  • Prior to the advent of microvascular surgery, conventional prosthetic rehabilitation offered limited success to re-establish the physiological function in oromaxillofacial reconstruction. Microvascular surgery provided a new frontier and there are multitude of flaps. Each flap has the benefits and limitation for the application to various defects. Advantage of the scapular flaps over other reconstructive methods include the ability to design multiple cutaneous panels on a separate vascular pedicle allowing improvement in three-dimensional relationship and osseointegrated implants can be palced to restore occlusal and masticatory function. Here I present the detailed description of the important surgical anatomy as well as graft dissection and clinical application of free vascularized scapular flap.

Factors Influencing Satisfaction on Home Visiting Health Care Service of the Elderly based on the degree of chronic diseases (만성질환 유병상태에 따른 노인 방문건강관리 서비스 만족도 영향요인 연구)

  • This study was conducted to derive factors that affect the satisfaction of home visiting health care services and to develop effective community care models by using the results of Seoul's outreach service which is the basis for Korean community care. The population of the study was the elderly aged 65 and 70 who participated in the Seoul's outreach community services 3rd stage (July 2017 - June 2018) and 4th stage (July 2018 to June 2019). 2,200 people were extracted by the proportional allocation method and home visit interviews were conducted on them. Subjects were divided into sub-groups based on chronic disease prevalence, and logistic regression was conducted to derive factors that affect the satisfaction of home visiting health care services. The results demonstrated that the elderly without chronic diseases were more satisfied when they received health education and counseling services, the elderly with one chronic disease were more satisfied when they received Community resource-linked services. In the case of elderly people with two or more chronic diseases, the service satisfaction level is increased when health condition assessment and Community resource-linked services are provided. Regardless of whether or not they have chronic diseases, service delivery time was a factor that increased satisfaction in home visiting health care. And the degree of explanation understanding was a factor that increased satisfaction for both single and complex chronic patients. Home Visiting health care services based on the community is a key component of the ongoing community care. In order to increase the sustainability and effectiveness of community care in the future, Community-oriented health care services based on the degree of chronic diseases of the elderly should be provided. In order to provide more effective services, however, it is necessary (1) to establish a linkage system to share health information of the subject held by the National Health Insurance Service to local governments and (2) to provide capacity-building education for visiting nurses to improve the quality of home visiting health care services. It is hoped that this study will be us ed as bas ic data for the successful settlement of community care.

The Correlation between Diabetes and Periodontitis in Korean Adults (한국 성인의 당뇨병과 치주질환의 관련성)

  • The purpose of this study was to evaluate the correlation between diabetes and periodontal diseases in Korean adults based upon the data of the 2007 Korea National Health and Nutrition Examination Survey. The subjects of this study were 190 adults aged ${\geq}19years$ whose CPI(Community Periodontal Index) and diabetes related medial histories including its component-specific examination data had been available, and the results of this study are summarized as follows: 1. Diabetes related characteristics associated with the prevalence of periodontal diseases include diabetes education, diabetes treatment, and glycosylated hemoglobin (HbA1c). The incidence rate of periodontitis was higher in the subjects who had not received diabetes education or treatment or who showed at least 7.0 of glycosylated hemoglobin (HbA1c) levels (apt to be uncontrolled). 2. General health behaviors associated with the prevalence of periodontal diseases include smoking and drinking. The incidence rate of periodontitis was higher in those who were currently smokers or who had higher frequencies of drinking per week. 3. Oral health behaviors associated with the prevalence of periodontal diseases include the history of oral health examination in the previous year, the use of proxabrush or dental floss, and perceived oral health status. The incidence rate of periodontitis was higher in those who had not received oral health examinations in the previous year, who had used neither proxabrush nor dental floss or who perceived that their own oral health was poor.

