Changes in Cross-sectional Area of Lumbar Muscle in Patients with Chronic Back Pain (만성요통환자의 요부근육 단면적의 변화)

  • Purpose: The purpose of this study was to characterize changes in back muscles in patients with chronic back pain. Accordingly, we studied 154 patients with chronic low back pain with regard to area, aspect and triggering position of the pain. We also determined muscle atrophy in painful areas. Methods: Subjects were questioned about pain and a pain provoking test was done. On Magnetic Resonance Imaging (MRI), we measured cross sections of the multifidus, erector spinae, iliopsoas and quadratus lumborum muscles at each spinal level. Results: Muscles in painful regions (multifidus and erector spinae muscles) decreased in area significantly more than nonpainful regions, and showed a significant difference (p<0.05) at levels L3, L4, and L5. Painful regions of the iliopsoas and quadratus lumborum did not change significantly more than non-painful regionsexcept at L5 (p<0.05). The group that had unilateral low back pain showed a significant decrease in cross section compared to the group that had central or bilateral pain (p<0.05). Conclusion: Chronic low back pain causes variable decreases in cross-sectional areas of some but not all back muscles, and at some but not all spinal levels.

The Effect of Improve the Waist Flexibility, the Waist Muscular Strength and the Waist Balance which Grafted in William & McKenzie Exercise with Swiss Ball (윌리엄 & 멕켄지 운동을 이용한 스위스 볼 운동이 요부유연성, 근력과 균형감각에 미치는 영향)

  • PURPOSE: The purpose of this research was comparing and analyzing the exercise effect for the public who was not suffering back pain exercise in improving the waist flexibility, the waist muscular strength and the waist sense of equilibrium after grafted in William & McKenzie exercise with swiss ball. METHODS: 16 people who were not in progress of the chronic low-back pain were recruited. They were grouped into 8 for each experiment(GSBE=Group which play swiss ball exercise, GWME=Group which William & McKenzie exercise). RESULTS: The following result was obtained through measurement of three items in both pre and post examination. CONCLUSION: Comparing the average result of flexibility improvements of each group after 12-week exercise program, the result showed that GSBE was more effective than GWME with increase of muscular strength. Comparing the average result of muscular strength of each group after 12-week exercise program, the result showed that GSBE was more effective than GWME with increase of flexibility improvements. Comparing the average result of sense equilibrium of each group after 12-week exercise program, the result showed that GSBE was more effective than GWME with increase of sense equilibrium.

The effects of Hamstring Stretching Exercise on Angle of Lumbar Flexion and Pain Reduction with Chronic Low Back Patients (슬괵근 스트레칭운동이 만성요통 환자의 요부 굴곡각도와 통증감소에 미치는 효과)

  • The purpose of this research is to study how the stretching exercise along with heat and electric therapy can effect on Angle of Lumbar Flexion and Reduction of Pain against the Hamstring Construction Symptom that can be found in the Chronic Low Back Patients. The target of research was 28 person selected out of the patients diagnosed as Lumbar Sprain and Lumbar Syndrome, who have positive from the active test of Hamstring construction and passive test. Their age range is 20 to 60 years old, 14 male and 14 female. As a research method, simple heat and electric therapy was applied to counter part while Hamstring Stretching Exercise including simple heat and electric therapy was applied to the experimental part. After 6 weeks treatment for each part, the results of effect on Angle of Lumbar Flexion and Pain Reduction was compared and analysed on the basis of the gravity angle meter and Visual Analogue Scale (VAS). The outcome of research is as follows: 1. Angle of Lumbar Flextion increased effectively not in the conventional counter group but the experimental group to which Hamstring Stretching was applied simultaneously. 2. remarkable effect was shown in the experimental part applied Hamstring Stretching. Consequently from the results of this study, we could find out the fact that the therapy combined preserve treatment and hamstring stretching not only produced increasing Lumbar Flexion Angle but also was more effective on reducing pain of the patients according to it than the conventional or preserve therapy.

