The Clinical Analysis of Patient with Patent Ductus Arteriosus (동맥관개존증의 임상적 고찰)

  • A clinical analysis was performed on 61 cases of patent ductus arteriosus experienced at Yeungnam University Hospital during 3 years period from Aprial, 1984 to December, 1986. Of the 61 patient of PDA, 13 patient were male and 48 patient were female and age ranged 2 months to 26 years old with the average age of 9.4 years. The chief complaints on admission were frequent URI and dyspnea on exertion. Main diagnostic procedure were Doppler Echocardiogram(in 53 among 61). All of the operations were performed the method of ligation of PDA. Three posterative complication were developed, but there was no operative mortality.

A Remote Medical Treatment System for Stroke Recovery using ZigBee-Based Wireless Brain Stimulator (ZigBee 기반의 무선 뇌자극기를 이용한 원격 뇌졸중 치료 시스템)

  • Stroke patients need regular medical treatments and rehabilitation training from their doctors. However, severe aftereffects caused by stroke allow them minimum activities, which make it difficult for them to visit doctor. Recently, electric brain stimulation treatment has been found to be better way compared to conventional ones and many are interested in using this method for the treatment of stroke. In this study, we have developed a remote medical treatment system using wireless electric brain stimulator that can help the stroke patients to get a treatment without visiting their doctors. The developed remote medical treatment system connects the doctors to the brain stimulator implanted in the patients via the internet and ZigBee communication built in the brain stimulator. Also, the system receives personal information of the connected patients and cumulates the total records of electric stimulation therapy in a database. Doctors can easily access the information for better treatment planning with the help of graphical visualization tools and management software. The developed remote medical treatment system can be applied to the electric stimulation treatments for other brain diseases with a minor change.

  • The radiologic imaging device was implemented. It can be installed in an ambulance or used when an accident happens. After an equipment which generates X-ray generating unit with a tube in DC 12[V] had been made, a trait experiment using an oscilloscope was made. An experiment was carried out where the generated X-ray was saved as a form of a file using a digital detector. In this experiment, as a result of generating X-ray and detecting it using a digital detector, 1.67[MB]-, jpg- radical rays information could be saved. One distinct advantage of the developed radiologic imaging device is the fact that we can efficiently deal with emergency cases too far from the hospital, difficult to diagnose but treat simultaneously. By using the radiologic imaging device at the urgent scene of an accident or in a moving ambulance, we can provide the patient's X-ray information with the emergency medical specialist who is in the emergent medical center and have the patients prescribed and treated appropriately. As a result the developed emergency medical treatment can be expected.

Development of medical/electrical convergence software for classification between normal and pathological voices (장애 음성 판별을 위한 의료/전자 융복합 소프트웨어 개발)

  • If the software is developed to analyze the speech disorder, the application of various converged areas will be very high. This paper implements the user-friendly program based on CART(Classification and regression trees) analysis to distinguish between normal and pathological voices utilizing combination of the acoustical and HOS(Higher-order statistics) parameters. It means convergence between medical information and signal processing. Then the acoustical parameters are Jitter(%) and Shimmer(%). The proposed HOS parameters are means and variances of skewness(MOS and VOS) and kurtosis(MOK and VOK). Database consist of 53 normal and 173 pathological voices distributed by Kay Elemetrics. When the acoustical and proposed parameters together are used to generate the decision tree, the average accuracy is 83.11%. Finally, we developed a program with more user-friendly interface and frameworks.

U-health Service Model for Managing Health of Chronic Patients in Multi-platform Environment (멀티플랫폼 환경의 만성 질환자 건강관리를 위한 유헬스 서비스 모델)

  • U-health services have been progressed as treatment and management for specific diseases and prevention services for providing the behavior management to customers according to the increase in chronic patients. The conventional U-health services provide required services and bio-information monitoring only through remote diagnoses and counsels and that represent limitations in preventing and managing metabolic syndrome patients like chronic patients. Thus, in this study a multi platform based U-health service model for managing the health of chronic patients is proposed. The multi-platform based U-health service model can provide continuous health information, diet, and exercise services regardless of the location of customers through PCs and smart phones. In addition, it is able to provide prescription services to doctors and nurses using a CDS (Clinical Decision Support) module based on clinical information. Doctors can identify the life pattern of patients through a behavior modification program and provide customized services to patients. The U-health service model provides effective services in multi-platform environments to customers and that will improve the health of chronic patients.

