Alterations in Mitochondrial DNA Copy Numbers and Mitochondrial Oxidative Phosphorylation (OXPHOS) Protein Levels in Gastric Cancer Tissues and Cell Lines (위암 조직과 세포주에서 mDNA와 OXPHOS 단백질 분석)

  • Alterations in mitochondrial DNA (mtDNA) copy numbers have been reported in patients with stomach cancer and suggested to play a role in gastric carcinogenesis or gastric cancer progression. However, changes in the levels of mitochondrial proteins or mtDNA-encoded oxidative phosphorylation (OXPHOS) proteins in gastric cancer remain unclear. In this study, we investigated mtDNA contents, mitochondrial protein levels, and mtDNA-encoded OXPHOS protein levels in gastric cancer tissues and cell lines. We correlated mtDNA copy numbers with clinicopathologic features of the gastric cancer samples used in this study and used quantitative PCR to analyze the mtDNA copy numbers of the gastric cancer tissues and cell lines. Western blot analysis was used for assessing the amounts of mitochondrial proteins and mtDNA-encoded OXPHOS proteins. Among the 27 gastric cancer samples, 22 showed a reduction in mtDNA copy numbers. The mtDNA content was increased in the other five samples relative to that in normal matched gastric tissues. Mitochondrial protein and OXPHOS protein levels were reduced in some gastric cancer tissues. However, mitochondrial protein and OXPHOS protein levels in gastric cancer cell lines were not always in line with their mtDNA contents. The mtDNA copy numbers were reduced in five gastric cancer cell lines tested in this study. In summary, this study reports a common reduction in mtDNA contents in gastric carcinoma tissues and cell lines, pointing to the possible involvement of mtDNA content alterations in tumorigenesis of the stomach.

Informational Needs of Postoperative Gastric Cancer Patients (위암 수술 후 환자의 정보 요구)

  • Purpose: Adequate health-related information provided by health professionals may help cancer patients overcome their uncertain situation and manage their healthcare. To provide information effectively, there is a need to understand the content of the patients' essential information. The purpose of this study was to identify recent informational needs of postoperative gastric cancer patients. Materials and Methods: Data were collected from 190 postoperative gastric cancer patients who attended the Stomach Cancer Patients' Day ceremony (18 November 2008) held by the Stomach Cancer Center of Samsung Medical Center with the use of a questionnaire which measured informational needs. A questionnaire with 37 items was comprised of domains of diagnostic tests, cancer therapy, prognosis, follow-up, sexual activity, stomach cancer-related information, and 7 single items. Results: The priorities of informational needs were the domains of prognosis, ways of healthcare during treatment (a single item), follow-up, stomach cancer-related information, cancer therapy, and diagnostic tests (in descending order). Items related to prognosis, diet, and management of their healthcare ranked in the top 10 informational need scores. As age decreased, the degree of informational needs about diagnostic tests, cancer therapy, sexual activity, and stomach cancer-related information increased. Conclusion: We suggest that prognosis-related information based on the accumulated institutional therapeutic outcomes and objective prognosis data should be incorporated in the current education program. Health professionals should provide comprehensible information content to cancer patients and caregivers and encourage patients to participate in their therapy with a more positive attitude.

The Prognostic Value of the Preoperative Lymphocyte Count in Patients with Gastric Cancer (위암환자에서 수술 전 말초혈액 림프구 수와 예후)

  • Purpose: The aim of this study was to evaluate the prognostic value of the peripheral blood lymphocyte count before surgery in those patients with gastric cancer. Materials and Methods: The study group was comprised of a series of 1,054 patients who underwent curative gastrectomy. The appropriate lymphocyte count cutoff value was determined. The prognostic factors were evaluated by univariate and multivariate analyses. Results: The lymphocyte count cutoff value was 1,500/ul. The patients were classified into two groups: Group A had a lymphocyte count $\geq$ 1,500/ul (n=765) and Group B had a lymphocyte count <1,500/ul (n=289). There were statistically significant differences between the groups according to their age (P<0.001), the tumor stage (P=0.038) and the tumor size (P<0.001). The 5- and 10-year survival rates of Group A were 80.1% and 76.6%, respectively and those of Group B were 72.4% and 63.5%, respectively (P=0.002). When multivariate analysis was performed by the Cox proportional hazards model, the lymphocyte count was not an independent prognostic factor. Conclusion: Although the prognosis of patients with a high lymphocyte count was better than that of the patients with a low lymphocyte count, our results did not support using the preoperative peripheral blood lymphocyte count as an independent prognostic factor for patients with gastric cancer.