The Effects of Combination Patterns of Proprioceptive Neuromuscular Facilitation and Ball Exercise on Pain and Balance in Chronic Low Back Pain Patients (고유수용성신경근촉진법의 결합패턴과 공 운동을 통한 요부안정화운동이 만성 요통 환자의 통증 및 정적 균형에 미치는 영향)

  • Purpose : This study aimed to compare the effect of two modes (proprioceptive neuromuscular facilitation combination patterns and ball exercise) of low back stability for chronic low back patients. Methods : The subjects were recruited fourty patients who had low back pain. All subjects were randomly assigned to PNF combination patterns group, ball exercise group. Measurements were performed fourtimes: pre test, 2weeks, 4weeks, and 6weeks. Main outcome measures comprised the visual analogue scale(VAS), balance performance monitor(BPM). Results : The results were as follows. In the comparison of VAS score, sway area, sway path, and sway velocity according treatment period, score was significantly reduced in both PNF combination pattern group and ball exercise group. In the comparison of the both VAS and sway area between groups, there were significant. PNF combination pattern group significantly more decrease than ball exercise group at 6 weeks. However, both sway path and sway max velocity between group, there were not significantly. Conclusion : These results of this study indicated that PNF combination pattern which performed for six weeks had a significant influence than ball exercise group on low back pain.

  • Complex regional pain syndrome type I of vascular origin is difficult to detect unless the classic symptoms and signs exist and/or overt extremity trauma has precipitated the pain. The diagnosis is confirmed by relief of pain following a sympathetic nerve blockade. A 36-year-old woman with arterial occlusive disease of the right lower extremity presented with burning pain and hyperesthesia after sprain had occurred which was accompanied by motor weakness of right ankle. A lumbar sympathetic ganglion blockade with 2% lidocaine 10 ml and triamcinolone 80 mg produced prompt improvement of the pain and motion.

Lumbar Sympathetic Ganglion Block with Alcohol for Plantar Hyperhidrosis (발 다한증 환자에서 알코올을 이용한 요부교감신경절 차단술 후의 결과)

  • Background: Chemical lumbar sympathetic ganglion block could potentially be used to treat plantar hyperhidrosis; therefore, we analyzed the outcome of lumbar sympathetic ganglion block using alcohol for the treatment of plantar hyperhidrosis. Methods: Between March 1992 and June 2003, 356 patients with plantar hyperhidrosis underwent lumbar sympathetic ganglion block using alcohol. All 356 patients were followed up for 2 years and the results evaluated. There were 185 and 171 male and female patients, respectively, with a mean age of 25.1 years, ranging from 15.3 to 56.5 years old. Lumbar sympathetic ganglion block using alcohol was performed with fluoroscopic guidance under local anesthesia. Results: The recurrence rate after 2 years was 34%. Compensatory hyperhidrosis, ejaculation failure, lower back pain and genitofemoral neuritis developed as complications in 132, 4, 12 and 2 patients, respectively. Of the 356 patients, 65% were satisfied. Conclusions: Lumbar sympathetic ganglion block using alcohol is an effective and safe method for the treatment of plantar hyperhidrosis, but more information about the complications and relatively high recurrence rates should be provided to the patient.

Effect of Lumbar Stabilizing Taping on The Electromyographic Activation of Trunk and Gluteal Muscles During One-Leg Standing (한발 서기 동작 시 요부 안정화 테이핑이 체간부와 고관절 주위근의 근활성도에 미치는영향)

  • Purpose: The purpose of this study was to investigate trunk and gluteal muscle activation during one-leg standing or two-leg standing with lumbar stabilizing taping using non-elastic tape. Method: The subjects of this study were twenty subjects(man=11, women=9) who be in good physical health and have not problem to back muscle and one leg standing. The surface electromyographic(EMG) data were recorded on external oblique(EO), gluteus medius(GMed), gluteus maximus(GMax), quadratus lumborum(QL) while pre- and post-lumbar stabilizing taping in two-leg standing and one-leg standing. The analysis of data was performed using the paired samples t-test to compare the difference of EMG activity of pre and post lumbar stabilizing taping. Result: Contrast of pre-lumbar stabilizing taping the muscle activity of QL in post-lumbar stabilizing taping is significant decrease on two-leg standing posture(p<.05), and the muscle activity of GMed is significant increase on one-leg standing posture(p<.05). Thus, we suggest that lumbar stabilizing taping using by functional tape will be able to affect on lumbar stability and gluteal muscle retraining.

