Parasitic Leiomyoma with Lymphatic Dilatation in Trocar Port-Site of Abdominal Wall: A Case Report (트로카 삽입 부위에 생긴 림프관 확장을 동반한 기생 평활근종: 증례 보고)

  • Uterine leiomyoma is the most common benign pelvic tumor in female and being symptomatic is an indication for surgical removal. As laparoscopic surgery has been developed, some cases related to parasitic leiomyomas in the port site have been reported. A 40-year-old female who a history of previous laparoscopic surgery to remove uterine myoma 2 years ago visited in outpatient clinic of general surgery with palpable mass in left lower abdomen. Contrast enhanced abdomen CT and pelvis MRI were done to evaluate the mass. It was diagnosed parasitic leiomyoma in pathologic study after surgical removal and parasitic leiomyoma should be considered when patient visited presenting abdominal mass with the history of laparoscopic myomectomy.

Immunohistochemical Diagnosis of Primary Renal Hemangiosarcoma in a Dog (개에서 발생한 신장원성 혈관육종의 면역조직화학적 진단)

  • A 7-year-old castrated male Yorkshire Terrier was presented with a palpable intra-abdominal mass. In radiography, a large radioopaque renal mass and small abdominal mass were found on dorsal area of the abdomen. Grossly, red to brown color mass and a cystic structure (hydronephrosis) were embedded in the right kidney. Histopathologically, the mass had many irregular shaped neovascular channels lined by polygonal or oval shaped endothelial cells. These vessels and neoplastic cells had great invasive tendency to adjacent connective or fat tissues. Small abdominal mass had identical morphologic features as in renal mass. According to immunohistochemistry, the neoplastic cells in renal mass demonstrated strong positive signals for vimentin and CD31, and weak positive for von Willbrand factor. However, there were no positive reactions for cytokeratin. Based on the gross, histopathology and immunohistochemistry, this mass was diagnosed as primary renal hemangiosarcoma in a Yorkshire Terrier dog.

Analysis of the Result and Merit of Computed Tomography Guided Percutaneous Needle Aspiration Biopsy of Focal Lung Lesion (CT 유도하 국소적 폐질환의 경피적 흡입생검의 장점과 결과 분석)

  • Percutaneous needle biopsy of pulmonary lesion with use of fluoroscopic guidance is well established as a diagnostic tool but limited by the small size and inaccessibility of certain lesions. However, percutaneous needle biopsy has been used increasingly in relation to advance and the safty of smaller biopsy needle and new imaging modalities such as ultrasound and CT. CT, because of its characteristics of high resolution, allows tissue sampling with considerable safety from area that heretofore could not be visualized under fluoroscopy. The authors summarized 44 pulmonary lesions that underwent CT-guided transthoracic biopsy with fine-needle over a 14 month period and analyzed the sensitivity of PTNB. CT-guided PTNB was done with 20 gauge or 22 gauge Westcott biopoy needle(Manan medical products, USA). A diagnosis was made in 27 of 44 cases(61%) including malignany in 19 of 24 cases and benignancy in 8 of 20 cases. The pulmonary mass lesions were located at the peripheral zone of the lung field in 33 cases and at the central zone in 11 cases. Complications were observed in 2 cases which were pneumothorax and hemoptysis each but specific therapy was not required. The sensitivity of PTNB by one session was 61%(27/44). The sensitivity of malignancy was 79%(19/24) and benignancy was 40%(8/20). These results suggest the usefulness of PTNB using fine needles be increased in earlier diagnosis and improved staging of pulmonary nodular lesions without significant complications.

  • Pleural masses may be caused by various conditions, including benign and malignant neoplasms and non-neoplastic tumorlike conditions. Primary pleural neoplasms include solitary fibrous tumor, malignant mesothelioma, and primary pleural non-Hodgkin's lymphoma. Metastatic disease is the most common neoplasm of the pleura and may uncommonly occur in patients with hematologic malignancy, including lymphoma, leukemia, and multiple myeloma. Pleural effusion is usually associated with pleural malignancy. Rarely, pleural malignancy may arise from chronic empyema, and the most common cell type is non-Hodgkin's lymphoma (pyothorax-associated lymphoma). Non-neoplastic pleural masses may be observed in several benign conditions, including tuberculosis, pleural plaques caused by asbestos exposure, and pleural loose body. Herein, we present a review of benign and malignant pleural neoplasms and tumorlike conditions with illustrations of their computed tomographic images.

  • A 12-year-old neutered male Shih Tzu presented with constipation and dyschezia. Abdominal radiographs showed distension of the descending colon and dorsal compression of the colon by a soft tissue mass. The mass was well-marginated with homogeneous soft tissue attenuation and showed no evidence of metastasis on computed tomography (CT). The dynamic CT showed a consistently mild contrast enhancement. The perfusion and capillary permeability were lower than those of the gluteal muscle. The tentative imaging diagnosis was a benign intrapelvic tumor, which rarely shows angiogenesis. The mass was excised, and a leiomyoma was confirmed by histopathologic examination.

