편측성대마비환자에 대한 피열연골내전술 (Arytenoid Adduction as a Surgical Treatment for Hoarseness with Unilateral Vocal Cord Paralysis)

  • 다양한 원인에 의하여 야기되는 편측성대마비에 의한 애성을 치료하기 위하여 현재 사용되고 있는 방법으로는 테프론이나 실리콘 등의 성대내 주입술, 갑상연골성형술 등이 있다. 그러나 성대부전이 너무 크거나, 양측성대의 위상차이가 심한 경우에는 상기 수술법으로는 만족스러운 결과를 얻기가 용이하지 않다. 이런 경우에 피열연골의 근돌기를 외과적으로 노출시킨 후 측전방으로 당겨줌으로써 성대의 내전을 유도할 수 있는 피열연골내전술(arytenoid adduction)로 음성개선을 도모할 수 있다. 최근 저자들은 애성을 주소로 영동세브란스병원 이비인후과로 내원하였던 편측성대마비 환자 3예에서 보다 만족할만한 음성개선의 결과를 도모하고자 피열연골내전술을 시행하여 좋은 결과를 얻었기에 보고하는 바이다.

양측성 성대 마비로 오인된 피열간 반흔 1예 (A Case of Interarytenoid Scar Disguising Bilateral Vocal Cord Palsy)

  • The patient suffered cardiac arrest 8 months before presentation. She has been suffering hoarseness and exertional dyspnea and nocturnal stridor. Upon flexible laryngoscopy, her vocal cords showed no motion and fixed in paramedian position. There was no causal finding on neck CT. EMG showed some muscular activity. Under the suspicion of crico arytenoid fixation, we performed suspension laryngoscopy, and found the arytenoid cartilage was fixed with short and stout scar, which was removed with scissors. Just after surgery she regained her voice and respiration.

갑상선 수술 후 양측성 성대마비 환자의 음성치료 1예 (A Case of Voice Therapy for Post-Thyroidectomy Syndrome)

  • Post-thyroidectomy syndrome refers to various voice changes experienced after the thyroid surgery. Among them, surgery or injection therapy has been prioritized over voice therapy for the bilateral vocal cord paralysis. However, if it is possible to determine whether voice therapy can be applied first, a faster treatment recovery will be possible. In this study, voice therapy was performed on a 59-year-old female patient with bilateral vocal cord paralysis after total thyroidectomy. This study is to presents the criteria for applying the voice therapy first for bilateral vocal folds paralysis and to share successful voice therapy sessions.

양측성 성대 마비의 치료 원칙 (Management Principles of Bilateral Vocal Fold Immobility)

  • Bilateral vocal fold immobility (BVFI) is a challenging condition which may result from diverse etiologies including vocal fold paralysis, synkinesis, cricoarytenoid joint fixation, and interarytenoid scar. Most patients present with dyspnea and stridor, but sometimes with a breathy dysphonia. Careful history taking, laryngoscopic evaluation under general anesthesia or awaken status, laryngeal EMG, and imaging studies with CT and/or MRI are helpful for providing a precise diagnosis and planning appropriate managements. In children, congenital neurological disorder is one of the most common etiologies, and spontaneous recovery has been reported in more than 50% of cases. Therefore, observation for more than 6 months while securing the upper airway with tracheostomy if needed is a generally accepted rule before deciding any destructive procedure to be undertaken. In children with advanced posterior glottic stenosis, laryngotracheal reconstruction with rib cartilage graft should be considered. In contrast to children, BVFI most commonly occurs as sequalae of surgical complication in adults. Diverse static or dynamic procedures can be applied; posterior cordotomy, vocal fold lateralization, endoscopic or open arytenoidectomy, arytenoid abduction, and reinnervation, electrical laryngeal pacing, which need to be carefully selected according to each patient's needs and pathophysiology of BVFI.

윤상피열관절과 후두신경을 동시에 침범하여 양측성대마비를 유발한 류마티스 관절염 1예 (A Case of Bilateral Vocal Fold Paralysis Induced by Concurrent Rheumatoid Arthritis Involvement of Laryngeal Nerve and Cricoarytenoid Joint)

  • Rheumatoid arthritis (RA) is a connective tissue disease involving the larynx in 30 % of the patients. Foreign body sensation, hoarseness, and cough are common symptoms in laryngeal involvement. An urgent tracheostomy is required when acute airway obstruction occurs in case of bilateral vocal fold paralysis. The most common cause of bilateral vocal fold paralysis in RA patients is a cricoarytenoid joint arthritis. Laryngeal nerve degeneration is rare cause of bilateral vocal fold paralysis in RA patients. In this case report, an emergent tracheostomy was performed on a 64-years-old male patient with acute dyspnea, and concurrent involvement of RA on laryngeal nerve and cricoarytenoid joint was revealed by laryngeal electromyography and histopathology. The vocal fold mobility was restored after 3-months medical treatment.

말단 비대증 환자에서 발생한 양측성대마비 1예 (A Case of Bilateral Vocal Fold Paralysis from Acromegaly)

  • Acromegalic patients can develop mild upper airway obstruction. However, the limitation of both vocal folds mobility developing dyspnea is rare. We report a case with bilateral vocal cord paralysis associated with acromegaly. The patient visited our clinic presenting dyspnea showing bilateral vocal cord hypomobility in laryngoscopy. The patient underwent a tracheostomy and a transsphenoidal resection of the pituitary adenoma. Thereafter, laser cordotomy with medial arytenoidectomy was done for the permanent treatment of glottal obstruction. The tracheotomy canula was successfully removed one month after the surgery.

  • Bilateral vocal cord palsy is uncommon but is serious because of airway obstruction. Treatments of bilateral vocal cord palsy are initially tracheotomy, vocal cord lateralization and vocal cord reinnervation. Recently, we experienced nerve-muscle pedicle reinnervation in 3 cases of bilateral vocal cord palsy, so reported it with a review of literature.
