Clinical Implications of the Epidermal Growth Factor Receptor Overexpression in the High-grade Astrocytomas (악성 성상세포종에서 표피성장인자 수용체 과발현의 임상적 의의)

  • Purpose : To determine the incidence and prognostic effects of EGFR over-expression in the high-grade astrocytomas. Materials and Methods : With 23 paraffin blocks of the high-grade astrocytomas (7 anaplastic astrocytomas and 16 glioblastoma multiforme), expression of EGFR were evaluated by immunohistochemical staining employing polyclonal antibody raised to short cytoplasmic domain of the molecule. Result : Two out of 7 anaplastic astrocytomas and 9 out of 16 glioblastoma multiforme patients showed overexpression of EGFR(p=0.44). Three out of 11 patients of age below 55 and 8 out of 12 patients of age over 54 showed EGFR overexpression(p=0.141). Median survival of the EGFR negative anaplastic astrocytoma patient was 37 months. Median survival of the glioblastoma multiforme Patients were 11 months in EGFR negative group and 7 months in EGFR positive group. But survival difference was not significant (p=0.17). Conclusion There was a marked trend of increasing overexpression of EGFR in older patients. But survival of the glioblastoma multiforme decreased by the overexpression of the EGFR without significant.

Radiotherapy for the Low-grade Astrocytomas (양성 성상세포종의 방사선치료)

  • Purpose : To evaluate the efficacy of radiotherapy for the low-grade astrocy-tomas and confirm the variables influencing treatment results. Materials and Methods : Forty-six patients with low-grade astrocytoma received radiotherapy after surgical removal (36 patients) or biopsy (10 patients) from 1979 to 1990. Twenty patients had grade I histology and 26 had grade II. External radiotherapy was done by conventional schedule with the total dose of 45 to 60 Gy (median: 54 Gy). The median follow-up period was 5 years. Results : The 2- and 5-year survival rates were $80\%$ and $72\%$, respectively and the 2- and 5-year progression-free survival was $75\%$ and $63\%$, respectively. The survival was influenced significantly by the histologic grade, the histologic type, and performance status. Major complication was not found. Conclusion: In spite of good survival, the local failure was still the major problem. Age and the extent of surgery as well as three favorable factors should be considered in the future treatments.

Evaluation of Metabolic Abnormality in Brain Tumors by In Viuo $^1$H MR Spectroscopy at 3 Tesla (3T 양성자 자기공명분광에 의한 뇌종양의 대사물질 이상소견)

  • To investigate differences between the metabolic ratios of normal controls and brain tumors such as astrocytomas and glioblastoma multiforme (GM) by proton MR spectroscopy (MRS) at 37 high field system. Using 3T MRI/MRS system, localized water-suppressed single-voxel technique in patients with brain tumors was employed to evaluate spectra with peaks of N-acetyl aspartate (NAA), choline-containing compounds (Cho), creatine/phosphocreatine (Cr) and lactate. On the basis of Cr, these peak areas were quantificated as a relative ratio. The variation of metabolites measurements of the designated region in 10 normal volunteers was less than 10%. Normal ranges of NAA/Cr and Cho/Cr ratios were 1.67$\pm$018 and 1.16$\pm$0.15, respectively. NAA/Cr ratio of all tumor tissues was significantly lower than that of the normal tissues (P=0.005). Cho/Cr ratio of glioblastoma multiforme was significantly higher than that of astrocytomas (P=0.001). Lactate was observed in all tumor cases. The present study demonstrated that the neuronal degradation or loss was observed in all tumor tissues. Higher grade of brain tumors was correlated with higher Cho/Cr ratio, indicating a significant dependence of Cho levels on malignancy of gliomas. This results suggest that clinical proton MR spectroscopy could be useful to predict tumor malignancy.
