Relationship between High Risk Human Papillomavirus and Young Patients and Nonsmokers in Squamous Cell Carcinoma of the Tonsil in Korea (한국인 편도 편평상피암종에서 고위험 인간 유두종 바이러스와 젊은 연령 및 비흡연자와의 관계)

  • 연구 및 목적 : 이번 연구의 목적은 한국인에서 편도 편평세포암종의 발생에 고위험 인간유두종 바이러스(high risk human pap-illomavirus)의 역할 및 관여 인자에 대해 밝히고자 한다. 재료 및 방법 : 연구는 편도 편평상피세포암종으로 진단받은 54명을 대상으로 하였다. 고위험 인간유두종 바이러스의 감염을 알기위해 in situ hybridization 방법을 이용하였다. 고위험 인간유두종 바이러스의 감염과 나이, 성별, 흡연, 음주, 병기, 병리학적 특징 등과의 관계를 분석하였다. 결 과 : 고위험 인간유두종 바이러스의 양성율은 31.5%(17/54)였다. 고위험 인간유두종 바이러스 감염은 젊은 연령(50세 미만), 비흡연자, 림프절 전이와 유의한 상관관계가 있었다(각각 p=0.008, p=0.042, p=0.027). 하지만 성별, 음주, 원발부위 병기, 종양분화도, 피막외침범, 혈관 및 신경 침범과 고위험 인간유두종 바이러스와의 관계는 통계적으로 유의하지 않았다. 6개월 이상 추적 관찰이 가능한 환자를 대상으로 조사한 5년 전체 생존율 및 질병 특이 생존율은 각각 60%와 62%였다. 고위험 인간유두종 바이러스의 양성율은 질병 특이 생존율과 유의한 상관관계가 있었다(p=0.019). 결 론 : 한국인의 고위험 인간유두종 바이러스 감염은 젊은 연령 및 비흡연자의 편도 편평세포암종의 발생에 관련이 있고 예후 인자로 중요한 역할을 한다.

A Clinicopathological Review of Verruca Vulgaris Occurring in the Oral Cavity (구강에 발생한 심상성 사마귀의 임상병리학적 특징)

  • Verruca vulgaris is one of the most frequent lesions of the skin. Evidences support that this lesion has strong association with low-risk human papillomavirus. Unlike skin lesion, oral verrucae vulgaris are not easily encountered and often misdiagnosed as squamous papilloma. Herein, we reported eight cases of oral verruca vulgaris and analyzed their clinicopathological features.

  • Verruca vulgaris of the larynx (VVL) is an extremely rare disease that can be easily misdiagnosed as papillary keratosis, squamous papilloma or verrucous carcinoma, which would have led to needless operation. We experienced a case of VVL in a 66-year-old man with 1-year history of progressive hoarseness. Whitish lesions in both vocal cords were observed by using indirect laryngoscopy and completely removed via microlaryngeal surgery with $CO_2$ laser.

  • Several investigators have shown that human papillomavirus(HPV) appear to play an etiologic role in oral and paranasal sinus carcinoma. It was known that 15-25% of head and neck squamous cell carcinoma(SCC) showed HPV-positive infection. Among them, HPV 16 was the most common type but HPV 18 was observed only 2-4% of HPV-positive head and neck cancers. In recent, we treated uncommon 2 oral SCC cases that associated with HPV infection. One is a case of tongue SCC after bone marrow transplantation(BMT), and the other is a case of SCC occurring with aspergillosis in the maxillary sinus. After surgery, HPV 16 and 18 were detected in the surgical specimens by the histological and polymerase chain reaction(PCR) examination. In this report, we present these cases with a review of literature.

