A Case of External Ear Osteoma as a Polyp in Dog (개에서 외이도내 발생한 폴립성 골종 증례)

  • A 2 year-old, 3.8 kg, intact female Maltese was presented with unilateral pain and reddish discharge from the left ear caused by a polyp in the horizontal ear canal. The polyp was diagnosed as osteoma histopathologically and removed through total ear canal ablation. Three months after surgery, osteoma was recurred and broken out of edematous skin lesion. It was considered that an inappropriate surgical removal of the tissue which attached base boarder of the polyp would be cause of the recurrence of the osteoma.

A Case of Neurilemmoma Originating From Arnold's Nerve (아놀드 신경에서 발생한 외이도의 신경초종 1예)

  • Neurilemmoma is a benign tumor that originates from the Schwann cell. It frequently occurs in head and neck regions, but neurilemmoma of external auditory canal is extremely rare. Here, we report a very rare case of 36-year-old male with a tumor located in the posterior wall of external auditory canal, preoperatively suspected as neurilemmoma originated from Arnold's nerve by temporal bone magnetic resonance imaging. The tumor mass was surgically excised and pathologically confirmed as a neurilemmoma.

A Case of Epidermal Cyst Occurred in the Bony External Auditory Canal Misdiagnosed as External Auditory Canal Carcinoma (외이도 암으로 오인된 외이도 골부에 발생한 표피 낭종 1예)

  • Epidermal cysts are generally benign tumors that usually originate from the skin caused by inflammation of hair cortex and proliferation of epidermal cells within the dermis; however, for these cysts to occur in the bony external auditory canal (EAC) is rare. They are often present as a solitary, painless lesion and usually asymptomatic and the diagnosis depends on the results of the histological examination. In treatment, the cyst wall must be completely removed surgically. We recently encountered a 82-year-old male with a mass in the right EAC. An otoscopic examination showed a polypoid mass on the bony EAC, which was finally diagnosed as epidermal cyst after an initial misdiagnosis as EAC carcinoma. We report the rare, unique case with literature review.

Role of Radiotherapy for Squamous Cell Carcinoma of the External Auditory Canal and Middle Ear (외이도 및 중이 편평상피암의 방사선치료)

  • Purpose: To investigate the role of radiotherapy for squamous cell carcinomas of the external auditory canal and middle ear. Materials and Methods: A series of 35 patients who were treated at a single institution from 1981 through 2007 were retrospectively analyzed. Thirteen patients were treated by radiotherapy alone; four by surgery only and 18 by a combination of surgery and radiotherapy. The total radiation dose ranged from 39~70 Gy (median, 66 Gy) in 13~35 fractions for radiotherapy alone and 44~70 Gy (median, 61.2 Gy) in 22~37 fractions for the combined therapy. Clinical end-points were the cause of specific survival (CSS) and local relapse-free survival (LRFS). The median follow-up time was 2.8 years (range, 0.2~14.6 years). Results: The 3-year CSS and LRFS rate was 80% and 63%, respectively. Based on a univariate analysis, performance status and residual disease after treatment had a significant impact on CSS; performance status and histologic grade for LRFS. Patients treated by radiotherapy alone had more residual disease following the course of treatment compared to patients treated with the combined therapy; 69% vs. 28%, respectively. Conclusion: Our results suggest that radiation alone was not an inferior treatment modality for CSS compared to the combined therapy for squamous cell carcinoma of the external auditory canal and middle ear. However, local failure after radiotherapy is the main issue that will require further improvement to gain optimal local control.

