Acute Epididymitis due to Urinary Reflux into Seminal Vesicle : A Case Report (요도정낭 역류로 인한 급성 부고환염 1례)

  • We experienced a 4-month-old boy presenting with fever and painful scrotal swelling. Diagnostic work-up showed the presence of urinary tract infection and concurrent acute epididymitis. On the voiding cystourethrography, vesicoureteral reflux and urinary reflux through the ejaculatory duct and the seminal vesicle were detected without obvious urethral obstruction. In general, urinary reflux into the seminal vesicle can take place with obstructive lesions of the urethra and may cause epididymitis in infants. We report a case of urinary reflux without urethral obstruction with a brief review of related literatures.

  • `성'에 대해서 바로 알면 자기 몸에 대한 소중함을 알아 자존감을 갖고, 잘못된 성으로 인해 가져올 수 있는 여러 가지 문제점들을 생각해 봄으로써 세상을 살아가는데 옳은 것과 그른 것을 가려낼 수 있는 능력이 생기게 된다. 성적 행위는 일순간에 끝날 수도 있지만 그 결과는 삶 전반에 영향을 미칠 수 있다는 것을 인식할 필요가 있다. 특히 성적인 관계가 말초적이고 감각적인 쾌락만을 추구하는 상품이 아니라 상대방과 사랑과 친밀감을 나누는 대화의 장으로서 인식할 수 있는 성교육이 실시되어야 한다고 본다. 더불어 자신과 상대방의 성적인 건강을 고려하면서 만남을 만들어 갈 수 있는 안전한 성에 대한 정확한 이해를 줄 수 있는 열린 성교육이 필요하다고 본다.

  • 양식 넙치의 스쿠티카충은 겨울에서 다음해 봄까지 육상넙치 양식장에서 발생하여 체표, 지느러미, 주둥이에 부종성 궤양을 형성하고 심한 경우에는 뇌에 침입하여 많은 폐사와 손실을 가져오는 난치성질병으로 알려지고 있다. 스쿠티카충에 관해서는 배양 및 분열(이 등, 1997), 분류·동정, 배양특성, 실험적 감염 및 발병에 미치는 물리·화학적 요인(지 등, 1999발표)에 대한 연구가 진행되고 있으며 일부에서는 실험적인 스쿠티카충 구제방법에 대한 연구(최 등,1997)가 있으나 양식현장에서의 치료률은 극히 낮은 실정이다.   • Imperforate hymen is, with an incidence of 0.1%, a rare female anomaly, which can appear with symptoms such as lower abdominal pain, primary amenorrhea, dysuria, anuria, caused by retention of menstrual blood after the onset of menstruation. Generally urinary retention is caused by psychological conditions, drug effect, infection or congenital anomaly causing acute urinary obstruction. We experienced a patient with symptoms of acute urinary retention, suggesting acute urinary obstruction. The cause for the retention turned out to be an imperforated hymen, which should therefore be mentioned in the literature as a possible cause in cases suspected of urinary retention.

Effect of Humenolepis nana infection on immunological responses of mice to sRBC (마우스에 있어서 왜소조충 감염이 sRBC에 대한 면역능에 미치는 영향)

