In this paper, we studied a computer based three dimensional implantation system of artificial hip joint. The system can be utilized for doctors to select a suitable artificial femur which is best-fit for the patient and to find out the optimal implanting position as well. We proposed a new numerical index to measure the fitness between the artificial hip joint and the patient's femur. The proposed fitness index accounts for the variance of the distance between the outer contours of artificial hip joint and the femur in addition to the conventional area comparison. A few simulation are run to show results of fitness measurement and compared to the conventional method.

The choice of suitable hip implant is one of important factors in total hip replacement (THR). In clinical view points, improper adaptation of hip implant might cause abnormal stress distribution to the bone, which can shorten the lifespan of replaced hip implant. Currently, interest in custom-designed hip implants has increased as studies reveals the importance of geometric shape of patient's femur in modeling and designing custom hip implants. In this study, we have developed the custom-designed hip implant models with various sizes in hip implant, and the stress distribution in the bone was analyzed using Finite Elements methods. It was found that minimizing the gap between implant stem and femoral cavity is crucial to minimize stress concentration in the bone.
Clinically, proximal bone resorption in the femur is frequently seen postoperatively on the follow up XI-rays after total hip replacement (THR). We developed the finite element model of cementless THR. The model is two dimensional side plate model, whereby the three dimensional structural integrity of the bone can be accounted for by a separate two dimensional mesh, a side plate. The subject of this article is the development and application of this two dimensional side plate FEM to study the reverse effect of the various degree of bone resorption of femur after THR. The results of this study indicates that 1) two dimensional side plate model is good and simple alternative to complex three dimensional model and 2) the severity of the proximal bone resorption has the effect of more increasing stress on the cortex at the level of femoral stem tip.

Hip joint simulator which Is an essential device for evaluating the wear of biomaterials (ultrahigh molecular weight polyethylene, Co-Cr alloy, alumina, etc.) used in total hip joint replacement was developed. This hip joint simulator mimics the joint motion and joint loading of human gait by adapting the 4 degree of freedom in kinematic motion (flexing/extension, adduction/abduction, Internal rotation/external rotation) and axial loading, Four stations are operated by 8 servo-motors and harmony drives. Joint leading was imposed by displacement control from a ball screw, LM guide, and spring system. Each kinematic link system operates separately or coupled modes. A heater and a thermocouple were installed for keeping the body temperature in each station.

Study Design: A retrospective clinical and radiographic review. Objectives: The purpose of this study was to suggest recycling bone autotransplantation with extracorporeal heat-treatment as one of favorable reconstruction method for malignant bone tumors of pelvis through 3 cases. Summary of Literature Review: There are many biologic and nonbiologic reconstruction method in pelvic reconstruction. Cases: Case 1- A 20-year-old women had chief complaint of right hip and thigh pain started 3 months ago and done curettage and bone cementing at right ilium at other hospital. She had impression of malignant bone tumor and undergone postoperative radiation therapy for 6 cycles. After that she was referred to our hospital and undergone wide excision of right ilium and recycling bone autotransplantation with extracorporeal heat-treatment at 132 degree celsius for 2 minutes internal fixation with pelvic reconstruction plate and total hip replacement arthroplasty (ABG$^{(R)}$). There was no evidence of distant metastasis and revealed well-differentiated osteosarcoma on postoperative pathology. Neither adjuvant nor neoadjuvant chemotherapy were done. Case 2- A 56-year-old women who suffered right thigh pain for 3 months was detected radiologic abnormality at right pelvis. After incisional biopsy, osteosarcoma was diagnosed. We had undergone wide excision of right ilium and recycling bone autotransplantation with extracorporeal low heat-treatment at 65 degree celsius for 30minutes internal fixation with pelvic reconstruction plate and total hip replacement arthroplasty (ABG$^{(R)}$). There was no evidence of distant metastasis and revealed high-grade osteosacoma which was fibroblastic type on postoperative pathology. Adjuvant chemotherapy (HDMTX, ADR, CDDP) was done immediate after wound healing was completed. Case 3- A 46-year-old women was incidently found mass at left ilium which was suspected chondrosarcoma. We had undergone wide excision of left ilium and recycling bone autotransplantation with extracorporeal low heat-treatment at 65 degree celsius for 30minutes internal fixation with pelvic reconstruction plate and total hip replacement arthroplasty (Protek$^{(R)}$). There was no evidence of distant metastasis and revealed chondrosarcoma which was graded II/III on postoperative pathology. Results: Oncologic and functional outcome at final follow-up were for case 1, final follow-up time was 7 years, is no evidence of disease and functional score is 53% according to Ennecking et al. During follow-up, evidence of radiologic union was at about 1 and 6 months after operation. The case had breakage of pelvic reconstruction plate and some resorption of autotransplated bone, but no symptom present. For case 2, final follow-up was 3 years and 6 months, is no evidence of disease and functional score is 60%. For case 3, final follow-up was 7 months, no evidence of disease and functional score is 63% which is improving state. Discussion: 3 cases which were undergone recycling bone autotransplantation with extracorporeal heat-treatment and total hip replacement arthroplasty had relatively successful oncologic and functional outcome. Taking account that difficulty in using allograft in Korea this method is thoght to be one of the useful way to reconstruct pelvis after resection of primary malignant bone tumor of the pelvis.

