K-means 알고리즘을 통한 연하 곤란 환자의 심각도를 확인하는 프로그램 개발 연구 (A study on the development of a program to check the severity of dysphagia patients using the K-means algorithm)

  • 현대인들은 과거에 비해 풍부한 먹거리와 다양한 삶의 형태를 가지게 되었으나 바쁜 생활 속에 아침을 거르게 되고, 제 시간에 식사를 하지 못하는 등의 올바르지 못한 식습관을 형성하게 되었다. 이러한 식습관은 장기간 유지되면서 소화기관 장애로 이어지게 된다. 그에 가장 쉽게 발생하는 증상이 역류성 식도염과 삼킴 장애라고 불리는 연하 곤란이 있으며, 그 중 연하 곤란은 다양한 합병증의 형태로 발전하거나 위암, 후두암등의 전조증상으로 확인되기도 하여 빠르고 정확한 진단이 요구된다. 이에 따른 진단 결과는 현재도 의사가 수동적으로 판단하며 그 결과가 제각각이다. 여기서 말하는 진단 결과는 중증 정도를 말하는 것이며, 그에 따른 치료법이나 합병증을 파악할 때의 잘못된 진단으로 과한 치료나 부족한 대처로 이어지게 될 수도 있다. 본 논문에서는 연하 곤란의 심각 정도를 파악하기 위해 연하 곤란 진단 과정에서 식도로 이어지는 구간에 후두개곡과 이상와 부에 남는 잔여 음식물을 확인하기 위한 X-ray 이미지 처리에 K-means 알고리즘을 사용하는 프로그램을 개발하는 것을 연구하였다.

입체조를 병행한 혀 근력운동이 노인의 연하장애와 연하력 및 설압에 미치는 효과 (Effects of tongue strength training program combined with oral exercises on swallowing disorder, swallowing ability, and tongue pressure in the elderly)

  • Objectives: The purpose of this study is to verify the usefulness and practicality of the program in the daily life of the elderly by performing a three-dimensional tongue muscle exercise for the elderly to identify the dysphagia, swallowing ability, and tongue pressure. Methods: The subjects of the study were 29 elderly people aged 65 or older living in Busan, divided into a oral exercises group and tongue strength training program combined with oral exercises group, and operated 16 times for 8 weeks twice a week, and the pre and post-program evaluation was conducted in the 1st and 8th weeks. For variable selection, 7 general characteristics, 5 oral health-related characteristics, 5 Likert scale for swallowing disorder, repetitive swallowing ability test for swallowing power measurement, and tongue pressure measurement was used. Data analysis was conducted using SPSS window program 25.0 version (IBM), general characteristics and homogeneity tests, oral health-related characteristics and homogeneity tests were chi-square test, swallowing disorder, swallowing ability, and change in tongue pressure were paired t-test, Wilcoxon signed rank test, t-test, Mann-Whitney U test for swallowing disorder and changes in swallowing ability and tongue pressure, and Spearman's correlation for the relationship between swallowing disorder and swallowing ability and tongue pressure. Results: As a result of tongue strength training program combined with oral exercise, both relieve swallowing disorder and improve low eating and tongue pressure than oral exercise, and the difference in improvement effects of both relieve swallowing disorder, swallowing ability, and tongue pressure is higher. Conclusions: Based on the results of this study, it is believed that simple and practical oral muscle function reinforcement products and practical use are needed, and institutional devices to seek oral health promotion programs for the elderly are needed.

연하장애가 있는 뇌졸중 환자의 영양상태, 자각증상 및 정보요구도 (A Study on the Nutritional Status, Symptoms, and Information Needs in Stroke Patients with Dysphagia)

  • Purpose: The purposes of this study were to reveal the nutritional status and symptoms related to dysphagia and to identify the information needs of the patient with post-stroke dysphagia for self care. Methods: Fifty one subjects were selected among patients being admitted from 3 tertiary hospitals. Data were collected using questionnaires, interview, and medical record. Data analysis was done using descriptive statistics with SPSS. Results: 1) The mean hemoglobin, serum albumin, and hematocrit level of the subjects were $11.2{\pm}1.8g/dL$, $3.2{\pm}0.6g/dL$, and $33.5{\pm}2.3%$, respectively. 2) The most common symptom due to dysphagia was 'slurred speech (86.3%)', followed by 'less flexible in tongue and mouth movement (80.4%)', 'difficulty chewing and swallowing (74.5%)'. 3) The highest score of information needs for patients/caregivers was to know which foods are suitable for dysphagic patients or not ($2.67{\pm}0.48$ of 3.0 score). They also want to know how to swallow safely ($2.65{\pm}0.59$), to administer medication safely ($2.63{\pm}0.59$), and to learn rehabilitative techniques ($2.61{\pm}0.57$). Conclusion: These findings would be useful information for staffs to do multidisciplinary approach and they would be necessary for stroke patients to manage their symptoms.

뇌졸중 후 연하장애 전침 치료의 임상연구에 대한 고찰 (Literature Review of Clinical Research Electro-Acupuncture Treatment on Post-stroke Dysphagia)

  • The aim of this study was to collect and analyze clinical studies on electro-acupuncture therapies of post-stroke dysphagia to suggest the methods of the study about electro-acupuncture therapy. In this study, electronic searches were performed with searching sites. Searched words such as '중풍', '뇌졸중', '연하장애', '전침', 'stroke', 'dysphagia', 'deglutition disorders', 'electro-acupuncture' were used as a single keyword or in combinations. Through searching, 27 studies were selected and analyzed. In comparison of results before and after electro-acupuncture treatment, there were statistically significant improvement in 26 studies by using assessment such as video fluoroscopy swallowing study, swallowing water function test. The study of electro-acupuncture treatment for post-stroke dysphagia needs to be standardized and high-quality study models should be introduced.