  • In an attempt to investigate the effect of Hymenolepis dana infection on immunological responses to sRBC in ICR strain of mice, cellular and humoral immune responses were chronologically monitored after sensitization with sRBC. Mice weighing about 20 g were allocated into artificial and natural infection groups. The shell-free eggs of H. dana were inoculated into mice on the day 0 (initial) and day 10 in the former group, and praziquantel (25 mg/kg/day) was administered for 3 days to the one half of the mice at the 15th day after the first inoculation and to all of the mice in natural infection group. In artificial infection group, the delayed-type hypersensitivity (DTH) to sRBC was considerably decreased on the day 10 after the first inoculation, and then elevated gradually to normal. Eosinophils in the peripheral blood increased slightly. The hemagglutinin (HA) and hemolysin (HE) titers during the early stage were shown to be more or less higher than those of control. Thereafter, the titers were returned to normal, followed by a transient decrease on the day 15 post-infection. The sRBC rosette and antibody-treated rosette-forming capacities on the day 15 post.infection were temporarily lowered but became higher thereafter. The mucosal mast cells (MMC) in the small intestine were gradually increased to make a peak on the day 10 post-infection and then maintained more or less at lower level. After praziquantel treatment, the DTH and the number of eosinophils were decreased slightly and the MMC number and sRBC rosette-forming capacity were considerably decreased. The titers of HA and HE and antibody-treated rosette-forming capacity, however, were elevated in general. In natural infection group, the DTH, the number of eosinophils, and MMC which were elevated due to H. dana infection were gradually returned to normal after prasiquantel treatment. The titers of HA and HE which were decreased by parasite infection were increased to normal after the treatment. However, the capacities of sRBC rosette or antibody-treated rosette formation were maintained at low levels in spite of the treatment. These results revealed that the immune responses to sRBC were significantly activated during H. dana infection, although they were transiently decreased during the days 10~15 post-infection.

Clinical Practice of COVID-19 and Infection Control by Dental Hygiene Students in Jeju Province (제주지역 치위생과 학생의 COVID-19와 감염관리 임상실습 수행 실태)

  • The purpose of this study was to investigate the factors affecting infection control practice by dental hygienist students in Jeju during clinical training amidst the COVID-19 pandemic. Online survey was conducted on 112 students and the results were statistically analyzed with frequency analysis, Student's T-test, one-way ANOVA, and regression analysis using SPSS 20.0. In the 'awareness and practice of infection control by year in college' section, 2nd year students scored significantly higher in 'cleaning and surface disinfection,' 'medical waste disposal,' and 'COVID-19 preventive measure' compared to 3rd year students. In the 'type of training institution' section, hospital setting scored significantly higher in 'cleaning and surface disinfection,' 'instrument disinfection and sterilization,' 'personal and patient protection,' and 'COVID-19 preventive measure' compared to dental clinic setting. In the 'location of institution' section, Seoul metropolitan region scored higher in 'medical waste disposal' compared to Jeju region. According to regression analysis, year in college (2nd year), type of training institution (hospital setting), location of training institution (Seoul metropolitan region), and difficulty using protective gear (no difficulty) were associated with better COVID-19 preventive measure This first study in Jeju provides an insight on the awareness and practice of infection control measures by dental hygienist students in Jeju during clinical training. Further investigation for improvement of clinical training manual is warranted.

Clinical Analysis of Intussusception Delayed in the Final Diagnosis (발병 초기에 진단하지 못한 장중첩증에 관한 고찰)

  • Purpose: In this study, we tried to evaluate the clinical characteristics or circumstances that lead to unintentionally the delay in the diagnosis of intussusception or to the wrong direction that prevent the proper management early. Methods: All the patients of intussusception with delayed diagnosis in the department of pediatrics or emergency room at Gyeongsang National University Hospital from 1990 to 2003 were enrolled and reviewed retrospectively. Results: There were 8 boys and 6 girls and their median age was 8 months (range 2 months to 10 years). Their initial symptoms and signs were vomiting, seizure, diarrhea, lethargy, irritability, bloody stool, palpable abdominal mass, foul odor of urine and tachycardia. Clinical diagnosis or impressions at admission consisted of acute gastroenteritis, shigellosis and toxic encephalopathy, convulsive disorders, urinary tract infections, sepsis, abdominal mass and intestinal obstruction. Eight patients were luckily diagnosed due to the delayed manifestations of cyclic irritability or currant jelly stool. Six patients were not paid attentions for the possibilities of intussusception and diagnosed serendipitiously by the abdominal sonography or CT during the evaluation of the abdominal mass or distension. Only five of 14 cases (35.7%) were successfully managed by barium or air reductions. The other 9 cases needed surgical operations. Conclusion: Delayed diagnosis of intussusception arise when doctors initially diagnose the patients incorrectly due to the unusual presentations or when they overlook the newly arising symptoms or signs suggestive intussusception after the admission because they are ardently attached to the first impressions or initial clinical diagnosis.