Simple radiography, CT and MRI were used for the diagnosis of the hip disease. However ultrasonography could be a good diagnostic tool for the lesion of tendons and bursae. Another advantage of ultrasonography is its usefulness for the follow-up of these lesions. Nowadays hip ultrasonography expanded the horizon to the follow-up study of total hip arthroplasty. However, the limitation of ultrasonography is that there is disparity in results according to the physician's experiences. In this article ultrasonographic findings of normal structures and pathologic lesions of the hip joint are introduced and explained briefly.

모든 생물은 늙어가면서 그 생물체를 이루고 있는 생체조직들이 낡게 되고 약해지기 마련이며, 이외에도 자동차 사고 등 재해에 의해 생체 장기의 손상을 가져올 수도 있다. 또한 인간의 평균수명 연장과 함께 소득 수준이 높아지고 또한 'quality of life'를 추구하는 고령화 시대로 접어듦에 따라, 인공골, 인공치아 또는 인공 고/슬관절 등의 골조직 대체재료의 수요가 빠르게 증가하고 있다. 이와 같은 골조직 대체, 즉 골이식은 크게 자기골 이식(autografting), 동종이식(allografing), 인공재료(man-made materials)의 이식으로 구분된다. 현재 대체물질의 약 58%를 차지하고 있는 자기골 이식의 경우, 거부면역 반응이 없어 임상성공율이 80%에 달한다는 장점을 가지고 있으나, 비용이 비싸고 감염과 통증의 위험이 있다. 또한 시신으로부터 골을 이식하는 동종이식의 경우 대체물질의 약 34%를 차지하고 있는데, 성공률이 떨어지고 질병 감염의 위험이 있는 단점이 있다. 이외에 약 8%를 차지하고 있는 인공재료 이식의 경우, 파단, 독성반응, 마모, 골조직의 remodeling 등이 일어나는 단점이 있으나, 앞에서 언급한 바와 같이 그 필요성이 급격히 증가함에 따라 보다 나은 치료법과 골이식 대체물질의 개발에 많은 노력이 경주되고 있다. 2003년 6월 미국 Financial Times에 의하면, 인체내 식립형 생체재료의 세계시장 규모는 약 650억 달러에 이르며, 매년 200% 이상씩 신장하고 있다고 한다. 따라서 세계 각국의 의학, 약학, 임상학, 생명과학, 공학 등의 관련 연구 분야에서는 이 수요를 충족시키기 위한 활발한 연구활동을 펼치고 있다. 한편 인체내 식립용 임플란트의 국내 시장규모는 치과 임플란트의 경우 2006년 현재 2000억원 규모로, 정형외과, 악안면 성형외과, 이비인후과를 포함하면 소위 'bone-anchored metal implant' 영역의 시장 규모는 4~5조원에 이를 것으로 추산 되고 있다. 또한 소비 신장률 10~15%를 감안하면 향후 시장 규모는 폭발적으로 증가될 것으로 예상된다. 이에 발맞추어, 최근 들어 선진국은 물론, 국내에서도 인체내 식립을 목적으로 하는 생체재료에 관한 연구개발이 활발히 진행되고 있으며, 일부는 실용화 단계에 진입하고 있다. 본 강연에서는 금속 임플란트의 시장현황과 앞으로의 추세에 대하여 조망하고, Ti 임플란트를 중심으로 이의 생체활성을 부여하는 표면개질 필요성 및 최근의 연구개발 동행에 대해 소개하고자 한다.
A ceramic articulating system in total hip replacement thought to be superior to metal-on-polyethylene due to its extremely low coefficient of friction and potential for high resistance to wear. But ceramic is brittle, which makes it mechanically and theoretically susceptible to fracture under certain mechanical conditions. In the current study, nine different models of ceramic ball heads were mechanically evaluated using 3D finite element(FE) analyses. It was found that the maximum stress in all ceramic models was lower than ceramic flexural strength, and it satisfied the requirements of the FDA Gaudience for artificial hip implant. Thus, ceramic ball head models introduced in the current study could be mechanically safe for clinical applications.

