The Study on Method of Speech Audiometry and the Korean Word Lists Part 1 : Normal Hearing Group (어음청력점사방법과 한국어음청력검사표에 관한 연구)

  • Among the various audiometric test batteries, pure tone and speech audiometry was carried as routine. During the past 20 years, there have been works on speech word list for Korean language. As a preliminary work for standardization of Korean speech word list, the authors are intended to study on the factors of test methods with these proposal speech word lists. Now, authors has studied as followings methods and materials and the results were obtained as followings. A. Materials and Methods a. Word List 1. Paik Chun Kee (Paik) 2. Soh Jin Myoung (Soh) 3. Cynn Kyu Shik (Cynn) 4. Lee Jong Dam (Lee) 5. Hahm Tae Young (Hahm) b. Test Method 1. ascending 2. descending c. Test Material 1. live voice 2. Recorded male voice 3. Recorded female voice B. Results 1. Score of ascending method was better result than descending method. 2. Speech reception threshold and discrimination score were better in recorded female voice, live voice and recorded male voice in that order. 3. Speech reception thresholds among tested word lists were Hahm, Lee, Soh, Cynn and Paik in that order. 4. Discrimination score was highest with Cynn and Lee, Soh, Hahm, Paik in that order.

  • 본 연구에서는 단일 청력검사 모듈에서 여러 가지 작업들을 동시에 운용할 수 있는 멀티스레딩 기법의 구현을 통하여, 청력검사 시스템에서 피검사자가 검사가 진행되는 동안 언제든지 응답 결과를 확인하거나 또는 수정할 수 있도록 하는 휴대용 어음청력검사 시스템을 구현하고자 하였다.

The Correlation Between Speech Reception Threshold and Pure Tone Audiometry (어음청취역치와 순음청력검사의 상관관계에 관한 연구)

  • Speech reception threshold is a base for word discrimination testing, but it also serves as a check for the reliability of pure tone audiogram. In order to investigate the correlation between SRT and PTA these tests were carried out in patients with conductive hearing loss and normal hearing, using Grason-Stadler 1702 Audiometer. The results were as follows; 1) The difference between the scores of SRT and PTA's was 2.4 dB with a range of -3.3 dB∼+8.3 dB in conductive hearing loss, and was 1.9 dB with a range of -6.7 dB∼+5 dB in normal hearing group. 2) The difference between the scores of SRT and each speech frequency of PTA was 6 dB at 500 Hz, 3 dB at 1,000 Hz and 8.8 dB at 2,000 Hz in conductive hearing loss, and 3 dB at 500Hz, 2 dB at 1,000Hz, and 5dB at 2,000Hz in normal hearing group.

CLINICAL STUDY OF HEARING LOSS IN CHRONIC RENAL FAILURE (혈액 투석요법을 받고 있는 만성 신부전증 환자의 청각학적 고찰)

  • 1987년 2월 1일 부터 1987년 3월28일까지 본 병원 인공신장실에서 혈액투석요법을 받고 있는 만성 신부전증 환자 20례 (남자 15례, 여자 5례)를 대상으로 2회의 청력검사를 시행하여 다음과 같은 결과를 얻었다. 1) 만성 신부전증으로 혈액투석요법을 받고 있는 환자 20례 중 7례 (35%)에서 청력손실을 보였다. 2) 연령분포상 20대에서는 2명중 청력손실을 보인 예는 없었으며, 30대는 7명중 1명, 40대는 5명중 4명, 60대는 1명중 1명에서 청력손실을 보였으며 7례 전례가 남자였다. 3) 청력손실의 양상은 전례에서 양측성이며 고음장애형이었다. 4) 임피던스 청력검사상 tympanogram A형이 2귀 였으며 2례(4귀)에서는 acoustic reflex가 나타나지 않았다. 5) 청력손실을 보인 7례중 3례에서 75% 이상의 SISI score를 나타내었고 어음청력검사상 25㏈ 이상의 청력손실을 보인 예는 없었다. 6) 전해질농도와 난청의 정도, 혈액투석회수와 청력손실의 정도에는 유의한 연관이 없었다.

Articulation Scores and Confusion Patterns of the 100 Monosyllable Korean Speech Sounds (우리말 100단음절의 명료도와 오청상에 관한 연구)

  • It is well known that speech signals are the most riliable materials for the hearing test and there are various difficult problems in the selection of these materials. Because of these difficulties, there is not a confirmed test material of Korean speech sound up to date. For the basis of the test materials, author had studied articulation scores and confusion patterns of 100 monosyllable korean speech sounds in normal listners, in normal listners under various noisy (white noise and speech noise) circumstances, and in patients with hearing loss, The results reveal as follows. 1. Except for perceptive deafness with poor articulation score, Confusion was occured among initial consonants, vowels and final consonants respectively according to their distinctive features under above various test conditions. 2. There is remarkable differences in articulation scores between different kindes of noise under some intensity levels.

