TREATMENT OF AN IMPACTED DILACERATED INCISOR BY SURGICAL REPOITION (외과적 재위치술에 의한 매복된 만곡절치의 치험례)

  • Dilaceration is a developmental disturbance in shape of teeth. It refers to an angulation, or a sharp bend or curve, in the root or crown of a formed tooth. Dilacerations are estimated to occur in 3% of all permanent dentitions. However, the etiology of dilaceration is not yet fully understood. Traumatic injury to the deciduous predecessors and ectopic development of the tooth germ are the two commonly cited causes of this anomaly. The involved tooth is usually the maxillary central incisor and it also shows high prevalence of impaction. At first, We tried to do surgical exposure with orthodontic traction but to no avail. After then surgical repositioning of the dilacerated tooth was performed. Finally, Satisfactory results were obtained at the end of treatment, with improvement in dental esthetics and functional aspects.

  • Traumatic intrusive luxation, an occurrence common in the primary dentition but one that occurs rarely in the permanent dentition, has a poor prognosis. There have been many treatment approaches such as allowing the tooth to reerupt spontaneously, surgical repositioning and immediate luxation, surgical luxation, and orthodontic repositioning; but all have their own drawbacks. Meanwhile, Turley et al. (1987) have proposed surgical and orthodontic combination therapy to treat intrusion. Surgical and orthodontic combination therapy means to apply the orthodontic traction force immediately after surgical luxation. If ankylosis occurs, orthodontic force may be applied after re-luxation repeatedly. But in cases of complete intrusive luxation, it would be not feasible to bond an orthodontic button or bracket on the tooth directly. Thus, in this case, traction of the tooth was attempted after surgically repositioning it close to the probable original socket site to promote better healing.

  • Intrusive luxation that takes approximately three percent of permanent teeth commonly occures at anterior teeth. This intrusion frequently leads to pulp necrosis, root resorption, marginal bone loss and these complications are influenced by depth of intrusion and stage of root development. Various treatment approaches have been suggested to manage of intrusive luxation. Techniques aiming to reposition the intruded tooth include an observation for spontaneous re-eruption, surgical or orthodontic repositioning. We report two cases with clinically satisfactory results for traumatically intruded maxillary central incisor. In one case which has a large open apex and mild intrusion depth, we observed for spontaneous eruption and then repositioning by forced eruption method. In other case, which has been completely intruded, was repositioned by surgical extrusion and followed by apexification.

  • Tooth anklylosis is defined as the adhesion state of alveolar bone to dentin or cementum. Trauma, disturbed metabolic disease, and congenital disease have been given as etiologic factors. Complications of tooth ankylosis are tipping of the neighboring teeth, space loss, and supraeruption of the opposing teeth. Particularly if dental ankylosis occurs in maxillary incisors of a growing child, the ankylosed tooth can not move vertically with subsequent disturbance in vertical growth of the alveolar process. With an appropriate treatment approach, an esthetic condition must be achieved especially in the maxillary anterior region. In this report, two cases are presented which were treated by the surgical repositioning method. One is treated by alveolar bone distraction osteogenesis which used a tooth-borne type distraction device and the other by single tooth osteotomy.

Orthodontic Traction and Decompression Method in Treating Impacted Permanent Mandibular First Molars : Case Reports (교정적 견인과 감압술에 의한 매복된 하악 제1대구치의 치험례)

  • Impacted teeth occur at higher frequencies in permanent than primary dentition. The most frequently affected teeth are the maxillary and mandibular third molars, whereas it is quite uncommon for the mandibular first molar to be impacted. Treatment methods for impacted teeth include continuous examination for independent eruption, surgical exposure, subluxation after surgical exposure, orthodontic traction, and surgical repositioning. If all of these treatments fail, tooth extraction may be considered. In the first case study, an 8-year-old boy was treated with surgical exposure, after which he was fitted with an obturator. His mandibular first molar then erupted successfully. In the second case, we treated a 12 year-old boy using orthodontic traction. This study describes children with tooth eruption disorders of the mandibular first molar in mixed dentition, and reports acceptable results regarding treatment of the impacted teeth.

  • Impaction of mandibular first molar is relatively rare and its overall frequency has been reported to be 0.01%. The etiology of impaction are lack of eruption space, physical obstacles such as supernumerary teeth, odontomas or odontogenic tumors, hereditary factors, functional disturbances of endocrine glands and traumas. Impaction of mandibular first molar can result in a short lower facial height, formation of a follicular cyst, pericoronal inflammation, resorption of the roots of neighboring teeth and malocclusion. The treatment options available for impacted teeth include surgical exposure, orthodontic forced eruption, surgical repositioning and surgical removal of unerupted molar. This report presents two cases of distally tilted and impacted mandibular first molars which were treated by surgical exposure. In these cases, we could observe spontaneous eruption of the impacted mandibular first molars after surgical exposure.

  • The first molar is important for mastication and also it plays roles to formation of vertical occlusion and growth of jaw bone after mixed dentition. Impaction of mandibular first molar can result in a short lower facial height, formation of a follicular cyst, pericoronal inflammation, resorption of the roots of neighboring teeth and malocclusion. The options of treatment plans are as follows; observation, surgical exposure, orthodontic traction, surgical relocation and extraction. Surgical exposure could be considered as a basic treatment plan. For surgical exposure it is important to maintain patent channel between the crown and the normal eruptive path into the oral cavity, many techniques including cementation of a celluloid crown, packing with zinc oxide-eugenol surgical pack are used. In these cases, we could observe spontaneous eruption of mandibular first molar using surgical exposure with or without removal of odontoma. Also we could obtain the main patency effectively and conveniently by using surgical pack and translucent retainer.

Management of Eruption Disturbances of the Mandibular First Molar : A Case Report (맹출장애를 가진 하악 제1대구치의 치료 : 증례 보고)

  • With a prevalence rate of 0.01%, the presence of eruption disturbances of mandibular first molar has rarely been in populations. Eruption disturbances of permanent molars have been usually manifested as impaction, primary retention, and secondary retention. The treatments of eruption disturbances are carried out by: periodic observation; surgical exposure; forced eruption after surgical exposure; forced eruption with luxation; surgical repositioning; and extraction. This case report show successfully erupted mandibular first molars by various treatment methods on five patients diagnosed with impaction, primary retention, and secondary retention, respectively. Eruption disturbances of the mandibular first molar can be properly diagnosed with impaction, primary retention, and secondary retention by clinical and radiographic examination at normal eruption time of the mandibular first molar. The treatment should be done synthetically, considering eruption state of affected tooth, the relationship between the affected tooth and the adjacent or opposite tooth, the patient's age, treatment compliance, and economic state.