본 연구에서는 전방항성 초음파 액추에이터의 제작, 시뮬레이션, 구동 드라이버 제작, 제어 방법에 대하여 연구하였다. 우선, 시뮬레이션 툴인 ATILA를 사용하여 전방향성 초음파 액추에이터를 설계하였고 구동 주파수, 어드미턴스 특성 그리고 액추에이터의 움직임을 분석하여 최적의 구동 주파수 영역을 파악하였다. 실제 구동을 확인하기 위하여 액추에이터 스테이지와 구동 드라이버를 개발 하였다. 전방항성 액추에이터 스테이지의 구조는 액추에이터와 4개의 볼 베어링이 알루미나 볼을 압착하는 형태이며 세라믹에 인가되는 사인파의 위상차에 의하여 4개의 방향으로 구동 할 수 있다. 또한, 액추에이터를 위치 제어를 위하여 로터리 엔코더 두 개를 사용하여 구의 움직임을 파악하였고 PID 제어기법을 사용하여 액추에이터의 움직임을 제어하였다. 스테이지 구조상 엔코더의 $70^{\circ}$ 내에서만 구동 가능하며, 실험 결과를 통하여 전방향성 초음파 액추에이터는 약 $1.2^{\circ}$의 분해능을 가지고 구동영역 내 원하는 좌표로의 이동이 가능했고, 향후 고 정밀 관절 및 인공 눈에 응용 가능할 것으로 사료된다.

A special mesenchymal tissue layer called perichondrium has a chondrogenic capacity and is a candidate tissue for engineering of cartilage. To overcome limited potential for chondrocyte proliferation and re-absorption, we studied a method of cartilage tissue engineering comprising chondrocyte-hydrogel pluronic complex (CPC) and cultured perichondrial cell sheet (cPCs) which entirely cover CPC. For effective cartilage regeneration, cell-sheet engineering technique of high-density culture was used for fabrication of cPCs. Hydrogel pluronic as a biomimetic cell carrier used for stable and maintains the chondrocytes. The human cPCs was cultured as a single layer and entirely covered CPC. The tissue engineered constructs were implanted into the dorsal subcutaneous tissue pocket on nude mice (n = 6). CPC without cPCs were used as a controls (N = 6). Engineered cartilage specimens were harvested at 12 weeks after implantation and evaluated with gross morphology and histological examination. Biological analysis was also performed for glycosaminoglycan (GAG) and type II collagen. Indeed, we performed additional in vivo studies of cartilage regeneration using canine large fullthickness chondrial defect model. The dogs were allocated to the experimental groups as treated chondrocyte sheets with perichondrial cell sheet group (n = 4), and chondrocyte sheets only group (n = 4). The histological and biochemical studies performed 12 weeks later as same manners as nude mouse but additional immunofluorescence study. Grossly, the size of cartilage specimen of cPCs covered group was larger than that of the control. On histological examination, the specimen of cPCs covered group showed typical characteristics of cartilage tissue. The contents of GAG and type II collagen were higher in cPCs covered group than that of the control. These studies demonstrated the potential of such CPC/cPCs constructs to support chondrogenesis in vivo. In conclusion, the method of cartilage tissue engineering using cPCs supposed to be an effective method with higher cartilage tissue gain. We suggest a new method of cartilage tissue engineering using cultured perichondrial cell sheet as a promising strategy for cartilage tissue reconstruction.
