Resonance Changes in the External Auditory Canal Associated with the Ear Canal Volume (외이도용적에 따른 외이도공명의 변화)

  • The external ear generates resonance gain because of anatomical characteristics. The ear canal resonance is influenced by the length and volume of the ear canal, the pinna, the concha cavity, the body trunk, and the speed of sound wave. This study is focus on the influence of the volume of ear canal. 17-healthy-adult (32 ears) were participated. They did not have any medical and ear disease history. The maximum resonance frequency of the ear canal was 2675 (${\pm}265$) Hz at azimuth $0^{\circ}$ and 2784 (${\pm}268$) Hz at azimuth $45^{\circ}$. The resonance gain was 18.1 (${\pm}3.9$) dB at azimuth $0^{\circ}$ and 17.9 (${\pm}3.8$) dB at azimuth $45^{\circ}$, respectively. The ear canal volume was 0.78 (${\pm}0.2$) cc and 1.32 (${\pm}0.8$) cc including static compliance. The ear canal resonance was changed depending on the ear canal volume. It was also statistically correlated at azimuth $0^{\circ}$ (p=0.038) and $45^{\circ}$ (p=0.013), respectively. The resonance gain was not correlated with the ear canal volume. The change of resonance frequency according to the ear canal volume will be useful information in the field of audiological rehabilitation especially for hearing aids fitting. In addition, we expected this study can provide the basic information for the study of the external ear resonance characteristics.

  • Malignant otitis externa (MOE) originates as inflammation of the epidermis in the external auditory canal, and spreads to the surrounding structures and neck, leading to abscess formation. MOE is associated with an immunosuppressive condition and diabetes. Patients with MOE suffer from otalgia, otorrhea and hearing loss. According to the literature, surgery to the temporomandibular joint is controversial as the treatment of choice.

  • 대학원생 김모씨(28세 여)는 귀에 갑작스런 통증을 느끼기 시작했다. 또 급격하게 저하된 청력으로 친구들의 말귀를 잘 못 알아들어 '사오정'이라는 별명도 얻게 되었다. 통증이 심해 병원을 찾아간 결과 외상성 고막천공이라는 진단을 받게 되었다. 이는 귀이개가 고막을 관통해 발생한 질병으로 귀이개로 귀를 후비던 중 실수고 너무 깊이 파 통증이 나타나게 된 것이다. 다행이 다른 부위는 손상이 없어 외래에서 간단한 수술 및 통원치료 후 증상은 좋아졌다. 또 직장인 박모씨(34세 남)는 어느 날부터 갑자기 귀에서 고름이 나오고, 고약한 냄새를 풍겨 병원을 찾았는데, 만성 외이도염이라는 진단을 받았다. 원인은 하루가 멀다 하고 귀지를 파내는 오래된 습관 때문이라는 것이다. 습관을 바꾸지 않으면 앞으로 청력도 떨어질 수 있다는 의사의 말에 귀이개부터 멀리하기 시작했다. 고려대 안산병원 이비인후과 조승현 교수는 "종종 속을 썩이는 것이 귀지"라며 "무리하게 후비다가는 고막까지 버릴 수 있으므로 함부로 파서는 안된다"고 지적한다.

  • The tinnitus retraining therapy(TRT) is an effective method for treating tinnitus patients. This therapy use the white noise to stimulate auditory cells for a wide frequency range. In this paper, the small white noise generator using the thermal noise is proposed. And frequency response controller which can compensate the frequency response changed by the human outer and middle ear system is proposed. We can know that proposed system is more proper type on a purpose of the tinnitus retraining therapy comparing with conventional white noise generator.

  • This study was compared with each 2 groups (Intermittent traction therapy alone group1: Control group and Intermittent traction therapy plus Auriculotherapy group 2: Experimental group) on 20 HIVLD(Hemiated Intervertebral Lumbar Disk) patients with sciatics Each subjects were divided to control and experimental. Each group were applied Intermittent traction therapy (HOld:25, Rest:15) for 15 minute and control group were not received auricular stimulation. However experimental groups were applied auricular stimulation simultaneosly. Auricular acupuncture points(max-17, min=13) were stimulated with low frequency. high intensity Electro-Acupunture stimulator for 20 seconds per each point. Treatment and measurements(R.P and VAS) was administered to each patient during the 10 times therapy per 2 weeks. The results were significant differences between the two groups. Experimental group appeared significantly greater than before in pain, and pain relief, and significant improvement in centralization phenomenon of radiating pain.