A Case of Canine Uterine Adenocarcinoma with Negative Estrogen and Progesterone Receptor Expression (개의 에스트로겐과 프로케스테론 수용체 발현이 되지 않은 자궁 선암종 증례)

  • A 12-year-old female mixed breed dog receiving a progesterone drug was referred for evaluation of an abdominal mass. Abdominal radiography and ultrasonography revealed a swollen uterus and an associated mass. Serum chemistry revealed hyperglobulinemia consistent with acute inflammation based on the results of serum protein electrophoresis. Fine needle aspiration of the mass guided by ultrasonography was performed for cytological evaluation. The cytological impression was consistent with adenocarcinoma. Exploratory laparotomy identified a uterine body mass, which was surgically removed for histopathology. Histology of the mass identified a uterine adenocarcinoma. Immunochemistry using anti-estrogen and progesterone receptor antibodies was performed and neoplastic cells were negative to both antibodies while some normal elements were reactive to both of them. Computer tomography demonstrated evidence of metastatic disease in the lung one week after the surgery and the dog died about 40 days after surgery.

Florid Reactive Periostitis of the Clavicle: A Case Report (쇄골에 발생한 개화성 반응성 골막염: 증례 보고)

  • Florid reactive periostitis (FRP) is a rare benign fibro-osseous proliferation, occurring mostly in the short tubular bones of hands and rarely in the long tubular bones. We report a surgically confirmed case of FRP involving the clavicle in a 26-year-old male. On MRI scans, a soft tissue mass with T2 high signal intensity was found that originated from the periosteum of the clavicle and included surrounding a periosteal elevation and perilesional soft tissue edema. Strong contrast enhancement was noted inside the mass and along the periosteum involving more than half of the circumference of the clavicle. Serial radiographs revealed a soft tissue mass without mineralization that turned into an ossified mass with a solid periosteal reaction within a month.

Is $^{99m}Tc$-MDP Mammoscintigraphy Useful in Patients with Breast Mass Lesions? ($^{99m}Tc$-MDP 유방 스캔은 유방 종괴의 감별진단에 임상적인 유용성이 있는가?)

  • Purpose: Piccolo et al. advocated the usefulness of $^{99m}Tc$-MDP breast scan in differential diagnosis of breast mass with high accuracy. But there are little or no follow-up studies about it as we know. We studied $^{99m}Tc$-MDP uptake pattern and lesion/background ratio in patients complaining palpable breast lesions to evaluate the clinical usefulness of $^{99m}Tc$-MDP scan. Materials and Methods: Total 34 patients were studied with physical examination, mammo-gram and $^{99m}Tc$-MDP scan prospectively. Anteroposterior and both lateral view of breast were obtained 5 minutes after iv injection of 740 MBq $^{99m}Tc$-MDP. Breast uptake pattern of $^{99m}Tc$-MDP was analyzed by a grade system: 0=no uptake, grade 1=bilateral diffuse uptake, grade 2=asymmetric faint uptake, grade 3=focal hot uptake. 20 cases were pathologically confirmed by excision biopsy or aspiration biopsy. 14 cases were normal in physical examination and mammogram. Results: Pathologic results showed 7 carcinomas, 6 benign solid tumors, and 7 fibrocystic changes. Grade 3 pattern of $^{99m}Tc$-MDP uptake was noted in 4/7 carcinomas, 3/6 benign solid tumors, and 1/7 fibrocystic changes. Grade 2 pattern was 217, 0/7, 3/7 respectively. The average L/B ratio was 1.66 in carcinomas, 1.68 in benign solid masses, 1.20 in fibrocystic diseases, 1.05 in normal patients. L/B ratio was higher in carcinoma and benign mass groups than in fibrocystic change and normal control groups(p=0.005). But there was no statistical difference between L/B ratio of malignant mass group and benign mass group. Conclusion: $^{99m}Tc$-MDP scan is not suitable to routine clinical use for breast mass diagnosis. It might be used in limited conditions when whole body bone scan is planned.

Intracapsular and Paraarticular Chondroma of the Infrapatellar Hoffa's Fat Pad: A Case Report (Hoffa씨 지방층의 관절강내 및 관절주위 연골종: 증례 보고)

  • Intracapsular and paraarticular chondroma is a rare benign lesion of the large joints (mostly the knee). We report a case of intracapsular and paraarticular chondroma in the infrapatellar Hoffa’s fat pad that presented as a painful palpable mass in 15-year-old woman. A physical examination revealed a firm, movable and tender mass in the infrapatellar area. Magnetic resonance images showed an ovoid, well-defined, soft tissue mass with focal calcification in the infrapatellar fat pad. The final pathology revealed an intracapsular and paraarticular chondroma.

  • Ganglionic cyst is the most common benign tumor in the wrist and hand, and easily diagnosed with ultrasonography. Ganglionic cyst in sonography usually appears as hypoechoic or anechoic with well-demarcated cystic mass. We report a case of the hyperechoic mass at the volar aspect of the wrist, which confirmed as ganglionic cyst by operative and histological findings.