The Factors Affecting Health Promotion Behavior among Stomach Cancer Patients (위암환자의 건강증진행위 및 관련 요인)

  • The purpose of this study is to examine the degree of health promotion behavior among stomach cancer patients and its relevant factors, such as belief, family support, and self-esteem etc. To investigate factors affecting health promotion behavior among stomach cancer patients, a survey was conducted through personal interviews with 153 stomach cancer patients who came to two hospitals in Daejeon from June to July, 2008. Questions were about respondents' general characteristics, properties related to stomach cancer, health belief & family support for the disease, and self-esteem. According to the results of multi-variate regression analysis, factors affecting health promotion behavior included age, treatment type, benefit, barrier, family support, and self-esteem which explained 66.6%. Public health and medical interventions are necessary to improve health promotion behavior among stomach cancer patients by improving benefit, family support, and self-esteem and reducing barrier.

Effect of Health Behaviors on Stomach Cancer Incidence in Resident of Local Area in Korea (국내 일부지역 주민의 건강행태가 위암발생에 영향을 미치는 요인)

  • 본 논문은 국내 일부지역 주민의 건강행태가 위암발생에 영향을 미치는 요인들을 규명하기 위해 시행하였다. 이를 위해 서울에 소재한 한 종합병원에 위암관련 검진을 받기 위해 내원한 환자로 조직병리검사상 위암으로 확진받은 91명을 환자군으로 선정하였다. 대조군도 환자군과 동일한 기간, 동일한 병원 내과에 내원하여 동일한 방법으로 검진한 결과 위암에 속하지 않는 것으로 진단된 환자군과 연령, 성별에 대해 짝짓기하여 선정된 91명이었다. 자료수집은 2008년 12월 20일부터 2009년 1월 30일까지 설문과 면접조사의 방법을 병행하여 진행하였다. 연구결과 첫째, 취미활동량은 취미활동을 전혀 하지 않는 군에 비해 일일 평균 300kcal 미만 또는 그 이상의 열량을 소모하는 군에서 교차비가 각각 0.27과 0.33으로 위암발생 위험을 유의하게 감소시켰다(P<0.05). 둘째, 음주경험이 없는 군에 비해 있는 군(OR=1.85, 95% Cl=1.02-3.36), 총 음주기간이 30년 이상인 경우(OR=5.91, 95% Cl=1.78-19.95)에서 유의하게 높았다. 셋째, 탄음식을 주 3회 이상 섭취할 경우(OR=4.85 95% Cl=1.61-9.84), 짠음식의 섭취빈도가 주 3회 이상인 경우(OR=1.49 95% Cl=0.26-0.91) 위암발생과 유의하게 관련되었다. 넷째, 다중 로지스틱 회귀분석결과 위암발생에 영향을 미치는 요인은 월평균소득, 10년 전 BMI, 취미활동량, 알코올 섭취량, 탄류 및 짠류 섭취였다. 이러한 연구결과를 토대로 적당한 취미활동이 육체적 활동량의 증가뿐만 아니라 건강행동변화를 유도하여 결과적으로 위암예방 효과를 높일 수 있을 것이라 본다.

Analysis of stage III stomach cancer using the restricted mean survival time (제한된 평균 생존시간을 이용한 위암 3기 자료 분석에 관한 연구)

  • The purpose of this study is to compare the effects of treatment on stage III stomach cancer data obtained from the SEER program of the National Cancer Institute and to identify the significant risk factors for the survival rates of stage III stomach cancer. Since the proportional hazards assumption was violated for treatment, we used the restricted mean survival time as an alternative to the proportional hazards model. The restricted mean survival time was estimated using pseudo-observations, and the effects of treatment were compared using a test statistic based on the estimated restricted mean survival times. We conducted the regression analysis using a generalized linear model to investigate the significant predictors for the restricted mean survival time of patients with stage III stomach cancer. We found that there was a significant difference between the restricted mean survival times of treatment groups. Age at diagnosis, race, substage, grade, tumor size, surgery, and treatment were significant predictors for the restricted mean survival time of patients with stage III stomach cancer. Surgery was the most significant predictor for increasing the restricted mean survival time of patients with stage III stomach cancer.
