DENTAL MANAGEMENT OF THE PATIENT WITH BILIARY ATRESIA : A CASE REPORT (담도폐쇄증 환아의 치과치료 : 증례보고)

  • Congenital biliary atresia with progressive sclerosis of the intra- and extra-hepatic duct system occurs in 1 : 10,000 live births, and has a poor prognosis with an expected survival of less than 5 years. Etiology of biliary atresia is unclear, however, it is believed a genetic or developmental cause. The clinical characteristics include pronounced jaundice, hepatosplenomegaly, pruritus, steatorrhea, xanthomas, growth retardation, portal hypertension, bleedings, ascites and respiratory infections. Oral manifestations have seldom been reported in patients with biliary atresia, but there may be enamel hypoplasia, delayed tooth eruption, and green teeth. Early diagnosis and surgical intervention have decreased morbidity. returned growth and development to normal and improved the prognosis for survival. Authors report the clinical and radiologic characteristics, proper managements about two cases with biliary atresia.

A Retrospective Study of Critical Success Factors in Regenerative Endodontic Treatment (재생근관치료의 성공에 영향을 미치는 결정 요인에 대한 후향적 연구)

  • This study evaluated critical factors influencing the success of regenerative endodontic treatment for necrotic immature permanent teeth. The study enrolled patients who had regenerative endodontic treatment in a necrotic immature permanent tooth through 1-year follow-up. Possible explanatory variables related to the success rates for 46 teeth were age, gender, dental stage at the initial appointment, etiology, treatment information, and clinical and radiographic outcomes. The dental stage at the time of regenerative endodontic treatment did not influence the success rate. The success rate was significantly related to the etiology of the necrotic immature tooth. The success rate according to etiology decreased in the following order: dens evaginatus, caries, and trauma. There was also a significant difference according to the type of antimicrobial. The success rate was influenced by appropriate disinfection of the root canal system, provision of a scaffold, and coronal sealing. Above all, careful case selection is necessary for treating a necrotic immature permanent tooth, especially in cases of trauma.

Antimicrobial Effect of Photodynamic Therapy Using Plaque Disclosing Agent (치면착색제를 이용한 광역동 치료의 항균 효과)

  • The aim of this study was to evaluate the antimicrobial effect of photodynamic therapy (PDT) using plaque disclosing agent, 10 - 20 mM erythrosine, as a photosensitizer. Multispecies cariogenic biofilms containing Streptococcus mutans, Lactobacillus casei and Candida albicans were formed on hydroxyapatite disc. 20 μM, 10 mM and 20 mM erythrosine were applied as a photosensitizer for 3 minutes, and then light-emitting diode (LED) irradiated for 24 seconds. Colony-forming unit (CFU) were measured and biofilms were observed using confocal laser scanning microscopy (CLSM). CFU were significantly decreased in the PDT groups using 10 - 20 mM erythrosine (10 mM, 20mM) and the results were also confirmed by CLSM. This study confirms the high antimicrobial effect of photodynamic therapy using plaque disclosing agent as a photosensitizer.

Bacteriological contamination of toothbrushes by dental plaque acidogenicity and related behaviors to toothbrush use (치면세균막 산생성도 및 칫솔관리행동별 칫솔 세균오염도)

  • One of the best way to prevent dental caries and periodontal disease is removing dental plaque. Toothbrushing is also one of the crucial ways to gel rid of dental plaque and improve dental hygiene. The purpose of th is study was to examine how many bacteria were attached to toothbrushes used by dental palque acidogenecity and related behaviors to toothbrush use. Subjects in this study were 30 college students in their twenties, on whom a survey was conducted to find out how they cleansed their toothbrushes, how long they cleaned toothbrushes after brushing, and how to keep them. And then dental palque acidogenecity by caries activity test was implemented and the number of bacteria attached to their toothbrushes was tested. The findings of the study were as follows : 1. Concerning correlations between dental palque acidogenecity and the number of remaining bacteria in the toothbrushes used by students, the number of bacteria was lower in the group of students with no caries activity than in the group with mild caries activity. 2. As for relations between the way of cleansing toothbrushes and the number of bacteria, there was no statistically significant gap between the group of students who wetted their toothbrushes with water and rubbed the heads of them and the group of students who just only wetted their toothbrushes with no further actions. 3. Regarding the relationship of toothbrush cleansing time to the number of bacteria, the number of bacteria was lower in the group of students who cleansed their toothbrushes for 15 seconds or more. 4. As to the influence of the number of toothbrush used by students, there was no significant difference between the group using just one toothbrush and the other group using two or more toothbrushes. 5. As for correlations between the way of keeping toothbrushes and the number of bacteria, no significant gap existed between the group thai kept their toothbrushes separately and the other that didn't.

