Intelligent Hospital Information System Model for Medical AI Research/Development and Practical Use (의료인공지능 연구/개발 및 실용화를 위한 지능형 병원정보시스템 모델)

  • Medical information is variously generated not only from medical devices but also from electronic devices. Recently, related convergence technologies from big data collection in healthcare to medical AI products for patient's condition analysis are rapidly increasing. However, there are difficulties in applying them because of independent developmental procedures. In this paper, we propose an intelligent hospital information system (iHIS) model to simplify and integrate research, development and application of medical AI technology. The proposed model includes (1) real-time patient data management, (2) specialized data management for medical AI development, and (3) real-time monitoring for patient. Using this, real-time biometric data collection and medical AI specialized data generation from patient monitoring devices, as well as specific AI applications of camera-based patient gait analysis and brain MRA-based cerebrovascular disease analysis will be introduced. Based on the proposed model, it is expected that it will be used to improve the HIS by increasing security of data management and improving practical use through consistent interface platformization.

  • Artificial intelligence (AI) technology is actively being applied for the interpretation of medical imaging, such as chest X-rays. AI-based software medical devices, which automatically detect various types of abnormal findings in chest X-ray images to assist physicians in their interpretation, are actively being commercialized and clinically implemented in Korea. Several important issues need to be considered for AI-based detection assistant tools to be applied in clinical practice: the evaluation of performance and efficacy prior to implementation; the determination of the target application, range, and method of delivering results; and monitoring after implementation and legal liability issues. Appropriate decision making regarding these devices based on the situation in each institution is necessary. Radiologists must be engaged as medical assessment experts using the software for these devices as well as in medical image interpretation to ensure the safe and efficient implementation and operation of AI-based detection assistant tools.

  • Recently, unexpected and more advanced cyber medical treat attacks are on the rise. However, in responding to various patterns of cyber medical threat attack, rule-based security methodologies such as physical blocking and replacement of medical devices have the limitations such as lack of the man-power and high cost. As a way to solve the problems, the medical community is also paying attention to artificial intelligence technology that enables security threat detection and prediction by self-learning the past abnormal behaviors. In this study, there has collecting and learning the medical information data from integrated Medical-Information-Systems of the medical center and introduce the research methodology which is to develop the AI-based Net-Working Behavior Adaptive Information data. By doing this study, we will introduce all technological matters of rule-based security programs and discuss strategies to activate artificial intelligence technology in the medical information business with the various restrictions.

  • Based on the accumulation of medical big data, advances in medical artificial intelligence technology facilitate the timely treatment of disease through the reading the medical images and the increase of prediction speed and accuracy of diagnoses. In addition, these advances are expected to spark significant innovations in reducing medical costs and improving care quality. There are already approximately 40 FDA approved products in the US, and more than 10 products with K-FDA approval in Korea. Medical applications and services based on artificial intelligence are expected to spread rapidly in the future. Furthermore, the evolution of medical artificial intelligence technology is expanding the boundaries or limits of various related issues such as reference standards and specifications, ethical and clinical validation issues, and the harmonization of international regulatory systems.

  • "Artificial Intelligence+" is a key strategic direction that has garnered the attention of several global medical device manufacturers and internet companies. Large hospitals are actively involved in different types of medical AI research and cooperation projects. Medical AI is expected to create numerous opportunities and advancements in areas such as medical imaging, computer aided diagnostics and clinical decision support, new drug development, personal healthcare, pathology analysis, and genetic disease prediction. On the contrary, some studies on the limitations and problems in current conditions such as lack of clinical validation, difficulty in performance comparison, lack of interoperability, adversarial attacks, and computational manipulations are being published. Overall, the medical AI field is in a paradigm shift. Regarding international standardization, the work on the top 10 standardization issues is witnessing rapid progress and the competition for standard development has become fierce.

  • The body shape measurement method using 3D images has been widely used due to the recent development of 3D measurement cameras and algorithms. Existing 3D imaging devices are expensive devices, and there is a limit to their universalization. Due to the recent spread of inexpensive 3D cameras and the development of various measurement methods, various possibilities are being shown. It is expected to have a great impact on the medical device market that requires accurate data collection. Various medical device products using artificial intelligence are emerging, and accurate data collection is the most important to develop accurate artificial intelligence algorithms. Collection equipment using 3D cameras is expected to act as a major factor in the development of artificial intelligence algorithms using 3D images.

