Nursing Quality Improvement for Acute Stroke Patients through the Use of NIHSS (NIHSS 활용을 통한 뇌졸중 환자 간호의 질향상 전략)

  • 문제: 뇌졸중 환자를 돌보는 간호현장에서는 급성기 뇌졸중 환자의 증상악화에 대한 조기사정과 적절한 대처가 매우 중요한 문제이다. 그러나 임상현장에서는 이를 객관화하고 수치화하여 모든 의료진이 의사소통할 수 있는 유용한 도구를 사용하고 있지는 못하는 실정이다. 이러한 문제를 해결하기위해 NIHSS(National Institute of Health Stroke Scale)를 신경과 진료의사를 중심으로 활용하고 있지만 아직 간호현장에서는 보편적으로 사용하지 않고 있다. 이에 뇌졸중 환자 간호에 NIHSS를 활용하여 뇌졸중 증상악화의 조기사정과 빠른 대처로 궁극적으로는 뇌졸중 환자 간호의 질을 보다 향상시키기 위하여 본 활동이 시도되었다. 목적: 첫째, 간호사의 NIHSS 측정 신뢰도의 현수준을 점검하고 이를 향상시키기 위한 프로그램을 개발 적용한다. 둘째, 간호사가 급성기 뇌졸중환자에게 NIHSS 측정을 통하여 뇌졸중 악화를 얼마나 예측할 수 있는지 분석하고 이를 향상시킬 수 있는 방법을 모색한다. 셋째, 궁극적으로 급성기 뇌졸중 환자의 증상악화의 조기 발견과 치료를 위한 의료진의 올바른 대처지침을 마련하여 이를 적용한다. 의료기관: 경기도에 소재한 대학병원의 뇌졸중 집중치료실. 질 향상 활동: 첫째, 간호사의 NIHSS 측정 신뢰도 향상을 위해 "NIHSS 간호사 교육 프로그램"을 마련하였다. 둘째, NIHSS 측정을 통한 뇌졸중 악화 예측율 향상을 위하여 "뇌졸중 악화 발생 시 의사, 간호사 대처 활동 프로토콜"을 개발하고 추적 관찰하였다. 개선효과: NIHSS 측정 신뢰도는 질 향상 활동 전 89%에서 92%로 향상되었다. 그리고 간호사의 뇌졸중 악화 양성 예측율은 58.7%에서 87.1%로 향상되었다(참고 : 간호사의 뇌졸중 악화 음성 예측율 100% 유지). 즉 "NIHSS 간호사 교육 프로그램 활용", "뇌졸중 악화 발생 시 의사, 간호사 대처 활동 프로토콜"의 적용으로 뇌졸중의 조기 발견 및 치료가 가능하였고, 향후 환자 예후에 긍정적인 영향을 줄 수 있을 것이라 기대된다.

A Study on Interorganizational Boundary Spanning Behaviors between Buyers and Sellers (유통경로 내 조직간 영역초월행동에 관한 연구)

  • Recently, both scholars and marketers have asserted the importance of boundary spanning behaviors, such as external representation, being vocal advocates to outsiders of the organization's image, goods, and services, internal influence, taking individual initiative in communications to the firm and co-workers to improve service delivery by the organization, co-workers, and oneself, and service delivery, serving customers in a conscientious, responsive, flexible, and courteous manner. However, there is lack of study dealing with bourdary spanning behaviors bewteen organizational dyads, in which boundary spanning behaviors are expected to have important roles. The objectives of this paper is to investigate these important concerns with prior research by developing a theoretical model predicting how distinct buyer's boundary spanning behaviors occur. To be concrete, this paper develops a seller characteristics-based model of the attitudinal antecedents of three conceptually distinct forms of boundary spanning behaviors, and tests the hypothesized differential effects of seller characteristics on the three forms of boundary spanning behaviors, and investigates the extent to which these relationships are mediated by relationship satisfaction and organizational commitment. For the purpose of empirical testing, 420 respondents of leading automobile dealers, dining franchisees, industrial material retailers in Korea were surveyed and the analysis utilizing structural equation model indicated that communication quality, fairness, and marketing program dynamism had positive effects on buyer's boundary spanning behaviors via relationship satisfaction and organizational commitment. In addition, boundary spanning behaviors occurred more in contractural and corporate distribution channel than in conventional distribution channel.

