A Study on Influence to Satisfaction of Outpatients in a General Hospital (일개 종합병원의 외래환자 만족도 영향요인에 관한 연구)

  • 본 연구는 병원을 이용하는 외래환자들의 불만족요인과 만족요인들을 조사하여 보다 높은 고객만족 경영을 위한 기초자료를 제시하고자 하였다. 연구대상은 대전에 소재하는 일개의 종합병원 외래환자를 대상으로 2011년 10월 24일 1일간 설문조사를 실시하였다. 자료 수집은 총 160부의 설문지를 배포하여 144부를 통계분석자료로 이용하였다. 연구결과 행정지원부문의 특성 중 전반적 만족도와 타인추천 의향에 대하여 가장 많은 영향을 미치는 요인은 접수 수납 절차의 신속 간편성인 편리성으로 나타났으며 검사실 외래 진료대기실의 쾌적성도 공통으로 영향을 주는 것으로 나타났다. 이상과 같은 결과 고객을 만족시킬 수 있는 고객지향적 병원경영시스템을 구축 운영한다면 병원의 경쟁력은 향상되고 병원의 지속가능성은 높아질 것이며 고객만족경영이라는 경영목표도 달성될 수 있을 것이라 사료된다.

Medical Service Factors Associated with Patient Satisfaction: The Difference between Inpatient and Outpatient of A Medium-sized Hospital (환자만족도에 영향을 미치는 의료서비스 품질 요인: A 중소병원 입원.외래환자 간 차이)

  • This study attempts to analyze medical service factors associated with patients' satisfaction, and its difference between inpatients and outpatients in A medium-sized hospital located in rural area. Questionaries have been collected from 411 patients of 96 inpatients and 315 outpatients. The survey was carried out for 4 weeks, from August 20, 2011. The study found that hospital awareness was only factor related to patients' satisfaction in inpatients. In the case of outpatients, several factors were related to patients' satisfaction. They are included qualified health care providers, infirmary accommodations, staff kindness, hospital awareness, and convenient procedure of treatment and admission. Based on the findings, it is desirable to differentiate the strategy in improving patients' satisfaction between patient type; either inpatient or outpatient to being competitive.

The Effect of Perceived Medical Services Quality by Outpatient on the Hospital Image, Satisfaction and Re-use Intention (외래환자가 지각한 의료 서비스의 질이 병원 이미지, 만족도 및 재이용 의도에 미치는 영향)

  • The purpose of this study is to investigate the effect of perceived medical service quality by outpatient on hospital image, satisfaction, and re-use intention in a general hospital. A total of 176 questionnaires were administered to outpatients who presented to a S general hospital in Daejeon. The results of this study are as follows. First, medical service quality had significant effects on re-use intention, satisfaction, and hospital image. Second, the mediating effect of satisfaction on the relationship between medical service quality and re-use intention was not significant but the mediating effect of hospital image on the relationship was significant. Third, satisfaction and hospital image had significant effects on re-use intention. Consequently, this study verified the influence of perceived medical service quality by outpatient on hospital image, satisfaction, and re-use intention in a general hospital. Further, considering that hospital image, rather than satisfaction, mediates the relationship between perceived medical service quality and re-use intention, it suggests that much more general satisfaction is required in the marketing activities of hospitals.

System Development for Tracking a UHF Passive RF1D Tag in an Outpatient Clinic (외래병원 환경에서 UHF 수동형 RFID 기술을 활용한 태그 추적 시스템 개발)

  • An RFID system has been widely applied in many areas over the initial SCM application. In the literature enormous RFID applications in healthcare are documented to improve patient safety, patient/provider logistics, and the efficiency of collecting data. Based on the proposed 4-layered RFID system architecture, we introduce a case that implemented an UHF passive RFID-based tracking system in an outpatient clinic. Particularly, we propose a method to process RFID data that contains noise and missing reads. The proposed method for processing unreliable RFID data is capable to locate the tag accurately and provide additional business information. We finally conclude the paper with identifying obstacles and what is necessary to ensure system reliability.

A Study on the design of the outpatient and inpatient conversion factors based on the medical expenditure budget system (진료비 예산에 기초한 외래 및 입원 환산지수 설계에 관한 연구)

  • In this paper, a theoretical model that separates one single conversion factor into two conversion factors for both outpatients and inpatients is introduced. By using hypothetical numbers, two conversion factors and an adjustment factor were calculated. The major implications are as follows. Firstly, by introducing two conversion factors, a minimum incentive mechanism for admitting outpatients to a clinic and high risk inpatients to a hospital was installed. Secondly, the introduction of two conversion factors decreased the drive to admit outpatients to a hospital by reducing the economic benefits for the hospital. Thirdly, it is possible to make explicit contracts for several factors rather than a single conversion factor, so that it can be used as an appropriate management tool for rapidly increasing medical expenses. Finally, this research can be used to set up policy tools to establish a proper healthcare delivery system in Korea by inducing behavioral changes in healthcare institutions.

Medical Care Utilization between National Health Insurance and Medical Assistance in Elderly Patients (건강보험과 의료급여 노인환자의 의료이용량 : 요양기관종별 분석)

  • The purpose of this study is to analyze the difference of medical care between medical assistance and health insurance patients to evaluate the increase of medical care costs due to the moral hazard of medical care patients and to provide a basis for rational medical care policy decision. For this purpose, we compared health insurance benefit data for Seoul citizens by gender, age, and type of medical institutions. The results of the analysis are as follows. First, all of the hospitalized and outpatient use of the advanced general Hospitals, medical assistance patients were less than those of the health insurance patients, so that the medical assistance patients could not use the high cost medical services. Second, in general hospitals, patients with health insurance are often hospitalized. On the other hand, medical assistance patients use a lot of outpatient services because they are less burdened. Third, in hospitals and clinics, medical benefits patients often use inpatient and outpatient services. Therefore, medical assistance patients are likely to use unnecessary medical care of outpatient and hospitalization clinics and hospitals, outpatient of general hospitals. But, in hospitalization and outpatient use in advanced general hospitals and medical assistance patients can not use due to excessive medical burden. Therefore, the policy to reduce the burden of medical expenses for patients with severe illness will continue, and the medical care patients using clinics and hospitals should be careful not to use unnecessary medical services.
