Retrospective Study of Survival Rates According to the Type of Dental Restoration of Proximal Caries in Primary Molars (치과용 수복재료에 따른 유구치부 인접면 수복물의 생존율연구 : 후향적연구)

  • Restorative dental materials have advanced rapidly, with improved physical properties that improve survival rates. Accordingly, various materials can be selected. Amalgam, composite resin, glass-ionomer cement, and preformed stainless steel crowns have all been used widely for the restoration of dental caries in primary molars. The various dental materials used to treat proximal caries in the primary molars have distinct advantages and disadvantages. However, few studies have examined their survival rates. This retrospective study examined the 2-year survival rates of more than 700 class II restorations of proximal caries in primary molars clinically and radiologically according to the type of restoration. The study results should help in the selection of class II restorations for molars, one of the biggest concerns of pediatric dentists.

  • The radicular cyst is the most common odontogenic cyst which is caused by pulpal inflammation, pulp death, and secondary to trauma or dental caries. Usually, the radicular cyst is asymptomatic, but a secondary inflammation can cause pain, swelling and redness. Getting larger, the radicular cyst can cause facial asymmetry and paresthesia by pressure on nerves. It requires conservative endodontic treatment or surgical approach. When the size of cyst is large or invasion of the adjacent tissue is not expected, cyst enucleation is carried out. And most of the case can be completely cured and shows low recurrence. In these radicular cysts cases, by cyst enucleation or apicoectomy after root canal treatment simultaneously, the infected teeth can be preserved successfully.

  • Patients who lost molars from dental caries or periodontitis have difficulty in maintaining their vertical dimension because their incisors and premolars also show vertical tooth movement. Missing posterior molars leads to occlusal interference and collapse of occlusal plane due to extrusion of opposite teeth and tipping of adjacent teeth. When this condition persists over long time, mandible moves forward, so it causes mobility and labioversion of upper incisors. This progress cannot be stopped spontaneously and the situation gets worse and worse. Therefore, for a patient with pathologic teeth migration caused by chronic periodontitis, interdisciplinary approach which is including periodontal treatment, orthodontic teeth alignment and prosthetic restoration of missing molars is required. This case report shows successful multidisciplinary approach to a patient who lost vertical dimension due to severe dental caries and periodontitis.

THE INFLUENCE OF pH ON THE FORMATION OF ARTIFICIAL ROOT CARIES IN ACID BUFFER SOLUTION (산 완충용액의 pH가 인공치근우식의 형성에 미치는 영향)

  • Recently root caries is reported as an increasing oral disease Many researchers studied in vivo and in vitro enamel lesions but due to its peculiar structure and different chemical composition compared to the enamel, little effort has been made to root caries study. The purpose of this study is to compare and to evaluate the effect of pH on the progression of artificial root caries lesion. Under the constant degree of saturation, the specimens were divided into 4 groups(pH 4.3, 5.0, 5.5, 6.0). Each group was immersed in acid buffer solution for 1, 3, 5, 7 days and examined using the polarizing microscope. The results were as follows: 1. The body of the lesion in the dentin showed higher degree of positive birefringence compared to sound dentin. 2. The rate of progression of the lesion slowed as the pH increased. 3. In all groups, the lesion progressed rapidly at the initiation or the experiment but increased gradually as time elapsed. In conclusion, the study shows that the pH in the acid buffer solution influenced the rate of progression of the lesion in artificial root caries. Compared to the previous enamel study, the initial pattern of the lesion progressed rapidly and as time elapsed increased gradually.

Antibacterial Effects of Extracts from Citrus Peels (귤피 추출물의 항균효과)

  • It has been reported that dried orange peel in a medicine prepared from crude drugs has antioxidant, anti-inflammatory, anti-allergic, anti-bacterial, and anti-viral functions, prevents circulatory diseases, reduces lipids, improves immunity, and strengthens capillary vessels. This study attempted to identify the antibacterial effects of dried orange peel extract on S. mutans causing dental caries, and its effects on GTase activation. The extract was put on the medium at the concentration of 5, 10 and 20 mg/ml and its effects on the inhibition of S. mutans growth were examined. Analysis found that the higher the concentration was, the more the number of colonies was reduced. In accordance with the measurement after 8 hours, the extract displayed a high growth inhibition rate: 92% at a concentration of 5 mg/ml, 95% at a concentration of 10 mg/ml and 99% at a concentration of 20 mg/ml. Furthermore, the GTase deactivation rate by the extract was measured. The higher the concentration of extract was, the more rapidly the deactivation rate increased. The deactivation rate of the extract was significant at 42% at a concentration of 5mg/ml.

