Diagnosis and treatment planning for removable partial denture: diagnostic cast analysis and abutment teeth selection (가철성 국소의치의 진단 및 치료계획: 진단모형 분석과 지대치 선정)

  • Diagnosis and treatment planning is initial step to fabricate removable partial denture, and one of most important factor influencing the prognosis. Partial edentulous patients have various condition of remaining teeth and oral mucosa. Therefore, for the proper diagnosis and treatment planning for each patient, getting many informations from patients during initial examination and fabricating accurate diagnostic cast are required. Especially, because the use of various diagnostic tools and surveying of diagnostic cast will be a guide for the abutment teeth selection, oral preparation and design of partial denture, dentist should understand and be able to apply this procedure during diagnosis.

  • Removable partial denture wearers are exposed to the risks that remaining teeth get damaged by caries, attritions, erosion, and fracture. In the case of damaged abutment tooth which should fit to Removable partial denture (RPD), the fabrication of surveyed crown is followed by the making of RPD. However, making new denture takes a long time, and needs several processes and costs. Also, patients should get used to new denture. If other abutment teeth and edentulous ridges provide the existing denture with support, retention, and stability, use of existing denture is considered clinically acceptable. In this situation, fabricating retrofit crowns to an existing removable partial denture makes patient use existing denture, cuts costs, and reduces discomfort. In this case, severely worn teeth were restored using monolithic zirconia crown which fit to an existing removable partial denture by CAD-CAM. Moreover, support, retention, and stability of the denture were improved, and both doctor and patient were satisfied with the result.

  • 의치(denture) 파절조각(fracture segment)의 조립을 정확하고 쉽게 부착시킬 수 있는 cyanoacrylate의 사용을 권장하고 싶다. 이 수리 방법은 환자가 오랜기간 동안 보철물 없이 존속하는 것을 막기 위해 석고모형(stone cast)을 부울 필요없이 신속하게 수리하는 방법이다. 더불어 의치(denture)가 많은 조각으로 파절 되었기 때문에 전통적인 방법에 의해서는 수리가 가능치 않다고 생각 될 경우에는 이 방법이 좋으리라 생각되어 소개하는 바 입니다.

A Study on the Relationship Between Halitosis and Dentures (불쾌감을 유발하는 구취와 의치장착의 상호관계)

  • The main purpose of this study was to observe the relationship of denture and halitosis and to investigate the influence with oral health indexes. Also we examined the contribution factors to subjective awareness of halitosis by denture condition. A total sample of 103 elderly people were answered questionnaire and all collected survey data were processed for data analyses. Based on general characteristics, the study of the halitosis showed higher score which is older age, oral health attitude, and living together. The result showed that the use of denture, dental intelligent quotient (dental IQ) and type of denture were significantly correlated with halitosis. These results indicated that halitosis was affected by denture and oral health index. According to these findings, it is necessary to develop and apply the oral health promotion program which is include the management of halitosis especially for elderly person.

  • The purpose of this study was to provide the data for discussion related to oral health promotion policies for the elderly by examining and analyzing the prosthesis conditions and necessity for the fixed and removable dentures among the Korean elderly. The data, obtained from 4,557 elderly aged at 65 or higher who were targeted for the 5th National Health Nutrition Survey, were analyzed through the complex sample frequency analysis, complex sample cross analysis, and complex sample logistic regression analysis. The results of analysis showed significant relevance of whole denture implant and the necessity thereof in older subjects, rural community, and subjects with lower education background, regarding the state of upper jaw/lower jaw prosthesis and the necessity for upper jaw/lower jaw fixed partial denture/whole denture. In addition, the necessity for prosthesis implant was found to have correlation with the income and subjective health condition, while the necessity for artificial teeth(denture) was found to have correlation with the gender, age, education, and subjective oral health condition. Therefore, it is considered necessary to map out the prevention and treatment policies designed to help maintain and promote oral health based on oral health education, along with the policies that aim to recover the neutralized oral health functions, in relation to the oral health of the elderly.