Effect of Aromatherapy and Apipuncture on Malassezia-related Otitis Externa in Dogs (개 말라세지아성 외이염에 대한 아로마치료와 약침의 치료 효과)

  • Aromatherapy and apipuncture was evaluated for its efficacy on canine Malassezia-related otitis externa. Ten dogs with Malassezia-related otitis externa were used in the present study. The control group was treated with ketoconazole, and the experimental group was treated with aromatherapy (topical aroma-oil drop) and apipuncture (injection-acupuncture of apitoxin). Clinical scores of the control group after 2-week treatment (p < 0.01) revealed a significant decrease compared with scores of pre-treatment. Experimental group scores after 2-week treatment revealed a significant decrease compared with scores of pre-treatment (p < 0.05). In the control group, ALT levels were significantly increased in the 1-week treatment (p < 0.05) and the 2-week treatment (p < 0.01), compared with the pretreatment levels. The experimental group ALT levels did not significantly change after 1-week and the 2-week treatments, compared with pre-treatment. ALT levels of the experimental group after the 1-week treatment (p < 0.05) and the 2-week treatment (p < 0.01), was significantly lower than those of the control group. In conclusion, a combination therapy of apipuncture and topical aroma-oil drop is not hepatotoxic and similar to the effect of ketoconazole treatment for Malassezia-related otitis externa in dogs.

Efficacy of Enrofloxacin and Silver Sulfadiaznine Topical Otic Suspension for the Treatment of Canine Otitis Externa (개 외이염에 대한 Enrofloxacin과 Silver Sulfadiazine 국소제제의 치료효과)

  • The aim of this study was to evaluate the in vivo and in vitro efficacy of enrofloxacin-silver sulfadiazine (Baytril$^{(R)}$ otic, Bayer, USA) for the treatment of otitis externa in dogs. Twenty-four dogs with otitis externa were included in this double-blinded, randomized study. The experimental group was treated with the Baytril$^{(R)}$ otic and the distilled water was applied to the control group. Both groups were administered each solution twice daily for 7 days and next 7 days off treatment. On days 0, 7 and 14, clinical signs, bacteriological and fungal counts were graded using semi-quantitative scales, respectively. For the evaluation of in vitro efficacy of Baytril$^{(R)}$ otic, we also performed Minimal Inhibitory Concentration (MIC) test by agar dilution method against Staphylococcus pseudintermedius, Pseudomonas aeruginosa and Malassezia pachydermatis. In the experimental group, the sum of clinical scores was decreased 81.0% and microbial scores were significantly reduced 87.0% at days 14, compared with day 0. The results of MIC testing were showed the concentration of enrofloxacin and silver sulfadiazine in Baytril$^{(R)}$ otic is high enough to kill for 3 infectious agents. No adverse reactions were observed in any of the dogs during this study. These results suggest that Baytril$^{(R)}$ otic are efficient and safe treatment for canine otitis externa.

Studies on Audiological Significance of the Bing Test (Being test의 임상청각학적 의의에 대한 고찰)

  • In normal auditory systems, the difference between the bone conduction thresholds obtained with the test ear opened and occluded in low frequency signals below 1, 000Hz is about 10~25 dB. But no difference in these thresholds suggests the presence of a lesion in the conductive mechanism. Sullivan added the differences in these thresholds at 250Hz, 500Hz and 1,000Hz and called "Occlusion Index". In oder to study the audiological significance, we have measured the occlusion index in 40 ears of normal persons, 20 ears of chronic otitis media patients and 20 ears of sensori-neural hearing impairment patients. We also measured the static compliance in 40 normal ears and observed the correlation with the occlusion index. The results are as follows: 1. Occlusion index was 33. $10\pm10.63dB$ in normal group, $3.10\pm3.03dB$ in chronic otitis media; group, $28.10\pm15.17dB$ in sensori-neural group. 2. Static compliance in normal group was $0.61\pm0.31cc$ (0.22~1.75cc) 3. Occlusion index showed inverse proportion to static compliance.

Auriculotherapy in Pain Management (동통관리의 방법 중에서 외이자극치료에 관한 고찰)

  • Pain is the most common symptom causing patients to seek physical therapy. Auriculotherapy has shown to be effective in pain management as well as in treating a wide variety of disorders. The pupose of this study were 1) to describe the historical, theoretical background and the effects of Auriculotherapy 2) to identify the applied method in clinic and 3) to encorage of the application in physical therapy.