  • The purpose of this study is to suggest a systematic procedural model for selecting competent project promoter. The proposed model was made by comparing domestic regulations and procedures with foreign nations', and through interviews with experts and literatures survey. The summaries of this paper are as follows. The current promoter selection procedure was evaluated by reviewing relevant papers, regulations and existing model. Some obstructions which hinder PPI project activation were identified, those are inadequacy of promoter qualification and negotiation process, lack of communication between parties, etc. Some alternatives which remove major obstructions are embodied in the model. The suggested model is comprised of PQ(prequalification), two phased evaluation for the technical proposals which include alternate, communication meeting, etc. To manage the overall procedure well, an involvement of professional "project promoter selection team" would be highly recommended. They will participate in each evaluation stage with full activities, and provide government with some technical materials for selecting promoter as coordinator.

  • In this paper, we built an Object Oriented Framework for Web collaboration system which contains high level analysis information and design knowledge for java applets and applications that enable web clients to communicate and collaborate each other. Components of framework contain design information, source codes and executable codes for reuse. We had defined a development method for domain framework in related works and built a web collaboration system framework following it. We defined subsystem of web collaboration system for the communication and collaboration between web clients. We also BBS, Q&A system, board service system for the communication and collaboration between web clients. We also developed visual tools for framework usability : source code generator, class editor, knowledge supporting tools.

A Study on the Correlation between Stress, Mother-adolescent Communication and Quality of Life in the Adolescents (청소년의 스트레스ㆍ어머니-자녀 의사소통과 삶의 질간의 관계)

  • The purpose of this study was to find out the correlations between mother-adolescent communication and quality of life in the adolescents. The study subjects consisted of 171 adolescents who were going to middle school and high school. The data included general characteristics, health related characteristics, stress, mother-adolescent communication and quality of life. The data were analyzed by frequency, mean, percentage, t-test, ANOVA and Pearson's correlation using the SPSS statistical program. The result were as follows: 1. The mean score for the stress of adolescent was 2.48(maximum score: 5) and there were significant difference according to demographic variables that is sex, grade, father's being and health related variables that is health status, frequently ill, illness severity and illness of family. 2. The mean score for the mother-adolescent communication was 3.27(maximum score : 5) and there were significant difference according to demographic variables that is father's and mother's being. There were no significant difference according to any health related variables. 3. The mean score for the quality of life of adolescent was 3.97(maximum score : 7) and there were no significant difference according to demographic and health related variables. 4. There was a negative correlation between the stress and the quality of life. There was a positive correlation between the quality of life and the mother-adolescent communication. There was a no correlation between the mother-adolescent communication and the quality of life. Consequently, the implication for nursing of this study is that there is a need to develop relief strategy of stress for the female adolescents. And there is a need to develop a promoting program of quality of life for the adolescents considering the stress and the mother-adolescent communication.

  • This study was conducted to provide basic data on the dental hygienists Happiness Index and identify factors influencing dental hygienists A self-administered questionnaire survey was conducted dental hygienists in Jeollabuk-do(Jeonju, Iksan, Gunsan) from October to November 2017. Data were analyzed using t-test, ANOVA and multiple regression. As the result of study, Happiness Index of socioeconomic level has increased when the more work career, and more monthly wage, and better interpersonal relationship. Happiness Index of psychological well-being has increased when younger group and more monthly wage, and better interpersonal relationship. Therefore, in order to increase the happiness index of dental hygienists, economic compensation such as incentives and if the education of the communication method of the cases where the most difficulty case of the communication situation in the human relationship is confirmed is preceded, the quality of the medical service provided to the patient due to the happy working life will be improved.

