인공매식치의 평가를 위한 치주지수간의 상관관계 (Interrelationship between periodontal parameters for the evaluation of clinically stable dental implants)

  • 자연치의 경우에는 치주지수간의 상관관계가 정립되어 병적인 상태에 대한 진단과 그에 따른 치료방법을 결정하는데 많은 정보를 제공하나, 인공매식치에서는 아직 논란의 여지가 많다. 이에 임상적으로 안정화되어 기능하고 있는 인공매식치에서 치주지수간의 상관관계에 관하여 연구하고자 하였다. 총 43명의 환자(평균 44.7세)에 식립된 178개의 인공매식치를 연구에 이용하였다. 인공치아매식술을 시행한 후 1년에서 6년이 경과한 인공매식치에서 상부보철물을 제거한 후 치태지수, 치은열구출혈지수, 각화점막지수, 치주낭깊이, Periotest Values(PTVs) 등을 측정하여 각각의 분포상황과 상관관계를 분석하였다. 이 연구의 결과는 다음과 같다. 1. 치태지수의 평균값은 $0.83{\pm}0.82$이었다. 2. 치은열구출혈지수의 평균값은 $1.04{\pm}0.86$이었다. 3. 각화점막지수의 평균값은 $2.47{\pm}0.95$이었다. 4. 치주낭깊이의 평균값은 $3.12{\pm}1.14\;mm$이었다. 5. Periotest Values(PTVs)의 평균값은 $-0.66{\pm}4.28$이었다. 6. 치태지수, 치주낭깊이가 증가함에 따라 치은열구출혈지수는 유의성있게 증가하였다(P<0.01). 7. 각화점막지수가 증가함에 따라 치은열구출혈지수는 유의성있게 감소하였다(P<0.01). 8. 치은열구출혈지수와 매식치동요도 사이에서는 유의성있는 상관관계를 발견할 수 없었다(P>0.05). 9. 치주낭깊이와 매식치동요도 사이에서도 유의성있는 상관관계를 발견할 수 없었다(P>0.05). 10. 인공매식치의 직경과 길이가 증가함에 따라 매식치동요도는 유의성있게 감소하였다(P<0.01). 11. 하악에서의 매식치동요도가 상악의 경우와 비교하여 유의성있게 작았다(P<0.01). 결론적으로, 인공매식치에서도 자연치에서와 동일한 양상으로 치태가 많을수록, 그리고 치주낭 깊이가 깊을수록 염증의 심도와 관련이 깊은 것으로 생각된다. 특히, 인공매식치의 경우에는 같은 양의 치태가 존재시에 각화치은이 충분히 있는 쪽이 염증발생이 적은 것으로 나타났다. 치주조직에 염증이 존재하는 경우, 자연치아에서는 치아의 동요도가 증가하는 것으로 알려져있으나, 이 실험의 인공매식치에서는 유의성있는 상관관계를 발견할 수 없었다. 임상적으로 안정화된 인공매식치의 동요도는 염증정도에는 큰 영향을 받지 않고 인공매식치의 직경과 길이가 증가함에 따라 감소함을 보여주고 있다. 또한, 인공매식치의 동요도는 상하악골의 골질에 따라 차이가 있음을 명확히 보여주고 있다.

5년간 시행된 인공치아매식술에 관한 일차보고 (A PRELIMINARY STUDY OF ENDOSSEOUS IMPLANT FOR 5 YEARS)

  • We experienced 152 endosseous implant surgery and prosthodontic restoration with edentulous jaw for 5 years from Mar. 1990. to Oct. 1995. 422 fixtures were implanted. the ratio of male to female was 1.38 : 1 and the range of age from 16 to 74 years. Most dominant group was 41 to 50 years group(36%). In our study, implant success rates showed 96% in mandible and 92% in maxilla. The major causes of implant failure of primary osseointegration(75%), overloading(15%), neurologic problem(5%), psychologic problem(5%).

하악 무치악 인공치아매식술시 하치조신경 전위술 (INFERIOR ALVEOLAR NERVE REPOSITIONING IN THE ATROPHIC POSTERIOR MANDIBULAR ALVEOLAR RIDGE)

  • Modified surgical technique for transposition of the inferior alveolar nerve followed by immediate placement of endosseous implants in mandibles with moderate to severe atrophy are presented. Five transpositions of the inferior alveolar nerve together with the installation of 10 implants were performed in four patients. The mean postoperative follow-up time was 17 months, with a range of 8 to 20 months. All implants with functioning pontics remained stable, with no mobility or symptoms of pain and infection during the follow-up period. Neurosensory evaluation was performed using the two-point discrimination test. Two patients had objective neurosensory dysfunction at postoperative, but all the nerve function were reported as normal by the patients 4 months postoperatively.