연하장애를 가진 노인에 대한 노인요양시설 간호사의 경험: 포커스 그룹 연구 (Nurses' Perception and Experiences at Nursing Home Residents with Dysphagia: Focus Group Interviews)

  • Purpose: This study aimed to describe Korean nursing home nurses' perceptions on dysphagia management and their working experiences of older adults with dysphagia. Methods: Using a purposive sampling design, 23 eligible nurses were interviewed as four focus groups from 4 facilities out of nursing homes. The qualitative data from focus group interviews were analyzed using thematic analysis to classify common themes into larger categories. Results: Two main themes on the attributes of caring nursing home residents with dysphagia were 'Need for nursing resources' and 'Need for support'. In addition, four subthemes emerged as a result of analysis: 'need for nursing protocols for assessing and managing dysphagia', 'need for emergency care skills of nurses and nurse assistants', 'need for prompt and accurate management by cooperation of physicians and therapists', and 'need for partnership upon comprehension of visiting family members concerning dysphagia'. Conclusion: Nursing home nurses fully recognized the importance of dysphagia management; however, they experienced substantial barriers due to lack of adequate nursing protocols or partnership with family caregivers, insufficient training for emergency care, and deficient support from medical staff. Development of nursing guidelines tailored to the nursing home context and based on partnership among medical experts and family caregivers is needed.

구강운동촉진기술(OMFT)을 적용한 조기 연하재활치료가 프래더 윌리 아동의 섭식 발달에 미치는 영향 - 발달에 근거하여 : 사례 보고 (The effect of Early Feeding Therapy by Oral Motor Facilitation Technique(OMFT) on Feeding development of Prader-Willi Syndrome baby - Based on the Development : Case study)

  • 본 연구는 섭식장애가 있는 프래더 윌리(PWS) 아동을 대상으로 구강운동촉진기술(OMFT) 적용 후 섭식발달의 변화를 확인하고 전반적인 발달과 비교·분석하여 OMFT가 섭식발달에 미치는 효과를 알아보고자 하였다. 대상 아동은 경관 식이로 섭식 중인 교정 연령 8개월 PWS 여자 1명으로 2020년 7월부터 2021년 6월까지 OMFT를 주 1회, 1년 동안 제공하였다. OMFT 적용 후 구강 섭식 전환 및 연령에 맞는 음식 섭취, 씹기 기능 증진, 빨대 사용, SSB 조절 향상 등의 섭식발달 단계 변화가 관찰되었다. 본 연구를 통해 OMFT가 PWS 아동의 섭식 발달 및 구강 섭취 전환에 효과적인 것으로 확인되었다. PWS 아동의 섭식 및 연하장애에 대한 조기 평가 및 치료 접근이 중요하며 근긴장도 및 근력 저하의 구강운동기능 문제 해결을 위한 OMFT 적용을 통한 섭식 발달 향상을 위한 접근이 필요할 것이다.

치과위생사의 연하장애 지식 및 예방 태도가 노인구강건강 관련 교육요구도에 미치는 영향 (Effects of dental hygienist knowledge of dysphagia and preventive attitudes on educational needs for oral health in the older adults)

  • Objectives: In this study, we aimed to examine the dysphagia knowledge, preventive attitudes toward dysphagia, and educational needs concerning geriatric oral health, of dental hygienists, and to provide fundamental information for recognizing the necessity of knowledge and education concerning geriatric oral health and for increasing educational needs. Methods: From April 1 to June 30, 2022, a questionnaire survey was administered to 198 dental hygienists via an online link. SPSS Statistics ver. 22.0 was used to conduct the frequency analysis, t-test, one-way ANOVA, Pearson's correlation coefficient, and multiple regression analysis. Results: Regarding the difference in knowledge of dysphagia depending on general characteristics, the study participants had more knowledge of dysphagia when they were more interested in dysphagia and had a higher geriatric treatment frequency. The preventive attitudes toward dysphagia were strong in those who graduated from graduate school or higher, had more prevention and education at work, became more interested in dysphagia, received more education about dysphagia, had intention to receive additional education on dysphagia, and had a higher geriatric treatment frequency. Positive correlations were observed between knowledge of dysphagia, preventive attitudes toward dysphagia, and educational needs related to geriatric oral health. The educational needs related to geriatric oral health were found to be, significantly and positively influenced by preventive attitudes toward dysphagia, having master's degree or higher, knowledge of dysphagia, and intention to receive additional education on dysphagia. Conclusions: It is necessary to improve the quality of oral health services offered to older patients by having accurate knowledge of dysphagia implementing appropriate prevention practices for dysphagia, and understanding the educational content needed by the oral hygienists and developing an effective education program to enhance their professionalism.

식도 스텐트 삽입후 발생한 식도 방선균증 - 수술 치험례 - (Esophageal Actinomycosis after Insertion of Esophageal Stent -A Case of Surgical Experience -)

  • 방선균증은 혐기성 그람 양성균인 Actinomyces israelii의 감염에 의한 만성 육야종성 질환으로 안면부, 폐, 목부에 주로 발생하며 농양, 누공 두터운 반흔의 형성을 특징으로 한다. 식도 방선균증은 매우 드물어 전세계적으로 단지 몇례 보고되어 있으며, 그 대부분이 후천성 면역결핍증 환자에서의 발병이다. 염산의 오연으로 인한 식도협착이 발생한 58세의 여자에서 식도 스텐트 삽입후 흉통, 연하장애, 연하통이 발생하였고 이에 대한 식도절제술후 병리조직학적으로 식도 방선균증으로 확진된 증례를 보고한다.