Effects of a School - Based Oral Health Care Program on the Prevalence of Dental Caries in Primary School Children (학교구강보건사업이 초등학교 아동들의 유치 및 영구치 우식실태에 미치는 영향)

  • Purpose: The aim of this study was to evaluate the effects of oral health care programs in 3 school-based oral health care center among primary schoolchildren. Methods: School-based oral health care programs included fluoride mouth rinsing, pit and fissure sealing for permanent premolars and molars, fluoride gel application and chewing of xylitol candy. All of the programs were carried out by one dental hygienist among 'D' primary schoolchildren in Daegu city under the supervision of a dentist. Baseline dental examinations were completed and preventive care was implemented for 544 children during one year. All of the children visited a school-based oral health care center every three months for a regular check-up. The final oral examination was conducted from March 15 to April 1, 2004. The data analysis data was made on the basis of SAS 8.01. Mean differences between 2003 and 2004 data were compared by paired t-test. Corresponding p-values were considered significant at values less than 0.05. Results: The DMF rate and DFT index were reduced to 8.0% and 8.4% during one year respectively, but there were no statistically significant differences. The DMF rate was significantly reduced (16.3%) after a one year program of school-based oral health care practice. The DMFT(Decay Missing Filling Tooth) index was also reduced compared to 2003 throughout the entire grade. Conclusion: School-based oral health care programs can reduce the prevalence of dental caries prevalence among schoolchildren during one year. This program also improved the oral health capacity of schoolchildren. It is recommend that the school-based oral health care program should be extended to every primary school in Korea.

A STUDY OF SALIVARY IMMUNOGLOBULIN AND DENTAL CARIES (타액 면역글로부린과 치아우식의 상관관계에 관한연구)

  • This study was undertaken to observe the relationship between salivary IgA, IgG, agglutinin titer & dental caries. The subjects divided into two groups, an active caries group (AC group) and caries-free and treated caries group (CFTC group). The AC group consisted of 36 subjects who had one or more carious lesions and the CFTC group of 12 subjects who had no evidence of caries or had filled teeth without present carious teeth for the last six months. The IgA, IgG and IgM levels in their saliva were measured by single radial immunodiffusion method using a disposable low-level immunodiffusion plate. The salivary agglutinin titers to Streptococcus mutans were measured by microtitration system. The results were as follow; 1. The mean value of IgA concentration in saliva of AC group was slightly higher than that of CFTC group, but its difference was slight. 2. The mean value of IgG concentration in saliva of AC group was slightly lower than that of CFTC group. The IgM concentration in saliva of both groups was neither below 1.1 mg/dl nor detected on LC partigen immunodiffusion plate. 3. There was no difference in the agglutinin titer to S. mutans antigen by serotypes, but low level agglutinin to type d was measured in both groups. 4. AC group showed low correlation between IgA, IgG & DMFT, but CFTC group revealed negative correlation. 5. The relationship between salivary IgA & agglutinin titers to S. mutans was low correlation in AC group, but CFTC group showed significant positive correlation. (P<0.05) 6. There were no specific correlations among the concentrations of salivary immunoglobulins, agglutinin titers to S. mutans, and the DMF teeth. They had no close concern to induce the dental caries.

Caries Preventive Effects on Permanent Teeth by Fluoride Mouthrinsing Program in Elementary School (초등학교 불소용액 양치사업의 영구치 우식예방 효과)

  • Objectives: This study was conducted to find out caries preventive effect on permanent teeth among children who brush teeth with fluoride-containing toothpaste by supervised weekly fluoride mouthrinsing program in elementary schools. Methods: The epidemiologic dental survey was performed on the elementary schoolchildren of supervised weekly fluoride mouthrinsing program (FMR) with 0.2 percent neutral sodium fluoride solution and control group in 2007. Caries preventive effect on permanent teeth by fluoride mouthrinsing program were calculated by DMFT index and DMFS index between FMR group and control group. Results: By DMFT index between FMR group and control group, caries rates on permanent teeth of the fourth-, fifth- and sixth-grade children participating in FMR program were 34.1%, 40.8% and 31.5%, respectively. By the DMFS index between FMR group and control group, caries preventive rates on permanent teeth of the fourth-, fifth- and sixth-grade children participating in FMR program were calculated 25.4%, 37.7% and 33.5%, respectively. Conclusions: We suggest that fluoride mouthrinsing program should be developed to all elementary schools to prevent dental caries.

