Background and Objectives:The Voice Handicap Index (VHI) evaluates the patients perception of impact of voice disorder in term of functional, physical, emotional factors. The purpose of this study was to evaluate the change of patient's subjective voice handicap index before and after laryngeal microsurgery for benign vocal cord lesions. Materials and Methods:We analyzed 55 patients who received laryngeal microsurgery for benign vocal cord disease from January 2011 to February 2013 retrospectively. There were 50 vocal nodules, 3 vocal polyps, 2 vocal cysts. VHI were analyzed before surgery and 3 months after surgery. Results:The VHI scores showed statistically significant reductions postoperatively in functional and emotional VHI (p=0.01 and p=0.034). Also, Emotional VHI score after microsurgery was higher in female than male [adjusted odd ratio (AOR)=0.292 ; 95% confidence interval, CI=0.098-0.869, p=0.01]. Conclusion:In other words, males experience significant more emotional effects that are improved after microscopic surgery.

병적인 고음성은 임상적으로 흔하지는 않지만 종종 볼 수 있는 질환으로서 치료에 대한 많은 논란이 있는 게 사실이다. 대개 음성교육에 의한 치료가 많이 시행되며 수술적인 치료로서는 제 3형 갑상연골성형술이 인정되어 온 것이 사실이다. 이러한 수술에 대한 적응증으로는 첫째 남자에서 오랫동안 너무 높은 음성(mutational voice disorder)을 가진 환자와 둘째 여러 가지 원인에 의해 성대가 너무 경직되어 고음이면서 애성(breathy voice)을 동반한 발성장애가 있겠다. (중략)

Sulcus vocalis remains a surgical challenge despite many recent advances in laryngomicrosurgeries. We previously reported that 585-nm Pulsed dye laser (PDL) exerts favorable outcome in treatment of sulcus vocalis due to its therapeutic effects of collagen rearrangement and improved wound remodeling. In spite of the usefulness of PDL glottoplasty for treating sulcus vocalis, the device is no more available in the country. It prompted us to focus another angiolytic laser ; 532-nm KTP laser which has similar mechanisms of action and has been used for treatment of other laryngeal lesions elsewhere. Herein, we present a case of sulcus vocalis successfully treated with KTP laser. A patient underwent laryngomicrosurgery with angiolytic KTP laser (KTP glottoplasty) by the same surgical procedure with PDL glottoplasty. After the surgery, the patient presented improved voice outcome in time without complications.

배경 및 목적: 전신마취를 위하여 시행한 기관내 삽관은 삽관튜브와 성대내면의 접촉에 의한 압력과 마찰로 후두 미세한 손상을 주게된다. 저자들은 단기간 기관내 삽관 전,후의 음성분석을 통하여 손상의 유무와 회복을 측정할 수 있는 객관적인 음성지표를 찾아보고자 하였다. 대상 및 방법: 만성 중이염 수술시 전신마취를 목적으로 경구기관 튜브를 거치한 성인 남자 10명과 여자 15명 환자를 대상으로 수술 1일전과 술후 24시간 후 각각 "a"음을 연장 발성시켜 CSL 4300B (KAY elemetrics Corp)의 MDVP(multidimensional voice program)을 이용하여 harmonic to noise ratio(NHR), Jitter, Shimmer, Fundamental frequency를 측정 비교하였다. 결 과: 남녀 모두에서 Jitter, Shimmer는 각각 평균 0.70%에서 1.06%, 1.92%에서2.28%로 증가되는 경향을 보였으나 통계학적 유의성은 없었다. Fundamental frequency는 여자에서 평균 220Hz에서 221Hz로 남자는 125Hz에서 128Hz로 변화를 보이지 않았고 harmonic to noise ratio(NHR)또한 평균 0.11로 수술 전, 후 변화를 관찰할 수 없었다. 결 론: 이상의 결과로 2내지 6시간의 단기간 삽관으로 인한 성대의 손상은 경미하여 24시간 이내에 회복되는 것으로 판단된다. 향후 6시간 이상의 기관내삽관이나 수일이상의 장기간 삽관후의 음성지표의 측정등의 연구가 필요할 것으로 사료된다.

변성발성장애는 변성기의 시기를 지났음에도 불구하고 적절한 음도저하를 습득하지 못함으로써 야기되는 음성장애의 한 가지로, 이런 환자들에 대해서는 제3형 갑상연골성형술을 통한 수술적 처치나 음성치료로 그 문제를 해결할 수 있다. 본 논문에서는 이중 음성치료적인 측면에 초점을 맞추어, 음성치료에 의한 변성발성장애 치료의 효율성에 대해 고찰해 보고자 한다. (중략)

성대의 마비는 그 원인과 정도에 따라 애성, 연하장애, 호흡장애 등 다양한 기능 결손을 초래하게 되므로, 마비된 성대에 의한 불편을 외과적으로 처치하기 이전에 반드시 마비의 원인에 대한 이해와, 환자의 불편에 대한 정확한 이해가 선행되어야만 적합한 술식을 선택할 수 있다. 예를 들어 두개저수술의 합병증으로 갑자기 발생된 성대마비와 갑상선암에 의하여 서서히 진행된 성대마비의 경우 그 예후나 경과가 전혀 다른 질환이라 할 수 있기 때문이다. (중략)
