The Impact of Delayed Interval Delivery on Neonatal Mortality and Morbidity (지연 분만이 신생아의 사망률과 이환율에 미치는 영향)

  • Purpose: To evaluate the impact of delayed interval delivery on neonatal outcomes. Methods: This was a retrospective study of infants who were born at Seoul National University Hospital by delayed interval delivery from June 2005 to July 2010. Outcomes (neonatal mortality and morbidity) of later babies were compared to those of the first babies and the control group whose gestational ages and birth weights were similar to them. Results: There were 4 twin and 5 triplet pregnancies. The first babies (group 1, n=9) were delivered at $22^{+6}$ to $27^{+5}$ weeks of gestational age, and the later babies (group 2, n=14) were born at $24^{+6}$ to $28^{+0}$ weeks. The mean interval between the first and later deliveries was 10 days, and there was no delay between the second and third deliveries in all triplet pregnancies. There were more small for gestational age (SGA) infants in group 1 than group 2 (66.7% and 21.4% respectively, P=0.03). Two of three babies who died in group 1 were born before 24 weeks of gestational age and expired within a week after birth. The mortality rate of group 2 (7.1%) was lower than group 1 (33.3%), but not significantly (P=0.106). The control group matched to group 2 consisted of 28 infants. There were no significant differences in neonatal mortality and morbidity between the two groups. Conclusion: Although there is a limitation to the number of infants in this study, it suggested that delayed delivery in a multiple pregnancy could decrease the incidence of SGA of the remaining fetuses and that prolonged gestation would not be harmful to those fetuses after birth.

Systemic Candida Infection in Very Low Birth Weight Infants : Epidemiological Features Over 5 Years (극소 저체중 출생아에서 전신성 칸디다 감염 : 5년간의 역학적 특성)

  • Purpose: Candida infection has increased in neonatal intensive care units (NICU). However, recent reports on systemic candida infections in preterm newborns are rare in Korea. The aim of this study was to examine the epidemiological features of systemic candida infection in very low birth weight infants (VLBW) over the past five years. Methods: We retrospectively reviewed the medical records of 19 patients with systemic candida infections in VLBW that were admitted to the neonatal intensive care units of three hospitals affiliated with the College of Medicine, The Catholic University of Korea from January 2004 to December 2008. We analyzed the birth weight, gestational age, age at diagnosis, risk factors, co-morbidity, antifungal treatment, and mortality rates among the 19 patients. Results: Systemic candida infections occurred in 19 cases (4.7%) among the VLBW infants. The mean birth weight and gestational age were 959.0$\pm$255.9 g and 26.7$\pm$2.1 weeks. The isolated Candida species were C. albicans (4), C. parapsilosis (9), C. glabrata (2), C. famata (2), and unkown subspecies (2). Most patients had various associated risk factors, including a central venous catheter, broad spectrum antibiotics, parenteral nutrition, intravenous lipid emulsion, endotracheal intubation and $H_2$ blocker therapy. There was no significant difference in the risk factors between newborns that survived and those that died with regard to the systemic candida infection, except for gestational age. Nine (47.4%) out of 19 patients with a candida infection died and four cases (21.2%) were directly related to the candida infection. Conclusion: The prevalence of systemic candida infection is increasing in VLBW infants. The majority of Candida species has shifted to C. non-albicans, especially C. parapsilosis. Because of the high mortality associated with candida infection in the NICU, prophylaxis and early treatment based on epidemiological features is necessary.