  • 목 적 : 두경부 암은 발생 순위에서 전체 6위에 해당하는 다빈도 암이나 최근 20여년 동안의 노력에도 불구하고 두경부 암의 독톡한 특성상 생존률에서 뚜렷한 향상을 보이지 못하고 있다. 특히, 하인두 암은 원발 부위의 점막하 침윤이 흔하며, 주변 림프절 전이와 원격 전이가 흔하고, 2차 원발 암종 발생이 흔하여 두경부 암 중에서도 가장 불량한 예후를 보이고 있는 악성 종양이다. 최근에 이러한 암을 치료하고 진단하기 위한 방법으로 분자생물학적 접근법들이 많이 시도 되고 있으며, 그 중 하나로 N-myc downstream regulated gene-1(Ndrg-1)이라는 유전자가 유방, 전립선, 방광 암 등의 타 악성 종양에서 종양의 전이 및 진행 양상과 관련되어 있다는 보고가 있었다. 이에 본 연구는 하인두 암에서의 Ndrg-1의 발현 양상을 살펴보고 이와 임상 양상과의 연관관계를 살펴보고자 하였다. 방 법 : 1996년부터 2003년까지 수술 받은 하인두 암 환자 56명을 대상으로 면역조직화학검사를 시행하여 Ndrg-1 발현을 확인하였고, 3명의 신선 조직을 대상으로 RT-PCR, Western blot을 시행하였다. 결 과 : Ndrg-1은 RT-PCR에서 정상 조직과 악성종양 조직 모두에서 비슷한 수준으로 발현되었다. 그러나 Western blot에서는 정상 조직에서 뚜렷한 증가 양상을 보여 타 연구와 동일한 결과를 보였고, 이는 불필요하며 비효율적인 mRNA수준에서의 발현이 있지만 최종적인 단백 산물 발현에서는 암종의 진행과 연계되어 악성 종양 진행군에서 발현이 억제되는 결과로 해석된다. 면역조직화학검사에서는 정상 상피조직에서 Ndrg-1 발현이 확인되었으며, 통계적으로 유의하지는 않으나 불량한 예후를 가진 그룹에서 대체로 발현이 억제되는 악성 종양과의 역 연관 관계를 확인할 수 있었고, 특히 림프절 전이를 보인 그룹과 그렇지 않은 그룹 사이에서는 통계적으로 유의미한 결과가 확인되었다. 결 론 : 즉, 림프절 전이가 없는 그룹에서 Ndrg-1이 종양의 전이에 관여할 것이라는 타 연구와 일관된 결과로 하인두 암에서도 그 역할이 있음을 나타내는 결과라 할 수 있다.

  • Parkinson's disease is one of the most frequent neurodegenerative diseases, which mainly affects the elderly. Parkinson's disease is often difficult to differentiate from atypical parkinson diorder such as progressive supranuclear palsy, multiple system atrophy, dementia with Lewy body, and corticobasal ganglionic degeneration, based on the clinical findings because of the similarity of phenotypes and lack of diagnostic markers. The accurate diagnosis of Parkinson's disease and atypical Parkinson disorders is not only important for deciding on treatment regimens and providing prognosis, but also it is critical for studies designed to investigate etiology and pathogenesis of parkinsonism and to develop new therapeutic strategies. Although degeneration of the nigrostriatal dopamine system results in marked loss of striatal dopamine content in most of the diseases causing parkinsonism, pathologic studies revealed different topographies of the neuronal cell loss in Parkisonism. Since the regional cerebral glucose metabolism is a marker of integrated local synaptic activity and as such is sensitive to both direct neuronal/synaptic damage and secondary functional disruption at synapses distant from the primary site of pathology, an assessment of the regional cerebral glucose metabolism with F-18 FDG PET is useful in the differential diagnosis of parkinsonism and evaluating the pathophysiology of parkisonism.

A Study on Analysis of Clinical Data and Telemedicine System for the Treatment of Acrophobia (고소공포증 치료를 위한 원격진료 시스템 및 데이터 분석에 대한 연구)

  • Acrophobia is a symptom of feeling an abnormal fear of heights. Medications or cognitive-behavior methods have been mainly used to treat the acrophobia. In these days the virtua1 reality technology has been applied to treat such an anxiety disorders. In this thesis, an telemedicine assistant system for treatment of acrophobia using biomedical signals and virtual reality technique is proposed. I made two virtual reality simulations for treatment of acrophobia and telemedicine system for communication between doctor and patient using personal computer. A virtual environment provides patient with stimuli which arouses phobia, and exposition to such environment makes him have ability to overcome the fear. Recently, the patient can take diagnosis from a medical doctor in distance with the telemedicine system. Multimedia conference service, on-line questionary, signal transfer system are needed to configure such system. Virtual reality simulation system that composed of position sensor, head mount display, and audio system, is also included in this telemedicine system. I added virtual environment update system to this virtual reality telemedicine system for treatment of acrophobia. Former acrophobia treatment systems use only patient's score of the questionary to appraise. The new system developed in this thesis uses not only patient's score of the questionary but also biomedical signals such as HR, GSR amplitude, GSR RT to increase the objectivity and quantitativity. The experimental results show that HR and GSR amplitude are useful for decision of acrophobia. We will apply this system to the acrophobia patient in distance and be able to offer better medical treatment for mental illness in near future.

Inflammatory Myofibroblastic Tumor of the Lung in a Child -A case report- (소아에시 발생한 폐 염증성 근섬유아세포종 -1예 보고-)

  • Inflammatory myofbroblastic tumor in the lung is a rare tumor. The etiology is not clear. This tumor in children is a benign tumor rarely presented with local invasiveness, recurrence, distant metastasis or malignant changes can occur. The complete surgical resection is chosen as the optimal management. A 12-years-old boy visited the outpatient clinic with a 4 cm sized pulmonary mass in left upper lung field. The patient underwent left upper lobectomy. Histopathologically, inflammatory myofibroblastic tumor was confirmed. The patient was discharged without any problems and there was no evidence of recurrence during 3 months follow-up.

Study on Development of Remote Mental Health Care Program with VR for Seafarers

  • Seafarers play an important role in shipping and logistics. However, seafarers are relatively vulnerable to mental illness because they have to board ships for a considerable period of time and work in isolation. In particular, in the pandemic situation caused by COVID-19, the crew change is delayed due to the closure of many ports around the world, increasing the mental burden on seafarers. The mental health management of the crew is important because these mental problems can lead to major accidents of lives and ships. This paper identified the necessity of mental health management of seafarers through a survey and identified problems with the currently operated mental health management program and curriculum. Especially, this study proposed VR-based programs to help crews receive mentally counseling treatment in a timely manner and to reduce the mental burden on them by preventing sensitive personal information exposure. Through this, it is expected to contribute to the stable development of the logistics industry by establishing a safe seafarers working environment.