A Study of Acoustic Masking Effect from Formant Enhancement in Digital Hearing Aid (디지털 보청기에서의 포먼트 강조에 의한 마스킹 효과 연구)

  • Although digital hearing aid algorithms have been developed to compensate hearing loss and to help hearing impaired people to communicate with others, digital hearing aid user still complain about difficulty of hearing the speech. The reason could be the quality of speech through digital hearing aid is insufficient to understand the speech caused by feedback, residual noise and etc. And another thing is masking effect among formants that makes sound quality low. In this study, we measured the masking characteristics of normal listeners and hearing impaired listeners having presbyacusis to confirm masking effect in speech itself. The experiment is composed of 5 tests; pure tone test, speech reception threshold (SRT) test, word recognition score (WRS) test, puretone masking test and speech masking test. In speech masking test, there are 25 speeches in each speech set. And log likelihood ratio (LLR) is introduced to evaluate the distortion of each speech objectively. As a result, the speech perception became lower by increasing the quantity of formant enhancement. And each enhanced speech in a speech set has statistically similar LLR, however speech perception is not. It means that acoustic masking effect rather than distortion influences speech perception. In actuality, according to the result of frequency analysis of the speech that people can not answer correctly, level difference between first formant and second formant is about 35dB, and it is similar to result of pure tone masking test(normal hearing subject:36.36dB, hearing impaired subject:32.86dB). Characteristics of masking effect is not similar between normal listeners and hearing impaired listeners. So it is required to check the characteristics of masking effect before wearing a hearing aid and to apply this characteristics to fitting.

Changes of Acoustic Reflex Thresholds and Speech-In-Noise Perception Using Personal Listening Device Under Subway Interior Noise (지하철 실내 소음 하에서 음향기기 사용에 따른 음향반사역치와 소음 속 어음인지 변화)

  • Although it is well-known that environmental noise can lead to hearing loss in individuals, the true extent of subway noise effects in the general population remains poorly understood. The purpose of the present study is to see changes of acoustic reflex thresholds and speech perception scores when passengers listen to music presented from their personal listening device in the subway. Forty subjects with normal hearing participated being divided into two groups, experimental and control groups. As a baseline, all subjects were measured by acoustic reflex thresholds in five test frequencies and Korean speech perception in noise (KSPIN) test at 0 and -5 dB SNR. In the experiment, the control group read newspaper or magazine in the subway noise, whereas the experimental group listened to music presented from their smartphone under the subway noise through speakers at 73.45 dBA for 60 min. After completing the experiment, two groups also conducted both acoustic reflex thresholds and KSPIN tests in the same condition as the baseline. The results showed that there was a significant difference of correct percent in speech-in-noise test between experimental and control groups and of that between two signal-to-noise ratios, which means the double noise exposure of 60 min might cause some degradation of speech perception when noise increases compared to only subway noise condition that was not statistically significant difference. We concluded that a risk of some degraded speech perception ability would be expected when passengers have a habit of listening to music in the subway noisy situation for a long duration.

Audiological Findings of Congenital Ichthyosiform Dermatosis with Sensorineural Hearing Loss (난청을 동반한 선천성 어린선상 피부병의 청각학적 소견)

  • The ichthyosiform dermatosis is consisted of a heterogenous group of hereditary disorders. Authors report a case of 26 year old female patient who has congenital ichthyosiform dermatosis associated with sensorineural hearing loss, neovascularization of cornea, partial alopecia and abnormalities of nails and teeth. The audiological findings of this patient were as follows: 1. Bilateral severe sensorineural hearing loss. 2. SISI scores: 100% at 1, 000 Hz and 4, 000 Hz on both ear. 3. Tone decay test: 25 dB decay at 1, 000 Hz and 4, 000 Hz respectively on both ear. 4. SRTs : 85 dB on both ear. 5. Speech discrimination scores : 80% at 95 dB hearing level(MCL) on both ear. With above results, cochlear lesion is suspected in this syndrome.

Application of sinusoidal model to perception of electrical hearing in cochlear implants (인공와우 전기 청각 인지에 대한 정현파 모델 적용에 관한 연구)

  • Speech consists of the sum of complex sine-waves. This study investigated the perception of electrical hearing by applying the sinusoidal model to cochlear implant simulation. Fourteen adults with normal hearing participated in this study. The sentence recognition tests were implemented using the sentence lists processed by the sinusoidal model which extracts 2, 4, 6, 8 sine-wave components and sentence lists processed by the same sinusoidal model along with cochlear implant simulation (8 channel vocoders). The results showed lower speech recognition for the sentence lists processed by the sinusoidal model and cochlear implant simulation compared to those by the sinusoidal model alone. Notably, the lower the number of sine-wave components (2), the larger the difference was. This study provides the perceptual pattern of sine-wave speech for electrical hearing by cochlear implant listeners, and basic data for development of speech processing algorithms in cochlear implants.

Acoustic Masking Effect That Can Be Occurred by Speech Contrast Enhancement in Hearing Aids (보청기에서 음성 대비 강조에 의해 발생할 수 있는 마스킹 현상)

  • In most of hearing aids, amplification algorithms are used to compensate hearing loss, noise and feedback reduction algorithms are used and to increase the perception of speeches contrast enhancement algorithms are used. However, acoustic masking effect is occurred between formants if contrast is enhanced excessively. To confirm the masking effect in speeches, the experiment are composed of 6 tests; test pure tone test, speech reception test, word recognition test, pure tone masking test, formant pure tone masking test and speech masking test, and for objective evaluation, LLR is introduced. As a result of normal hearing subjects and hearing impaired subjects, more making is occurred in hearing impaired subjects than normal hearing subjects when using pure tone, and in the speech masking test, speech reception is also lower in hearing impaired subjects than in normal hearing subjects. This means that acoustic masking effect rather than distortion influences speech perception. So it is required to check the characteristics of masking effect before wearing a hearing aid and to apply this characteristics to fitting curve.
