A study on the change of limb muscle atrophy by the period of limited activities following the heart surgery in congenital heart disease children (선천성 심장질환아의 수술후 활동제한 기간에 따른 사지근 위축 변화에 관한 연구)

  • In order to investigate the effects of a decreased activity on skinfold thickness, circumference and muscle strength of the extremities during the recovery period following heart surgery, skinfold thickness, circumference and muscle strength of the extremities were measured on days 0, 3, 6, and 9 following the surgery, and compared with those on the arrival day of intensive care unit. Skinfold thickness was measured using a skinfold caliper(Saehan Cor., Korea), circumference of the limbs were measured with a tape measure, upper extremity strength was determined using the Takei grip dynamometer and lower extremity strength was measured by pressing the flatfoot on an electronic digital health meter while tying on a bed. Results from this study were thus : 1. Skinfold thickness of triceps, quadriceps and gastrocnemius muscle on days 3, 6, 9 following the heart surgery was not significantly different from that of on the day of operation. 2. Circumference of midupperarm and midthigh on days 3, 6, 9 following the heart surgery was not significantly different from that of on the day of operation. Circumference of midcalf on days 3, 6 following the heart surgery was not significantly different from that of on the day of operation, while that of midcalf on day 9 following the surgery decreased significantly compared with that of on the day of operation. 3. Muscle strength of the upper extremity was not significantly different from that of on the day of operation, while that of the lower extremity on day 9 following the surgery decreased significantly compared with that of on the day of operation. From these results, it may be concluded that circumference and muscle strength of lower extremity can be decreased due to the postoperative inactivity following heart surgery in congenital heart disease children.

Human endogenous retroviruses and neurologic disorders (인간 내인성 레트로 바이러스와 신경학적 장애)

  • Human endogenous retroviruses (HERVs) are fossil viruses that began to be assimilated into the human genome some 30~40 million years ago, and now constitute nearly 8% of the human genome. These ancient retroviruses have since accumulated mutations that have rendered them defective; thus, they have been termed junk DNA. However, recent research indicates that not all HERVs remain silent passengers. Although they have not been shown to be causative of any human disease, endogenous retroviral sequences may become expressed under select pathological circumstances such as neurological disorders, including multiple sclerosis (MS), schizophrenia, and Amyotrophic Lateral Sclerosis (ALS); viral infections, including human immunodeficiency virus (HIV) and herpesvirus; and multiple types of cancers. This review focused on the possible interactions of HERVs and neurological diseases.

Revision Rotator Cuff Repair (회전근 개 봉합술 후 재수술)

  • The primary purposes of revision repair for a failed rotator cuff repair are a relief of pain and functional improvement. Therefore, revision repair is most proper in patients with the functional deficit accompanied with the shoulder weakness as well as the persistent pain. The important factor that is considered in revision repair is a quality of torn cuff. Especially, Care must be taken to ensure that the revision repair is possible, considering the size of tendon defect, atrophy of the muscle, fatty infiltration and extent of the retraction of tendon. Revision repair of a failed rotator cuff repair is more difficult, and the functional results are less satisfactory than those of primary repair, because excessive bursal scarring and tendon retraction may be exhibited, a large or massive tear is often detected, tear has usually been present for a long time, and a quality of muscle-tendon may be poor. So, we discuss our experiences related to revision repair after a failed cuff repair that has been recently introduced through the articles.

Effect of Low Frequency Electrical Stimulation on VEGF Expression of Suspension Induced Atrophic Muscle (뒷다리 현수 유도 위축근에 대한 저빈도 전기자극의 VEGF 발현에 미치는 영향)

  • Purpose : The purpose of this experiment was to evaluate the pre-application effect of low frequency electrical stimulation(LFES) on VEGF expression of atrophic muscle and to determine the optimal pre-application period of LFES for prevent muscle atropy Methods : Twenty-five adult sprague-dawley rats were randomly assigned to weight bearing group, hindlimb suspension for 14 days group, hindlimb suspension with pre-application of LFES for 14 days group, hindlimb suspension with pre-application of LFES for 11 dsys group and, hindlimb suspension with pre-application LFES for 7 dsys group. 16Hz of Biphasic pulse current was applied to gastrocnemius for 15min per days. Results : VEGF were decreased expression in HSG groups, whereas VEGF were significantly increased in HS+ES14G, HS+ES11G, HS+ES7G groups Conclusion : LFES during the hindlimb suspension showed a positive effect in VEGF induction and early application is strongly encourage VEGF induction. This indicated that pre-application of LFES could prevent muscle atrophy.

Preoperative Shoulder MRI Findings to Predict Subscapularis Tendon Tear Requiring Surgical Repair (수술이 필요한 견갑하건 파열을 예측하기 위한 수술 전 어깨 MRI 소견)

  • Purpose This study aimed to investigate which indirect parameters on preoperative MRI were the principal predictors of subscapularis tendon tears (STTs) requiring surgical repair. Materials and Methods Preoperative MRI scans of 86 patients were retrospectively reviewed for visual assessment of the STT, pathology of the long head of the biceps tendon (LHBT), posterior decentering (PD) of the humeral head, humeral rotation, fatty degeneration, and subscapularis muscle atrophy. To evaluate atrophy, visual grading using the anatomical line connecting the coracoid tip to the glenoid base, designated as the base-to-tip line (BTL), and thickness measurements were performed in the en-face view. Results Arthroscopically, 31 patients (36%) exhibited Lafosse type III or IV STT and underwent surgical repair. LHBT pathology (p = 0.002), PD of the humeral head (p = 0.012), fatty degeneration (p < 0.001), and BTL grade (p = 0.003) significantly correlated with STT. In the multivariate analysis, PD of the humeral head (p = 0.011, odds ratio [OR] = 5.14) and fatty degeneration (p = 0.046, OR = 2.81) were independent predictors of STT. Conclusion PD of the humeral head and fatty degeneration of the subscapularis can help to diagnose clinically significant STT. Interpretation of these findings may contribute to the planning of an optimal surgical strategy.