THE PREVALENCE OF DOUBLE TEETH AND CONGENITAL MISSING TEETH IN PRIMARY DENTITION AND THEIR CORRELATION WITH THE PERMANENT DENTITION (유치열의 이중치 및 결손치의 발생빈도와 영구치열과의 상호관계)

  • The purpose of this study was to investigate the relationship between morphology and number of deciduous teeth and the occurrence of other dental anomalies in their successors, and to evaluate the necessity of early diagnosis of dental anomalies in the primary dentition. Prevalence of double teeth and congenital missing teeth was investigated in 254(134 boys, 120 girls) panoramic radiographic films, taken by 2 to 7-year-old children in Chonnam National University Hospital from 2000 to 2005. And then it was examined that relationship of anomalies of the primary dentition and their successors. Among them 11 children(6 boys, 5 girls) had double teeth or missing teeth. And prevalence of the double teeth was 1.6% and missing teeth was 3.1%. One subject had double teeth in in the mandible and missing teeth in the maxilla. Of the 11 cases of dental anomalies in primary dentition, 7 cases had congenital missing tooth in their successors. This study suggests that the dental anomalies in the primary dentition induced high prevalence of the congenital missing of permanent successors in the permanent dentition.

The Association of Asthma and Osteoporosis in the Residents of the Local Community (지역사회 주민을 대상으로 한 천식과 골다공증과의 연관성)

  • Objectives: Corticosteroids, usually as an inhaled form, are usually used to treat narrowed airway in the treatment of asthma. However, the use of corticosteroid commonly associated with the development of osteoporosis. In this study, we studied the association of asthma and osteoporosis in the residents of the local community. Methods: As a part of the Local Community Health Study Chungnam 2008, the residents are surveyed with the formal questionnaire. We analyzed the association between asthma and osteoporosis data with the questionnaire. Results: We analyzed total 6,348 data from the residents in Chungnam province. Of them, 203(3.2%) were asthma patients and 465(7.3%) were osteoporosis patients. Patients with asthma have more advanced age(p<0.001), higher incidence of hypertension(p<0.001) and diabetes(p=0.001), lower incidence of exercise(p=0.002) and economic activity(p<0.001). Patients with osteoporosis have more advanced age (p<0.001), female predominance(p<0.001), higher incidence of hypertension (p<0.001), diabetes(p=0.001), and fractures (p<0.001), lower incidence of exercise (p=0.002) and economic activity (p<0.001). In total population, asthma is associated with the increased incidence of osteoporosis, especially in patients treated with anti-asthmatic medications. Conclusions: Through this study, we demonstrated the presence of asthma is closely associated with the increased incidence of osteoporosis, especially in patients treated with anti-asthmatic medications. Therefore, further and broad study will be needed to evaluate the association of asthma and the types of anti-asthmatic medications in the development of osteoporosis.

THE PREVALENCE OF DOUBLE TEETH AND CONGENITAL MISSING TEETH IN PRESCHOOL CHILDREN OF IKSAN CITY (익산지역 유치원 아동의 이중치와 결손치의 발생빈도에 관한 조사 연구)

  • The double teeth include gemination and fusion. The congenital missing tooth is the absence of the tooth. The purpose of this study was to investigate the prevalence of double teeth and congenital missing teeth in preschool children of Iksan city. The study population consisted of 1,031 children, 569 boys and 462 girls, aged from 17 to 84 months. The double teeth possess a variety of diagnostic and treatment problems. Gemination is the partial splitting of a single bud into two distinct entities that remain joined in a Siamese twin fashion. Fusion is the joining of two buds. Both may be normal, or one may be a supernumerary tooth. Congenital missing is the absence of one or a few teeth. Twenty three(11 boys and 12 girls) of the 1,031 children had double teeth and 17(8 boys and 9 girls) exhibited congenital missing of teeth. The prevalence of double primary teeth was 2.2%. The prevalence of congenital missing teeth was 1.6%. One subject had double teeth and congenital missing tooth at the same time. Of the 17 cases of congenital missing teeth, one girl showed it in the maxilla and aye boys and six girls had them in the unilateral side of the mandible. There were no significant differences in the prevalence of double teeth and congenital missing teeth between the boys and the girls.