Long-term outcomes of infantile spasms (영아 연축 환아의 장기적 예후에 관한 고찰)

  • Purpose : The aims of this study were to investigate the long-term outcomes in children with infantile spasms (IS) and to identify the prognostic factors influencing their neurodevelopment. Methods : We retrospectively evaluated seventy two children over five years old who were treated for IS at Asan Medical Center, Seoul, Korea, between 1994 and 2007. Forty-three children were contacted by telephone or medical follow-up to assess their current neurodevelopmental status. Multiple logistic regression was used to calculate odds ratios (ORs) and 95% confidence interval (95% CIs) of risk factors for unfavorable outcomes.Results : The mean follow-up duration for these 43 children was $7.2{\pm}1.5$ years (range, 4.5 to 13.0 years). Of these, 13 (30.2%) had cryptogenic and 30 (69.8%) had symptomatic IS. Eleven (25.6%) children were initially treated with adrenocorticotrophic hormone (ACTH) therapy, with a mean treatment lag of $1.3{\pm}1.9$ months (range; 0.1 to 7.0 months). Eighteen (41.8%) children clinically responded to initial treatment, as shown by EEG response. Overall, 22 (51.2%) children had at least moderate neurodevelopmental disorders and 2 (4.8%) died. In univariate analysis, etiology (symptomatic) and poor electroclinical response to initial treatment were related to long-term unfavorable outcomes. In multivariate analysis, response to primary treatment was the sole significant independent risk factor with a high OR. Conclusion : Overall prognosis of children with IS was poor. Electroclinical non-responsiveness to initial treatment was related to unfavorable long-term outcomes, indicating that initial control of seizures may be important in reducing the likelihood of poor neurodevelopment.

The Relationship between GDI(Gender Related Development Index) and the maternal and Child Mortality (여성관련개발지수와 모성 및 영아 사망률과의 관계)

  • Studies on the relation between socio-economic factors and metermal and child health have found that poverty, lack of edcation, inappropriate health serives are affecting to maternal and child health. The Gender Related Development Index (GDI) focuses on equality between men and women as well as on the average achiement of all people taken together, using same cariables as the Human Development Index (HDI) which are life expectancy, literacy rate, and per capita GDP. This research is to inverstigate whether HDI and GDI are useful determinants for maternal infant mortality. Using 146 UN member countries date, we condented multiple regression analysis for maternal and infant mortality with three models which are Model(individual variables-literacy rate, per capita GDP), Model(HDI) and Model(GDI). The results showed that HDI and GDI are powerful determinants of both maternal and infant mortality, respectively HDI($\beta$=-1.18, t=3.3; $\beta$=1.04, t=5.1) GDI($\beta$=-1.44, t=3.9; $\beta$=1.28, t=6.5) The higher power in model with GDI for both maternal and infant mortalities represented that GDI was more powerful determinant of maternal and infant mortality, than HDI respectively HDI($R^2$=0.824, $R^2$=0.842), GDI($R^2$=0.834, $R^2$=0.865). In conclusion, the maternal and infant mortalities are explained by GDI than HDI and may be lower in the societies where there are less discimination between men and women.

  • The neuronal ceroid-lipofuscinoses (NCLs) are a kind of neurodegenerative storage disorders. The NCLs are charecterizated by accumulation of autofluorescent lipofuscin or lipopigment in the brain. All NCL group belongs to in lysosomal storage disorders (LSDs), except Northern epilepsy. NCLs are the most common group of progressive neurodegenerative disorders in childhood, with an incidence as high as I in 12,500 live births. Four main clinical types have been described based on the onset age : infantile, late infantile, juvenile and adult types. Clinical symptoms of NCLs include loss of vision, seizures, epilepsy, progressive mental retardation and a premature death. Although mutation causes neurodegeneration in NCLs, the molecular mechanism by which mutation leads to neurodegeneration remains unclear. In this paper, we review the characteristics of these NCLs.

Estimating Direct Effects of Economic Benefits on Aquatic Animal Disease Prevention Project in South Korea (수산생물방역사업에 대한 경제적 편익의 직접적 효과분석)

  • The success or failure of aquaculture heavily depends on how effectively to manage aquatic animal diseases. The paper is to estimate the direct effects of economic benefits on aquatic animal disease prevention program using economic surplus analysis. The economic benefits include changes in consumer and producer surplus owing to a reduction effect of aquatic animal mortality and the consumption recovery effect. The annual changes in producer surplus for flounder, rockfish and others cultured is estimated as 2.33 billion won totally (0.87 billion won, 1.01 billion won and 0.45 billion won respectively) and the annual changes in consumer surplus 10.15 billion won totally (6.36 billion won, 1.85 billion won and 1.94 billion won respectively). Therefore, total annual economic benefit is 12.5 billion won.

