Simultaneous Aortobifemoral and Bilateral Femoropopliteal Artery Bypass Graft for Multilevel Lower Extremity Occlusive Disease - 2 cases report - (다단계 폐쇄성 하지동맥경화증에서 복부대동맥-양측 대퇴동맥 및 양측 대퇴동맥-슬와동맥간 우회술의 동시 시행 - 2예 보고 -)

  • Atherosclerosis has more than 60% of the causes of arterial occlusive diseases. The abdominal aorta and lower extremity arteries are the most common sites of occlusion. We have treated surgically 2 cases who had intermittent claudication and were diagnosed as simultaneous aortobifemoral and bilateral femoropopliteal obstruction by angiography, but had ineffective results from medical treatment or angioplasty. Simultaneously aortobifemoral bypass using Hemashield Y graft and bilateral femoropopliteal bypass using autologous greater saphenous vein were done. After operations, the symptom disappeared and there were no specific post-operative complications except abdominal wound dehiscence. In postoperative angiography, we had obtained good patency of bypass graft. We are following up patients through the out patient department without recurrence up to 16 months.

Effects of Combined Postural Correction Exercises on Selected Muscle Activity and Stiffness in Children With Cerebral Spastic Diplegia: Case Study (융합적 자세교정치료가 뇌성경직양측마비 아동의 선택된 근육 활성도와 강직도에 미치는 영향: 사례연구)

  • The aim of this study was to investigate the short-term effects of combined postural correction exercises (CPCE) on muscle stiffness and muscle activity in children with cerebral spastic diplegia(CSD). The subjects were 3 children (2 male and 1 female) with CSD from 4 to 7 years old. The measurement sites were upper trapezius muscle(UT), rectus abdominis muscle(RA), and gluteus maximus muscle(Gmax), muscle stiffness and muscle activity were measured using Myoton and surface EMG, and the symmetry of both sides was also examined. As a result, children with severe compensatory action showed decreased muscle stiffness and muscle activity in UT and RA and increase in Gmax after CPCE intervention. Especially, the left and right symmetry of the muscles was decreased. In this study, despite some limitations, CPCE have shown a positive effect in posture correction of children with CSD.

  • A localized tenosynovial giant cell tumor rarely affects the bilateral knee joint compared to the diffuse form. Only a few cases can be found in the literature, and there is no case report of the bilateral localized form of giant cell tumor causing patellofemoral pain syndrome in Korea. This study experienced a case of bilateral localized giant cell tumor, causing patellofemoral pain syndrome. The tumor was excised arthroscopically and confirmed pathologically. This paper reports this case with a review of the relevant literature.

Effects of Occupation-Based Bilateral Upper Extremity Training for Chronic Stroke: Pilot Study (뇌졸중 환자의 작업기반 양측성 상지훈련 효과: 예비연구)

  • Objective : The purpose of this study is to observe effects of Occupation-Based bilateral UE training for chronic stroke. Methods : 4 patients with hemiplegic stroke were divided into two groups: experimental group and control group. From May to August 2016, we conducted a occupation - based bilateral UE training and a traditional bilateral UE training for 5 weeks, 3 times a week, and 60 minutes per session for each subject. EMG, accelerometer, ARAT, Y-BAT, SIS, and COPM were performed before and after the intervention. Results : The experimental group showed a large changes before and after intervention in ECR, DA and all items except for hand function and memory of SIS, compared with the control group. Also, the experimental group showed a large change before and after the intervention compared to the control group in the accelerometer measuring the amount used of bilateral hand and grasp, pinch, and gross movement items of ARAT and COPM, Y-BIT. Conclusions : Occupation-based bilateral upper extremity training was effective in the recovery of upper limb function and social participation through neurological changes. Future research will be required development studies to ensure a high content validity of the Occupation-based bilateral upper extremity training protocol.

The Early Results of CABG with Bilateral Internal Thoracic Artery (양측 내흉동맥을 이용한 관상동맥우회술의 조기 결과)

  • Background: It has been known that internal thoracic artery grafting has a better patency rate compare to other graft conduits in coronary revascularization. Better patency rates can be expected in more coronary arteries with the use of bilateral internal thoracic artery. However, there were some debates on the complications after the use of bilateral internal thoracic artery. The purpose of our study was to reveal the results of bilateral internal thoracic artery. Material and Method: The 26 coronary artery bypass operations with bilateral internal thoracic artery were performed from July 2001 to May 2002. We compared the results of 8 diabetic patients to those of 18 non-diabetic patients. We compared the results of BITA (bilateral internal thoracic artery) group to those of SITA (single internal thoracic artery) group that were 20 patients and performed during same period. Result: There was no mortality. There was one wound complication in the diabetic group and one in the non-diabetic group. There were no significant differences in operation time, duration of mechanical ventilation, amount of bleeding, infusing duration of cardiotonics, and complication between two groups. There were no significant differences in results between the BITA group and the SITA group. Conclusion: There were no significant differences in early results between the BITA group and the SITA group, and there were no significant differences in results between the diabetic group and the non-diabetic group. We think coronary artery bypass grafting with the use of bilateral internal thoracic artery is considered in diabetic patients.

Bilateral Sequential Single Lung Transplantation for Eisenmenger's Syndrome (Eisenmenger's 증후군이 동반된 동맥관 개존증환자에서의 순차적 양측 폐이식술 - 1례 보고 -)

  • Eisenmenger's syndrome is the disease of right to left shunt developing from the increased pulmonary vascular resistance caused by excessive pulmonary blood flow in patients with abnormal connections of systemic to pulmonary blood passage. The heart-lung transplantation was the only curative method in early transplantation period, but good results after bilateral lung transplantation have been reported as the fact that right heart function improved by only lung transplantation. We successfully carried out bilateral sequential single lung transplantation in a 34-year-old female patient with Eisenmenger's syndrome with large PDA. We report this case with a brief review of the literature.

Sequential Surgical Treatment of Hemoptysis Caused by Bilateral Aspergilloma -A case report- (양측 폐상엽 진균구에 의한 객혈의 순차적 수술 치험 1례)

  • We experienced a case of hemoptysis caused by bilateral upper lobe aspergilloma. He was 66 years old and had a history of old unclear pulmonary tuberculosis with irregular medication 20 years ago and intermittent hemoptysis for several years. In x-ray study, there was a bilateral upper lobe aspergilloma with cavity. He received bilateral wedge resection through thoracotomy with some interval to reduce postoperative complications. We selected the priorty of operation through the bronchoscope in the operation room. Both sides had the same pathology of aspergilloma and he was discharged after an uneventful postoperative course.
