소의 해면형 뇌증(Bovine Spongiform Encephalpathy; BSE)은 우리나라에서의 발생보고는 없으나, 우리에게 그리 생소한 질병만은 아니다. 저자중 한사람인 진 영 화 박사는 일찌기 소 해면형 뇌증의 발견 초창기인 1987년도에 UNDP 가축위생강화계획에 의하여 영국 중앙수의연구소(CVL)에 파견되어 소 해면형 뇌증의 진단병리 연구에 참여한 바 있으며, 강 영 배 박사는 전국 각 시도 가축위생시험소장을 인솔하여 영국 중앙수의연구소를 3차례 다녀온 바 있다. 본편에서는, 1991년 해외출장 기간 중 영국정부를 통하여 입수한, 새로운 질병 즉 소의 해면형 뇌증(Bovine Spongiform Encephalpathy)에 대한 Video tape 자료중, 'BSE 이야기 - 모든 합리적인 대책방안(MAFF V710)'과 'BSE 임상증예 5예 보고(MAFF V659)'를 기본으로하여 사진 또는 영문자료와 함께 소개하고자 한다. BSE의 잠복기는 2년 이상으로 매우 길며, 3-5세의 성우에서 다발한다. 주요 임상증상은, 정신상태, 정서 및 운동의 이상으로 특징지어 지는데, 광증과 침울 등 행동이상을 나타내며, 특히 후지파행, 비틀거림, 미끄러짐, 넘어짐 등의 보행이상을 나타낸다. 초기의 관찰사항으로는, 출입구 통과나 착유실 입실을 거부하는 등 불안 상태를 흔히 나타내는 것이다. 건드리거나 소리에 민감하게 반응하는 지각과민증, 경증의 보행실조, 발로차기, 동물이나 관리자에 대한 공격적 자세, 그리고 공포자세가 흔히 수반된다. 임상소견은 발병기간 중 일관되게 유지되며, 개체별로는 수주일부터 수개월간 지속된다. 젖소에서는 산유량이 떨어지며, 6개월내지 1년 정도 경과후 100$\%$ 폐사되는 것이 특징이다. 치료시도는 전혀 효과가 없다.
Objectives The aim of this study was to examine significant improvement of gait ataxia, vertigo and tremor after treatment with Sipyimigwanjung-tang, Hyangsayangwi-tang in a Soeumin Greater Yin Symptomatology patient Diagnosed as Cerebellar ataxia. Methods The patient was diagnosed with Soeumin Greater Yin Symptomatology and treated with Soeumin's constitutional medications and acupuncture. The primary outcome measures for this study were the Unified Multiple System Atrophy Rating Scale(UMSARS) and Cerebellar function test to assess the overall function of patient. Secondary outcome assessment included Global Assessment Scale(GAS), change of patient's sleep, stool and digestion. Results The symptoms of gait ataxia, vertigo and tremor decreased from GAS 100 to GAS 0~40 after treatment, and the UMSARS score decreased in Part I, II. Conclusions This case showed that Sasang Constitutional medicine treatment can be effective treatment method for cerebellar ataxia. We consider that consistent treatment can contribute to improve the patient's quality of life.
This study reported on the effect of traditional Korean medicine on ataxia, gait disturbance, tremor, and dizziness with cerebellar atrophy. The patient was treated with traditional Korean medicines, such as acupuncture, moxibustion, and herbal medicine (Kuibiondam-tang-gami). The effectiveness of the treatment was evaluated with the scale for the assessment and rating of ataxia (SARA). After treatment, the symptoms had improved. The SARA score decreased by 18 points after Korean medicine treatment. According to this study, traditional Korean medicine can be effective to treat ataxia, gait disturbance, tremor, and dizziness in patients with cerebellar atrophy.
Objective: The aim of this study was to report a clinical case suggesting the potential effects of traditional Korean medicine for the treatment of sporadic spinocerebellar ataxia in a patient with herniated intervertebral lumbar discs. Methods: The patient was treated with a series of Korean medicine treatments including the herbal medicine Zhenganxifeng-tang-gagam, acupuncture, pharmacoacupuncture, and electroacupuncture along with herniated intervertebral lumbar disc treatments. Results: Improvements of Numeric rating scale (NRS) and European Quality of Life-5 Dimensions (EQ-5D) score were observed after the treatment. Conclusion: Traditional Korean medicine has potential benefits for sporadic spinocerebellar ataxia patients with herniated intervertebral lumbar discs.