Factors Related to Dental Caries in Deciduous Teeth of Preschool Children (미취학아동의 유치우식 관련 요인)

  • Objectives: This study was taken to investigate factors related to dental caries in deciduous teeth of preschool children. Methods : this study was completed by oral examination on 623 kindergarten pupils and questionnaire on their mothers in Sangju city from April 1 to May 20, 2001. Study design is Cross-sectional Study. Results: The mean values for decayed teeth indexed(dt index), filled teeth index(ft index), and decayed and filled teeth index(dft index) was 2.46, 1.85, and 4.30 respectively. 2.05 for girl was significantly lower than 2.77 of boy in gender (p<0.01). As child ren got older, values were higher(0.7 and 2.84 of three years old, 1.6 and 4.01 of four, 2.42 and 5.02 of five) in ft and dft index(p<0.001). Also, Mother age was almost same, that is, 4.12 dft value for 30~34 years was smaller than 6.17 for above forties(p<0.001). 2.07 and 4.68 for mothers who graduated from high school were higher than 1.36 and 3.34 for university in the ft and dft index(p<0.05), that is, the more they had a high educational level, the lower they got a values. Value 2.76 for children who had non cariogenic food was lower than 5.11 for those who had cariogenic food in dft index(p<0.001). In dft index, 3.82 of children who have learned the education on oral health from mother frequently have lower than 45 of children who have never learned (p<0.05). The ft value of children who had a snack food with mother(1.29) was a smaller than children had it at out of home(1.97)(p<0.05). The variables related 10 dft index were children and mothers age, educational level, and kinds of snack food (p<0.05). Conclusion : This study reveals that younger girl, younger mother, higher educational level, and non cariogenic foods showed lower dft index.

Perception and attitude for the expansion of the adjusted water fluoridation program in Gimhae, Korea (수돗물불소농도조정사업 확대 시행에 대한 김해시민의 의식)

  • Objectives : The purpose of this study was to obtain the information for the improvement of the support in oder to expand the adjusted water fluoridation program to Myeong-dong water treatment plant in Gimhae city, Korea. Methods : Questionnaires were distributed to the parents of elementary schoolchildren and their responses were collected. All collected data were analyzed using the SPSS program. Results : The percentage of respondents using the tap water was 47.0% for drinking and 54.4% for cooking, respectively. The percentage of respondents using the water purified the tap water with the home water purifier was 45.1% for drinking and 36.2% for cooking, respectively. Respondents who knew the adjusted water fluoridation program was 48.3%. Respondents who knew the implementation of the adjusted water fluoridation program at the moment in Gimhae was 27.0%. 90.2% of residents agreed on the expansion of the adjusted water fluoridation program to Myeong-dong water treatment plant, Significant variables in the approval rates of the adjusted water fluoridation program were age, residence period, occupation, recognition of the program. Conclusions : It is desirable to expand the adjusted water fluoridation program to Myeong-dong water treatment plant in Gimhae as the percentage of respondents who agreed on the introduction of the program was high.

Effects of school incremental dental care program (학교구강보건실 운영 효과에 관한 조사)

  • Objectives : This study was conducted to purpose the fundamental data for further developments of the school dental clinic program(SDCP) through comparing results of oral health promotion program for 4 years relating to compare oral health status of children who had experienced and had not experienced the program. Methods : The subjects were 66 children of elementary school in Busan Metropolitan City who had benefited school dental clinic program(SDCP) from 2004 to 2007 and control group was 98 children of elementary school who had not benefited school dental clinic program(SDCP). Data analysis were done with SPSS program (ver 13.0) through basic statistical data, t-test. Results : The DMF rate of elementary school students who had benefited program was 41.5% and it was lower than that of control group(52.5%), however it was not significant statistically. The DMT index of elementary school students who had benefited program was 1.81 and it was lower than that of control group(2.44)(p<0.05). The DMFT rate of elementary school students who had benefited program was 4.44% and it was lower than that of control group(7.45%)(p<0.05). Conclusions : In oder to promote oral health of children, school dental clinic in elementary school should be extended to more elementary schools as possible and the operating system should be changed to improve the efficiency. Oral health educational materials should be developed and utilized to improve educational effect.
