The Effects on 3-Dimensional Exercise of Lumbar Stabilization for Chronic Low Back Pain (3차원 요부 안정화 운동이 만성요통에 미치는 효과)

  • Purpose: This study is intended to evaluate the effects of lumbar stability after 3-dimensional exercises of lumbar stabilization. Methods: Total subjects with chronic low back pain(age ranged from 20 to 60) were recruited. All subjects received 3-dimensional exercise of trunk stabilization during 4 weeks in 00 orthopedic clinic, from May 15 to October 15 in 2006. All measurements of each patients were measured before and after lumbar stabilization exercise. Results: After lumbar stabilization exercise by CENTAUR(R), the muscle power was increased from 57.99kNm to 72.01kNm, there were statistically remarkable differences(p<0.05). VAS was lessened from 6.35 to 3.26, there were statistically remarkable differences(p<0.05). After lumbar stabilization exercise by CENTAUR(R), the temperature was increased from $27.68^{\circ}C$ to $28.26^{\circ}C$, there were remarkable differences statistically(p<0.05). Conclusions: It has been turned out that lumbar stabilization exercise has positive effect on the muscle strengthening, pain index and thermal change.

The Effects of Spinal Stabilization Exercise using Gravity on patients with Degenerative Disc Disease (중력을 이용한 요부안정화 운동이 만성요통을 가진 노인환자에게 미치는 영향)

  • Purpose: The purpose of this study was finding out the effects of spinal stabilization exercise using Centaur which is a 3D spinal stabilization sports implement on Chronic low back pain patients over 8 weeks. Methods: 30 patients with DDD were observed during the study. Their average age was 66.88years, height was 152.12cm and average weight was 58.91kg, 4 males and 26 females were involved. 8 various investigations were performed and varied values were compared with reinvestigation done after having exercised 8 weeks using 3-D CENTAUR We used VAS(visual analog scale) in order to see the variation of pain intensity, MOI(modified oswestry index) in order to see limitation of daily life. Results: VAS was lessened from 7.57 to 2.63, limitation of routine life(MOS) from 23.48 to 11.30, there were remarkable differences statistically(p<0.05). As a result of muscular investigation for static spinal stabilization by 8 variations of body deflection, muscular strength were all increased and there were signigicant differences statistically(p<0.05). Conclusion: It has turned out that pain and limitation of routine life was lessened, as a result of 8 weeks exercise using CENTAUR, and deep muscular power was increased. Thus it has turned out that 3-D spinal stabilization exercise has an effect on the strengthening spinal muscles and alleviation of their pain for old patients with DDD.

  • Dysfunction of the anterior and dorsal muscles of the trunk have been studied in relation to low back pain of many years. Many muscles of the trunk are capable of contributing to the stabilization and protection of the lumbar spine, recent evidence has suggested that transversus abdominis may be critically involved and has been the focus of rehabilitation. The delay in onset of contraction of trunk muscles associated with movement of the upper or lower limb in patients with low back pain indicates a significant deficit in the automatic motor command for control of disturbance to the spine. The function of transversus abdominis has been largely ignored in the evaluation of spinal stabilization and protection. The most essential stabilizing muscles for the lumbar column are the transversus abdominis and the multifidus. Sling exercise therapy(SET) concept consists of a system of diagnosis and treatment. The system of diagnosis involves testing the muscle's tolerance through progressive loading in open and close kinetic chains. The SET system contains elements such as relaxation, increasing the range of movement, traction, training the stabilizing musculature, sensorimotor exercises, training in open and close kinetic chains, dynamic training of the mobilizing musculature, cardiovasc+ular exercises, group exercise, personal exercise at home. Sensorimotor training is an essential element of the SET concept. The emphasis is on closed kinetic chain exercises on an unstable surface, thereby achieving optimum stimulation of the sensorimotor apparatus.

The Effects of Lumbar Stabilization Exercise on Muscle Activity and Isokinetic Muscle Strength of Female Patients with Chronic Low Back Pain (요부안정화 운동이 만성요통여성 환자의 근활성도와 등속성 근력에 미치는 영향)

  • PURPOSE: The purpose of this study to investigate the effects of exercise on lumbar stabilization in muscle activity and isokinetic muscle strength of female with chronic low back pain. METHODS: The candidates was chose to twenty women in their 30s and 40s complaining back pain for over 12 weeks and consist of 10 people for lumbar stabilization and general physical therapy group(PL group), another 10 people for general physical therapy group(GP group). Lumbar stabilization exercise was conducted for 8 weeks and was comprised of 60 minutes for two times a week. In order to examine the effects of lumbar stabilization, results in the present study were analyzed maximal voluntary isometric contraction (MVIC) using electromyogram to measure muscle activity and isokinetic performance including peak torque and average power at the pre to post. RESULTS: The following are results in this study. The MVIC and isokinetic muscle strength were gradually increased in all group. As the result of the test of the MVIC and isokinetic muscle strength, the difference of lumbar stabilization and general physical therapy group is statistically more significant than that of general physical therapy group. CONCLUSION: In the present study, results indicate that lumbar stabilization helps to improve the muscle activity and isokinetic muscle strength.