The autosomal dominant spinocerebellar ataxias (SCAs) are a group of neurodegenerative diseases, clinically and genetically heterogeneous, characterized by degeneration of spinocerebellar pathways with variable involvement of other neural systems. At present, 27 distinct genetic forms of SCAs are known: SCA1-8, SCA10-21, SCA23, SCA25-28, DRPLA (dentatorubral-pallidoluysian atrophy), and 16q-liked ADCA (autosomal dominant cerebellar ataxia). Epidemiological data about the prevalence of SCAs are restricted to a few studies of isolated geographical regions, and most do not reflect the real occurrence of the disease. In general a prevalence of about 0.3-2 cases per 100,000 people is assumed. As SCA are highly heterogeneous, the prevalence of specific subtypes varies between different ethnic and continental populations. Most recent data suggest that SCA3 is the commonest subtype worldwide; SCA1, SCA2, SCA6, SCA7, and SCA8 have a prevalence of over 2%, and the remaining SCAs are thought to be rare (prevalence <1%). In this review, we highlight and discuss the SCA7. The hallmark of SCA7 is the association of hereditary ataxia and visual loss caused by pigmentary macular degeneration. Visual failure is progressive, bilateral and symmetrical, and leads irreversibly to blindness. This association represents a distinct disease entity classified as autosomal dominant cerebellar ataxia (ADCA) type II by Harding. The disease affectsprimarily the cerebellum and the retina by the moderate to severe neuronal loss and gliosis, but also many other central nervous system structures as the disease progresses. SCA7 is caused by expansion of an unstable trinucleotide CAG repeat in the ATXN7 gene encoding a polyglutamine (polyQ) tract in the corresponding protein, ataxin-7. Normal ATXN7 alleles contain 4-35 CAG repeats, whereas pathological alleles contain from 36->450 CAG repeats. Immunoblott analysis demonstrated that ataxin-7 is widely expressed but that expression levels vary among tissues. Instability of expanded repeats is more pronounced in SCA7 than in other SCA subtypes and can cause substantial lowering of age at onset in successive generations termed ‘anticipation’ so that children may become diseased even before their parents develop symptoms. The strong anticipation in SCA7 and the rarity of contractions should have led to its extinction within a few generations. There is no specific drug therapy for this neurodegenerative disorder. Currently, therapy remains purely symptomatic. Cellular models and SCA7 transgenic mice have been generated which constitute valuable resources for studying the disease mechanism. Understanding the pathogenetic mechanisms of neurodegeneration in SCAs should lead to the identification of potential therapeutic targets and ultimately facilitate drug discovery. Here we summarize the clinical, pathological, and genetic aspects of SCA7, and review the current understanding of the pathogenesis of this disorder. Further, we also review the potential therapeutic strategies that are currently being explored in polyglutamine diseases.
본 연구는 일회성 호흡훈련 피드백이 중년여성의 HRV-자율신경시스템 변화에 미치는 영향을 분석고자 하였다. 연구대상자는 중년여성(40-60세) 총 24명을 호흡군 12명, 통제군 12명으로 무선할당하여 일회성 호흡 훈련을 실시하였다. 호흡군의 피드백 훈련은 총 15분 동안 실시되었으며, 10분 간 전문가 지도에 따른 호흡 자각 훈련 후 5분 간 자율호흡 훈련을 실시하였다. 자료의 분석은 SPSS WIN 20.0으로 반복측정 변량분석(Repeated Measures ANOVA)을 실시하였으며, 이를 통해 얻어진 결론은 다음과 같다. 중년여성들은 일회성 호흡 훈련 후 SDNN, RMSSD, LF, HF가 유의하게 높아졌으며, 호흡군이 통제군보다 SDNN, RMSSD, LF, HF이 유의하게 높아진 것으로 나타났고, Mean HR과 LF/HF는 집단과 시기의 주효과, 집단과 시기의 상호작용효과 모두 유의한 차이는 나타나지 않았다. 이상의 결과로 일회성 호흡훈련 피드백은 중년여성들의 SDNN, RMSSD, 교감활성, 부교감활성에 효과적이며, 이러한 호흡 훈련 프로그램은 자율신경계 균형 능력을 향상시켜 중년여성들의 스트레스 완화 및 자율신경실조증을 회복할 수 있는 운동중재의 효용 가치를 기대할 수 있다고 사료된다.
