MRI Study of the Degenerative Radial Tear of Medial Meniscus (내측 반월상 연골판 후각부의 퇴행성 파열에서 MRI를 이용한 두께의 변화)

  • Purpose: To evaluate the thickness of the posterior horn of the medial meniscus accompanying with degenerative radial tear. Materials and Methods: We retrospectively reviewed 170 cases which show degenerative meniscal tear with variable degree of meniscal degeneration from February 2000 to February 2010. All cases were older than 40 years and 57 cases were men and 113 cases were women. Mean age were 55-year-old. We grouped the cases into 3 categories. Group A were composed with cases which showed horizontal and radial tear in posterior horn of medial meniscus. Group B showed horizontal tear only and group C showed intrasubstance degeneration without meniscal tear. Results: The mean thickness of medial meniscus posterior horn in group A, B, C were 7. 44 mm, 6.52 mm, 6.04 mm respectively. Group A showed significant increase of the thickness of medial meniscus posterior horn than group B, C. Group B also showed significant increase of thickness than group C. The degree of meniscal degeneration was highest in group A, which showed significant higher meniscal degeneration than group B and C, and, group B showed higher degeneration than group C, however, there was no statistically difference between group A and B regarding the degree of meniscal degeneration. Conclusion: The thickness of medial meniscus posterior horn was increased when accompanied with radial tear, which may elicit pain caused by meniscal impingement.

Change in the Levels of Intracellular Antioxidants during Aging of Articular Chondrocytes and Cartilage (연골세포 및 관절연골의 노화 과정에서 세포내 항산화 인자들의 변화)

  • Cartilage diseases, such as rheumatoid arthritis (RA) and osteoarthritis (OA), are associated with the loss of chondrocytes and degradation of articular cartilage. Recent studies revealed that inflammatory reactive oxygen species (ROS) and age-related oxidative stress can affect chondrocyte activity and cartilage homeostasis. We investigated changes in the levels of intracellular antioxidants during cellular senescence of primary chondrocytes from rat articular cartilages. Cellular senescence was induced by serial subculture (passages 0, 2, 4, and 8) of chondrocytes and measured using specific senescence-associated ${\beta}$-galactosidase ($SA-{\beta}-gal$) staining. ROS production increased significantly in the senescent chondrocytes. In addition, total glutathione (GSSG/GSH) and superoxide dismutase (SOD) levels and heme oxygenase-1 (HO-1) expression increased in senescent chondrocytes induced by serial subculture. Analysis of changes in intracellular antioxidant levels in articular cartilage from rats of different ages (5, 25, 40, and 72 wk) revealed that total glutathione levels were highest after 40 wk and slightly decreased after 72 wk as compared with those after 25 wk. SOD and HO-1 expression levels increased in accordance with age. Based on these results, we conclude that intracellular antioxidants may be associated with cartilage protection against excessive oxidative stress in the process of chondrocyte senescence and age-related cartilage degeneration in an animal model.

Medial Discoid Meniscus - A Report of Two Patients - (내측 원판형 연골 - 2명 보고 -)

  • Although the lateral discoid meniscus has been observed frequently, the medial discoid meniscus is an extremely rare anomaly. The incidence of lateral discoid menisci ranges from 1.4% to 15.5% and the incidence of medial discoid menisci ranges from 0.06% to 0.3%. We present 1 patient of bilateral medial discoid menisci with tear on left side and 1 patient of unilateral medial discoid meniscus with tear, that were diagnosed by magnetic resonance imaging (MRI) and arthroscopy. We performed partial meniscectomy on 2 cases of symptomatic medial discoid menisci under arthroscopy and meniscus-related symptoms were relieved. We report 3 cases of 2 patients with medial discoid meniscus.

Anomalous Insertion of the Anterior horn of the Discoid Lateral Meniscus into the Anterior ACL - A Case Report - (외측 원판형 연골 전각부의 전방십자인대 전방부로의 부착변형에 관한 증례보고)

  • The lateral meniscus is morphologically more variable than the medial meniscus. An abnormal lateral meniscus also varies with respect to its size, shape and stability, and such variations can occur in any patient of any age. The most common variant is a discoid lateral meniscus. We recently encountered a patient with a discoid lateral meniscus for which the anterior horn of the meniscus was anomalously inserted into the anterior ACL. The patient was forty five years old women who had persistent pain for three to four years without any specific trauma history. She was preoperatively diagnosed as having discoid lateral meniscus by MRI, and was confirmed the presence of the complex tear and anomalous insertion of the anterior horn of the discoid lateral meniscus into the anterior ACL by arthroscopic examination. Arthroscopic subtotal meniscectomy was performed and the symptoms were improved after surgery.

  • Extraskeletal chondroma is a benign soft tissue tumor which is composed of hyaline cartilage but arises from the fibrous stroma rather than from mature cartilaginous or osseous tissue. Extraskeletal chondroma is relatively rare and occurs most frequently in the soft tissue around the joints of hands and feet. We present one case of extraskeletal chondroma in a finger of a young woman.

