Hourglass-Like Constrictions of Peripheral Nerve in the Upper Extremity (상지에서 발생한 말초 신경의 모래시계형 협착)

  • Nerve torsion is a rare condition that causes sensory abnormalities and decreased muscle strength due to a nerve-twisting phenomenon in the peripheral nerves. To date, the progression of the condition from its cause to diagnosis and treatment has not been established. The authors report three cases of good results from the treatment of nerve torsion in the anterior interosseous and radial nerves with epineurotomy and neurolysis.

  • Most acute patella dislocations can be reduced closely using a spontaneous or simple maneuver, but it may not be possible if accompanied the by anomalies, such as rotation of the patella, osteophyte ridge protruding from the femur, or anatomical variations. This case report outlines irreducible acute patella dislocation with anatomical variations, a notched patella. If the reduction of an acute patella dislocation fails, an additional radiological examination and reduction under general anesthesia may be necessary with the possibility of an anatomical variant in mind.

Pseudoaneurysm of Tibioperoneal Trunk of Tibial Artery Caused by Minor Blunt Trauma (경한 둔상에 의하여 발생한 경골 동맥의 경골비골동체의 가성동맥류)

  • Pseudoaneurysm which is usually caused by high energy trauma or penetrating injury results in serious complications if the diagnosis is delayed. The authors experienced a 63-year-old patient who slipped on the stairs with simple contusion and skin laceration. But, he was finally diagnosed with pseudoaneurysm of tibioperoneal trunk of tibial artery on magnetic resonance imaging and 3-dimensional computed tomography angiogram performed in two weeks due to aggravation of the symptoms. To the best of my knowledge, pseudoaneurysm of tibio-peroneal trunk caused by minor blunt trauma has not been reported till now in Korea. So we report this case with a review of the relevant literature.

  • Isolated fractures of lower sacrum are commonly treated conservatively in most cases because of low energy damage and stable fracture. However, surgical treatment is required in displaced unstable fracture. But the surgical technique is not established and even case reports are not common. We reported a case of displaced transverse fracture of the lower sacrum that was treated with an open reduction and dual plate fixation.

  • The radiographic findings of an osteolytic lesion in the knee may indicate numerous possible impressions. Furthermore, osteolysis is a possible cause if there is a surgical history of total knee arthroplasty (TKA). The authors diagnosed osteolysis of the fibular head after aseptic loosening of the tibial component of a cemented TKA in an 83-year-old female patient who visited with right knee pain and report their treatment with revision TKA along with a literature review.

  • Unstable pelvis ring trauma can accompany urologic injuries such as bladder, ureter, or urethral injuries. In very rare cases, testicular dislocation can accompany a pelvic ring injury. Testicular dislocation causes pain, but can be regarded as the pain caused by pelvic ring injury. If not detected early, testicular dislocation may result in gangrenous testes. The authors report a case of pelvic ring injury with testicular dislocation.

  • A diagnosis of gout is often straightforward because gout has well known clinical presentations, laboratory analyses, and radiologic features. On the other hand, gout can mimic other diseases by showing a range of atypical clinical manifestations. This paper reports a 35-year-old male with no prior history of gout who developed tophaceous gout at his previously repaired Achilles tendon 11 years after surgery. He was initially misdiagnosed with cellulitis because of his atypical clinical features. This case is presented with a review of the relevant literature.

  • Acupuncture, which is a procedure of inserting needles in a specific portion of the body to control pain, is conducted widely. The procedure is considered relatively safe because of its minimal side effects and low occurrence (1 in 200,000). On the other hand, its side effects include neurological damage, infection, hematoma, emphysema, etc. The authors report a case of cervical neurological damage that occurred after an acupuncture procedure.

  • Anaplastic large cell lymphoma (ALCLs) are a group CD30-positive mature T-cell lymphomas, an uncommon subtype of non-Hodgkin lymphomas, characterized by diverse clinical and genetic features. Among the types of ALCL, anaplastic lymphoma kinase (ALK)-negative ALCL, though typically involves the lymph nodes, can infrequently invade other tissues. When soft tissue involvement occurs, it may mimic the clinical presentation of infectious diseases, leading to potential misdiagnosis. Therefore, a histological examination is necessary to differentiate between ALK-negative ALCL and similar phenotypes associated with infectious conditions. This paper reports a case of ALCL, initially misdiagnosed as an infection.

  • Slipped capital femoral epiphysis (SCFE) is a rare disorder of the proximal femur in adolescents. The pathogenesis of SCFE is unclear, but it is believed to be the result of a mechanical insufficiency of the proximal femur physis. SCFE appears to caused by mechanical, endocrinal, and immunological abnormalities. Few cases have reported the SCFE associated with endocrine disorder in the domestic literature, particularly in patients over 20 years old. This paper reports two cases of SCFE associated with hypopituitarism in adults over 20 years old.