피질하 낭종을 동반한 거대뇌성 백질뇌병증은 뇌백질의 희귀성 질환으로 1990년대에 처음 보고되었다. 이는 자기공명 영상(MRI)에서 미만성의 백질 이상 및 다양한 정도로 발견되는 피질하 낭종과 함께 뇌의 종창을 보이는 것이 전형적이며, 이 병변은 특징적으로 대뇌 측두엽의 전반부를 침범하고, 심부의 뇌백질, 대뇌 기저핵, 시상과 소뇌는 상대적으로 보존되는 경향을 보인다. 피질하 낭종을 동반한 거대뇌성 백질뇌병증이 있는 환아는 큰 머리와 함께 운동 장애, 실조증, 강직과 인지 기능 장애 등의 신경학적 결함을 보인다. 저자들은 큰 머리와 함께 경련, 발달 지연, 뇌 MRI 의 T2 강조영상에서 양측성으로 전두, 측두, 두정엽에서 고신호강도의 백질 병변을 보이는 피질하 낭종을 동반한 거대뇌성 백질뇌병증에 합당한 소견을 보이는 20개월의 남자 환아를 경험하였기에 보고하는 바이다.
목 적 : Ataxia-Telangiectasia (AT) 증은 여러 가지 유전적 결함을 갖는 질병으로 방사선 민감도가 비정상적으로 상승되어 있는 것이 특징이다 AT 환자에서 공통적으로 존재하는 ATM 유전자는 현재까지 방사선 신호전달에 관여하는 것으로 알려진 Pl-3 kinase와 유사한 구조임이 알려져 ATM이 방사선 신호전달경로에 중요한 작용을 할 것으로 추정하게 되었다. 본 연구에서는 AT 세포와 정상세포에 PKCI를 과발현 시킴으로써 방사선 신호전달에 관여하는 PKC를 억제하여 이것이 방사선 민감도에 미치는 영향을 관찰하고, 방사선에 의해 유도되는 early response gene인 c-fos transcription의 차이를 측정하여 ATM과 PKCI에 의한 신호전달이 c-fos 유전자 전사에 미치는 영향을 분석하고자 하였다. 대상 및 방법 : PKCI expression vector를 작제한 후 정상세포인 LM217과 AT세포인 AT5BIVA에 transfection 시킨 후 plasmid의 genomic DNA에 결합된 것은 polymerase chain reaction (PCR) 방법으로 확인하였고 PKCI의 mRNA 발현 여부는 northern blotting으로 확인하였다. 방사선 민감도는 아포토시스로 측정하였으며 PKCI가 과발현된 각 세포주에 5 Gy의 방사선을 조사한 후 48시간에 세포를 모아 TUNEL방법으로 아포토시스 세포의 수를 측정하였다. c-fos 유전자의 전사는 reporter 유전자로 c-fos CAT plsmid를 $\beta$-gal expression vector와 같이 각 세포주에 transfection 시키고 36시간이 지난 후 CAT assay를 하여 activity를 측정하고 동시에 $\beta$-gal assay를 시행하여 transfection 효율을 보정해 주었다. PKCI, Ras의 영향을 보기 위하여는 PKCI, Ras expression vector와 c-fos CAT plasmid를 cotransfection하고 CAT activity로 측정 하였다. 결 과 : 이 실험의 결과 LM과 AT 세포에서 PKCI가 방사선 민감도에 미치는 영향과 c-fos 전사에 미치는 영향을 처음으로 보여주었다. PKCI의 과발현이 LM 세포에서는 방사선 민감도를 증가시켰지만 AT세포에서는 오히려 약간 감소시키는 작용을 나타내었다. c-fos 전사는 AT 세포에서 LM 세포에 비하여 70배 낮게 나타났는데 PKCI가 과발현 됨으로써 LM 에서는 c-fos의 전사가 감소되었지만 AT 세포에서는 영향이 없었다. Ras 단백으로 c-fos를 유도시키고 여기에 PKCI 발현 백터를 contransfection 하면 LM세포에서는 induction 이 감소되었지만 AT 세포에서는 영향이 없었다. 즉 LM과 AT 세포에서의 PKCI에 의한 반응의 차이는 Ras와 관련된 signal transduction pathway라는 것을 알 수 있었다. 결 론 : PKCI는 정상세포에서는 방사선에 의한 세포 손상을 증가시키지만 AT 세포에서는 별 영향을 보이지 않는 것을 알 수 있었으며, 두 세포간의 이러한 차이는 c-fos proto-oncogene의 전사차이로 설명할 수 있겠다. 이러한 차이가 AT 세포의 방사선 민감도의 한 원인일 것으로 생각된다.