Effect of Lumbar Stabilization Exercise on Back Pain, Physical Fitness, Sleep, and Depression in Middle-Aged Women with Chronic Back Pain (요부 안정화 운동이 만성요통을 가진 중년여성의 요통, 체력, 수면 및 우울에 미치는 효과)

  • Purpose: The study was to examine the effect of lumbar stabilization exercise on back pain, physical fitness, sleep, and depression in middle-aged women who have chronic back pain. Methods: With one group pre-post test design, 18 subjects who have had more than 6 months of back pain were recruited by convenience sampling in D metropolitan area. Lumbar stabilization exercise was based on Tai Chi for back pain program developed by Lam(2003). This program was mainly focusing on strengthening lumbar stabilizing muscle for one hour per session, twice a week for 20 weeks. Degree of back pain, back pain disability, sleep, and depression were measured at three time points(pretest, 12 weeks, and 20 weeks) by a structured study questionnaire. Physical fitness variables including waist flexibility, mobility, and back muscle strength were measured three times at health promotion center located in the university hospital. Data were analyzed by frequency, descriptive statistics, and repeated measures of ANOVA. Results: Mean age of subjects was 52 years(SD = 6.4). Most of them(90%) had suffered from back pain more than one year and 67% was taking medicine to relieve their back pain. Results showed that back pain score and disability score were not significantly decreased after the exercise. On physical fitness variables, however, waist flexibility(F = 3.50, p = .04) and mobility(F = 3.31, p = .04) were improved after the exercise. Quality of sleep(F = 4.48, p = .03) was improved gradually and depression scores were also decreased(F = 3.12, p = .05) after the exercise. Conclusion: This lumbar stabilization exercise was not able to reduce chronic back pain, but could improve physical fitness and psycho-social variables for community dwelling women who have chronic back pain. Further study is needed to replicate this exercise with control group to confirm the positive effect.

Comparisons between Global Postural Re-education Exercise and Lumbar Stabilization - Focuhing on Eldery Women (Global Postural Re-education 운동과 일반적인 요부 안정화 운동(LSE)의 비교 -여성노인을 대상으로-)

  • Purpose : The purpose of this study was to compare 2 different interventions, global postural re-education (GPR) and specific spinal stabilization exercise in the healthy elderly of women. Methods : A total of 28 subjects were randomized into 2 treatment groups: GPR, where therapy involved muscle global chain stretching, or specific spinal stabilization exercise with conventional static stretching (GPR group: n=14, specific spinal stabilization exercise group: n=14). Both groups received exercise intervention 3 times a week for 12-weeks. Results : Outcome was based on trunk endurance, trunk flexibility, Pressure Bio Feedback, Oswestry Disability Index measured immediately before and after intervention. Trunk flexibility, Pressure Bio Feedback, Oswestry Disability Index improved more in GPR group after intervention. There were no differences between the 2 exercise approaches for any of the trunk endurance. Conclusion : As the above results, GPR appear to improve Trunk flexibility, Pressure Bio Feedback, Oswestry Disability Index to the elderly of women. The efficacy of the GPR program used in this study should be further investigated in a long period study and objective outcomes.

The Effect of Short-term Lumbar Stabilization Exercise for Lumbar Muscle Strength and Postural Balance on Chronic LBP (단기간의 요부안정화 운동이 만성요통환자의 근력과 자세균형에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate the effect of short-term lumbar stabilization exercise to patients suffering from chronic low back pain(CLBP) on the strength of their lumbar flexor and extensor and the postural balance of them and to suggest effective treatment duration for them. METHODS: Study subjects were 20 patients with CLBP who's been reported the pain for more than 12 weeks. For these subjects, muscle strength of both extensor and flexor of lumbar and the balancing ability were measured before and after the intervention. Lumbar stabilization exercise was composed of matt, ball and sling exercise. The intervention was applied for total 12 sessions for 4 weeks (3 times a week) and each session was for 40 minutes. RESULT: For muscle strength of both extensor and flexor of lumbar, there was significant differences after the application of lumbar stabilization exercise(p<.05). For balancing ability, stabilization shows significant differences in all of the positions except PC(eyes closed on pillows)(p<.05). Distribution of weight shows no significant differences but it was generally distributed within normal range after the exercise and Fall index shows significant differences between before and after the exercise(p<.05). CONCLUSION: These results could have positive effect on deciding different periods of therapeutic intervention.