Investigation of Detected by Recent Various Human Papillomavirus from General Hospital in Seoul Area (최근 서울지역 종합병원에서 다양한 인유두종바이러스의 검출에 대한 실태조사)

  • Human papillomavirus (HPV) infection is a necessary precondition of cervical cancer. A change from cytology to molecular HPV testing is, however, challenging. A new HPV DNA chip test for the infection of 22 HPV genotypes were developed in Korea. The purpose of this study was to investigate the prevalence and genotype distribution of HPV infection in the Seoul area. Over the last year, a total of 5,614 samples were tested. Using a chip test, HPV genotypes were detected in 1,596 (28.4%); of which, 679 (42.5%) were considered as high risk and low risk HPV were 152 (9.5%). 831 were single positive samples (n=1596). The most frequently found genotypes in all HPV-single positive samples (n=831) were HPV-16 (16.5%), 58 (15.2%), 52 (8.8%), 51 (7.1%) and 56 (5.9%). Mixed genotypes (n=219) were detected in 2 (n=176, 11.0%), 3 (n=37, 5.9%), and 4 (n=2, 0.1%) positive samples (n=1596). This study demonstrated that epidemical investigated HPV infection in patients of general hospital. These findings could be used to indicate a nationwide distribution of HPV and the adoption of vaccines. It is hoped that additional epidemiological research regarding the outcomes that are important to decision makers will be conducted.

Correaltion of Human Papilloma Virus Infection Status with Tonsillar Squamous Cell Carcinoma (편도암의 발암 원인으로 Human Papilloma Virus를 통한 발암 기전과의 상관 관계)

  • Background:Squamous cell carcinoma(SCC)of the palatine tonsils represents approximately 15-23% of all intraoral SCC. The most frequently reported risk factors for oropharyngeal cancer are smoking and alcohol. In a recent overview of HPV and tonsillar squamous cell carcinoma(TC), 51% contained HPV DNA, and HPV-16 being the most frequent type. We aimed to clarify whether HPV directly effects on the oncogenesis and biologic behavior of TC by comparison with infection prevalence, and physical status of virus. Material and Method:We used HPV genotyping DNA chip(Biocore, Korea, Seoul) arrayed by multiple oligonucleotide probes of L1 sequence of 26 types of HPV and HPV genotypes are identified by fluorescence scanner. The copy numbers of HPV E2 and E6 open reading frames(ORF) were assessed using a TaqMan-based 5'-exonuclease quantitative real-time PCR assay. The ratio of E2 to E6 copy numbers was calculated to determine the physical status of HPV-16 viral gene. Results:We observed a significant difference in HPV prevalence between 52 TCs and 69 CFTs(73.1% vs. 11.6%), and most of the HPVs were type 16(87.2%)and non-episomal(94.1%) state. Conclusions:This study regarding HPV infection prevalence and mechanism in the largest population of palatine tonsillar squamous cell carcinoma with chronic follicular tonsillitis revealed significant difference pf HPV prevalence between TC and CFT. Most of HPV were 16 type and integrated or mixed, HPV-16 integration could be directly related to tonsillar carcinogenesis.

Update of Head and Neck Cancer Staging in the 8th Edition Cancer Staging Manual of the American Joint Committee on Cancer (두경부암 병기 설정의 최신 변화: AJCC 암 병기설정 매뉴얼8판)

  • The recently released the $8^{th}$ edition of the American Joint Committee on Cancer (AJCC) Staging Manual introduces significant modifications from the prior $7^{th}$ edition. In this paper, the contents of the new changes in the decision of cancer of the head and neck is summarized except changes in staging of skin and thyroid cancer. In addition to the 8th edition, 1) Addition of extracapsular involvement in metastatic lymph nodes (N category) 2) Oral cancer T classification change, 3) Staging of the pharyngeal cancer was divided into 3 chapters: high-risk human papilloma virus (HR-HPV) associated oropharyngeal cancer (OPC), non HR-HPV associated OPC and hypopharynx cancer (HPC), and nasopharynx cancer (NPC) 4) Changes in T and N classification in NPC, 5) In the case of cancer of unknown primary, P16-positive case is defined as HR-HPV related OPC, and EBV-positive case is defined as NPC. The process that led to these changes highlights the need to collect high-fidelity cancer registry-level data that can be used to confirm prognostic observations identified in institutional data sets. Clinicians will continue to use the latest information for patient care, including scientific content of the 8th Edition Manual. All newly diagnosed cases through December $31^{st}$ 2017 should be staged with the 7th edition. The time extension will allow all partners to develop and update protocols and guidelines and for software vendors to develop, test, and deploy their products in time for the data collection and implementation of the 8th edition in 2018. The 8th edition strikes a balance between a personalized, complex system and a more general, simpler one that maintains the user-friendliness and worldwide acceptability of the traditional TNM staging paradigm.