  • The purpose of This study was to survey the present state and prospect of geriatric denture prosthetic dentistry among community dental hygienists in Korea. Questionnaires about the present state and prospect of geriatric denture prosthetic dentistry were distributed to the community dental hygienists in Korea, July 2006. Collected questionnaires from 89 cases were analyzed. All collected data were analyzed using the SPSS program. The significance of differences among groups were assessed using chi-square test or Fisher's exact test. The results of this study were summarized as follows: The public relations of the geriatric denture prosthetic dentistry were enforced good, mainly by village offices. Appointed dental clinics made a production of full dentures connecting in Korean Dental Association. Above findings suggest that further study about the present state and prospect of geriatric denture prosthetic dentistry is necessary in improving the oral health related quality of life in elders.

Evaluation of the dimensional change of 3D-printed complete denture after post-curing (후경화에 따른 3차원 프린팅 의치의 체적변화)

  • Purpose: The purpose of this study was to assess the dimensional change of 3D-printed dentures after post-curing. Materials and Methods: The upper and lower dentures were designed in Exocad DentalCAD software and exported as STL files. The upper and lower dentures were printed from 10 STL files using a DLP-type dental 3D printer. The printed upper and lower dentures were cleaned, and a scan file was created using a model scanner before and after post-curing. The dimensional change was evaluated by superimposing the scanned denture files before and after post-curing and measuring the distance between measurement points on the denture. SPSS was used for statistics, and the level of significance was 5%. Results: The maxillary denture reduced in size during post-curing, with the most notable color change occurring in the posterior palatal region. The reduction in anteroposterior maxillary denture length (A-D, A-E, A-F), as well as the distance between the first molars on both sides (B-C), was statistically significant. After post-curing, the mandibular denture showed more noticeable color change in the posteriorly buccal and lingual region. The decrease of length on the posterior (A-M, A-D, A-E, A-L, A-H, A-I, H-I) and lingual (J-K, L-M) sides of the denture were statistically significant. Conclusion: There was significant dimensional change in both the length and width of the 3D-printed maxillary and mandibular dentures after post-curing in this experiment. Consequently, it is seemed necessary to develop post-curing techniques and materials that reduce such denture deformation.

  • To fabricate removable partial denture which provide patient's comfort and maintain oral health, cast fabrication which reproduce anatomical and functional state of mouth is needed. Diagnostic cast and master cast are produced through the process of preliminary impression and final impression taking. Both of remaining teeth and edentulous area should be well taken in removable partial denture impression. According to the supporting type of partial denture, impression material and method can be changed. Therefore, understanding of impression procedure is essential for the accurate clinical application.

  • When wearing complete dentures, patients want to function naturally within a physiologically stable range. To do this, recovery of esthetics, biologically stable arrangement and contour, and occlusal contacts with denture stability are necessary. In this case report, a complete denture patient of adverse conditions was presented. To increase stability of the dentures, functional impression was made by border molding using the neutral zone. The dentures were checked for physiological centric relations and stable occlusion. The clinical results showed satisfactory results on function and esthetics.

  • This clinical report presents the case using overlay dentures to treat a patient with cleft lip and palate and malocclusion. This patient requires vigorous treatment such as orthodontic treatment and combined orthodontic/oral surgery procedures. Overlay denture can be used as an alternative treatment option. Intraoral and radiographic examinations were done. The vertical dimension of occlusion was evaluated for proper prosthetic procedures and the surveying in the diagnostic cast was done. Considering the tissue and teeth undercut, the alterations of teeth shape were done. Final preparation and impression was performed. After the evaluation of vertical dimension and occlusion with wax-denture, the dentures were polymerized. Clinical remounting and occlusal adjustment were done on the articulator. Overlay dentures were delivered and the patient was recalled for relining. The overlay dentures satisfied patient's esthetic and functional requirements and provided a stable occlusion. Overlay dentures in this case were a reversible and relatively inexpensive treatment for this patient, however the potential possibilities of caries and periodontal disease as a result of poor oral hygiene should be prevented with periodic recall.