The Effect of Non Elastic Taping During the Lumbar Stabilizing Exercise in Chronic Low Back Pain (비탄력 테이핑을 적용한 요부안정화 운동이 만성요통환자의 통증과 기능에 미치는 영향)

  • PURPOSE: The purpose of this study was to compare the effects of a combined non elastic taping and lumbar stabilizing exercise against lumbar stabilizing exercise only during the lumbar stabilizing exercise in chronic low back pain. METHOD: Thirty-one patients of low back pain were randomly allocated to 2 groups: lumbar stabilizing exercise group (n=16) and nonelastic taping group(n=15) with lumbar stabilizing exercise. Taping and stabilizing exercise were performed twice a week for 4 weeks. The patients were assessed using by visual analog scale (VAS) and Korean version of Oswestry disability index (KODI). The measurements of each patients were measured before the intervention and 2 weeks post-experiment and 4weeks after the intervention. All data were analyzed using by SPSS 12.0 software for Window, the experimental data was analyzed using by paired samples t-test and repeated ANOVA. All statistical tests in this study were conducted at the .05 level of significance. RESULTS: The results of this study are in the nonelastic taping group, significant difference were found in th VAS between pre-test and post-test (p<.05). In addition, there were significant differences in the VAS between the two groups at post-test(p<.05). And in the nonelastic taping group, significant difference were found in the KODI between pre-test and post-test (p<.05). However, there were no significant differences in the KODI between the two groups at post-test (p<.05). CONCLUSION: These findings suggest that combination of nonelastic taping and lumbar stabilizing exercise is more effective for low back pain than stabilizing exercise alone. In conclusion, this study indicates that stabilizing exercise combined with nonelastic taping would be recommended in the clinic.

The Effect of Lumbar Stabilization Exercise with Proprioceptive Neuromuscular Facilitation Technique to Balance and Gait in Chronic Stroke Patients (고유수용성 신경근 촉진법을 적용한 요부 안정화 운동이 만성 뇌졸중 환자의 균형과 보행에 미치는 영향)

  • Background: The purpose of this study is on finding the effects of the lumbar stabilization exercise applied with the proprioceptive neuromuscular facilitation technique (PNF) to the balance and the gait of chronic stroke patients. Methods: An experiment was conducted using two sets of behavioral measures with 30 chronic stroke hospitalized patients in the rehabilitation center located in Incheon city. The 15 participants in group A were instructed to apply the traditional exercise therapy. And the other 15 participants were assigned to apply the lumbar stabilization exercise with the PNF technique. It was conducted for 30 minutes per three days for six weeks, which had eighteen times. Using the balance system, it was checked about the static and dynamic balance. Using the Gaitrite, it was checked about the ability of the gait. Results: There was a better effect on both groups especially with the static balance. Those who were applied the lumbar stabilization exercise with the PNF technique had a better result on the dynamic balance and the gait than those applied the traditional exercise therapy. Conclusion: This research shows that the Lumbar Stabilization Exercise applied with the PNF technique is more effective on dynamic balance and the gait performance ability of the chronic stroke patients.

Effects of Lumbar Stability Exercise Program on Trunk, Lower Extremity of Muscle Activity and Balance in Soccer Player (요부 안정화 운동 프로그램이 축구선수의 체간 및 하지 근활성도와 균형에 미치는 영향)

  • Purpose: The purpose of this study is to provide an efficient and scientific basis for muscle activity (%MVIC) of RA, EO, VL, HS muscles and balance in soccer players through dynamic lumbar stability exercise and static lumbar stability exercise. Methods: This study included 23 soccer players belonging to D University of J province who attended the program for 30 minutes at a time and three times a week for 4 weeks. Of these 13 attended the dynamic lumbar stability exercise (DLSE) program and 10 the static lumbar stability exercise (SLSE) program. The differences between the effects of the dynamic lumbar stability exercise program and static lumbar stability exercise program were analyzed. Results: To increase muscle activity (%MVIC) and balance (WPL), the dynamic lumbar stability exercise program was more effective than was the static lumbar stability exercise program. 1) The %MVIC of trunk muscle (RA &EO) and lower extremitys muscle (VL & HS) increased from before training to after training in the case of the participants who performed the dynamic lumbar stability exercise. 2) The whole path length (WPL) decreased from before the training to after the training. The 2 groups significantly differed in this regard. Conclusion: Dynamic lumbar stability exercise program helps to improve the balancing ability and muscle activity in a soccer players who requires both muscle activity and balance than does any other players.