  • 본 연구의 목적은 건축프로젝트에 있어서 디자인분야 개선요소의 특성을 파악하는데 있다. 기존 연구에서 디자인요소 중 열여덟 개의 개선요소들이 정리되었으며 이러한 개선요소들을 중심으로 건설관계자들의 설문조사를 통하여 주요도를 결정하였다. 연구결과는 각 파트별 협조가 가장 큰 개선요소로서 나타났으며 경영개선의 리더십, 팀워크, 시공능력 순으로 나타났다. 또한 각 그룹별 공통적인 개선요소로서 설계사무소와 시공자와의 원활한 의사소통이 상위개선요소에 지적되었다. 디자인에 있어서 초기 공사와 완공 후 유지관리를 반영한 시공성에 중점을 둔 설계에 대한 관심도 크게 나타났다.

  • 이 논문은 다문화 가정의 구성원인 결혼이주여성들이 사회통합을 하는데 필요한 교육정책에 관한 논의인데, 그 중에서 한국사회를 구성하는 기존 구성원들과의 정보리터러시 격차해소를 위한 교육정책에 주목하였다. 지금까지 다문화 가정의 결혼이주여성들을 대상으로 한 교육과정들은 대부분 언어격차의 해소를 위한 것이었다. 기본적으로 그들에게 의사소통을 위한 언어격차의 해소도 매우 중요하다. 그러나 정보화 사회에서 정보의 활용능력과 정보의 습득이 곧 여러 가지 삶의 질이나 다양한 기회의 획득에 중요하게 작용하므로 결혼이주여성들이 빠른 사회통합을 할 수 있도록 하기 위해서는 언어교육뿐만 아니라 정보리터러시 격차해소를 위한 교육정책이 필요하다. 이 논문에서는 결혼이주여성들의 정보격차 해소를 위한 정보교육의 현황을 일부지역을 중심으로 파악하였다. 그리고 정보리터러시 기준에 기초하여 정보교육의 커리큘럼 개발을 위한 구성요소 및 세부항목을 추출하여 정보교육 커리큘럼 개발을 위한 기초자료를 제시하고 정보교육의 활성화 방안에 대해 제언하였다.

  • 수작업으로 이루어지고 있는 환경의 영향이나 작업의 영향에 따른 정신피로나 신체피로의 주관적인 평가를 자동화하기 위한 방법에 대하여 논하였다. 사람의 가장 자연스러운 의사소통인 평가어를 척도로 하여 평가가 이루어지는 음성인식기술을 응용한 주관평가법에 대하여 연구하였다. 주관평가의 자동화를 위하여 우선, 평가어에 대한 음성 인식을 한 후 인식된 평가 결과 데이터를 이용하여 설문지를 자동 생성시킴과 동시에 파일 형태로 저장시켰다. 음성 인식 알고리즘으로는 DTW(Dynamic Time Warping)인식 알고리즘을 사용하였고. 설문지 질의 내용은 집중도 평가를 이용하였다. 인식실험은 설문에 대한 응답에 필요한 평가어를 대상으로 하였다.

Patients' Perception of Quality of Health Service and Utilization considering convergence factors: A Survey of Elderly Patients of Public Health Centers, Seoul, Republic of Korea (융합적 요소를 고려한 의료서비스 질 인식도와 의료이용의 관련성: 서울시 보건소를 이용하는 노인을 대상으로)

  • Health service quality is increasingly emphasized, and primary care service is no exception in this. This study identified patients' perception of quality considering convergence factors associated with increased utilization among patients over the age of 65 using community health centers. A total of 307 respondents were included in the analysis. The results showed that positive perceptions on facility and environment, patient-centered care, technical knowledge and skills of physicians, and emotional support with nurses and non-medical workers were statistically and significantly associated with higher utilization. In order to increase utilization of primary care service at community health centers, they need to manage and improve these factors.