A Kinetic Modeling for the Dynamics of Hybridoma Cells in Suspension Culture (현탁배양 하이브리도마 세포의 속도론적 모델링)

  • Batch suspension cultures of hybridoma cell were performed with various initial glutamine concentrations to investigate the effects of glutamine on cell growth and death, monoclonal antibody production, glucose and glutamine consumption, and the production of lactate and ammonium ion. An mathematical kinetic model was formulated to describe the kinetics of cell growth, the consumption of nutrients (glucose and glutamine), and the production of monoclonal antibody and waste metabolites (lactate and ammonium ion) based on experimental data. An equation for the specific growth rate was developed such that superimposed Monod equation in glucose and glutamine, with non-competitive type inhibition relations in ammonium ion and lactate. The inhibition constant for lactate was inversely proportional to the lactate concentration. The specific death rate was considered to be a function of glucose, glutamine, ammonium ion and lactate concentration.

  • Acute liver failure is a devastating disease in children. Most cases of acute liver failure in children are indeterminate; however, metabolic liver disease is one of the main causes in the pediatric age group. Though a major symptom of acute liver failure is hepatic encephalopathy, this is very difficult to diagnose, particularly in younger children. Liver transplantation has improved the chances of survival dramatically; however, it is not known which patients are ideal candidates for liver transplantation. Because patients may deteriorate rapidly, arranging care in a center with expertise will secure the best possible outcomes.

Surgical Treatment of Congenital Cystic Lung Disease (선천성 낭성 폐질환의 수술적 치료)

  • 배경: 폐격리증, 선천성 낭성 유선종 기형, 기관지 낭종 선천성 엽기종 및 낭성 기관지 확장증 등의 폐에 발생하는 선청성 낭성 질환은 않으며 유사한 발생학적 및 임상적 양사을 가진다. 대상 및 방법: 1972년부터 1999년까지 본원 흉부외과에서 선청성 낭성 폐질환으로 수술받은 46명의 환자를 대상으로 병상 기록을 통한 임상상, 치료 및 병리소견을 검토하였다. 결과: 환자의 평균 연령은 16.2세 였고 남녀비는 17:29였다. 주 증상은 감염에 의한 발령이 11례, 호흡곤란이 10례 그리고 흉통과 가래 등이였고, 증상이 없었던 경우가 13례 였다. 질환은 폐격리증이 13례, 선청성 낭성 유선종 기형이 12례, 기관지성낭종이 12례, 낭성 기관지 확장증이 5례 그리고 선천성 엽기종이 4례였다. 수술은 단순 절계 8례, 폐구역 절제술 6례, 폐엽 절제술 31례 그리고 전폐 전제술 1례를 시행하였고 수술 사망은 폐엽 절제술은 시행한 7세된 기관지 낭종 환자로 수술직후 발생한 뇌경색으로 1례에서 발생하였으며 술후 합병증으로는 폐렴 4례와 지속적 공기누출과 농흉이 각 1례씩 이었다. 결론; 선청성 낭성 폐질환을 가진 환자들은 진단 즉시 수술하여 좋은 결과를 얻었다.

Clinical Approaches to Patients with Congenital Malformations (선천성 기형의 임상적 접근)

  • Congenital malformation is observed in about 2-5% of newborns and is a leading cause of infant mortality. The prognosis of malformation is dictated mainly by proper treatment followed by correct diagnosis at an early age. In practice, etiological consideration and classification of a malformation is critical for diagnosis. Malformations can be classified as belonged to minor or major anomaly. It is clinically important to clarify the pathogenesis of the anomalies among malformation, deformation, disruption, and dysruption. Genetic counseling aids this process by helping patients or family members understand and the nature of the malformation and risk assessment.