A Morphological Study on the Changes in Rat's Gastrocnemius (쥐의 비복근 섬유의 변화에 대한 형태학적 연구)

  • Twelve Spraque-Dawley healthy male rats(average weight ; 250g)were used to study the morphological changes of mitochondria, myofibril, muscle cell nucleus, triad. They were devided into 3 groups : normal daily activity (Group 1), 2weeks immobilization (Group 2), 4 weeks immobilization(Group 3). Left ankle of Group 2 and 3 were immobilized with plaster cast in $65^{\circ}$ plantarflexed position. The gastrocnemius were removed from 12 rats. Muscle fibers were observed electronmicroscopically by double staining with uranyl acetate and lead citrate, All the variables of Group 2 and 3 that selected in this study were significantly decreased when decreased with control value (p<.05) but also muscle fibers showed extensive damage, characterized by irregularity of mitochondrias and wide separation of myofibrils. irregularity and thinness of myofilaments and abnormal shape of muscle cell nucleus and unclear triad. Especially, sarcomere length of Group 3 were singnificantly decreased when compared with Group 2(p<.01).

  • Amyotrophic lateral sclerosis(ALS) is a progressive disorder that causes degeneration of motor neurons of the brain and spinal cord. It is characterized by loss of motor neurons leading to muscle weakness, and sensation and mental function stay intact during the course of the disease. Diagnostic tests include magnetic response imaging(MRI), electromyogram(EMG), muscle biopsy, and blood tests. In order to a definitive diagnosis of ALS, damage must be evident in both upper and lower motor neurons. When three limbs are sufficiently affected, the diagnosis is ALS. There is no cure for ALS. We experienced one case of ALS, the patients was diagnosed as ALS by EMG and symptoms. We diagnosed her as ShinEumHur(SEH) and treated by Choakwiyeum(左歸飮). We report the change of her symptoms through oriental medical treatment.

  • 성대구는 성대점막 고유층의 일부 결손 또는 위축성 함입으로 유리연이 내함되어 성대궁형(bowing cord)이 형성되기 때문에 발성시 정상점막 파동이 소실되고 성문폐쇄부전을 보이며 그 결과 애성을 초래하게 된다. 이러한 성대의 내함을 교정하기 위하여 구절제술, 성대 상하순(upper and lower lip) 점막의 위치조절 둥이 시행되고 있으나 성대궁형이 교정되지 않아 음성개선에 실패하는 경우가 많다. 저자들은 구절제술 후 성대궁형을 교정할 목적으로 연골막을 포함한 연조직을 성대근 내측으로 삽입하여 음성을 개선하는데 도움을 얻고자 하였다. 이러한 방법으로 음성수술을 시행한 5례의 경험과 그 효과 및 문제점을 문헌고찰과 더불어 보고하고자 한다.

  • $\ulcorner$기업활동 규제완화에 관한 특별조치법$\lrcorner$에 의한 산업안전보건 관련 규제의 대폭적인 완화로 인하여 사업장의 안전보건업무의 위축을 가져와 산업재해를 감소시키는데 많은 지장이 초래될 것으로 우려되고 있다. 이에 대한 대책을 마련하기 위한 일환으로 본 연구에서는 안전보건관련 당사자들을 대상으로 집중면담(Focus Group Interview; FGI) 및 이를 바탕으로 개발한 설문조사를 통하여 이들의 기업 활동 규제완화에 대한 의식을 심층적으로 분석하였다. 대체적으로 규제완화사항에 대해 안전관리자나 보건관리자들이 잘 인지하고 있었으며 이러한 규제완화가 산업재해 감소에 부정적인 영향을 미칠 것으로 생각하고 있었다. 특히 안전 및 보건관리자의 겸직허용이 현장의 안전보건관리에 미치는 영향이 매우 높은 것으로 파악되었다.

Factor Analysis of the Korean-Child Behavior Checklist in Children with Autism Spectrum Disorders (자폐 범주성 장애 아동에서 아동·청소년 행동평가척도의 요인분석)

  • The purpose of this study was to examine validity of the Korean-Child Behavior Checklist: K-CBCL) as measures for emotional and behavioral problems for use with children with autism spectrum disorders. In present study, the factor of the K-CBCL was investigated, using data of 248 children with autism spectrum disorders, with 11.17 mean ages. The two factor model of Internalizing problems (Withdrawn, Somatic Complaints, Anxious/Depressed) and Externalizing problems (Delinquent Behavior, Aggressive Behavior) was investigated by the confirmatory factor analysis. The two factor model of K-CBCL was adequate for children with autism spectrum disorders. The inter-item consistency for the sub-factor of K-CBCL demonstrated on adequate reliability of the measure. Although the inter-item consistency of Withdraw, Social problems, Delinquent Behavior was not acceptable, the inter-item consistency of Internalizing, Externalizing and total problems were good. This results supported validity and reliability and suggested that K-CBCL is used to assess for emotional and behavioral problems in children with autism spectrum disorders.
