외골격 고정법은 이미 외국에서는 골절의 고정, 관절의 일시적 또는 영구적인 고정, 골격 이상 교정 등에 널리 사용되고 있다. 그럼에도 불구하고 국내에서는 소수의 수의사를 제외하고는 별로 사용되고 있지 않아 외골격 고정 장치에 관해 소개해 드리고자 한다.

Purpose: We report case of neglected elbow dislocation for three years. Material and method: A 45 years old female patient presented with neglected elbow dislocation for three years. Since the patient had contracture in both lateral and medial collateral ligament of elbow, dissection was done. After total separation of posterior articular capsule and incision of anterior articular capsule, by manual manipulation, reduction of radiohumeral and ulnohumeral joints were obtained. We applied modified Morrey type hinged external fixation in the elbow and done early elbow exercise. Result and Conclusion: Since we have experienced a satisfactory result in the case with neglected elbow dislocation for 3 years by using open reduction and hinged external fixation, we report it with the literature.

This study is about university students' perception of intergenerational communication and its affecting factors. Based on literatures on intergenerational communication, the effect of stereotype on intergenerational satisfaction was testified and the mediating role of intergroup anxiety and the moderating role of communication competence were also analysed. Results showed that the effect of stereotype was mediated by intergroup anxiety, and the paths were different depending on the gender and communication competence. In the case of the male student group and the group with high communication ability, stereotypes affected intergroup anxiety, but intergroup anxiety did not directly affect intergenerational communication satisfaction. On the other hand, in the case of the female student group and the group with low communication skills, intergroup anxiety had significant effect on communication satisfaction. Also, it was found that the content of stereotype had an important effect on intergenerational communication satisfaction.

Purpose: We evaluated the effectiveness of temporary using the extendible external fixator (EF) for lengthening of soft tissue that contracture caused by tumor prosthesis removal in the treatment of complications after limb salvage surgery like deep infection and loosening. Materials and Methods: Five patients six cases were included who underwent extendible EF (Dyna-extor(r)). EF was applied after insertion of half pin to the proximal and distal bone of defect area. EF lengthening started at third day of post-operation, above 2-3 mm per day in the range of no neurological sign. Results: The treatment area was three in femur and two in tibia. Mean age when the time of EF apply was 22.2 years old (range 15-29), but its primary limb salvage operation had done in 13.4 years old (range 9-19), therefore mean times of interval between initial tumor prosthesis reconstruction and temporary EF apply was 8.8 years (range 3-14). One patient had EF for 150 days with 7.2 cm lengthening. Others 5 cases of 4 patients had EF for mean 37 days (range 25-50) and mean soft tissue lengthening was 5.8 cm. Three patients underwent re-insertion of tumor prosthesis and two patients underwent knee fusion as final operation and showed no evidence of infection through mean 22 months follow up period. Conclusion: Temporary using of extendible EF is an effective method for correction of leg shortening which occurred by soft tissue contracture in the complications of limb salvage operation or their treatment process, and it could be provide easily application of tumor prosthesis and knee fusion as final operation.

족관절 고정술은 족관절에 발생한 심한 관절염(end-stage arthritis)에서 전통적으로 시행하던 술식으로 현재 사용되고 있는 인공 족관절 치환술이 발달되기 전까지는 gold standard 로 알려져 있었으며 현재에도 심한 족관절의 변형이나 파괴, 족관절 인공 관절의 실패시에 시행되고 있다. 1897년 Albert가 처음으로 족관절 고정술을 소개한 이후로 30여가지가 넘는 방법이 행해지고 있으나 개방적 관절 고정술의 단점인 수술 후 염증, 유합 지연, 연부조직의 손상에 의한 피부 괴사 등을 극복하고 유합물을 높이기 위해 다양한 내고정 및 외고정 장치를 사용하고 있다. 관절경 술식이 발달하면서 Schnider가 1983년 최초로 관절경을 이용한 족관절 고정술을 발표한 이후로 관절경하의 족관절 고정술은 개방적 족관절 고정술 보다 높은 유합율과 적은 수술 합병증으로 인해 관심의 대상이 되었지만 족관절의 변형이 심한 경우에는 시행하기 어려운 단점이 있는 것으로 알려져 왔다.

The treatment of a brachymetacarpia using conventional distraction osteogenesis requires holding an external fixator following distraction for stability, which causes prolonged discomfort that adversely affects the patient's daily activities. This paper reports a case of a 20-year-old male of brachymetacarpia treated with distraction osteogenesis combined with a plate reducing the period of an external fixator, allowing rapid return to the daily activities, and presenting good clinical results.

The notable findings are as follow: First, men show higher level of perceiving open marriage, sex-role, acceptance of extra-marital relationship and intention of extra-marital sex than that of women. Second, the effects of men's perception of sex-role and open marriage, and the acceptance of extra-marital relationship have direct effects on their intention of extra-marital sex. The acceptance of extra-marital relationship shows the mediation effect on the path between that of open marriage and extra-marital sex among men. However, men's perception of sex-role does not affect the acceptance of extra-marital relationship. Lastly, women's perception of sex-role and open marriage, and the acceptance of extra-marital relationship directly influence the intention of extra-marital sex each, including the mediating effect of accepting extra-marital relationship between that of open marriage and extra-marital sex which are similar to the men's trend. Women's perception of sex-role show the direct effect on the acceptance of extra-marital relationship unlike man's trend. Therefore, the fact demonstrates that the effect of perceiving of sex-role on the intention of extra-marital sex is different according to gender. Some practical and political implications are discussed based on the study's findings.

In this paper, we proposed a new method of pre-operative planning for tibial deformity correction using double hexapod external fixator in pseudoacondroplasia. The 3-D computer graphic model of deformed tibia was reconstructed from 3 mm sliced CT data, and CAD model of double hexapod external fixator was developed. The fixator was composed of 170 mm diameter of three rings and 90 mm of twelve struts. The bone deformities and the osteotomy lines for double osteotomy were measured using X-rays, and the necessary joint values to correct the given deformities were obtained by inverse kinematics analysis. The computer graphic simulation was performed to visualize the deformity correction process and evaluate the analysis result. By examining the pre-op and post-op X-rays, the simulation result was in good agreement with the clinical outcomes.