  • Dental caries is the most common oral disease. There are many factors contributing to its development, but complete understanding and prevention are not fully known. However, it is possible to remineralize the early enamel curious lesion by fluoride containing remineralization solution. Recently the pH-cycling model has been used to examine the effect of fluoride solution on remineralization of artificial caries in vitro as it can closely simulate the conditions encountered in vivo within a carefully controlled environment. The aim of this study was to evaluate the remineralizing effects of supersaturated buffer solutions under pH-cycling model. The specimen with 3mm-diameter was made using mature bovine incisors which has no caries and has sound enamel surface. Early curious lesions were produced by suspending each specimens into demineralization solution at pH 5.0 for 33 hours and the specimen whose surface hardness value ranged from 25 to 45 VHN were used. The pH cycling treatment regimen consisted of 5 min soaks of three treatment solutions four times per days for 15 days and the continuous cycling of demineralization and remineralization were carried out for 15 days. Following the pH-cycling treatment regimen, the specimens' surface microhardness were measured by the Vickers hardness test (VHN) and analyzed by ANOVA and Duncan's multiple-range test. 1. The surface microhardness value of supersaturated solution, Senstime, and Gagline groups were increased after pH cycling, and that of supersaturated solution was significantly Increased compared to saline group(P<0.05). 2. The surface remineralization effect of fluoride containing solutions was accelerated by saliva under pH-cycling mode 3. The pH cycling model was considered appropriate to mimic the intra-oral pH changes when evaluating demineralization and remineralization in vitro. Under the results of above study, salivary remineralization effect can be improved by fluoride containing remineralization solution.   • 목적 - 최근 복합레진의 미세변연누출을 줄이기 위한 새로운 광중합 조사방법이 연구되고 있다. 또한 구치용 복합레진이 개발되면서 그 제조회사에서는 5mm 깊이까지 광중합이 가능하다고 소개하고 있다. 본 연구에서는 논리적 가설에 근거한 몇가지 광중합 조사방법이 구치부 class II 와동의 복합레진 충전시 미세변연누출에 어떠한 영향을 미치는지를 관찰하였다. 재료 및 방법 - 100개의 우식증이 없는 사람의 상하악 대구치를 사용했다. 각 치아에 교합-치은방향으로 4mm, 협설 4mm, 깊이 2mm의 class II 와동을 형성하여 인접치와 함께 베이스플레이트 왁스에 매몰하고 구치용 복합레진인 Surefil을 제조 회사의 지시대로 충전하였다. 이때 5가지의 광조사방법을 이용하여 5개의 군으로 나누어 중합하였다(Table 1). 수복된 치아들을 5$^{\circ}C$와 55$^{\circ}C$의 수조에서 번갈아 1분씩 총 500회의 온도변화를 주어 thermocycling을 실시한 후 근첨을 폐쇄하고 수복와동의 치은경계를 제외한 전 표면에 nail varnish를 2회 도포하였다. 2%의 methylene blue용액에 24시간 침적시키고 흐르는 물에 세척한 후 시편을 투명한 에폭시 레진에 매몰하였다. 매몰된 시편을 치아 장축에 평행하게 절단 연마하였다. 입체 현미경으로 미세변연누출을 관찰하고 Kruskal-Wallis One Way ANOVA 와 Dunn's Method로 통계처리 하였다. 결과 - 1. 미세누출은 1군, 4군과 5군, 2군 3군 순으로 증가였다. 이때 1군, 4군과 5군은 통계적 유의성이 없었다(P>0.05). 2. 2군의 경우 1군, 4군, 5군에 비교하여 유의성 있게 미세누출이 많았으며(P<0.05) 3군에 비하여 유의성 있게 적게 나타났다(P<0.05). 3. 3군의 경우 다른 방법들에 비교하여 통계적으로 유의성 있게 미세누출이 많았다(P<0.05).

  • Shortened dental arch (SDA) as a treatment goal is the concept that stable occlusion and enough masticatory force can be achieved by restoration to the second premolars when the situation is not favorable. SDA could be applied both natural teeth and implant supported fixed prostheses. This case dealt with a patient who has grade 2 intellectual disability and a lot of missing teeth. Because of intellectual disability, patient cooperation during treatment could not be expected. Therefore every treatment should be done under general anesthesia. In addition to that, ridge resorption around molar area was severe and there were maxillary sinus pneumatization and maxillary sinusitis which increased failure probability. SDA concept was adopted to reduce risk factor and minimize general anesthesia. After the treatment, functional and esthetic improvement was achieved and oral hygiene was fortified by periodic recall check and education.

A CLINICAL STUDY FOR CARIES ACTIVITY OF PRESCHOOL CHILDREN USING CARIOSTAT (Cariostat를 이용한 아동의 우식활동에 관한 임상적 연구)

  • The purpose of the study was to determine the ability of the caries activity test, 'Cariostat' to identify preschool children with dental caries. The subjects of this study were 76 children of 3 to 5 years of age. Oral examination was carried out to assess the caries experience using a mirror and explorer. In all subjects buccal plaque samples were obtained, incubated, and scored as the manufacturer's instructions for the Cariostat test. Statistical analyses were used with the Crosstabulation. The test group exhibited mean dft index 5.21, while 27.63% of subjects were caries free and had no restorations. The sensitivity of the Cariostat test was found to be 98.8% while the specificity was found to be 19.05% The results showed that caries activity measured by visual reading was highly significant to the Cariostat score and dft index(P<0.001). The Cariostat method has indicated the future possible occurrence of caries. The use of this method is meant to require in the caries prevention program.

  • Pit and fissure sealants has been proven as safe and effective method of caries prevention. But improper application of pit and fissure sealant may masking the caries process in occlusal fissure. Dentists have to understand the correct and exact application method of pit and fissure sealant for he good result of caries prevention. A key factor to enhanced the effectiveness of caries prevention is retention of pit and fissure sealant. Deep penetration of material into fissure and least marginal leakage around the fissure orifice are the major concerns for dentists to achieve the successive application of pit and fissure sealants. This paper reviewed the literature on the pit and fissure sealants under the following subtitles for enhanced retention : (1) application timing, (2) Indication, (3) Occlusal prophylaxis, (4) Materials, (5) Penetration, (5) Recall check Dental profession must perform the exact application of pit and fissure sealant because this procedure is one of the most technique-sensitive one in dental field.
