Degree of Interest for Dental Caries Prevention and Child's Oral Health Behaviors in Korean Adults: Gallup Survey (한국 성인의 우식예방 인식과 자녀의 우식예방 실천행위조사: 한국갤럽자료분석)

  • This study was to evaluate the influence of dental caries prevention-related concerned and perceived in Korean adults and oral health behavior of their first child if they had children. A cross-sectional study in 1,014 adults over 19 years old were conducted in Korean adults, asked to answer a computer-assisted telephone interview regarding their oral health-related knowledge, behavior, concerned of caries prevention and child's behaviors such as frequencies of tooth brushing, chewing the xylitol gum and dental checkup. The statistical analysis was done by using the SPSS 19.0 program (IBM Co., Armonk, NY, USA). Awareness of caries prevention in the female than male, 40~49 years old age group was higher. Parents of school-age children were higher level of awareness of the dental caries prevention than other group. As knowledge and behavior of dental caries prevention were higher, the child's conduct regular oral health checkups, fluoride topical application and dental sealant was significantly higher. The higher the concerned of caries preventive, the intention of oral health behaviors and child's oral health behaviors was more increased. Awareness of caries prevention had effect on the perception of the impact of the child's oral health behaviors.

  • Early childhood caries prevalence has increased significantly in children ages 2~5 years. ECC disproportionately affects lower socioeconomic and minority groups, is a predictor for future decay, but is preventable and manageable. Caries risk assessment systematically derives a patient's caries risk and is important during an infant oral health visit beginning at age 1. Information obtained through a risk assessment can guide a disease management care path tailored to an individual's age and risk to effectively treat and manage one's caries disease process.

  • 지난 20세기 동안, 치아 우식에 대한 치료는 G.V. Black이 제안한 '예방 확대' 개념에 따른 와동형성 원칙에 따라 외과적 치칠 삭제의 형태로 수행되어져 왔다. 즉, 치아 우식 병소에 발견되면 치과의사들은 망설임 없이 핸드 피스를 들고 우식 부위 치질을 삭제하며, 수복재료에 따라 적합한 형태로 건전한 치질을 삭제하여 수복 와동을 형성하였다. 그러나 치아 우식에 대한 많은 연구 결과, 원인균과 산 생산 및 탈회와 재석회화 등에 대한 정보가 알려졌고, 접착 수복이 비약적 발전을 하여 임상 적용이 일반화되면서, 지난 10여년 이래 치아 우식에 대한 치료 개념이 점차 최소 치질 삭제와 건전 치질 보존으로 전환되어 왔고 이러한 개념을 통칭하여 Minimal Intervention Dentistry라 하고 있다. 21세기에는 치아 우식에 대한 치료가 minimal intervention dentistry의 개념 하에 점차 폭넓게 임상에 적용될 것으로 예상되며, 다음과 같은 원칙들이 제시되고 있다.(중략)

  • Current treatment concept of dental caries has been changed, because it has been proved that it is a preventable disease. The philosophy has been changed from purely restorative treatment to preventive caries control. Therefore the methods or criteria of oral examination has been changed. The clinician have to detect not only cavitation, but also the lesion of non-cavitation stage. International Caries Detection and Assessment System II (ICDAS II) was developed recently, which is a new criteria of classification of dental caries. This system was based on the current concept of prevention, early detection and patient-centered management of caries. Therefore this philosophy is in accord with the perspective of pediatric dentistry. The purpose of this article is to introduce this system for oral examination of children.

Trends of Dental Caries Prevalence in Children Under 14-Year-Old Using a Health Insurance Database (건강보험 데이터를 이용한 14세 이하 소아청소년의 치아 우식 유병률 경향성)

  • The purpose of this study is to analyze trends in the prevalence of dental caries and demand for dental caries treatment among children under 14 years old using Health Insurance Review and Assessment data. The analysis was conducted using treatment records from a random sample of approximately 1 million pediatric patients from a population that included all children and adolescents for each year from 2011 to 2020. In this study, the number of children diagnosed with K02 dental caries and the number of children receiving dental caries treatment across all ages have increased. However, the number of children aged 10 to 14 who received pulp treatment or extraction has decreased. In the National Survey of Children's Oral Health, the decay-missing-filled teeth index for 5- and 12-year-olds has stagnated or increased slightly, but the percentage of the population with active dental caries has decreased. Accessibility and local environments for dental caries treatment have generally improved compared to the past, but preventive dental care has stagnated over the past decade. Therefore, it is necessary to evaluate the effectiveness of oral health programs implemented in Korea to promote and prevent dental caries among children.
