Analysis of Health Insurance Standards and Utilization of MRI in Korea: Based on Health Insurance Claim Data (한국의 MRI 건강보험 급여기준 및 진료이용에 관한 연구)

  • This study analyzed the current status of MRI (frequency, amount of treatment) based on the history of application of the MRI health insurance benefit standard and health insurance claim data. MRI examinations began as a health insurance benefit in 2005. In 2005, the indications were restricted for some diseases, but coverage for benefits in 2010, 2013, 2016, and 2018 was expanded. In 2021, the Ministry of Health and Welfare decided to apply health insurance for all MRI examinations. From 2010 to 2017, the number of MRI examinations increased by 86.7% in 2017 compared to 2010, and the amount of treatment increased by 53.5%. According to general characteristics, the number of MRI examinations was higher in women than in men. By age, the number of examinations was the highest among ages 70-79. Outpatient examinations were more frequent than inpatient examinations, and the number of examinations in the tertiary hospitals was the highest among the types of hospitals. The number of brain MRI examinations was the highest in each exam site. In December 2013, the standard of MRI was expanded for heart disease and Crohn's disease, the number of cardiac MRI and abdominal MRI examinations increased in 2014 compared to 2013. However, the number of examinations is small and not associate with the disease, it would be difficult to say that it affected the increase in the total number of MRI examinations. To assess health insurance sustainability and policy effectiveness, monitoring will be necessary.

Suggestion of Learning Objectives in Social Dental Hygiene: Oral Health Administration Area (사회치위생학의 학습목표 제안: 구강보건행정 영역)

  • The purpose of this study is to propose learning objectives in social dental hygiene by analyzing and reviewing learning objectives in oral health administration area of the existing public oral health. This study is a cross-sectional study. The subjects of the study selected with convenience extraction were 15 members of the social dental hygiene subcommittee of the Korean Society of Dental Hygiene Science. Data collection was conducted by self-filling questionnaire. The research tool is from 48 items of A division in the book of learning objectives in the dental hygienist national examination, and this study classified each of them into 'dental hygiene job relevance', 'dental hygiene competency relevance', 'timeliness', and 'value discrimination of educational goal setting' to comprise 192 items. Also, to collect expert opinions, this study conducted Delphi survey on 7 academic experts. Statistical analysis was performed using the IBM SPSS Statistics ver. 23.0 program (IBM Co., Armonk, NY, USA). Recoding was performed according to the degree of relevance of each learning objective and frequency analysis was performed. This study removed 18 items from the whole learning objectives in the dental hygienist national examination in the oral health administration area of public oral health. Fifteen revisions were made and 15 existing learning objectives were maintained. Forty-five learning objectives were proposed as new social dental hygiene learning objectives. The topics of learning objectives are divided into social security and medical assistance, oral health care system, oral health administration, and oral health policy. As a result of this study, it was necessary to construct the learning objectives of social dental hygiene in response to changing situation at the time. The contents of education should be revised in order of revision of learning objectives, development of competency, development of learning materials, and national examination.

Prospective Supply and Demand of Medical Technologists in Korea through 2030 (임상병리사 인력의 수급전망과 정책방향)

  • The purpose of this study is to provide policy recommendations for manpower planning by forecasting the supply and demand of Medical Technologists. Supply was estimated using an in-and-out movement method with a demographic method based on a baseline projection model. Demand was projected according to a demand-based method using the number of clinico-pathologic examinations taken for Medical Technologists. Over- or undersupply of Medical Technologists will depend on the productivity scenario and assumptions and ultimately on governmental policy direction. In other words, whether the production of Medical Technologists is higher or lower than the current level depends on the government policy to consider insurance finances. In this study, we assessed 'productivity scenario 3' based on the productivity as of 2012, when the government's policy direction was not considered. Based on the demand scenario using the ARIMA model, the supply of Medical Technologists is expected to be excessive. This oversupply accounts for less than 10% of the total and therefore should not be a big problem. However, given that the employment rate of Medical Technologists is 60%, it is necessary to consider policies to utilize the unemployed. These measures should expand the employment opportunities for the unemployed. To this end, it is necessary to strengthen the functions of laboratories in the public health center, to increase the quota of Medical Technologists, to assure their status, to establish a permanent inspection system for outpatient patients, and to expand the export of Medical Technologists overseas.
