}2.98{\mu}V$, t = -4.37, p<0.001 ; recovery 2 : $7.88{\pm}4.25{\mu}V$ vs. $5.17{\pm}1.99{\mu}V$, t = -5.27, p<0.001). Change in heart rate across phases of the stress reactivity test were higher in normal controls than in insomnia patients (stress 1-baseline : $6.48{\pm}0.59$ vs. $3.77{\pm}0.59$, t = 3.22, p = 0.002 ; recovery 1- stress 1 : $-5.36{\pm}0.0.59$ vs. $-3.16{\pm}0.47$, t = 2.91, p = 0.004 ; stress 2-recovery 1 : $8.45{\pm}0.61$ vs. $4.03{\pm}0.47$, t = 5.72, p<0.001 ; recovery 2-stress 2 : $-8.56{\pm}0.65$ vs. $4.02{\pm}0.51$, t = -5.31, p<0.001). Conclusion: Psychophysiological profiles of insomnia patients in a stress reactivity test were different from those of normal healthy controls. These results suggest that the sympathetic nervous system is more highly activated in insomnia patients.

  • Nonlinear characteristics of cellular counterflow diffusion flame near the radiative extinction limit at large Damk$\ddot{o}$hler number are numerically investigated. Lewis number is assumed to be 0.5 and flame evolution is calculated by imposing an infinitesimal disturbance to a one-dimensional(1-D) steady state flame. The early stage of nonlinear development is very similar to that predicted in a linear stability analysis. The disturbance with the wavenumber of the fastest growing mode emerges and grows gradually. Eventual, an alternating pattern of reacting and quenching stripes is developed. The cellular flame temperature is higher than that of 1-D flame because of the gain of the total enthalpy. As the Damk$\ddot{o}$hler number is further increased, the shape of the cell becomes circular to increase the surface area per unit reacting volume. The cellular flames do not extinguish but survive even above the 1-D steady state extinction condition.

  • PWSCC(Primary Stress Corrosion Cracking) in Alloy 82/182 butt welds is the problem affecting safety and integrity of nuclear power plant. PWSCC can be occurred in the area that is at high magnitude of tensile residual stress, such as Alloy 82/182 dissimilar metal welds in PZR(pressurizer) nozzles. There have been a number of incidents recently at the dissimilar metal welds in overseas nuclear power plants. Overlay weld is the one of the effective methods to decrease tensile residual stress of inside surface, which will result in preventing PWSCC. In this paper, overlay weld conditions on the purpose of preventing PWSCC was explained and the benefit of the overlay weld was confirmed performing finite element analysis.

The Recovery of Carbon Source from Municipal Primary Sludge using Pilot Scale Elutriated Acidogenic Fermentation (Pilot scale 세정산발효를 이용한 하수 일차슬러지의 산발효)

  • Pilot scale study was carried out to produce Volatile Fatty Acids with primary sludge from municipal wastewater treatment plant. An acid fermenter was operated at pH 9, $35^{\circ}C$, SRT of 3.5-4.25d, using a final effluent as elutriating water(Mode-I) and pH 9, SRT 5d, temperature of $35^{\circ}C$(Mode-II), $55^{\circ}C$(Mode-III), using a primarily treated water as elutriating water. Although solubilization rate was enhanced with the increase of temperature, the VFAs production rate was decreased. The VS reduction was shown approximately 56%, and the sludge volume reduction was 93%. The optimal conditions for solubilization was obtained at pH 9, $35^{\circ}C$ and SRT of 5d.

  • A computer program is developed in order to investigate the fluid flow and heat ransfer of a water body in a 2-D vertical rectangular coordinate. The specific purpose of this study is to obtain a physical insight of several fluid flow problems which occur in a lake and the water and wastewater treatment facility like a primary clarifier. The performance of computer program developed is successfully evaluated by the comparison of other two experimental and computational data in open literature : the first comparison is made against the numerical data associated with the cooling water discharge and the other is numerical and experimental works for the primary clarifier of Sarina City at Ontario. Further, the computer program is applied to investigate the feature of lake flow, say lake turnover, and 2-D vertical channel flow in terms of temperature, wind velocity and flow rate, etc. The computational results appear to be physically acceptable and consistent. The computer program developed in this study shows the possibility of the viable tool to figure out the flow characteristics of water reservoir.