  • Clinical outcomes of dogs treated for end-stage otitis extema with total ear canal ablation (TECA) combining lateral bullar osteotomy (LBO) at Veterinary medical teaching hospital of Seoul National University from 2001 to 2003 were evaluated. In 14 dogs, 23 ears, the operation was successful in alleviating all clinical signs of otitis externa and media. The main complication after TECA combining LBO were facial nerve paralysis and the others were recurrent infection and the formation of fistula and cyst. Two cases were tumors. One was ceruminous gland adenoma and the other was squamous cell carcinoma. The operation successfully resolved the original problems in 100% (23 of 23) of the surgically treated ears of these dogs.

The comparison of the voice between the free field and the external auditory canal (음장과 외이도 내부에서의 음성 비교)

  • The purpose of this study was to examine some acoustic characteristics in the ear canal. It was assumed that a sound outside the external auditory canal could be different from the sound inside the external auditory canal. The acoustic signals were captured by a probe microphone placed at a distance within 1 cm from the tympanic membrane, and a reference microphone was placed over the upper pinna. Three vowels /a/, /i/, /u/ were recorded from a normal adult male speaker. The parameters such as the formant frequency ($Fl\simF5$) and the peak intensity were measured using a speech analyser, PCquirer. It was found that the entering part of the external auditory canal functions as a narrowing point as to the speech that passes through the free field. Results show that acoustic characteristics were changed for speech discrimination rather than speech perception.

  • According to Teed, the first report of oseoma in the literature was by Veiga in 1586. There was no satisfactory explanation of its etiology. True osteomas may occur in the mastoid and temporal bone, but they are relatively rare. These tumors are benign, slow growing and composed predominantly of mature bone. Treatment is indicated for symptomatic osteoma. The authors experienced a case of osteoma of the temporal bone which had revealed a progressive posterior auriular swelling, obstructed bony ear canal, chronic ear discharge and hearing loss for about 18 yrs in right side.

치료용 레이저를 이용한 대상포진 (Herpes Zoster)치료 19례에 대한 보고

  • 문헌에 의하면 대상포진의 원인은 바이러스에 대한 세포성 면역이 저하될 때, 또는 악성종양 등으로 면역억제제 치료를 받아서 2차적으로 면역이 저하될 때 체내에 잠재해 있던 바이러스가 재활되어 발병하는 것으로 추정하고 있다. 본 저자도 이에 대하여 의견을 같이하고 있는데 노령층의 $60\~70$대 여자환자수가 압도적으로 많았다는 점과, $20\~39$세 사이의 청${\cdot}$장년층에서는 단1명의 대상자가 없다는 점뿐 아니라, 본 대상자중 11세의 소녀는 7세때 뇌종양 수술을 2차례에 걸쳐 실시한 후 면역제제를 계속적으로 사용하는 있는 점 등은 문헌의 발병원인과 상당히 일치하였다. <전파 양식> 비말 감염 (droplet infection)으로 전파되며, 수두와 달리 전염성이 높지 않다. <잠복기> 잠복해 있던 바이러스가 언제 재활할지 알 수 없으므로 불명이다. 그러나 본조사에서는 대상포진에 대한 병력을 가진 사람이 없었고, 발생 원인을 본인 자신도 모르고 있었다. <합병증> Ramsay Hunt의 증후에 의하면 합병증은 외이도의 수포, 안면신경마비, 혀의 2/3부분의 미각상실, 간혹 청각 및 평행장애를 일으킨다고 한다. <치료> 대상포진의 대증요법으로 calamine lotion Burrow solution의 wet dressing 진통제 및 cytosine arabinoside나 adenosine arabinoside와 같은 항히스타인제, 최근에는 Acyclovir가 많이 사용되고 있는 것으로 알려졌다.