A Study on Effect by Lumbar Extension Exercise (요부신전운동에 의한 효과에 관한 연구)

  • The purpose of this study is to improve the effect of Lumbar extensor exercise program and develop the Lumbar extensor exercise program which will be suitable to the patients characteristics. In this study the experimental group was made up of 38 subjects. They are the patients with low back pain using the Lumbar extension exercise program in C hospital. The lumbar extension exercise program was given two times a week for 8 weeks. The results were estimated by Lumbar extensor strength by diagnosis agent. The results were measured three times, one time pre-treatment and two times post-treatment at 4weeks and 8weeks by lumbar extensor curve angle(0, 12, 24, 36, 48, 60, 72). The results were compared at pre-test 4weeks and 8weeks. The muscle strength measured at both 4weeks and 8weeks with the greater strength was shown at 4weeks. The muscle strength of patients with M.strain, Laminectomy and HNP was increased at all angles except for patients with stenosis. The results of this study indicated that diagnosis influenced the muscle strength in Lumbar extensor exercise program.

  • Purpose : The purpose of this study is to examine the influence of a core stability exercise program using Swiss Ball on muscle activity, muscle thickness, maximum muscular strength, and pain in the trunk region during a 6-weeks program involving participants who have lower back pain. Methods : A total of 21 males between the ages of 20 and 33 years old were divided into 3 Swiss Ball exercise groups. Group 1 performed isometric exercises, group 2 performed isotonic exercises, and group 3 performed mixed exercises. Measurements were taken prior to starting the exercise program and after completing the program at the 6-week period using ME6000 to measure muscle activity and VAS to measure pain reduction. Comparisons were made using a paired t-test and ANOVA on SPSS 10.0. Results : There was a statistically significant effect in muscle activity for group 1 and group 3. Secondly, there was a statistically significant effect in pain reduction for group 1, group 2, and group 3. Conclusion : We found that isometric exercises performed by group 1 were effective in improving muscle activity and pain reduction.

The Effect of Lumbar Stabilization Exercises with Neurodynamic Techniques on Lumbar Muscular Strength and Oswestry Disability Index in Lumbar Disc Herniation Patient's (요추 안정화 운동과 결합된 신경역동학기법이 요추 추간판 탈출증 환자의 요부근력과 오스웨스트리 장애지수에 미치는 영향)

  • Background: The purpose of this study was to investigate the effects of lumbar stabilization exercise with neurodynamic techniques on lumbar muscular strength and Oswestry index in lumbar disc herniation patient. Method: The Lumbar stabilization exercise with neurodynamic techniques was performed by 30 pt's in G hospital. The subjects were randomly organized into a study group 15 and control group 15. The exercises and neurodynamic technique were conducted for 40 minutes and, 3 times a week (total 24 times for 8 weeks). General characteristics of the Lumbar muscular strength and Oswestry disability index (ODI) were measured before the training and at 4 weeks and 8 weeks after the intervention. Comparison of the time dependent variable for each group was Calculated by a one way repeated analysis of variance (ANOVA). Comparison between the two groups was Calculated by an analysis of covariance (ANCOVA). Result: There was a significant difference in the Lumbar muscular strength and ODI (p<.05). Similarly in the inter-group analysis, significant differences (p<.05) occurred. Conclusion: In this study, we demonstrated that lumbar stabilization exercises with neurodynamic techniques are an effective therapy for Lumbar muscular strength and ODI in lumbar disc herniation patient's.