인공치아 매식술 계획에 있어서 상이한 단층두께의 나선형 단층사진의 비교 (A Comparative Study of Spiral Tomograms with Different Slice Thicknesses in Dental Implant Planning)

  • Purpose: To know whether there would be a difference among spiral tomograms of different slice thicknesses in the measurement of distances which are used for dental implant planning. Materials and Methods: 10 dry mandibules and 40 metal balls were used to take total 120 Scanorailll tomograms with the slice thickness of 2 mm, 4 mm and 8 mm. 3 oral radiologists interpreted each tomogram to measure the distances from the mandibular canal to the alveoalr crest and buccal. lingual and inferior borders of mandible. 3 observers recorded grades of 0, 1 or 2 to evaluate the perceptibility of alveolar crest and the superior border of mandibular canal. ANOVA with repeated measure. Chi-square tests and intrac!ass correlation coefficient(R₂, α) were used For statistical analysis. Results: There was not a statistically significant difference among spiral tomograms with different slice thicknesses in the measurement of the distances and in the perceptibility of alveolar crest and mandibular canal(p>0.05). All of them showed a good relationship in the reliability analysis. The perceptibility of alveolar crest and mandibular canal was almost similar and an excellent relationship was seen on all of them. Conclusions: There would be no significant difference. no matter which spiral tomogram of any slice thickness may be used in dental implant planning. considering the thickness of dental implant fixture.

인공치아 매식술을 위한 하악관, 이관, 이공의 골내 주행에 관한 방사선학적 연구 (Radiologic study of intraosseous path of the inferior alveolar cantal, mental canal, and mental foramen for endosseous implants)

  • The purpose of this study was to examine the anatomic structures of the mandible-inferior alveolar canal, mental foramen, mental canal-with panoramic radiography and conventional tomography and to compare both radiographic techniques in conjunction with endosseous implants. In this study 14 adult dentulous mandibles -27 cases of right and left side of mandibles- were examined and the results were as follows. 1. The distance between superior border of the inferior alveolar canal and the alveolar ridge crest showed a decreasing tendency from the mental foramen to 4cm posterior to the mental foramen. 2. The mean diameter of the inferior alveolar canal was $4.11{\pm}0.50mm$ with panoramic radiography and $3.29{\pm}0.59mm$ with conventional tomography. 3. The inferior border of the inferior alveolar canal and inferior border of the mandible was closest at 2cm posterior to the mental foramen but it was not statistically significant. the mean distance was $1l.64{\pm}2.95mm$ in panoramic radiography and $1l.68{\pm} 2.91mm$ in conventional tomography. 4. The inferior alveolar canal located lingually in bucco-lingual direction 16%(mental foramen), 54%(lcm posterior to the mental foramen), 68%(2cm posterior to the mental foramen), 50%(3cm posterior to mental foramen), 55%(4cm posterior to the mental foramen). 5. Mean length of the anterior loop of the mental canal was 2.73mm, and the loop below 2mm was 35% and 15% of mental canal was invisible in panoramic radiography. 6. The minimum interforaminal distance was 56.7mm, the maximum distance was 73.2mm and the mean distance was 66.42mm in panoramic radiography. 7. The mean distance between midpoint of the mental canal and alveolar ridge crest was 16.24mm and the mean buccolingual angulation of the mental canal was $52.98^{\circ}$ in conventional tomography. 8. In comparison of panoramic radiography and conventional tomography, inferior alveolar canal is better visualized with conventional tomography than panoramic radiography from the mental foramen to the 2cm posterior to the mental foramen, while visiblity of conventional tomography prominently decreased in 4cm posterior to the mental foramen and alveolar ridge crest is better visualized with panoramic radiography than conventional radiography at the mental foramen and at 4cm posterior to the mental foramen. In radiologic examination of anatomic structures of the mandible for endosseous implants, panoramic radiography and conventional tomography can be effectively used when it is used to overcome the anatomic limitations.

치주 수술 및 인공 치아 매식술 후 $Myprodol^{(R)}$의 동통 억제 효과에 대한 연구 (Pain Control Effects of $Myprodol^{(R)}$ after Periodontal Surgery and Dental Implant Surgery)

  • Although various analgesics have been administrated for postoperative pain control, postoperative pain has not been adequately controlled . The purpose of this study was to evaluate the effects and patient's satisfaction of $Myprodol^{(R)}$(combination analgesics with codeine, ibuprofen, paracetamol) compared to Acetamionphen and placebo drug after periodontal surgery and dental implant surgery. We studied 98 cases of outpatients which were composed of 67 cases of flap operation(which separated to 3 groups: Placebo group(n=25), $Myprodol^{(R)}$ group(n=22), Acetaminophen group(n=20)) and 21 cases of dental implant surgery(which separated to 3 groups : Placebo group(n=10), $Myprodol^{(R)}$ group(n=12), Acetaminophen group(n=9)). We evaluated the postoperative pain(Pain 1), Pain after first drug administraion(Pain 2), the degrees of pain reduction(pain 3), patient's satisfaction for drug, and side-effects. We obtained following results; 1. In Pain 1, making a comparison among groups, there was no significant difference in both cases of flap operation-group and dental implant surgery-group 2. In Pain 2, establishing a comparison among groups, there was no significant difference in flap operation-group, but significant difference was seen between placebo group and $Myprodol^{(R)}$ group in cases of dental implant surgery group(P<0.05). 3. In Pain 3, making a comparison among groups, $Myprodol^{(R)}$ group showed significant differences compared to placebo group and Acetaminophen group in both cases of flap operation group and dental implant surgery group(P<0.05). 4. In patient's satisfactory score, making a comparison among groups, there were significant differences between placebo group and $Myprodol^{(R)}$ group in cases of flap operation group and between $Myprodol^{(R)}$ group and Acetaminophen group in cases of dental implant surgery group(P<0.05). 5. Making a comparison in side-dffect, no significant differrence was seen. Our conclusion is that $Myprodol^{(R)}$ is a effective oral analgesics to the patients who underwent periodontal surgery or implant surgery for it's synergism among three dugs.