  • 골관절염은 관절연골의 퇴행성 변화로 연골기질의 분해로 인하여 연골 강도와 cushion으로서의 능력이 감소되는 질환이다. 골관절염의 대부분 약물치료는 통증과 염증을 감소시키는 목적으로 사용되며, 근본적인 치료효과를 주는 약물은 현재까지 개발되어 있지 않다. 영양약학 제품에 대한 질병의 예방적, 치료 보조적 차원에서 인체에 대한 기초적인 생리활성의 중요성을 인식하게 됨으로써 그 역할이 중요하게 인식되고 있다. 특히, 골관절염 치료에 쓰이는 glucosamine은 proteoglycan (PG)와 glucosaminoglycans(GAGs)의 합성의 전구물질로서 연골세포 생성을 자극하며, 통증, 염증의 경감 및 진행과정을 억제시키고, 관절기능 회복을 촉진하는 것으로 알려져 있다. 그러나 glucosamine이 어떤 기전에 의해 관절연골세포에 직접적인 영향을 주는지 밝혀져 있지 않다. 본 연구에서는 glucosamine sulfate가 연골세포에 미치는 영향을 규명하고자 하였다.

  • Meniscal allograft transplantation is considered to restore important functions of the meniscus after total and subtotal meniscectomy. There are patients who need meniscal allograft transplantation have concomitant lesions; chronic anterior cruciate ligament tear, chondral defect, and malalignment of the knee. Therefore proper selection of the patients and thorough preparation of operative procedure are imperative to get satisfactory results.

  • 최근에 스포츠 손상이 증가하고 관절경을 이용한 치료가 발전하면서 조기에 더 적극적인 치료를 하는 추세이다. 또한 방사선 소견에 비하여 관절경하에서는 연골이 불안정하거나 분리되어 있는 경우도 있으므로 기존의 수술 적응증보다는 좀 더 광범위하게 관절경 검사 및 수술적 치료가 요구된다고 생각한다. 치료 방법을 하면 $1.5cm^2$ 이하의 병변을 가진 50세 이하의 환자는 관절경을 이용하여 변연 절제술, 연골하 천공, 연마, 미세 골절술, 소파술 등의 방법으로 치료할 수 있다. 같은 방법으로 50세 이상의 $3cm^2$ 이하의 병변을 가진 환자 중 mosaicplasty와 자가 연골 세포 이식술을 적용할 수 없는 환자에서 시도해볼 수 있다. $1.5\sim3cm^2$의 병변을 가진 50세 이하의 환자, 그전의 관절경적 치료로 실패한 경우에는 자가골 연골 이식 또는 자가 연골 세포 이식술을 이용하여 치료해야 한다. $3cm^2$ 이상의 병변을 가진 50세 이하의 환자는 자가 연골 세포 이식술이나 동종 골 연골 이식을 이용하여 치료하며, 50세 이상의 환자는 관절 고정술이나 족근 관절 인공치환술을 고려하는 것이 바람직하다.

Promoting effects of Transforming growth Factor-$\beta$2 on Chondrogenic Differentiation in vitro (Transforming Growth Factor-$\beta$2에 의한 연골세포 분화 촉진 효과)

  • 계배 limb bud 간충직 연골원성 세포로부터 연골세포로의 분화에 미치는 transforming growth factor-f2(TGF-$\beta$2)의 영향을 알아보기 위하여, Hamburger-Hamilton stages 23/24의 간충직 세포들을 미세배양법으로 배양하면서. TGF-$\beta$2의 농도 및 처리시간에 따른 연골세포의 분화에 미치는 영향을 조사하였다 그 결과 TGF-$\beta$2는 배양 첫 24시간 동안 1-2 ng/ml의 농도로 처리하였을 때 가장 효과적으로 연골세포의 분화를 촉진하였으며, 또한 TGF-$\beta$2의 처리군에서 배양 3일째에 tsss) sulfate의 glycosaminoglvcan으로의 유입량이 현저히 증가함을 보였다 한편. 배양 48시간내에TGF-$\beta$2를 처리한 경우 분화를 촉진 유도한 반면, 배양 48시간 이후에 처리하였을 때에는 분화 촉진 효과가 나타나지 않았다. 이상의 결과로부터 TGF-$\beta$2는 연골원 세포의 분화 초기단계에 세포외기질의 합성을 촉진시켜 세포응축을 유발하고. 세포-세포 및 세포-세포외기질의 상호작용을 증대시킴으로써 연골세포로의 분화를 촉진시킬 것으로 추정되었다.

  • Posterior root tear of the medial meniscus are well known. Although very rare, there are reports of anterior root tears of the medial meniscus but no reports on traumatic bony avulsion of the anterior medial meniscus root. This paper reports a case of an isolated bony avulsion of the anterior medial meniscus root, which was successfully repaired arthroscopically.
