The Effect of Maternal Parenting Style and Sensitivity on Infant Development (어머니의 양육유형 및 민감성이 영아의 발달에 미치는 영향)

  • A total of 31 mothers of 5-month-old infants(18 boys and 13 girls) were observed and interviewed at home via a questionnaire format. Results revealed that perceived maternal parenting style had an effect on general infant general development. As mother exhibited more didactic and limit setting behaviors, the infants showed more receptive, expressive, and language development characteristics. Furthermore, the effects of maternal sensitivity on infant general development and language skills were also ascertained. The infants who showed more positive general development had mothers who interacted sensitively with them. Maternal sensitivity was the only factor that had a major effect on infant general development. The overall findings of this study indicate that perceived maternal parenting style and observed maternal sensitivity were found to have positive effects on general infant general development and language skills.

Long-Term Effects of the DHA Supplementation on Physical and Brain Development in Full-Term Infants (장기간에 걸친 DHA 보충이 영아의 신체발육 및 두뇌발달에 미치는 영향)

  • Recent research indicates that the n-3 fatty acid , docosahexaenoic acid(22 : 6n 3, DHA) plays an essential role in infant brain development . DHA is highly concentrated in brain and retinal tissues and accumulates during late fetal and early neonatal life. Diets deficient in DHA are associated with reduced levels of DHA in brain and retinal tissues. The purpose of this study is to investigate the long term effects of DHA supplementation on the growth and mental development of full-term infants. THirty four healty infants were recruited from those who were delivered at Kyung Hee Medical Center. The experimental groups were the breast milk+DHA(-) group who were fed human milk for 20 weeks after birth and thereafter were fed placebo formula for 28 weeks, the breast milk+DHA(+) group who were fed human milk for 20 weeks after birth and thereafter were fed DHA supplemented formula for 28 weeks, DHA(-) group who were fed placebo formula for 48 weeks, and DHA(+) group who were fed DHA supplemented formula for 48 weeks. The daily average intake of DHA for the breast milk+DHA(-) , breast milk+DHA(+), DHA(-) and DHA(+) groups were 39.1mg, 89.9mg, 17.7mg, and 160.224mg, respectively. The results showed that measurements of infant weight, length, head, and chest circumferncewere all in normal range and they were not influenced by the DHA supplements in their diets. There was a significant correlation between dietary DHA intake and erythrocyte DHA level. The results of flash visual evoke potential (VEP) test were not correlated with eerythrocyte DHA and dietary DHA levels at 48 weeks of age. No differences were found in Bayley mental and Psychomotor Development lndex scores among the four experimental groups at 48 weeks of age. Unlike the short-term effects there was no long-term effect of relatively small amounts of dietary DHA supplements on the scores for flash VEP and Bayley test, even thour호 there was an elevated DHA supplements on the scores for flash VEP and Bayley test, even through there was an elevated DHA content in the infants erythrocytes.

The Effects of Early Cumulative Risk Factors on Children's Development at Age 3 - The Mediation of Home Learning Environment - (유아기 발달에 대한 생애 초기 가족 누적위험요인의 영향 - 가정학습환경을 매개로 -)

  • The purpose of this study was to examine the structural models in which early cumulative risk factors affect children's language(indicated by expressive vocabularies) and social development(indicated by peer competence) at age 3 thorough their effects on the home learning environment. To examine the hypothesized models, the data of 1,725 families from the second and the fourth waves of the Panel Study of Korean Children was used. Correlation analysis and structural equation modeling were conducted to test the models. First, the cumulative risk factors at age 1 and 3 were highly correlated, implying the stability of the risk factors over time. The more cumulative risk factors at age 1 predicted the lower level of the home learning environment at age 3, which, in turn, was significantly related to both language and social development at age 3. However, the early cumulative risk factors did not directly influence later developmental outcomes. Moreover, the cumulative risk factors at age 3 were directly related to the child's language development, but neither social development northe home learning environment. In addition, the mediational role of the home learning environment (i.e., cumulative risk factors at age 1${\rightarrow}$home learning environment${\rightarrow}$language and social development) was statistically supported. In conclusion, the early cumulative risk factors in infancy indirectly predicted children's development at age 3 through the home learning environment. The practical implications for the early intervention and support for the families with infants who are experiencing multiple risk factors were discussed.

Long-term outcomes of infantile spasms (영아 연축 환아의 장기적 예후에 관한 고찰)

  • Purpose : The aims of this study were to investigate the long-term outcomes in children with infantile spasms (IS) and to identify the prognostic factors influencing their neurodevelopment. Methods : We retrospectively evaluated seventy two children over five years old who were treated for IS at Asan Medical Center, Seoul, Korea, between 1994 and 2007. Forty-three children were contacted by telephone or medical follow-up to assess their current neurodevelopmental status. Multiple logistic regression was used to calculate odds ratios (ORs) and 95% confidence interval (95% CIs) of risk factors for unfavorable outcomes.Results : The mean follow-up duration for these 43 children was $7.2{\pm}1.5$ years (range, 4.5 to 13.0 years). Of these, 13 (30.2%) had cryptogenic and 30 (69.8%) had symptomatic IS. Eleven (25.6%) children were initially treated with adrenocorticotrophic hormone (ACTH) therapy, with a mean treatment lag of $1.3{\pm}1.9$ months (range; 0.1 to 7.0 months). Eighteen (41.8%) children clinically responded to initial treatment, as shown by EEG response. Overall, 22 (51.2%) children had at least moderate neurodevelopmental disorders and 2 (4.8%) died. In univariate analysis, etiology (symptomatic) and poor electroclinical response to initial treatment were related to long-term unfavorable outcomes. In multivariate analysis, response to primary treatment was the sole significant independent risk factor with a high OR. Conclusion : Overall prognosis of children with IS was poor. Electroclinical non-responsiveness to initial treatment was related to unfavorable long-term outcomes, indicating that initial control of seizures may be important in reducing the likelihood of poor neurodevelopment.