  • Artificial intelligence-based health data verification has become an essential element not only to help clinical research, but also to develop new treatments. Since the US Food and Drug Administration (FDA) approved the marketing of medical devices that detect mild abnormal diabetic retinopathy in adult diabetic patients using artificial intelligence in the field of medical diagnosis, tests using artificial intelligence have been increasing. In this study, an artificial intelligence model based on image classification was created using a Teachable Machine supported by Google, and a predictive model was completed through learning. This not only facilitates the early detection of cataracts among eye diseases occurring among patients with chronic diseases, but also serves as basic research for developing a digital personal health healthcare app for eye disease prevention as a healthcare program for eye health.

A Study on the Decision Factors for AI-based SaMD Adoption Using Delphi Surveys and AHP Analysis (델파이 조사와 AHP 분석을 활용한 인공지능 기반 SaMD 도입 의사결정 요인에 관한 연구)

  • With the diffusion of digital innovation, the adoption of innovative medical technologies based on artificial intelligence is increasing in the medical field. This is driving the launch and adoption of AI-based SaMD(Software as a Medical Device), but there is a lack of research on the factors that influence the adoption of SaMD by medical institutions. The purpose of this study is to identify key factors that influence medical institutions' decisions to adopt AI-based SaMDs, and to analyze the weights and priorities of these factors. For this purpose, we conducted Delphi surveys based on the results of literature studies on technology acceptance models in healthcare industry, medical AI and SaMD, and developed a research model by combining HOTE(Human, Organization, Technology and Environment) framework and HABIO(Holistic Approach {Business, Information, Organizational}) framework. Based on the research model with 5 main criteria and 22 sub-criteria, we conducted an AHP(Analytical Hierarchy Process) analysis among the experts from domestic medical institutions and SaMD providers to empirically analyze SaMD adoption factors. The results of this study showed that the priority of the main criteria for determining the adoption of AI-based SaMD was in the order of technical factors, economic factors, human factors, organizational factors, and environmental factors. The priority of sub-criteria was in the order of reliability, cost reduction, medical staff's acceptance, safety, top management's support, security, and licensing & regulatory levels. Specifically, technical factors such as reliability, safety, and security were found to be the most important factors for SaMD adoption. In addition, the comparisons and analyses of the weights and priorities of each group showed that the weights and priorities of SaMD adoption factors varied by type of institution, type of medical institution, and type of job in the medical institution.

Analyze Technologies and Trends in Commercialized Radiology Artificial Intelligence Medical Device (상용화된 영상의학 인공지능 의료기기의 기술 및 동향 분석)

  • This study aims to analyze the development and current trends of AI-based medical imaging devices commercialized in South Korea. As of September 30, 2023, there were a total of 186 AI-based medical devices licensed, certified, and reported to the Korean Ministry of Food and Drug Safety, of which 138 were related to imaging. The study comprehensively examined the yearly approval trends, equipment types, application areas, and key functions from 2018 to 2023. The study found that the number of AI medical devices started from four products in 2018 and grew steadily until 2023, with a sharp increase after 2020. This can be attributed to the interaction between the advancement of AI technology and the increasing demand in the medical field. By equipment, AI medical devices were developed in the order of CT, X-ray, and MR, which reflects the characteristics and clinical importance of the images of each equipment. This study found that the development of AI medical devices for specific areas such as the thorax, cranial nerves, and musculoskeletal system is active, and the main functions are medical image analysis, detection and diagnosis assistance, and image transmission. These results suggest that AI's pattern recognition and data analysis capabilities are playing an important role in the medical imaging field. In addition, this study examined the number of Korean products that have received international certifications, particularly the US FDA and European CE. The results show that many products have been certified by both organizations, indicating that Korean AI medical devices are in line with international standards and are competitive in the global market. By analyzing the impact of AI technology on medical imaging and its potential for development, this study provides important implications for future research and development directions. However, challenges such as regulatory aspects, data quality and accessibility, and clinical validity are also pointed out, requiring continued research and improvement on these issues.

  • Artificial intelligence is being applied in various industrial fields to the development of the fourth industry and the construction of high-performance computing environments. In the medical field, deep learning learning such as cancer, COVID-19, and bone age measurement was performed using medical images such as X-Ray, MRI, and PET and clinical data. In addition, ICT medical fusion technology is being researched by applying smart medical devices, IoT devices and deep learning algorithms. Among these techniques, medical image-based deep learning learning requires accurate finding of medical image biomarkers, minimal loss rate and high accuracy. Therefore, in this paper, we would like to compare and analyze the performance of the Cross-Entropy function used in the image classification algorithm of the loss function that derives the loss rate in the chest X-Ray image-based deep learning learning process.
