Ubiquitous health care system, which is one of the developing solution technologies of IT, BT and NT, could give us new medical environments in future. Implementing health information systems can be complex, expensive and frustrating. Healthcare professionals seeking to acquire or upgrade systems do not have convenient, reliable way of specifying a level of adherence to communication standards sufficient to achieve truly efficient interoperability. Great progress has been made in establishing such standards-DICOM, IHE and HL7, notably, are now highly advanced. In this paper, there are suggestions that provide a test bed, demonstration and specification of how standards such a IHE, HL7, ASTM can be used to provide an integrated environment.

4월 7일은 세계보건기구(WHO) 발족을 기념하는 '세계 보건의 날'이다. 고령화사회가 됨에 따라 의료기기 시장이 커지고 있다. 국제표준화 동향에 대한 조사 분석과 정보를 관련 산업체에 제공하는 등 의료기기 국제경쟁력 확보를 위한 지원이 필요하다.

영화 로보캅, 아이언맨 등에 등장하는 외골격기구(exoskeleton)는 이제 더 이상 먼 미래의 얘기도 아니고 딴 나라 얘기도 아니다. 외골격기구 중 특히 하지외골격기구는 Baytheon Sarcos, Berkley Bionics, Exohiker, Bleex, HAL5 등의 이름으로 버클리, 스탠퍼드 등 해외 우수 연구집단에서 이미 개발을 성공하였고 상용화를 앞두고 있다. 늦은 감은 있지만 국내에서도 재활의료용, 군수용 하지외골격기구 연구가 진행되고 있다. 본 저자는 최근 기존연구와 다른 접근방식으로 고민을 시작하였고 본 연구팀이 상표출원한 $PLE^{TM}$, $LeSATA^{TM}$의 개발을 서두르고 있다. 이 글을 통해 그간의 고민들을 간략히 털어놓을까 한다.

2001년 카타르 도하에서 열린 세계무역기구(WTO)의 각료회의 결과 2005년까지 보건의료 서비스분야의 시장개방 논의를 마쳐야 한다. 이에 국내 의료계는 의료공동대책위원회를 구성하여 대응방안을 모색하고 있으며, 치협도 특별위원회를 구성하여 국민 구강보건에 최선의 방향을 찾기 위해 노력을 하고 있다. 그런 노력의 일환으로 치협은 치과의사들이 현 의료시장 개방이라는 명제 앞에서, 얼마나 처해진 상황을 잘 파악하고 있으며 무엇을 원하는 가를 알기 위해 회원들의 의견을 물었다. 이번 호와 다음 호에서는 치협이 지난 4월에 회원 및 치대생 등에게 설문 조사한 결과를 중심으로 모아진 회원들의 견해를 중심으로 구강보건정책연구회에서 발표한 자료를 게재한다.

In this study, we have examined the possibility to improve the material properties and biocompatibility of the 630 stainless steel for the application of medical instruments. The evaluation of mechanical properties and biocompatibility of the 630 stainless steel were studied by aging, nitriding, SEM measurement, Vicker's hardness, tensile strength and MTT cytotoxicity using IGF cells, respectively. The results showed that the tensile strength and Vicker's hardness of 630 stainless steel was increased with aging and ion nitriding time. The cytotoxicities of the 630 stainless decreased compared with the 420 stainless on MTT cytotoxicity using IGF cells. Ion nitriding of 630 stainless led to an enhanced application of medical instruments and biocompatibility performance.

의료용기구를 수입에만 의존하던 우리나라 의료업계는 IMF영향으로 큰 타격을 받고 있다. 이처럼 외환위기에 시달리는 때 연세대의대 김덕원 교수는 치아 신경치료에 필수적으로 사용되는 치아신경관 측정기를 개발해 각광을 받고 있다. 김교수는 또 휴대폰에서 나오는 전자파가 의료기기 작동에 미치는 영향이 크다는 사실을 확인하고 '휴대폰 유해'연구도 계속하고 있다.

Implementing health information systems can be complex, expensive and frustrating. Healthcare professionals seeking to acquire or upgrade systems do not have a convenient, reliable way of specifying a level of adherence to communication standards sufficient to achieve truly efficient interoperability. Great progress has been made in establishing such standards-DICOM, IHE and HL7, notably, are now highly advanced. IHE has defined a common framework to deliver the basic interoperability needed for local and regional health information networks. It has developed a foundational set of standards-based integration profiles for information exchange with three interrelated efforts. HL7 is one of several ANSI-accredited Standards Developing Organizations operating in the healthcare arena. Most SDOs produce standards (protocols) for a particular healthcare domain such as pharmacy, medical devices, imaging or insurance transactions. HL7's domain is clinical and administrative data. HL7 is an international community of healthcare subject matter experts and information scientists collaborating to create standards for the exchange, management and integration of electronic healthcare information. The ASTM specification for Continuity of Care Record was developed by subcommittee E31.28 on electronic health records, which includes clinicians, provider institutions, administrators, patient advocates, vendors, and health industry. In this paper, there are suggestions that provide a test bed, demonstration and specification of how standards such a IHE, HL7, ASTM can be used to provide an integrated environment.
